      • The First Case of Pure 3D Laparoscopic Living Donor Hepatectomy in Kazakhstan

        Aims: Laparoscopic living donor hepatectomy is performed selectively in some countries because of the small sized structures and anatomic variations which requires more precise technique. In Kazakhstan about 200 cases of living donor liver transplantation were performed starting from December 2011.We now present the first case of laparoscopic living donor hepatectomy in Kazakhstan using a 3D laparoscopic device with colleagues from Seoul National University Hospital, Korea by professor KwangWoong Lee. Methods: A 23-year-old male volunteered for living donor to his older brother who had combined hepatitis B, D and C related cirrhosis without any history of antiviral treatment. The recipient age was 36 and His model for end-stage liver disease score was 22. Brief morphologic characteristics of donor and recipient were as follows: donor - height 177 cm, weight 74 kg, BMI 23.64 kg/m<sup>2</sup>standard liver volume 1348 ml; recipient - height 172 cm, weight 78 kg, BMI 26.44 kg/m<sup>2</sup>, standard liver volume 1350ml.Donor graft volume was 820ml and left remnant liver volume was 32%. Results: The surgery was performed in October 2016 with 5 port insertion using an ultrasonic dissector, Cabitron Ultrasonic Aspirator, and clips. The liver was extracted via 10 cm sized suprapubic incision. V5 and V8 were reconstructed to the right hepatic vein using the PTFE graft. The Donor’s operation time was 295 minutes and estimated bleeding was 700ml. The recipient had sanguinous drain and bleeding control was performed postoperative day 1 due to bleeding from hilar plate. Both donor and recipient were discharged after 15 and 28 days and the donor was not eventful. Conclusions: 3D laparoscopic hepatectomy has been progressively developed along the past decade. High quality studies allied with high-level surgical training are required to base surgical practice and to disseminate the benefits of to many centers as possible. Future applicability of new technologies such as robot assistance and 3D surgery is still under investigation in Kazakhstan.

      • BEZ235 (PIK3/mTOR inhibitor) Overcomes Pazopanib Resistance in Patient-Derived Refractory Soft Tissue Sarcoma Cells

        <P><I>BACKGROUND:</I> Although pazopanib treatment has become the standard chemotherapy in salvage setting for metastatic sarcoma patients, most patients progress after pazopanib treatment in 4 to 6 months. After failure to pazopanib, patients have limited options for treatment. Therefore, subsequent therapy in patients who failed to pazopanib is urgently needed and the use of patient derived cells or patient derived tumors for accompanying testing with various pharmacological inhibitors could offer additional treatment options for these patients. <I>METHODS:</I> Patient derived tumor cells were collected from ascites at the time of progression to pazopanib and a 13-drug panel was tested for drug sensitivity. We confirmed the results using <I>in vitro</I> cell viability assay and immunoblot assay. We also performed the genomic profiling of PDX model. <I>RESULTS:</I> The growth of patient derived tumor cells was significantly reduced by exposure to 1.0 μM AZD2014 compared with control (control versus AZD2014, mean growth = 100.0% vs 16.04%, difference = 83.96%, 95% CI = 70.01% to 97.92%, <I>P</I> = .0435). Similarly, 1.0 μM BEZ235 profoundly inhibited tumor cell growth <I>in vitro</I> when compared to control (control versus BEZ235, mean growth = 100.0% vs 7.308%, difference = 92.69%, 95% CI = 78.87% to 106.5%, <I>P</I> < .0001). Despite the presence of CDK4 amplification in the patient-derived tumor cells, LEE011 did not considerably inhibit cell proliferation when compared with control (control vs LEE011, mean growth = 100.0% vs 80.23%, difference = 19.77%, 95% CI = 1.828% to 37.72%, <I>P</I> = .0377). The immunoblot analysis showed that BEZ235 treatment decreased pAKT, pmTOR and pERK whereas AZD2014 decreased only pmTOR. <I>CONCLUSION:</I> Taken together, upregulation of mTOR/AKT pathway in sarcoma patient derived cells was considerably inhibited by the treatment of AZD2014 and BEZ235 with downregulation of AKT pathway (greater extent for BEZ235). These molecules may be considered as treatment option in STS patient who have failed to pazopanib in the context of clinical trials.</P>

      • GAITRite 시스템을 이용한 건강한 노인의 시 공간적 보행변수 분석

        본 연구는 60대 노인들의 보행패턴을 분석하기 위하여 GAITRite 보행분석기를 이용하여 20대와 비교하여 노화에 따른 보행패턴 변화를 알아보고, 보행 장애가 없는 노인들의 보행 자료를 통해 근골격계 및 노인성 질환에 따른 보행문제를 임상적 평가수단으로 활용하여 하지분절의 역학적 구조 및 발과 보행에 관한 기초자료를 제공하는데 연구목적이 있다. 본 연구의 결과로 다음과 같은 결론을 내릴 수 있다. 첫째, 60대 노인의 보행특징은 시간적 변수에서는 성별에서 차이가 없으며, 공간적 변수에서는 성별에서 보장, 활보장, 양뒤꿈치 기저면에서 유의한 차이가 있었다. 둘째, 60대는 20대에 비해 양하지 지지기 비율이 길어, 입각기 비율이 길어지고, 유각기는 짧아 보행속도가 느리며, 분당 걸음수도 느린 특징을 나타냈으며, 보장과 활보장은 짧으며, 토아웃각이 큰 특징을 나타냈다.

        레이저 간섭계(ESPI)에 의해 측정된 플립칩 열변형의 유한요소해석 모델링을 통한 솔더볼의 유동곡선 평가

        The goal of this study was to determine the uniaxial flow curve for solder balls in a flip-chip from experimental-computational algorithms based on finite element modeling (FEM) of in-plane thermal displacement data measured by electronic speckle pattern interferometry (ESPI). In order to measure the deformation of such tiny components as the solder balls in the flip-chip, the spatial resolution of ESPI was increased to submicron scale by magnifying the areas studied. The flow curve for solder balls in the flip-chip was determined by the algorithm, which effectively matches the simulated solder deformation by FEM to the measured deformation by ESPI. The algorithms were applied to Sn-36Pb-2Ag flip-chip solder balls. The flow curve obtained for flip-chip solder was compared with those for bulk solder. The microstructure was also studied to clarify the flow curve results.

      • Ziegler-Natta 촉매를 이용한 선형저밀도 폴리에틸렌의 제조

        For the preparation of linear low-density polyethylene (LLDPE), the copolymerization of ethylene and 1-butene has been carried out with various Ziegler-Natta catalysts in slurry phase. The effects of catalyst components, concentration of catalyst components, polymerization time on the yields and compositions of copolymer have been examined. Furthermore the density and thermal properties of obtained copolymer have been measured for the correlation to the 1-butene contents. It has been found that the titanium tetra n-butoxide-diethyl aluminum chloride cata-lyst system was the most suitable for yield and 1-butene content of copolymer. The yield of copolymer was increased with the concentrations of two catalyst components but the 1-butene content did not show any tendency for catalyst concentration. The diluent of aliphatic hydrocarbon showed better results than other diluents. The density of copolymer was decreased with the increase of 1-butene content. The melting point and heat of fusion of copolymer were decreased with 1-butene content.

        대구광역시 소재 대형할인점의 여성고객편의시설에 대한 현황조사 및 분석에 관한 연구

        As educational level of women has become strong and various job classifications have been created, social climate has been changed to give important part to women. Only construction, however, seems to be not melting this social climate. In other words, while things are trending toward making of exclusive spaces or The Facility for Female Customers, this Facility has been designed without care for women. So this study is to find out the existing conditions of The Facility for Female Customers through the survey and then analyzes degree of satisfaction of Female Customers and Zoning Types of The Facility for Female Customers by survey. This study focused on Large-scale Discount Stores in Daegu. The result of The Improvement of the Current Facility for Female Customers in Large-scale Discount Stores can be summarized as follows : ① The Attached Parking Space in Large-scale Discount Stores should be more safe for Female Customers through improvement of illuminance of artificial lighting and reasonable arrangement of security. ② The Restroom for Women should be designed that the facility and space are more expanded than the present state. ③ The Nursery Room should be designed to take privacy that the sense of sight and hearing will be guaranteed. ④ The Indoor Playground should be set up and managed installing safety equipment and arranging security focused on free Indoor Playground. ⑤ The Culture Center in Large-scale Discount Stores should be used lively as facility for life-long education of residents. ⑥ The Facilities for Female Customers should be connected closely each other through considering women's behavior.

        치료 저항성 환청을 나타내는 만성 정신분열병 환자에서 측두두정부 반복 경두개자기자극의 효과

        Objectives : It has been reported that repetitive transcranial magnetic stimulation (rTMS), which stimulate the focal region of brain may relieve auditory hallucination in patient with schizophrenia. This study was aimed to evaluate the effect of rTMS on left and right temporopahetal cortex in schizophrenic patients with treatment-resistant auditory hallucination. Methods : Twenty-six patients with chronic schizophrenia with treatment -resistant auditory hallucination were randomly a1-located to rTMS group on left and right temporoparietal head regions. The rTMS was applied to TP3/4 region by EEG 10-20 international system at 1 Hz for 20 minutes per day for 101reatment days. A Figure of eight TMS coil with a solid core was used for rTMS at 100% of individual motor threshold. Efficacy was evaluated with Auditory Hallucination Rating Scale (HRS), Positive and Negative Symptoms Scale (PANSS), Clinical Global Impression (CGI) Scale on 5 and 10 days of treatment. Results : Ten days administration of l-Hz rTMS to temporopahetal cortex significant improved frequency (p<0.001) and attentional salience (p=0.038) of auditory hallucination, Positive (p<0.001) and negative symptom (p=0.02), and CGI-impro-vement (p<0.001). There were no significant differences between right and left side stimulations. Conclusion : These results suggest that the low frequency rTMS on temporopahetal cortex may be useful in treating Schizo-phrenic patients with treatment-resistant auditory hallucination and it could be a useful novel treatment option.

        심정지환자에서 epinephrine투여시 혈중 catecholamine의 변화

        Background : Cardiac arrest is a potent stimulus for the release of endogenous catecholamine and high plasma catecholamine concentration has been reported during cardiopulmonary resuscitation(CPR). Few data were present about the clinical effect of high plasma catecholamine and endogenous catecholamine response to exogenous epinephrine administration during CPR. Purpose : This study was designed to evaluate endogenous catecholamine response to cardiac arrest and exogenously administered epinephrine during CPR. Method : In 21 cardiac arrest victims, blood sample for epinephrine and norepinephrine was drawn before and 1 minute after each epinephrine administration during CPR. 1mg of epinephrine was administered immediately after intravenous access and 3 minute interval during CPR. Plasma catecholamines were quantitated by high-performance liquid chromatography. Result : Baseline plasma epinephrine concentraqtion was higher in patients with cardiac arrest than normal controls (985±1627 vs 194±173 pg/ml, p<0.05). Baseline plasma norepinephrine concentration was also elevated in patients with cardiac arrest than normal controls, although statistically insignificnat(1965±4915 vs 360±250 pg/ml). Plasma epinephrine concentration in cardiac arrest patients was significantly elevated after exogenous administration of epinephrine, but its response was blunted after the third does of epinephrine. Plasma norepinephrine concentration in cardiac arrest patients was also elevated, but its response was observed only after the first epinephrine administration. Plasma epinephrine concentration was persistently higher in patients with return of spontaneous circulation (ROSC) than patients without ROSC. there was no significant difference of plasma norepinephrine concentration whether spontaneous circulation was restored or not. Endogenous epinephrine response to exogenous epinephrine was not observed in patients having prolonged arrest time over 20 minutes. Conclusion : Results of this study suggests that significant amount of catecholamine is released by exogenous epinephrine administration in spite of high plasma catecholamine concentration, and degree of endogenous catecholamine response influences resuscitation outcome.
