Multiple lines of evidence link elevated blood pressure with diabetes mellitus. Specifically, it has been proposed that resistance to insulin-stimulated glucose uptake and hyperinsulinemia may play a central role in the cause and clinical course of hypertension. In diabetes mellitus, insulin resistance and/or hyperinsulinemia is an common finding and it is interesting whether the hyperinsulinemia may play a signigicant role in hypertension with DM in the same way as in essential hypertension without diabetes mellitus. The object of this study was to compare insulin and c-peptide levels between age, sex, and obesity matched two groups(DM with DM without hypertension). Method The study group consisted of 55 male diabetes mellitus patients, aged between 45-55 (years). Patients with obesity (body mass index · 30 ㎏/m2), renal disease (proteinuria · 300 ㎎/24hr urine), and secondary hypertension were excluded. Insulin and c-peptide were measured in overnight fasting state and after oral administration of glucose(75 gm). In the fasting, venous plasma glucose levels were similar in the hypertensive and control group(132 ±7 ㎎/dl vs 135 ±8 ㎎/dl). In the fasting, venous plasma insulin levels were higher in the hypertensive than in the control group (10.9 ±5.3 μIU/ml vs 5.5 ±3.9 μIU/ml, p = 0.0001). After loading with 75 gm glucose, venous plasma insulin level seems to be higher in the hypertensive patients than in the control patients, but the difference was not signigicant statistically (27.2 ±17.5 μIU/ml vs 19.9 ±18.9 μIU/ml, p = 0.1297). The mean insulin concentration of the essential hypertensive patients with diabetes mellitus was twice that of the normotensive patients with diabetes mellitus. In control group, there was a direct relations between insulin level and c-peptide, in both fasting state (R = 0.617, p = 0.0001) and glucose-loaded state (R = 0.531, p = 0.001). But in hypertensive group, there was no relations between insulin level and c-peptide, in both fasting state (R = 0.257, p = 0.2738) and glucose-loaded state (R = 0.307, p = 0.1885).
영아자(Phyteuma japonicum Miq.)의 성분 조성

산채식물인 영아자(Horned Rampion ; Phyteuma japonicum Miq.)의 영양학적인 가치를 평가코져 야생 및 평지재배 시료를 잎과 줄기로 구분하여 일반성분, 비타민 C, 유리당, 무기물, 핵산관련물질, 구성 아미노산 및 유리아미노산을 분석하였다. 야생과 재배 영아자의 회분은 1.2∼2.7%의 범위였고, 조지방과 조단백질은 재배시료가 조섬유는 야생시료에서 더 높은 함량으로 정량되었고, 전당은 두 시료간에 대차를 보이지 않았다. 비타민 C는 줄기보다는 잎에서 재배시료보다는 야생시료에서 더 높게 정량되었다. 유리당은 야생 및 재배시료 모두 glucose, frucose 및 sucrose가 잎보다 줄기에서 높게 정량되었다. 무기물은 총 9종이 분석되었는데 이중 칼슘의 함량이 가장 높아 재배시료의 경우 잎은 34374.0㎎/㎏, 즐기는 9584.1㎎/㎏였고, 그 다음으로 칼륨, 마그네슘의 순으로 많았으며, 야생시료도 비슷한 경향을 보였다. 핵산 관련물질은 CMP, UMP, IMP, AMP 및 hypoxanthine이 동정되었는데 잎과 줄기 모두 야생시료에서는 hypoxanthine이, 재배시료에서는 AMP가 월등히 높게 정량되었다. 구성아미노산은 총 17종으로 야생시료에서는 glutamic acid, 재배시료는 잎의 경우 aspartic acid, 줄기의 경우 glutamic acid의 함량이 가장 많았다. 유리아미노산은 총 29종이 동정되었고 야생시료의 잎에서는 glutamic acid, 줄기에서는 γ-aminoisobutyric acid, 재배시료의 줄기에서는 asparagine이 가장 높은 함량으로 정량되었다. This research is to establish the basic data of the nutritive value and improve our diet. In the part of th leaf and stem of the wild cultivated horned rampion(Phyteuma japonicum Mig), the components such as chemical composition, vitamin C, free sugar, mineral, nucleotide and its related compounds, composition and free amino acid were analyzed one after another. Content of the crude lipids and proteins was determined much higher in its wildness than in its cultivated horned rampion ; while, that of carbohydrates was higher in the former than in the latter. The content of vitamin C was retained higher in the leaf than in the stem horned rampion. And the content of calcium among the detected minerals was outstanding in all of the samples collected, and potassium and magnesium was the next ones in its order. The main components of free sugars in both the wild and cultivated horned rampion were glucose and fructose, and their content was higher in the stem than in the leaf. Nucleotide and its related compounds were identified with 5 kinds of nuciotides such as CMP, UMP, IMP, AMP and hypoxanthine(Hx), and the content of Hx and AMP was the highest in the wild and cultivated samples, respcetively. In the composition amino acid of the wild horned rampion, glutamic acid, aspartic acid and phenylalanine was outstandingly abundant; while, such amino acid as methionine and proline was small and besides cysteine couldn't be detected in the stem. Total amounts of composition amino acid in the leaf was 2118.0 and 1120.1㎎% in the wild and cultivated sample, respectively. In the free amino acid of horned rampion, the total amount ranged from 8.5 to 50.1㎎%, which were lower level than that of composition amino acid. But the number of free amino acid was 29 kinds, which was bigger in its number than of composition amino acid detected 17 kinds.

Purpose: To evaluate treatment outcomes for patients with stage II/III pancreatic cancer who are treated with radiation therapy (RT) with or without chemotherapy (CTx) following surgery. Methods: We retrospectively analyzed data from 17 patients who underwent surgery and post-operative RT with or without CTx between January 2000 and December 2008. Seven patients (41%) had stage II cancer and 10 (59%) had stage III cancer. Most were male (13 of 17; 76.5%). Age at diagnosis ranged from 42 to 82 (median 69) years. Whipple’s operation was done in 9 patients (53%), distal pancreatectomy in 7 (41%), and subtotal pancreatectomy in 1 (6%). All patients received RT using a three-dimensional RT technique to spare critical normal structures. Median radiation dose was 54 Gy (range, 50.4∼55.8 Gy). Variable CTx regimens were combined in 10 patients (58.8%); 5-FU in 4, UFTE-G in 4, gemcitabine in 1, and xeloda in 1. Acute toxicity was evaluated according to RTOG toxicity criteria. Survival analysis was done using the Kaplan-Meyer method. Univariate and multivariate prognostic factor analysis were done, respectively, using a log-rank test and Cox’s proportional hazards model. Results: The median follow-up period was 12.6 months. Locoregional and distant failures occurred in 8 (47.1%) and 8 patients (47.1%), respectively. Five patients (29.4%) developed both loco-regional recurrence and distant metastasis. The metastatic sites were liver in 4 patients, lung in 3, peritoneum in 1, and kidney in 1. Median overall survival (OS) was 12.6 months. The 1- and 2-year OS rates were, respectively, 58.8% and 24.5%. Median disease-free survival (DFS) was 8.3 months and the 1- and 2-year DFS rates were 46.3% and 30.9%, respectively. The 2-year OS was not different between RT and RT with CTx : survival rates were 28.6% and 17.5%, respectively (p=0.764). T stage and a postoperative CA 19-9 level of ≥180 U/ml were significant prognostic factors for OS in both univariate and multivariate analysis: the 2-year OS for T3 and T4 were 34.1% and 16.7%, respectively (p=0.0022), the 2-year OS for <180 and ≥180 U/ml were 32.5% and 0%, respectively (p=0.0142) Acute toxicities were RTOG grade 1 (G1) nausea in 1 patient (5.9%), G1 vomiting in 2 (11.8%), and G1-2 enteritis in 5 (29.4%). The hematologic toxicities were G1 leukopenia in 5 patients (29.4%), G2 leukopenia 1 (5.9%), G1 thrombocytopenia in 1 (5.9%), and G1∼2 anemia in 6 (35.3%). Conclusion: Survival results of the present study are comparable to those in other reports with acceptable toxicity. Significant prognostic factors for overall survival in pancreatic cancer are tumor stage and postoperative CA 19-9 level.

Recently railway operators are doing a lot of researches and studies in order to apply reliability technologies to their maintenance tasks. The maintenance in the aviation and the munitions industry has been developed enough to be benchmarked with the high quality reliability technologies; however railway industry is still situated in a rudimentary stage with insufficient & limited data. 5678 Seoul Metropolitan Rapid Transit Corporation which has 17 year experience of the EMU maintenance and system development has made a constant effort for appling RCM (Reliability Centered Maintenance) to the maintenance for a few years. In this connection the case study is to be introduced. This paper is based on 'RCM Gateway to World Class Maintenance' by Anthony M. Smith". The reliability technology are applied to the specific EMU and its system by 7 stages; accordingly applying SMRT's maintenance experience a unique standard for FMEA(failure mode effect analysis) & LTA(logical tree analysis) is established. Moreover for reasonable and effective preventive maintenance tasks the case considering an analysis of failure effects is selected in the final step 7. SMRT will develop reliability technologies through the application of the results to all the EMU systems.

Thermal pasteurization has been effectively used for decades as a method to extend the shelf life of milk and to inactivate any pathogenic bacteria that it may contain; however, it can negatively affect the nutritional properties of milk. In recent years, the food industry has sought new, less aggressive technologies that affect food freshness and its nutritive and health benefits less significantly. Various means have been used to extend the shelf life of dairy foods, such as high-pressure processing, irradiation, ohmic heating, and pulsed electric field (PEF) technologies. Of these, PEF technologies are potential alternatives to traditional thermal milk pasteurization, owing to their advantages in minimizing sensory and nutritional damage. In this review, we have primarily focused on the feasibility of applying PEF technologies to the sterilization of dairy products and briefly discussed whether they should be adopted for use in the dairy beverage industry in the future.
