        Purpose: The gastrointestinal (GI) tract is the most common site of extranodal non-Hodgkin's lymphoma (NHL), which is increasing in incidence, but there is no established optimal treatment modality. Thus, this study was investigated the clinicohistologic feature, the therapeutic modalities, and the prognosis for GI-NHL, as well as the factors affecting it. Methods: We retrospectively analyzed 45 patients who had been diagnosed as having GI-NHL and had been followed up from July 1994 to February 2005 at Soonchunhyang University Hospital. The patients were divided into groups according to the site of origin and to various other features, and the survivals of the various groups were compared. The modified Ann Arbor system and WHO classification were adopted for staging and histopathologic classification, respectively. Results: GI-NHL of the stomach, small bowel, ileocecal region, and colon occurred in 28 patients (62.2%), 5 patients (11.1%), 3 patients (6.7%), and 8 patients (17.8%), respectively, In one patient, the entire gastrointestinal tract was diffusely involved. The median age of patients was b5 years (25~78 years), and male-to-female ratio was 1:1.1. Fourteen patients were in stage Ⅰ, 24 in stage Ⅱ, 4 in stage Ⅲ, and 3 in stage Ⅳ. Surgical resection was performed in 19 patients, and combination chemotherapy was performed in 43 patients. Surgical resection only was performed in 4 patients, Chemotherapy only was performed in 26 patients. The expected overall 5 year survival of 45 patients was 39.6%, and there was a significant survival difference between the stages, but between sites of origin (p=0.842). The most important factors influencing the survival was the stage and other factors were not significant. Conclusion: The stomach was the most common site of GI-NHL. Most GI-NHL were localized Stage was the most important prognostic factor. However, Prospective randomized studies are needed to approve the therapeutic modality.

        WWW상에서 음란물 검색기법 : Obscene Material Searching Method in WWW

        WWW은 기존의 인터넷이 단순 문자 자료 중심의 데이타 교환을 멀티미디어화 하기 위한 프로토콜이며, 자료들을 하이퍼텍스트 형태로 저장함으로써 초보자들도 쉽게 원하는 자료를 찾고, 접근할 수 있도록 되어 있다. 이러한 WWW의 멀티미디어 데이타의 검색 및 접근의 용이성은 음란물 데이타가 보편화, 멀티미디어화 되는데 결정적인 역할을 하였으며 음란물의 상업화를 가능케 하는 사회적 문제를 야기하였다. 한편, 이러한 문제를 해결하기 위해 음란물을 제공하는 사이트를 효율적으로 차단하기 연구가 활발하게 진행되고 있다. 본 논문에서는 이러한 음란물을 제공하는 사이트를 효율적으로 검색하여, 미성년자의 음란성 사이트 접근을 차단하기 위한 기법을 제시하고 이를 구현하였다. 제안된 기법은 링크를 기반으로 정보 검색 기능을 수행하며, 가장 정확한 결과를 보여 주는 것으로 알려진 확률 모델과 비교한 결과 제안된 모델(Link-Based Model)이 확률 모델보다 평균 재현율과 정확율에서 12%와 8% 성능이 우수하였다. 특히 텍스트 이외의 데이타와 적은 링크를 가진 문서들을 검색하는데 크게 효율성이 향상되었다. World-Wide Web(WWW) is a protocol for changing information exchanges which is central to text documents in the existing network to make a multimedia data exchanges. It is possible for a beginner to search and access data which he wants to find as data were stored in the form of hypertext. The easiness for searching and accessing the multimedia data in WWW makes a important role for obscene materials to be toward generalization and multimedia and occurs social problems for them to be commercialized, while other researchers have actively studied the way to block effectively the site providing obscene materials for solving such problems. This paper presents and implements the blocking method for the sites having obscene material as it effectively search them. The proposed model was based on Link-Based information retrieval method and proved that it accomplished more efficient retrieval of relevant documents than probabilistic model when compared the one with the other which is known to generate the most correct results. The improvements in the average recall and precision ratio were shown as 12% and 8% respectively. Especially, the retrieval capability of relevant documents which include non-text data and have a few links increased high]y.

        <P>X-linked deafness type 3 (DFN3), the most prevalent X-linked form of hereditary deafness, is caused by mutations in the POU3F4 locus, which encodes a member of the POU family of transcription factors. Despite numerous reports on clinical evaluations and genetic analyses describing novel POU3F4 mutations, little is known about how such mutations affect normal functions of the POU3F4 protein and cause inner ear malformations and deafness. Here we describe three novel mutations of the POU3F4 gene and their clinical characterizations in three Korean families carrying deafness segregating at the DFN3 locus. The three mutations cause a substitution (p.Arg329Pro) or a deletion (p.Ser310del) of highly conserved amino acid residues in the POU homeodomain or a truncation that eliminates both DNA-binding domains (p.Ala116fs). In an attempt to better understand the molecular mechanisms underlying their inner ear defects, we examined the behavior of the normal and mutant forms of the POU3F4 protein in C3H/10T1/2 mesodermal cells. Protein modeling as well as in vitro assays demonstrated that these mutations are detrimental to the tertiary structure of the POU3F4 protein and severely affect its ability to bind DNA. All three mutated POU3F4 proteins failed to transactivate expression of a reporter gene. In addition, all three failed to inhibit the transcriptional activity of wild-type proteins when both wild-type and mutant proteins were coexpressed. Since most of the mutations reported for DFN3 thus far are associated with regions that encode the DNA binding domains of POU3F4, our results strongly suggest that the deafness in DFN3 patients is largely due to the null function of POU3F4.</P>

      • 비혈연간 동종 골수이식술 후 발생한 혈전성 미세혈관병증 1례

        혈전성 미세혈관병증은 골수이식 후 발생하는 중한 합병증 중의 하나이다. 저자들은 22세의 급성 림프구성 백혈병(ALL, L3)을 진단 후 비혈연간 동종 골수이식을 받은 환자에서 발생한 혈전성 미세혈관병증을 경험하였다. 그는 비혈연간 동종골수이식술을 받았으며 이식의 전처치로 busulfan, cyclophosphamide, 전신방사선조사를 받았고 이식편대숙주반응을 막기 위하여 CsA와 methotrexate을 투여받았다. 이식 후 28일째 말초혈액도말검사상 Burr cell, schistocyte, tear drop cell이 관찰되었으며, 이식 후 31일째 혈변소견이 보여 S결장 내시경상 장관 이식편대 숙주질환 소견이 보였고 혈청검사상 거대세포바이러스가 양성소견을 보여 steroid와 gancyclovir를 투여받았고 2주 후에 장관 이식편대 숙주반응이 호전되었다. 이식 후 68일째 임상적으로 혈전성 미세혈관병증이 발생되었고 Cr 수치가 증가되어 TPE와 혈액투석을 격일로 시행하였으나 악화되었다. 이식 후 82일째 다발성 출혈로 사망하였다. Thrombotic microangiopathy (TMA) is one of the serious complications of bone marrow transplantation (BMT). We experienced a 22-year-old male with acute lymphoblastic leukemia (ALL, L3) who developed post BMT-TMA. He received pretransplant conditioning chemotherapy with busulfan, cyclophosphamide, and total body irradiation (TBI). He received cyclosporine (CsA) and methotrexate (MTX) for graft versus host disease (GVHD) prophylaxis. Peripheral blood smear revealed burr cells, schistocytes and tear drop cells on post BMT day 28. Hematochezia was developed on post BMT day 31. Biopsy specimen of sigmoid colon revealed grade III to IV acute GVHD and cytomegalovirus (CMV) culture was positive in blood on post BMT day 43. He received steroid pulse therapy and gancyclovir so that improved after 2 weeks. TMA was developed clinically on post BMT day 68 and creatine (Cr) was increased. In spite of undergoing plasma exchange with fresh frozen plasma and hemodialysis every other day, TMA progressed. He died due to multiple hemorrhage on post BMT day 86.

        Anti-bacterial and Anti-viral Activities of Extracts from Terminalia chebula Barks

        Effects of methanol extract of Terminalia chebula on antibacterial activity and the generation of superoxide radical in Bacillus subtilis, as well as on syncytium formation and cytopathic activity in virus-infected baby hamster kidney cells were examined. The extract effectively inhibited syncytium formation in a concentration-dependent manner, and infectious virus production was markedly reduced. However, glycoprotein synthesis was not affected. These results collectively indicate that methanol extract of T. chebula potentially inhibit glycosylation by acting as a suppressor of intracellular glycosylation trafficking.

        Antimycin A sensitizes cells to TRAIL-induced apoptosis through upregulation of DR5 and downregulation of c-FLIP and Bcl-2

        <P>Tumor necrosis factor-related apoptosis-inducing ligand (TRAIL) has been the focus as a potential anticancer drug, because it induces apoptosis in a wide variety of cancer cells but not in most normal human cell types. In this study, we showed that combination treatment with sub-toxic doses of antimycin A (AMA), an inhibitor of electron transport, plus TRAIL induced apoptosis in human renal cancer cells, but not in normal tubular kidney cells. Treatment of Caki cells with AMA upregulated the death receptor 5 (DR5) protein and downregulated c-FLIP and Bcl-2 proteins in a dose-dependent manner. AMA-induced decrease of c-FLIPL and c-FLIPs protein levels which were caused by increased protein instability, which was confirmed by the result showing that treatment with a protein biosynthesis inhibitor, CHX, accelerated degradation of c-FLIPL and c-FLIPs proteins caused by AMA treatment. We also found that AMA induced upregulation of DR5 and downregulation of Bcl-2 at the transcriptional level. Pretreatment with N-acetyl-l-cysteine (NAC) partly recovered the expression levels of c-FLIPL and c-FLIPs proteins were downregulated by the AMA treatment, suggesting that AMA appears to be partially dependent on the generation of ROS for downregulation of c-FLIPL and c-FLIPs. Collectively, this study demonstrates that AMA enhances TRAIL-induced apoptosis in human renal cancer cells by upregulation of DR5 as well as downregulation of c-FLIP and Bcl-2. Furthermore, this study shows that AMA markedly increases sensitivity to cisplatin in Caki human renal cancer cells.</P>

        <P>OBJECTIVE: Lipid accumulation in vascular endothelial cells may play an important role in the pathogenesis of atherosclerosis in obese subjects. We showed previously that alpha-lipoic acid (ALA) activates AMP-activated protein kinase (AMPK) and reduces lipid accumulation in skeletal muscle of obese rats. Here, we investigated whether ALA improves endothelial dysfunction in obese rats by activating AMPK in endothelial cells. METHODS AND RESULTS: Endothelium-dependent vascular relaxation was impaired, and the number of apoptotic endothelial cells was higher in the aorta of obese rats compared with control rats. In addition, triglyceride and lipid peroxide levels were higher, and NO synthesis was lower. Administration of ALA improved all of these abnormalities. AMPK activity was lower in aortic endothelium of obese rats, and ALA normalized it. Incubation of human aortic endothelial cells with ALA activated AMPK and protected cells from linoleic acid-induced apoptosis. Dominant-negative AMPK inhibited the antiapoptotic effects of ALA. CONCLUSIONS: Reduced AMPK activation may play an important role in the genesis of endothelial dysfunction in obese rats. ALA improves vascular dysfunction by normalizing lipid metabolism and activating AMPK in endothelial cells.</P>

        Microstructural evaluation of oxide layers in CaO-added Mg alloys

        <P><B>Abstract</B></P> <P>A transmission electron microscopy analysis was conducted on the oxide layers of Mg–5.5Al–0.25Mn (AM60) alloys with different levels of CaO content formed in a thermogravimetric analyzer (TGA) system at 450°C for 7h. It was found that the oxide layer in AM60+CaO grew as a columnar structure and, resultantly, that the oxide layers had different thicknesses with different levels of CaO content. The columnar oxide grains that were formed on the Mg+CaO alloy grew on the [100]<SUB>MgO</SUB>, resulting in a more compact and thicker initial oxide layer compared to the normal oxide layer. The columnar growth of the oxide layer in AM60+CaO formed due to the Ca, which dissolved from CaO.</P> <P><B>Highlights</B></P> <P> <UL> <LI> Oxidation layer thickness of CaO added AM60 increases with increasing amounts of CaO. </LI> <LI> Effect of CaO is that the columnar grains on AM60 grow along the [100]<SUB>MgO</SUB>. </LI> <LI> A initial oxidation layer thickness and ignition temperature is closely connected. </LI> <LI> The columnar oxide layer formed due to Ca, which dissolved from CaO. </LI> </UL> </P>

        <P>578-nm yellow light with an output power of more than 10 mW was obtained using a waveguide periodically-poled-lithium-niobate crystal as a nonlinear medium for second harmonic generation, which is the highest output power at this wavelength using second harmonic generation of a solid state laser source without an enhancement ring cavity, to our knowledge. To achieve this result we made a high power 1156-nm external-cavity diode laser with the maximum output power of more than 250 mW. This system is expected to be an excellent alternative to the system using the sum-frequency generation with the advantage of simplicity and cost-effectiveness, and will be used as a clock laser of the ytterbium optical lattice clock with robust and reliable operation.</P>
