      • The Impact of Inflammation Grade of Liver Histology on the Improvement of Liver Stiffness Assessed by Transient Elastography

        Aims: Transient elastography (TE; Fibroscan) is now almost indispensable tool to estimate liver fibrosis. Although many clinical factors are known as confounding factors of liver stiffness (LS), there is no knowledge of who will achieve an improvement of liver stiffness if they have a similar liver fibrosis stage. The aim of this study is to see whether baseline hepatic inflammation may affect accurate LS measurement and which factors are associated with improvement of LS in Fibroscan. Methods: This retrospective study included consecutive 678 patients who underwent baseline liver biopsy and sequential LS assessment from 2006 to 2016 at 6 tertiary hospitals in Korea. Liver fibrosis and inflammation were graded on the basis of standard guideline proposed by the Korean Study Group for the Pathology of Digestive Diseases. LS measurement was performed at baseline and 1, 3, 5 years. Improvement of LS was defined as decreased LS value compared with baseline. Logistic regression was used to evaluate factors associated with improvement of LS in Fibroscan. Results: Mean age of the patients was 47.12±12.25 years and 48.5% were male. Six hundred 2 patients had viral hepatitis (419 HBV; 183 HCV), 76 non-viral hepatitis. Fibrosis stages 0, 1, 2, 3 and 4 were identified in 13 (1.9%), 96 (14.2%), 132 (19.5%), 186 (27.4%) and 251 (37.0%) patients, inflammation grade 0, 1, 2 and 3 were in 28 (4.1%), 278 (14.0%), 279 (41.2%), and 93 (13.7%), respectively. Baseline inflammation grade was correlated with baseline LSM value, and showed linear correlation with ΔLSM. In addition, as the grade of inflammation increased, the higher percentage of patients showed improvement of LSM. A multivariate analysis showed that higher degree of hepatic inflammation was an independent good predictor for LS improvement (adjusted hazard ratio, 3.33; 95% confidence interval, 1.20-9.26; P=0.021) after adjustment for fibrosis stage, platelet count, total bilirubin and alanine aminotransferase level. The association of LSM and hepatic inflammation was more significant in viral hepatitis compared with non-viral etiology. Conclusions: Baseline hepatic inflammation has significant impact on LS value and improvement of LSM, and should be considered as one of the confounding factors of measuring liver stiffness using Fibroscan.

      • Current Status of Ultrasonography Examination Operated by National Cancer Surveillance Program for Hepatocellular Carcinoma: Multi-Center Large-Scale Research

        Aims: Abdominal ultrasonography (USG) is recommended as a surveillance test for the high-risk group of hepatocellular carcinoma (HCC). However, there is lack of information about how USG examination is actually performed in clinical practice, and how the quality control related to USG examination is managed. We investigated the incidence of HCC by a reference point of 6 months after USG was conducted. Methods: We collected data of surveillance USG from high-risk group of HCC (liver cirrhosis, or chronic hepatitis B or C over 40 years) group during 2017 in 8 tertiary hospitals. To determine the accuracy of the ultrasound examination itself, patients who had a prior history of HCC or who had CT or MRI scan within 6 months of ultrasound were excluded. If ultrasound was performed more than twice during 2017, the results of the first test were used. Results: In 2017, 45 experienced hepatologists or radiologists performed 8,512 ultrasounds. Mean age was 53.5 ± 11.0 years and, 4817 (56.6%) were males. Chronic hepatitis B was the most common indication (6319, 74.2%), and 2429 patients (28.5%) had liver cirrhosis. Doctors had 15.0 ± 8.3 years of experience, and proportion of hepatologists was higher than that of radiologists (61.4% vs. 38.6%). The time taken for the ultrasound scan was 4.1 ± 3.3 minutes excluding the preparation time. The detection rate of HCC through surveillance USG was 0.3% (29 patients). During 27 months of follow-up, a total of 60 patients (0.7%) developed new HCC. Of these, 15 patients (0.2%) were diagnosed with HCC within 6 months of the surveillance USG, and 45 patients (0.5%) were diagnosed after 6 months of the surveillance test. Conclusions: This is the first study on the current status of USG examination as a surveillance modality for HCC. It is necessary to develop quality indicator and quality assessment of USG to increase the detection rate of HCC.

        A Study on the Safe Route through the Analysis of the Density of the Gill Netters in the West Sea

        본 연구는 서해안에서 상선의 어구 손괴로 인한 추진기 장애를 예방하기 위해 연안자망어선의 월별분포를 제시하였다. 분석 방법은 2014년 1년간의 어선위치발신장치 데이터를 이용하였다. 조업 해구도를 기준으로 경·위도 30′× 30′크기의 해구별 간격으로 격자를 설정하였다. 총 56개 해구를 대상으로 격자를 구성하였다. 연구 결과 밀도지수가 가장 높았던 194해구는 어구 손괴가 없었다. 반면, 밀도지수가 상대적으로 작은 193해구는 상선의 주 통항경로에 어구가 설치되기 때문에 손괴가 발생했다. 이 분석을 바탕으로 어구 손괴는 밀도지수에 비례하지 않고 상선의 주 통항경로에 따라 손괴위험이 존재하는 것으로 나타났다. 따라서 상선의 안전항해를 위해 5월부터 9월까지 위도 34.5° ~ 35.5°, 경도 125.67° ~ 126°를 항행경보구역으로 통보하면 추진기 장애와 어구손괴와 같은 해양사고를 예방할 것이다. 따라서 상선이 193해구와 203해구를 항해할 때는 안전한 항해를 위해 위도 34.5° ~ 35.5°, 경도 125.5° ~ 125.67°을 추천한다. This study attempted to present the monthly distribution of the inshore gill netters for the prevention of propeller failure caused by the damage of fishing gear by the merchant vessels in the West Sea. This study was conducted using the transmitting device of fishing location (V-Pass) data for 1 year in 2014. The grid intervals are 30 minute × 30 minute (latitude × longitude) based on the section of the sea. A total of 56 sections were analyzed by constructing a grid. The results revealed that the section no. 194 with the highest density index showed no fishing gear damage accident. But the section no. 193 with a relatively lower density index showed more frequent fishing gear damage accident. It is because the section no. 193 is included in the main route of the merchant vessels. Based on this analysis, it is found that the fishing gears are not damaged in proportion to density index but risk of damage exists according to the main routes of merchant vessels. Therefore, for the safe navigation of the merchant vessels, notifying the waters of 34.5° ~ 35.5° at latitude and 125.67° ~ 126° at longitude, as a navigational warning area from May through September, will prevent marine accident, such as propeller failure and fishing gear damage. Accordingly, when the merchant vessels navigate in the section no. 193 and 203, the area of 34.5° ~ 35.5° at latitude and 125.5° ~ 125.67° at longitude is recommended for the safe navigation.

      • Loss-of-function screens of druggable targetome against cancer stem–like cells

        <P>Cancer stem–like cells (CSLCs) contribute to the initiation and recurrence of tumors and to their resistance to conventional therapies. In this study, small interfering RNA (siRNA)-based screening of ∼4800 druggable genes in 3-dimensional CSLC cultures in comparison to 2-dimensional bulk cultures of U87 glioma cells revealed 3 groups of genes essential for the following: survival of the CSLC population only, bulk-cultured population only, or both populations. While diverse biologic processes were associated with siRNAs reducing the bulk-cultured population, CSLC-eliminating siRNAs were enriched in a few functional categories, such as lipid metabolism, protein metabolism, and gene expression. Interestingly, siRNAs that selectively reduced CSLC only were found to target genes for cholesterol and unsaturated fatty acid synthesis. The lipidomic profile of CSLCs revealed increased levels of monounsaturated lipids. Pharmacologic blockage of these target pathways reduced CSLCs, and this effect was eliminated by addition of downstream metabolite products.       • Lanthanum Doping Enabling High Drain Current Modulation in a p-Type Tin Monoxide Thin-Film Transistor

        <P>Effects of lanthanum (La) loading on the structural, optical, and electrical properties of tin monoxide (SnO) films were examined as a p-type semiconducting layer. La loading up to 1.9 atom % caused the texturing of the tetragonal SnO phase with a preferential orientation of (101), which was accompanied by the smoother surface morphology. Simultaneously, the incorporated La cation suppressed the formation of n-type SnO<SUB>2</SUB> in the La-doped SnO film and widened its optical band gap. These variations allowed the 1.9 atom % La-loaded SnO film to have a high hole mobility and carrier density, compared with the La-free control SnO film. The superior semiconducting property was reflected in the p-type thin-film transistor (TFT). The control SnO TFTs exhibited the field-effect mobility (μ<SUB>SAT</SUB>) and <I>I</I><SUB>ON/OFF</SUB> ratio of 0.29 cm<SUP>2</SUP> V<SUP>-1</SUP> s<SUP>-1</SUP> and 5.4 × 10<SUP>2</SUP>, respectively. Enhancement in the μ<SUB>SAT</SUB> value and <I>I</I><SUB>ON/OFF</SUB> ratio was observed for the TFTs with the 1.9 atom % La-loaded SnO channel layer: they were improved to 1.2 cm<SUP>2</SUP> V<SUP>-1</SUP> s<SUP>-1</SUP> and 7.3 × 10<SUP>3</SUP>, respectively. The reason for this superior performance was discussed on the basis of smoother morphology, suppression of disproportionation conversion from Sn<SUP>2+</SUP> to Sn + Sn<SUP>4+</SUP>, and reduced gap-state density.</P> [FIG OMISSION]</BR>

        Specific Inhibition of the Distribution of Lobeglitazone to the Liver by Atorvastatin in Rats: Evidence for a Rat Organic Anion Transporting Polypeptide 1B2–Mediated Interaction in Hepatic Transport

        <P>Cytochrome P450 enzymes and human organic anion transporting polypeptide (OATP) 1B1 are reported to be involved in the pharmacokinetics of lobeglitazone (LB), a new peroxisome proliferator-activated receptor gamma agonist. Atorvastatin (ATV), a substrate for CYP3A and human OATP1B1, is likely to be coadministered with LB in patients with the metabolic syndrome. We report herein on a study of potential interactions between LB and ATV in rats. When LB was administered intravenously with ATV, the systemic clearance and volume of distribution at steady state for LB remained unchanged (2.67 +/- 0.63 ml/min per kg and 289 +/- 20 ml/kg, respectively), compared with that of LB without ATV (2.34 +/- 0.37 ml/min per kg and 271 +/- 20 ml/kg, respectively). Although the tissue-to-plasma partition coefficient (K-p) of LB was not affected by ATV in most major tissues, the liver K-p for LB was decreased by ATV coadministration. Steady-state liver K-p values for three levels of LB were significantly decreased as a result of ATV coadministration. LB uptake was inhibited by ATV in rat OATP1B2-overexpressing Madin-Darby canine kidney cells and in isolated rat hepatocytes in vitro. After incorporating the kinetic parameters for the in vitro studies into a physiologically based pharmacokinetics model, the characteristics of LB distribution to the liver were consistent with the findings of the in vivo study. It thus appears that the distribution of LB to the liver ismediated by the hepatic uptake of transporters such as rat OATP1B2, and carrier-mediated transport is involved in the liverspecific drug-drug interaction between LB and ATV in vivo.</P>

        Estimating Suitable Probability Distribution Function for Multimodal Traffic Distribution Function

        The purpose of this study is to find suitable probability distribution function of complex distribution data like multimodal. Normal distribution is broadly used to assume probability distribution function. However, complex distribution data like multimodal are very hard to be estimated by using normal distribution function only, and there might be errors when other distribution functions including normal distribution function are used. In this study, we experimented to find fit probability distribution function in multimodal area, by using AIS(Automatic Identification System) observation data gathered in Mokpo port for a year of 2013. By using chi-squared statistic, gaussian mixture model(GMM) is the fittest model rather than other distribution functions, such as extreme value, generalized extreme value, logistic, and normal distribution. GMM was found to the fit model regard to multimodal data of maritime traffic flow distribution. Probability density function for collision probability and traffic flow distribution will be calculated much precisely in the future.
