과립상 세포종은 Schwann 세포 기원으로 생각되며 인체에 비교적 드물게 발생한다. 과립상 세포종은 전신 어느 곳에서나 발견될 수 있으나 주로 혀, 구강, 피부 혹은 유방 등에서 호발하며 드물게 위장관에서 발견된다. 위장관에서는 식도에서 가장 호발하며 다음으로 위, 대장 순이다. 과립상 세포종은 대부분, 특히 위장관에서는 양성이며 소수의 악성 병변이 보고되었다. 이러한 이유와 함께 수술 전의 진단이 어렵기 때문에 과립상 세포종에 대한 근본적인 치료는 현재까지 외과적 절제술이다. 최근에 시도되는 치료방법들로는 내시경적 레이저 치료, 용종절제술, 내시경적 점막 절제술 등이 있다. 저자들은 상부 소화관 내시경검사를 시행하여 식도 과립상 세포종을 진단하고 내시경적 점막 절제술을 시행하여 합병증 없이 퇴원하여 현재 재발없이 경과 관찰중인 1례를 경험하였기에 보고하는 바이다. Granular cell tumors, which occur infrequently, are probably of Schwann cell origin. They can occur almost anywhere in the body but usually affect the tongue, oral cavity, skin, or breasts and are rarely found in the gastrointestinal tracts. The esophagus is the most frequent gastrointestinal site, followed by the stomach and the colon. Granular cell tumors are generally benign, especially in the gastrointestinal tract, some malignant lesions have been reported. For this reason, and also because preoperative diagnosis is difficult, the standard treatment for granular cell tumor has until now been surgical excision. In recent years, other therapeutic methods is endoscopic laser therapy (ELT), polypectiomy, endoscopic mucosal resection (EMR). We report a case of esophageal granular cell tumor which was diagnosed by an endoscopy and managed using an endoscopic mucosal resection without complication.
Fournier’s Gangrene: A 10-Year Clinical Experience at a Tertiary Academic Medical Center

Purpose: This retrospective study examined the factors influencing clinical outcomes and mortality in patients with Fournier's Gangrene (FG). The medical history, diagnostic procedures, treatment approaches, complications, and mortality factors associated with FG were analyzed. Materials and Methods: This study retrospectively analyzed the medical records of 40 patients with FG treated over 10 years. The collected data included the patient demographics, comorbidities, vital signs, laboratory tests, Fournier’s Gangrene Severity Index (FGSI) scores, wound swab culture results, treatment approaches, and length of hospitalization. Results: Among the patients with FG, diabetes mellitus and hypertension were the most common comorbidities. The hemodialysis dependence has been identified as a significant risk factor of mortality. In addition, septic shock and an FGSI >9 were associated with increased mortality. Escherichia coli was the most prevalent bacterium in wound swab cultures, and the presence of antibiotic-resistant bacteria was significantly higher in the non-survivors. Treatment involved broad-spectrum antibiotics, emergency surgical debridement, and subsequent adjustments based on culture results. Conclusions: Early diagnosis and prompt initiation of treatment are essential for improving the outcomes of patients with FG. Hemodialysis dependency, septic shock, FGSI scores, and the presence of antibiotic-resistant bacteria are important factors associated with mortality in patients with FG. Further research will be needed to validate these findings and explore adjunctive therapies to enhance the patient outcomes and improve FG management.
CASE REPORT : A Case of Ulcerative Colitis Patient with Colonic Stenosis and Enterocutaneous Fistula

Ulcerative colitis (UC) is one of the chronic inflammatory bowel diseases (IBD), characterized by a diffuse mucosal inflammation limited to the colon. Complications of UC include stricture, colorectal cancer, and toxic colitis. UC patients rarely present with a stenosis or fistula, and strictures develop in less than 5% of patients with UC. We present a patient with UC, accompanied by unusual complications that involved not only a stricture but also a fistula and abscess. A 49-year-old female was presented with a left flank pain and fever that had begun two weeks before admission. She had received a diagnosis of UC 20 years ago and had it treated for 2 years in a local hospital. However, she arbitrarily stopped visiting the hospital and relied on home remedies. An abdominopelvic CT scan revealed luminal narrowing and extra-peritoneal fistula formation in the descending colon. Fistula was connected with a subcutaneous abscess in the left flank. She had undergone total colectomy and ileo-anal anastomosis. On the pathologic exam, the long standing UC with severe stenosis was observed without malignant change. It cannot be emphasized enough that a correct therapeutic approach and an appropriate follow-up schedule are very important for patients with UC. (Intest Res 2012;10:388-391)

Recently many dentists would like to select the implant installation for the restoration of partially or fully edentulous patient. Usually, it is needed at least 8mm of alveolar bone height for implant installation. But, in many cases, the implant installation is impossible due to anatomic limitation such as maxillary sinus, floor of nose, mandibular canal, lack of width, etc. So, dentists must come over these limitations by methods of sinus lift, nerve repositioning, transmandibular implant etc. Especially, a lot of ridges have inadequate bone height at the upper molar edentulous areas, and to insert implant for such sites represents low success rate. So, autogenous or allogenic or alloplastic bone can be grafted before or at the same thme with implant installation. We report a case of 22 year-old woman who was performed implant installation successfully by using sinus lift technique with iliac block bone graft to a patient whose low alveolar bone height made common implantation impossible. Now, she exerts good occlusive function at the time of 15 months after the operation.

<P><B>Abstract</B></P> <P>Piezoelectric energy harvesting generally demonstrates low output power because its output current is low compared to its high output voltage. The low current and high impedance limit the applications of piezoelectric energy harvesting systems. Thus, it is necessary to increase the output current and reduce the internal impedance. This study presents the fabrication of multilayer piezoelectric thick films with high output currents. Single-layer and five-layer piezoelectric devices are prepared using the tape-casting process. The material properties of each lead zirconate titanate (PZT) ceramic are measured using an impedance analyzer and a d<SUB>33</SUB> meter. The electrical properties of the piezoelectric devices were evaluated by a controllable strain module using vibration exciter. The capacitance of the single-layer device was 7.33 nF and that of the five-layer device was 241.04 nF, which was 32.88 times higher than that of the single-layer device. The open circuit voltage of the five-layer device decreased 6.09 times compared to the single-layer, but the short circuit current increased 5.30 times. The impedance matching load of the five layer device for maximum power transfer was averagely 34.05 times smaller than that of single-layer. Under maximum power transfer conditions, the current of the five-layer PZT device was 6.61 times larger than the single-layer, even though the output power of both devices was similar. Because of this capacitance and current increase, the energy stored in the 220 μF capacitor for 10 s using the five-layer device was 920% larger compared to the energy stored using a single-layer.</P> <P><B>Highlights</B></P> <P> <UL> <LI> Low current and high impedance limit piezoelectric-energy-harvester applications. </LI> <LI> Multilayer piezoelectric thick films with high capacitance are fabricated. </LI> <LI> The electrical properties of the piezoelectric devices were evaluated by a controllable strain module. </LI> <LI> Output current is observed to be proportional to the number of layers. </LI> <LI> The energy stored in the capacitors using the five-layer device was 920% larger than that stored using the single-layer. </LI> </UL> </P>
