        The goals of this paper are not only to reduce of noise using anisotropic diffusion filter post-processing but also to represent 3-D image using 2-D image slices by volume rendering. A 3D shape image can be obtained as the 2-D images are continuously acquired, during the object is removed noise by filtering. The 3-D shape images are displayed on computer monitor by use of OpenGL Software.

        당뇨환자에서 발생한 두개저의 침습성 아스페르길루스증 1예

        두개저의 침습성 아스페르길루스증은 매우 드물지만 치명적인 질환으로 침습성 아스페르길루스 부비동염에 합병될 수 있다. 침습성 아스페르길루스증의 치료 원칙은 병변의 외과적 적출술과 장기간의 항진균요법으로 되어 있으나, 적절한 치료 기간과 치료 반응의 경과 판정에 대한 지침은 없는 실정이다. 저자들은 부비동염 증상을 주소로 내원하여 아스페르길루스종을 진단받고 4개월 전에 수술받은 적이 있는 당뇨 환자에서 두 개저의 침습성 아스페르길루스증을 진단하였으며, 병소의 부분적인 수술적 제거와 5개월 이상의 전신적 항진균제 사용으로 환자의 임상 경과가 호전되었으며 치료전과 치료 과정에서 시행한 갈륨스캔에서 방서성동위원소의 섭취가 점차 감소하여 일치하는 소견을 관찰할 수 있었다. 이에 저자들은 당뇨를 포함한 면역저하자에서 발생하는 아스페르길루스 부비동염은 조직 침습 유무에 대한 정확한 진단이 매우 중요하며, 추후에도 침습성 아스페르길루스증으로 진행할 수 있음을 유의해야 할 것을 강조하며, 또한, 갈륨스캔이 그러한 감별과 치료 경과 판정에 유용함을 경험하여 보고하는 바이다. Invasive aspergillosis of skull base is a rare but potentially lethal disease which complicates fungal sinusitis and requires surgical debridement, and long term antifungal therapy. However, optimal duration of antifungal therapy and standard method to monitor of clinical response are not known. A 55-year-old diabetic female patient presented with headache and otatgia 4 months after surgery for non-invasive aspergilloma of sinus. Brain MRI findings revealed bone destructions at the skull base and mastoid process. Mastoidectomy and tympanoplasty were performed, and the resected specimen showed fungal hyphae invading the tissue. Amphotericin B was given for 9 weeks (a total dose of 3.3g) with oral itraconazole, followed by itraconazole (400 ㎎ daily) alone for another 12 weeks. Serial gallium scans, taken before treatment and at 8th and 16th weeks following therapy, showed gradual reduction in uptake of the lesions. We emphasize that non-invasive Aspergillus sinusitis may progress to invasive disease, and strong suspicion of invasiveness is mandatory, especially in immunocompromised patients including diabetes mellitus. Also, we suggest that gallium scan is useful for determining the presence and extent of the disease, and monitoring clinical response following therapy.

        <P>Precise graphene patterning is of critical importance for tailor-made and sophisticated two-dimensional nanoelectronic and optical devices. However, graphene-based heterostructures have been grown by delicate multistep chemical vapor deposition methods, limiting preparation of versatile heterostructures. Here, we report one-pot synthesis of graphene/amorphous carbon (a-C) heterostructures from a solid source of polystyrene <I>via</I> selective photo-cross-linking process. Graphene is successfully grown from neat polystyrene regions, while patterned cross-linked polystyrene regions turn into a-C because of a large difference in their thermal stability. Since the electrical resistance of a-C is at least 2 orders of magnitude higher than that for graphene, the charge transport in graphene/a-C heterostructure occurs through the graphene region. Measurement of the quantum Hall effect in graphene/a-C lateral heterostructures clearly confirms the reliable quality of graphene and well-defined graphene/a-C interface.       • S-240 Hs-CRP as a Predictor of De Novo AF in Patients Treated with Moderate-Intensity Statin

        ackground: Inflammation affects development of atrial fibrillation (AF) by means of atrial remodeling. Statins are known to have a protective effect on this relationship. Previous studies have been centered on postoperative new-onset AF or post-ablation recurrence of AF, but whether inflammation induces de novo AF in dyslipidemia patients on chronic statin therapy for primary prevention is uncertain.?Methods: A total 4803 consecutive dyslipidemia patients (pts) without AF were given moderate-intensity statin for primary prevention from March 2004 to May 2014. Patients were divided into 4 groups according to high sensitivity C-reactive protein (hsCRP) levels, and the development of AF identified by echocardiography was followed for 9 years.?Results: Compared to the lowest quartile (<0.46 mg/L, n=1224), the highest quartile (>1.86 mg/L, n=1194) of hsCRP was associated with a higher incidence of de novo AF [OR= 3.76, (95% CI 2.12-6.66), p<0.001]. In proportional hazard cox-regression adjusted by age, gender, cardiovascular co-morbidities, chronic kidney disease, lipid parameters, fasting glucose, HbA1c, Creatinine, apolipoproteins, and medications including types of statins, there was significantly higher incidence of new-onset AF in the highest quartile of hsCRP, compared to the lowest quartile [HR= 3.28, (95% CI 1.82-5.88), p<0.001, figure].?Conclusions: In our study, high hsCRP levels strongly predicted the development of new-onset AF in moderate-intensity statin-treated dyslipidemia pts during 9-year follow-up.

        Objective: The purpose of this study is to choose a treatment plan and equipment to maximize tangential irradiation to protect the normal brain tissues as much as possible during total scalp irradiation. Subjects and Methods: After zoning the total scalp of a phantom and selecting a target area for treatment, the study made a Helical TomoTherapy(HT) plan, a Helical TomoTherapy with a Complete Block(HTCB) plan, and a Volumetric Modulated Arc Therapy(VMAT) plan. All of these plans made sure that the volume of a treatment plan with 95% of a prescription dose(40 Gy) would not exceed 95% of the entire volume and that Dmax would not be more than 110% of the prescription dose. The therapy plans compared doses among organs at risk of damage including the brain. Doses in the brain tissues were assessed based on the volumetric criteria for normal tissues in Emami et al. Results: HT, HTCB, and VMAT had a dose of 21.68 Gy, 13.75 Gy, and 20.89 Gy, respectively, in brain tissues at D<sub>33%</sub>, a dose of 7.06 Gy, 3.21 Gy, and 7.84 Gy, respectively, at D<sub>67%</sub>, and a dose of 3.14 Gy, 1.75 Gy, and 3.84 Gy, respectively, at D<sub>100%</sub>. They recorded a Dmean of 16.64 Gy, 11.78 Gy, and 16.64 Gy, respectively. These results show that the overall dose was low in the HTCB plan. When the volume of a low dose was calculated based on 5 Gy, they recorded 87%, 49%, and 96%, respectively, in V<sub>5Gy</sub>. In addition, the maximum dose in the remaining organ(brain stem, hippocampus, and both lenses) except for the optic pathway was the lowest in HTCB Conclusion: The findings demonstrate that TomoTherapy with a complete block minimized a dose in organs at risk of damage including the brain and hippocampus on both sides and accordingly reduced the probability of side effects such as radiation-induced brain injuries and a resulting decrease in neurocognitive functions. In addition to total scalp irradiation, if additional studies on ring targets treated in various areas are conducted to establish the benefits of tangential irradiation, it is believed that TomoTherapy using Complete Block can be used to maximize tangential irradiation in treatment planning.

      • Galvanic synthesis of three-dimensional and hollow metallic nanostructures

        <P>We report a low-cost, facile, and template-free electrochemical method of synthesizing three-dimensional (3D) hollow metallic nanostructures. The 3D nanoporous gold (3D-NPG) nanostructures were synthesized by a galvanic replacement reaction (GRR) using the different reduction potentials of silver and gold; hemispherical silver nanoislands were electrochemically deposited on cathodic substrates by a reverse-pulse potentiodynamic method without templates and then nanoporous gold layer replicated the shape of silver islands during the GRR process in an ultra-dilute electrolyte of gold(III) chloride trihydrate. Finally, the wet etching process of remaining silver resulted in the formation of 3D-NPG. During the GRR process, the application of bias voltage to the cathode decreased the porosity of 3D-NPG in the voltage range of 0.2 to -0.62 V. And the GRR process of silver nanoislands was also applicable to fabrication of the 3D hollow nanostructures of platinum and palladium. The 3D-NPG nanostructures were found to effectively enhance the SERS sensitivity of rhodamine 6G (R6G) molecules with a concentration up to 10<SUP>-8</SUP> M.</P><P><B>Electronic supplementary material</B></P><P>The online version of this article (doi:10.1186/1556-276X-9-679) contains supplementary material, which is available to authorized users.</P>

        Objective:?We evaluated the impact of diabetes mellitus (DM) and DM control status on each layers of carotid artery in asymptomatic population.?Methods:?This is an observational cohort study consisted of 1,479 patients. The carotid images were sent to the Korea Research Institute of Standards and Science for core laboratory analysis using specialized software which can measure intima and media thickness respectively.?Results:?DM patients (n=634, 42.9%) were likely to be older, higher prevalence of male, hypertension and dyslipidemia, and higher creatinine level than non-DM patients (n=845, 57.1%). There was no significant demographic difference according to DM control status in DM patients. DM patients showed higher carotid intima-media thickness (CIMT, 0.70±0.15mm vs. 0.66±0.16mm, p<0.001) and media thickness (CMT, 0.41±0.12 mm vs. 0.36±0.12mm, p<0.001) than non-DM patients, whereas intima thickness (CIT) showed no significant difference (0.29±0.07 mm vs. 0.30±0.06 mm, p=0.067) between 2 groups. Well controlled DM patients (HbA1C<7.0%, n=232, 47.4%) showed higher CIT (0.30±0.08mm vs. 0.27±0.06 mm, p=0.003) than poorly controlled DM (HbA1C ≥ 7.0%, n=257, 52.6%). Old age and LDL-cholesterol were the independent factors for CIMT, CIT and CMT in total asymptomatic adults as well as DM patients. DM control status was not significant independent factor for CIMT.?Conclusions:?The increased CIMT in DM patients was mainly due to the increased CMT. DM control status did not impact on carotid arterial wall thickness in this study. Lipid control rather than glucose control may be the most important factor to decrease atherosclerosis progression in subclinical adults.
