      • 최근 10년간 원광의대병원에서 시행한 소아 심장혈관 수술의 임상적 고찰

        We reviewed 10 years experiences of the clinical aspects and early postoperative results in 544 pediatric patients(age less than 16 years old). Among them. 529 cases had the congenital heart diseases, and 15 had the acquired diseases. Open heart surgery was performed in 413 patients. 115 cases were treated with non-open heart surgery. 16 with interventional catheterization. Annual increase of the cardiac patients was not significant in recent 10 years, except slight increase in 1994. The mean age of the study patients had been evidently changed to younger year by year. Total mortality rate of the open heart surgery was 6.4%. The cyanotic congenital heart diseases were high in early postoperative mortality by 25.7%, whereas 4% in acyanotic group. The age group less than one month was most highest in surgical mortality(66.7%). There were no deaths in patients with patent ductus arteriosus(103 cases), atrial septal defect(83 cases) or pulmonary stenosis(17 cases). Among 220 patients with ventricular septal defects(VSD). 10(4.5%) were died early postoperatively. In cyanotic group, the patients with pulmonary atresia with ventricular septal defects(PAVSD), transposition of the great arteries(TGA), interrupted aortic arch(IAA), complex cardiac anomalies with isomerism seemed to be most susceptible to an early death.

        김종오, 윤진환. 저항성 운동이 노화 흰쥐 해마 및 소뇌의 신경가소성 인자 발현 및 미세구조와 신경학적 기능에 미치는 영향. 운동과학, 제23권 제3호, 217-228, 2014. 본 연구는 노화로 인해 나타나는 기억력 감소와 균형감각의 저하를 저항성 운동이 지연시킬 수 있는지와 그 기전을 밝힘으로써, 노인운동처방학적 운동기전을 제시하고자 하는데 있다. 이러한 목적을 달성하기 위해 노화 흰쥐를 대상으로 tower climbing 운동을 12주간 실시한 후 신경기능학적 향상, 신경가소성 인자인 BDNF, TrkB의 발현 증가와 시냅스 관련 단백질인 synapsin I의 발현을 유도할 수 있는 지와 해마의 연접형성수준의 증가에 어떠한 영향을 미치는 지를 확인하고자 하였다. 본 연구에 이용된 실험동물은 Sprague-Dawley 계열의 노화 흰쥐 72주령 수컷과 대조군으로 10주령 수컷이 이용되었으며, 이들은 실험 전 일주일 동안 운동 환경에 대한 적응기간을 거친 후, 무작위 표본추출에 의하여 노화 쥐는 노화 대조군(aging control group, ACG; n=15)과 노화 운동군(aging and exercise-trained group, AEG; n=15)으로 분류되었고, 10주령의 정상 대조군(normal control group, NCG; n=15)이 비교군으로 이용되었다. 저항성 운동은 tower climbing protocol을 이용하여 1일 약 30분씩 3회(09시, 14시, 20시) 12주 동안 훈련시켰다. 공간학습과 기억력과 균형감각과 신경학적 기능평가는 Morris water maze와 balance beam 검사가 이용되었고, BDNF, TrkB와 synapsin I과 같은 단백질 분석은 Western blotting 방법이 이용되었다. 또한, 해마부위 연접형성수준의 변화는 투과전자현미경(TEM)적 관찰을 통해 확인하였다. 그 결과 본 연구에 수행된 저항성 운동인 tower climbing 운동의 12주간 훈련은 노화로 인해 해마, 소뇌 부위의 신경 및 연접가소성인자 감소로 인해 저하된 공간학습기억능력과 평형감각기능을 지연시키는데 효과적인 운동임을 확인할 수 있었던 연구이다. Exercise Science, 23(3): 217-228, 2014. Aging results in cognitive and balance decline both in humans and animals. Neurogenesis in the hippocampus and cerebellum also declines with age. Traditionally, exercise has been used for the treatment of several neuro degenerative disorders. Thus, the purpose of this study was to investigate the effects of resistance exercise on spatial learning memory ability, the synapses and the synaptic-plasticity neurotrophic factor in hippocampus and cerebellum in aged-rats. The experimental animals were divided into three groups (n=15 in each group): the normal control group (NCG), the aging control group (ACG), the aging and exercise-trained group (AEG). Animals in the exercise group were performed to daily exercise on the tower climbing during 12 weeks. The spatial learning and memory capability was evaluated by Morris water maze in a separate set of rats. Also, the balance function was evaluated by Balance beam test. Western blotting analysis for the expression of BDNF, TrkB and synapsin I was performed. Electron microscopy were used to evaluate synaptic plasticity and quantitative electron microscopy was used to investigate morphological changes in the hippocampus. Measurements taken from analysis of the electron micrograph sets were used to calculate the spine synapses in hippocampus. Here in this study, it have shown that both synaptic plasticity neurotrophic factor expression and spine synapses were suppressed in aged-rat, whereas tower climbing exercise alleviated the aging-induced suppression of both synaptic-plasticity neurotrophic factor expression and spine synapses in the hippocampus of rats. The results of the present study indicate that tower climbing exercise may facilitate recovery from the CNS complications associated with aging by inducing enhanced the morphological development of synapses in the hippocampus via the augmentation of synaptic plasticity neurotrophic factor expression in the h ippocampus a nd t he cerebel l um.

        현행 중등학교 과학 실험 , 실습 교육 실태 조사 및 그 운영 진단 (Ⅱ) : 고등학교 과학 실험 , 실습 교육을 중심으로

        본 연구는 96년도에 실시한 우리 나라 중학교 과학 실험·실습 교육의 실태 조사 연구에 따른 계속 연구의 일환으로 고등학교 과학 실험·실습 교육의 실태를 조사하기 위한 것이다. 연구내용은 고등학교 과학 교과서의 분석, 실험·실습 실시 현황 분석, 현행 실험·실습 교육의 문제점 분석, 현행 학교 구성원들의 실험·실습 교육 운영 실태 조사 및 현행 실험·실습 교육의 개선을 위한 연구과제 도출을 주요 연구내용으로 다루었다. 연구방법은 전국의 고등학교 80개교를 대상으로 이들 학교의 학생 1,977명, 교사 165명, 학교장 80명에게 적용하였다. 본 연구에서 밝혀진 결과를 중심으로 우리 나라 고등학교 과학 실험 실습 교육의 정상화를 위한 실험 내적인 연구과제를 제시하면 다음과 같다. (1)교재 개발에 대한 연구, (2)실험 킷트 개발에 대한 연구, (3)교사교육 및 재교육 프로그램 개발에 대한 연구, (4)교실 밖 실험·실습 활동 프로그램 개발에 대한 연구, (5)과학 실험·실습 소재의 개발에 대한 연구. This study was accomplished to analysis and survey on the experimental and practical science education of high school in korea for the consecutive study of the an analysis and survey on the experimental and practical science education of middle school in korea(Lee, Yoon-Jong et al., 1997). The status of facilities, management for the experiment, practices, teaching methods in high schools have been investigated. The present status and reasonable management of the high school science education have been grasped from the questionaires. To do this 165 high school science teachers, 1977 students and 80 principals of high schools in Korea are administered questionaires of Science Education Research Institute of Kyungpook National University(1997). The results of this study are as follows : The reasonable management for experiments and practices of science education were scanty in the high school around the urban and rural school owing to the shortage of facilities and equipments, crowded class, excessive class works for teacher, excessive contents of present textbooks and insufficiency of the administrative supports etc. The current teaching method of high school science has emphasized knowledge. This fact does not satisfy the objective of learning due to lack of the teaching method. Desirable directions for the improvement of present status of high school science education were proposed in this paper.

        내인성 혈장 리튬 농도와 정신분열병

        Objectives : The aim of this study was to compare endogenous plasma lithium concentrations among schizophrenic patients classified by DSM-IV subtype and control groups and to investigate the correlation of endogenous plasma lithium concentration and psychotic symptoms in schizophrenia. Methods : Schizophrenic patients were selected among psychiatric inpatients without lithium medication and then classified by DSM-IV schizophrenia subtype, Schizophrenic patient groups were composed of 15 disorganized type, 15 paranoid type and 15 undilferentiated type schizophrenic patients. The control group was composed of 15 healthy subjects without any psychiatric disease,Endogenous plasma lithium concentrations were estimated by inductively coupled plasma atomic emission spectrometer. The psychotic symptoms in schizophrenic patients were classified as positive symptoms, negative symptoms and disorganized symptoms according to andreasen classificationl about SANS and SAPS items. Endogenous plasma lithium Con-centration among three subtypes of schizophrenia and control group was compared, and correlation between endogenous plasma lithium concentrations and psychotic symptoms was examined. Rcsults : 1) Schizoprenic patients showed higher endogenous plasma lithium concentration than control groups (p=0.033). Endogenous plasma lithium concentrations were significantly different among three subtypes of schizophrenia (p=0.001). Compared with the control group, disorganized type showed higher endogenous plasma lithium concentration, but paranoid type and undifferentiated type were not significantly different. 2) Disorganized symptoms correlated with endogenous plasma lithium concentration (r=0.416, P=0.004), but negative symptom and positive symptom did not significantly correlate with endogenous plasma lithium concentration (r=0.202, P=0.184. r= -0.216, P=0.155). Conclusion : These results suggested that schizophrenic patients with disorganized symptom show the differences in utilization or distribution of endogenous lithium.

        운동형태의 차이가 Alloxan유발 당뇨쥐의 혈당 및 인슐린 농도에 미치는 영향

        The Purpose of this study was to investigate the effect of exercise intensity on blood glucose and insulin levels in diabetic rats with alloxan. Twenty sprague-dawley male rats were assigned to power training groups(n=10) and endurance training groups(n=10). Diabetes was induced by single injection of alloxan(50 mg/kg B.W) Blood glucose and insulin were determined every week for 4-weeks. The conclusion of this study follows as below: 1) There was significantly different on blood glucose levels in two groups. 2) There was significantly different on insulin levels in two groups These results suggest that proper exercise intensity in diabetic rats can significantly decrease blood glucose and insulin level and make good glycemic control. In conclusion, it has been found that regular prolonged endurance training with diabetic rats, improves insulin and blood glucose.

      • 接觸酸化法을 利用한 LAS 除去效率에 關한 硏究

        This study was performed to know LAS concentration in the small urban stream and estimate the LAS removal efficiency when apply to natural purification method with variable media. When we used gravel and concrete as media, the average LAS concentration of influent was 819.7㎍/ℓ (112.0 ∼ 1671.0 ㎍/ℓ). The average LAS concentration of effluent gravel media reactor was 58.5 ㎍/ℓ (0 ∼ 158.4 ㎍/ℓ) and average removal efficiency was 93.8%. The average LAS concentration of effluent concrete media reactor was 33.1 ㎍/ℓ (0 ∼ 136.8 ㎍/ℓ) and average removal efficiency was 96.1%. And when we used tire and scrubber, the average LAS concentration of influent was 693.7 ㎍/ℓ (241.5 ∼ 1330.3 ㎍/ℓ). The average LAS concentration of effluent tire media reactor was 88.4 ㎍/ℓ (47.8 ∼ 131.8 ㎍/ℓ) and average removal efficiency was 83.9%. The average LAS concentration of effluent scrubber media reactor was 26.1 ㎍/ℓ(0 ∼ 58.9 ㎍/ℓ) and average removal efficiency was 95.3%.

        Supine Bicycle Ergometer 운동이 뇌파와 기분상태에 미치는 영향

        In this study we investigated the effects of supine ergometer exercise on physiological and psychological relaxation. Seven healthy young men(aged 24.4±2.00years) volunteered for the experiment. The experiment consisted of the following three successive segments" A pre-exercise period of 10 min, during which the subjects rested in a supine posture with their eyes closed for the final 5 min; an supine ergometer exercise period of approximately 10 min, during which the subjects performed intensity of 50%HRmax exercises; A post-exercise recovery of 5 min, and a recovery of 10 min, which was similar to the pre-exercise rest period. The electroencephalo-graphic(EEG) activity of the central cortex of the brain were measured pre-exercise, post-exercise 5 min, and recovery phase 10min. Fast Fourier Transformation of the EEG was used to obtain power spectrum areas in the delta(0.5~4 Hz), theta(4~8 Hz), alpha(8~13 Hz) and beta(13~30 Hz) frequencies. We compared the relative power values(power %) of the electroencephalogram alpha bands(8~13 Hz) and korean edition of the profile of mood states(K-POMS) before and after the supine ergometer exercise. We also estimated the percentage of maximal heart rate(%HRmax) throughout the experiment to ascertain the intensity of the supine bicycle ergometer exercise. The results of %HRmax indicated that the intensity of supine exercises practised in the experiments ranged from low to moderate. The power % of EEG alpha bands had increased significantly after the supine ergometer exercise compared with the pre-exercise rest(p<0.05). From the POMS results, we observed that positive mood(vigour) increased and negative mood(tension, depression, and total mood)decreased significantly after the supine ergometer exercise(p<0.05). This study found that the subjects showed increased physiological and psychological indices of relaxation after supine bicycle ergometer exercise.

      • 지하철 운행구간중 D철교 주변지역의 소음도 예측에 관한 연구

        This study was carried out to predict and evaluate the outdoor noise levels around the D railway bridge of the subway line. The counter-measure for noise control was reviewed as the tunnel. The outdoor noise levels are generally calculated by subtracting sound transmission loss of the tunnel envelope from indoor noise levels. The coefficient of sound absorption of tunnel envelope was NRC 0.6 and sound transmission loss STC 23. Prediction of the noise levels around the D railway bridge was achieved by dividing the circumference into 10m×l0m regular grid spaces. The indoor noise levels at the tunnel ranged from approximately 45dB(A) to l07dB(A) and the outdoor noise levels 37dB(A) to 67dB(A) as peak sound level. In this study the maximum sound level at all locations around the subway line should not exceed 60dB(A) as energy equivalent sound level(Leq). As a result all points around the D railway bridge satisfied the noise level limit with the above tunnel.

        동형접합성 단백 C 결핍 환아의 치과적 치험례

        단백 C 결핍증은 항응고인자인 단백 C의 결핍으로 혈전 색전증의 위험성이 높다. 선천성 단백 C 결핍증 중 동형접합성 단백 C 결핍증은 단백 C의 활성도가 측정되지 않을 정도로 낮고 1/25만~50만의 발생 빈도를 가지는 희귀한 질환이다. 동형접합성 단백 C 결핍증의 주요 증상은 자반성 및 괴사성 피부 병변, 반상출혈, 설명, 중추신경계의 혈전증 등이다. 본 증례는 동형접합성 단백 C 결핍증인 만 4세 여아로 치아의 전반적인 우식을 주소로 내원하였다 본 환아는 와파린 복용중이었고 치과치료시 합병증의 예방을 위해 소아청소년과에 협진 의뢰하였다. 와파린 복용 중단 시 혈전으로 인한 심각한 합병증이 예상되어 와파린을 적절하게 (INR 3~5) 복용하되 치과 치료 중 과다 출혈 시 신선냉동혈장으로 조절할 것을 권고받았다. 이 환아는 설명을 동반한 중증 장애 아이로 행동조절이 어려웠고 과다 출혈시 신속한 처치가 가능하도록 하기 위해 전신마취 하에 치과치료를 시행하였다. 저자는 치아의 전반적인 우식을 주소로 내원한 만 4세 단백 C 결핍 환아에 대하여 전신 마취 하 치과치료를 시행하였으며 다소의 지견을 얻었기에 보고하는 바이다. Protein C deficiency increases the risk of thrombosis due to the lack of anticoagulant factor protein C. Among the numerous congenital protein C deficiencies, homozygous protein C deficiency has an especially low protein C activity level, that it is almost undetectable. It is a rare disease with a probability of 1:250000~500000. The signs and symptoms of homozygous protein C deficiency include purpuric, necrotic dermatosis, ecchymosis, blindness, and thrombosis in central nervous system. A 4-year-old girl was brought to the clinic with a chief complaint of extensive caries. The child was under warfarin medication in order to prevent possible complications during dental treatment. We consulted the pediatric department. Without warfarin intake, serious complications may occur due to thrombosis during dental treatment. Therefore, certain warfarin dosage (INR 3~5) and fresh frozen plasma as a backup for excessive hemorrhage were recommended. This child was a severely disabled child with the loss of vision, and it was difficult to manage her behavior effectively. Thus, dental treatment was carried out under general anesthesia, where bleeding control would be also easier to achieve.This report presents the case of a 4-year-old girl with protein C deficiency, who has received dental treatment for extensive caries under general anesthesia.

      • 알코올 섭취 후 운동이 시간 경과에 따른 혈중 알코올 농도와 간 기능 효소에 미치는 영향

        This study was done in order to investigate the effects of performing the light exercise right after alcohol drinking on the changes of blood alcohol and ALT, AST concentration, All seven healthy collegiate students volunteered to two sections of experiment, Each subject drank one bottle of Soju(360ml, 75.6g ethanol) For forty minutes at the same speed of drinking with the same kind and volume of carbohydrate snacks. Each section of the study was conducted by a two-cross-over design with four days wash-out period, All volunteers performed brisk walking exercise corresponding to about 50-60%HRmax of exercise intensity For 30minutes immediately after alcohol drinking. Blood sampling was conducted by drawing about 10ml venous blood from antecubital vein at pre -alcohol drinking, and post- 1, 2, 4 hour alcohol drinking. Blood alcohol concentration reduced significantly at all time course in exercise group compared to Control group. However, the concentration of serum aspartate aminotransferase and alanine aminotransferase, the indicator of liver cell damage, showed no significant difference between two groups. In conclusion, performing light exercise such as brisk walking after alcohol drinking may somewhat help to reduce the concentration of blood alcohol Faster by influencing the alcohol metabolism.
