Objective-The functional strategic mechanisms in the brain during performing visuospatial working memory tasks, especially tasks with heavy load, are controversial. We conducted the functional magnetic resonance imaging (fMRI) while sixteen subjects were performing face- and location-matching n-back tasks to examine causal relations within the frontoparietal networks. Methods-We applied a sophisticated method, the structural equation modeling (SEM), to the fMRI data. The imaging data were analyzed by extracting the task-related eigenseries using the principal component analysis (PCA) and then by applying a form of data-driven model called the automated search method. Results-The SEM analyses revealed a functional shift of network connectivity from the right to the left hemisphere with increasing load in the face-matching n-back tasks while the location-matching tasks required bilateral activation. In the locating matching n-back tasks, a pattern of parallel processing was observed in the left phonological loop and the right inferior parietal regions. Furthermore, object working memory-related activities in the left hemisphere reliably contributed to performance of both the face- and location-matching 2-back tasks. Conclusion-Our results are consistent with previous reports in terms of demonstrating parallel and distributed information processing during performing working memory tasks with heavy loads. Our results additionally suggest a dynamic shift between the fast imagery circuit (right hemisphere) and the stable verbal circuit (left hemisphere), depending on task load.
Integrative package for low back pain with leg pain in Korea: A prospective cohort study

<P><B>Summary</B></P><P>Back pain significantly affects both patients and society through personal suffering, supporting burden, work loss, and incurred expenses. With no unequivocal support for surgery versus conservative treatment, an integrative approach has become popular in Korea.</P><P><B>Objectives</B></P><P>To investigate the outcomes of an integrative package for low back pain with leg pain.</P><P><B>Methods</B></P><P>A prospective cohort study involving patients with low back and leg pain and confirmed disc herniation was carried out at an outpatient clinic in Korea. The treatment package comprised of herbal medicines, acupuncture, bee venom acupuncture, and a Korean version of spinal manipulation (Chuna). Study participants were evaluated at baseline and every 4 weeks for 24 weeks. Low back and leg pain intensity levels were measured on a visual analog scale (0–10), back function was evaluated with the Oswestry Disability Index (0–100), and the overall quality of life was assessed using the SF-36 Health Survey (0–100 in 8 different subcategories).</P><P><B>Results</B></P><P>Out of 150 patients, 128 completed the 24 weeks of therapy. Patients reported improvements in all outcome measures. At the completion of the study, low back pain scores improved by a mean of 3.3 (95% CI=2.8 to 3.8), and leg pain scores improved by a mean of 6.3 (95% CI=5.9 to 6.6). Significant improvements in ODI and SF-36 scores were observed at 4 weeks and sustained throughout.</P><P><B>Conclusions</B></P><P>This integrative package was effective in the treatment of LBP with leg pain and warrants further rigorous investigations.</P>
Recurrent parvovirus B19 infection-associated pure red cell aplasia in a kidney transplant patient

Posttransplant anemia is a common complication after kidney transplantation. Parvovirus B19 (PVB19) infection can induce pure red cell aplasia (PRCA) in immunosuppressed transplant patients. We herein report a case of recurrent PVB19-associated PRCA in a kidney transplant patient. A 49-year-old woman presented with anemia and normal renal function 1 year after a deceased-donor kidney transplantation for immunoglobulin A nephropathy- related end-stage renal disease. She received desensitization therapy, and 2 years later, she underwent transplantation with thymoglobulin induction. Despite repeated red cell transfusion and erythropoietin therapy, her anemia aggravated progressively. Bone marrow biopsy revealed normocytic normochromic PRCA. Real-time polymerase chain reaction detected a high plasma load of PVB19. Administration of intravenous immunoglobulin (IVIG) at 2 g/kg with adjuvant reduction of tacrolimus and discontinuation of myfortic acid effectively treated the anemia. However, the PVB19 load remained high, and PRCA recurred 7 months after the initial IVIG treatment. Tacrolimus was switched to cyclosporine in the second IVIG treatment, which successfully improved PRCA and reduced the PVB19 load. Our case suggested that PVB19-associated PRCA should be suspected when persistent anemia is observed in kidney transplant patients with heavy immunosuppression and that PVB19-associated PRCA can recur in the presence of persistent PVB19 viremia.

Determination of Illegally adulterated compounds in Foods and Dietary Supplements

In this work, we developed an analytical method for determining 18 illicit compounds and analyted for screening and confirmation of 76 illicit compounds in foods and dietary supplements. We monitored 292 food items categorized into sexual enhancement, weight loss, muscular strengthening or relaxing products in online markets. This study was to monitor illegally adulterated foods were assayed using a simultaneous analytical method involving high performance liquid chromatography and LC coupled with tandem mass spectrometry. In 66 of 292 samples, we detedcted 20 different illegal compounds, including Sildenafil, Tadalafil, Yohimbine, Melatonin, Synephrine and Andarine. Approximately 22 % of the tested samples were identified as having been illicitly adulterated. When purchasing products marketed for foods and dietary supplements, ulmost care should be taken owing to the possible presence of these illegal compounds. These results indicate that a continuous monitoring of illegal compounds is required for the safety management in various foodstuffs.
