      • Delta Neutrophil Index is a Predictor of Disease Severity in Patients with Acute Pyelonephritis

        Background: Delta neutrophil index (DNI) is the fraction of immature granulocytes provided by a complete blood count analyzer. Previous studies demonstrated that DNI is a novel marker to predict outcomes in sepsis patients. Therefore, this study aimed to evaluate DNI as a predictor of disease severity in patients with acute pyelonephritis (APN). Methods: Patients diagnosed with APN at Severance Hospital from December 2009 to July 2012 were retrospectively investigated. DNI levels were measured at the time of admission. The patients were classified into two groups according to the median value of DNI at baseline. Severe APN was diagnosed with one or more of these conditions including bacteremia, acute kidney injury, hypotension requiring use of vasopressors and admission of intensive care unit. Independent risk factors for severe APN were determined by multiple logistic regression analysis. Receiver operating characteristic (ROC) curves were plotted for DNI, white blood cell (WBC) count and C-reactive protein (CRP). Results: A total of 258 patients were included in this study. The median age was 61.0 (19.0-87.0), 237 patients (91.9%) were female, and the median DNI level was 2.5%. Severe APN was significantly more prevalent in high DNI group. Multivariate analysis showed that DNI, WBC count and CRP level independently predicted severe APN. DNIwas a better predictive marker of severe APN than WBC count, as reflectd by area under the ROC curve analysis (0.697 [95% CI, 0.637-0.753] vs. 0.562 [95% CI, 0.499-0.624], p=0.0013). DNI was not inferior to CRP levels to predict severe APN (0.697 [95% CI, 0.637-0.753] vs. 0.633 [95% CI, 0.571-0.692], p=0.1185). Conclusions: DNI level at admission is a valuable predictor of disease severity in patients with APN.

        A Hexane Fraction of Guava Leaves (Psidium guajava L.) Induces Anticancer Activity by Suppressing AKT/Mammalian Target of Rapamycin/Ribosomal p70 S6 Kinase in Human Prostate Cancer Cells

        This study was carried out to evaluate the anticancer effect of guava leaf extracts and its fractions. The chemical compositions of the active extracts were also determined. In the present study, we set out to determine whether the anticancer effects of guava leaves are linked with their ability to suppress constitutive AKT/mammalian target of rapamycin (mTOR)/ribosomal p70 S6 kinase (S6K1) and mitogen-activated protein kinase (MAPK) activation pathways in human prostate cancer cells. We found that guava leaf hexane fraction (GHF) was the most potent inducer of cytotoxic and apoptotic effects in PC-3 cells. The molecular mechanism or mechanisms of GHF apoptotic potential were correlated with the suppression of AKT/mTOR/S6K1 and MAPK signaling pathways. This effect of GHF correlated with down-regulation of various proteins that mediate cell proliferation, cell survival, metastasis, and angiogenesis. Analysis of GHF by gas chromatography and gas chromatography-mass spectrometry tentatively identified 60 compounds, including <TEX>${\beta}$</TEX>-eudesmol (11.98%), <TEX>${\alpha}$</TEX>-copaene (7.97%), phytol (7.95%), <TEX>${\alpha}$</TEX>-patchoulene (3.76%), <TEX>${\beta}$</TEX>-caryophyllene oxide (CPO) (3.63%), caryophylla-3(15),7(14)-dien-6-ol (2.68%), (E)-methyl isoeugenol (1.90%), <TEX>${\alpha}$</TEX>-terpineol (1.76%), and octadecane (1.23%). Besides GHF, CPO, but not phytol, also inhibited the AKT/mTOR/S6K1 signaling pathway and induced apoptosis in prostate cancer cells. Overall, these findings suggest that guava leaves can interfere with multiple signaling cascades linked with tumorigenesis and provide a source of potential therapeutic compounds for both the prevention and treatment of cancer.

        부산 항만물류산업의 실태에 관한 연구

        본 연구는 부산시의 항만물류산업의 실태분석에 목적을 두고 있다. 항만물류의 개념 및 기능에 기반을 두고 항만물류산업을 체계적으로 분류하고, 실태분석을 한 결과, 부산시 항만물류산업은 업종별 편중이 극심한 문제점을 가지고 있고, 게다가 항만물류산업의 전형적인 산업인 하역과 보관업이 다른 업종에 비해 영세성을 면치 못하고 있는 실정임을 알 수 있었다. 또한, 항만물류산업이 부산시의 지역경제에 상당한 비중을 차지하나 외부로의 유출에 대한 조사가 좀 더 정확히 조사된다면 일반적이 예상과 달리 그다지 크다고 볼 수 없을 것으로 사료된다. 따라서, 본 연구의 분석결과로부터, 항만물류산업이 지역경제에 보다 긍정적인 역할을 하기 위해서는 항만물류에 반드시 필요한 산업이나 영세성을 면치 못하는 다수의 업종에 대한 다양한 지원과 종합적인 육성책을 마련해서 물류산업전체의 균형적인 활성화 대책의 수립이 무엇보다 중요한 것으로 사료된다. 다음으로, 부산시의 항만물류산업은 관련업종에 따라 서로 다른 여건과 실적이 매우 다르므로 종래의 포괄적 차원의 지원책보다 차별화된 지원책의 수립이 효과적으로 것으로 사료된다. The aim of this research is to analyze actual condition of port logistics industries in Busan. For this, this research identified port logistics, based on the concept and definition of port and logistics, and then, divided them into six categories, transportation, cargo candling, storage, service, packing, and information. The results of the paper are, first, it was found that there are much differences among industries, in revenue scale, and number of employee and firm. Secondly, it indicated that actual occupation rate of these industries in was necessary to build different policies according to competitiveness level of firms, and strategies to promote many firms with low occupation rate.

        최근 정보화는 사회전반에 걸쳐 핵심 전략수단으로 부각되고 있으며, 항만에 있어서도 정보화 및 정보기술의 중요성은 예외없이 강조되고 있다. 그러나, 이러한 항만 정보화의 중요성에도 불구하고 이에 대한 기반연구가 부족하고, 게다가 항만의 정보화 수준을 제대로 측정할 수 있는 기준과 그 수준을 분석한 연구는 전무한 실정이다. 본 연구는 이러한 점에 주목하여 항만의 정보화 수준을 파악하기 위한 기준을 수립하고, 정보화 수준 평가지수 개발하는 것을 연구의 목적으로 한다. 그리고, 수립된 지수모델을 바탕으로 그 적용가능성을 가늠해 보고자 한다. In recent, ITs(Information Technologies) have been as essential methods for port operation. However, despite of the importance of ITs, there has not been any research that evaluate and analyze current level of ITs performance and their utilizations. Thus, objective of this study is to develop integrated indicators to evaluate current level of ITs performance and their utilization in container terminal. To confirm the effectiveness of the suggested indicators, current level of ITs performance in a container terminal was measured, and then some implications based on findings were presented.

        현재 국내 대도시권은 도시의 외연적 확장과 도로 중심의 교통물류체계 및 통행패턴으로 인하여 도로운송 시스템은 이미 포화상태에 도달하여 교통 혼잡으로 인한 물류비용의 증가, 도로 시설 유지보수 비용 증가 등 사회 경제적 비용 부담이 가중되고 있다. 화물운송시스템도 도로운송에 의존하고 있어 이산화탄소의 배출량이 많아 대기오염의 주된 요인으로 작용하며, 야간 운송에 따른 소음 유발, 교통사고 발생, 도로파손 등 사회문제를 야기하고 ‥‥(중략)‥‥.

        Structural characterization of the putative DNA-binding domain of CP2c and its relevance to zinc binding

        The transcription factor CP2c has been recently validated as an oncogenic protein that can serve as a promising target for anticancer therapy. We have recently documented that a recombinant protein corresponding to the putative DNA-binding region (residues 63-244) of CP2c adopted two different conformers, one of which is dominated by zinc binding. However, in the present study, a longer construct encompassing residues 63-302 appeared to form a single structural domain. This domain could be considered to adopt a functionally relevant fold, as the known specific binding of a dodecapeptide to this protein was evident. Hence, the residues 63-302 region rather than 63-244 can be regarded as a natively folded structural domain of CP2c. In addition, it was confirmed that zinc ions can bind to this putative DNA-binding domain of CP2c, which resulted in reduced stability of the protein. In this context, it is suggested that the mode of action of CP2c would resemble that of tumor suppressor p53.

        후뇌동맥에 발생한 蛇行性 內管 巨大動脈瘤 : 1예 보고 Csae Report

        본원 신경외과에서는 최근 후뇌동맥에 발생한 사행성 내관 거대 동맥류를 하측두엽 도달법으로 후뇌동맥의 포획 및 동맥류 완전절제(trapping & excision)를 하였음에도 시야결손등의 신경학적 장애소견없이 치유된 경험을 하였기에 문헌고찰과 함께 보고하는 바이다. Aneurysms of the posterior cerebral artery are rare, furthormore giant serpentine aneurysms-partially thrombosed aneurysms containing a tortuous vascular channel-of the posterior cerebral artery are extremly rare. The author et al. were able to remove 3.9㎝×2.9㎝×3.0㎝ sized giant serpentine aneurysm of the distal PCA totally, without any significant postoperative neurological sequela except transient ipsilateral third cranial nerve paresis and transient hemihypesthesia. Various kinds of surgical treatment were proposed by several authors in managing unclippable giant aneurysms : excision including partial excision ; clipping of feeding artery and excision ; trapping and excision ; revascularization and excision : anastomosis only. We took subtemporal approach under the surgical microscope and trapping & excision was performed in several steps : proximal clipping of the PCA distal to the Pcom-PCA junction : reducing volume of the mass through piecemeal resection of the aneurysmal wall : distal clipping of the PCA ; total excision and final trapping just proximal and distal to the origin of the aneurysm. We are discussing the characteristics of the giant serpentine aneurysm, precognition of rich collateral circulation of the PCA distal to trapping and surgical management of the giant serpentine aneurysm.

        Caffeic Acid Phenethyl Ester Inhibits Cell Proliferation and Induces Apoptosis in Human Ovarian Cancer Cells

        The phenethyl ester of caffeic acid (CAPE), an active component of honeybee propolis extract, is shown to inhibit cancer growth previously. However, studies on human ovarian cancer are largely obscure. This study evaluated the effects of CAPE as a potential anti-proliferative and pro-apoptotic agent in the human ovarian cancer line, OVCAR-3. CAPE treated OVCAR-3 cells showed inhibition of cell viability and proliferation in a dose-dependent manner by WST-1 assay, LDH assay and bromodeoxyuridine (BrdU) incorporation assay. Furthermore, CAPE-mediated OVCAR-3 cell growth inhibition was associated with apoptotic changes as evident by cell cycle arrest and accumulation of cells in the apoptotic phase and DNA fragmentation. Taken together, CAPE inhibits cell proliferation via DNA synthesis reduction and induces apoptotic cell death via DNA damage, thus elucidating a novel, plausible mechanism of CAPE anti-tumorigenic property in OVCAR-3 cells.
