Primary unruptured appendiceal mucocele is a rare disease. It is very difficult to diagnose preoperatively because of the rarity of classical symptoms and low incidence. Usually it is discovered incidentally during abdominal ultrasonography(US), computed tomography(CT), or appendectomy for acute appendicitis. If untreated, one type of mucoceles may rupture producing a potentially fatal entity known as pseudomyxoma peritonei, therefore preoperative diagnosis is very important. With the advent of colonoscopy, abdominal US and CT, it has been possible to preoperatively diagnose mucocele of appendix. We experienced a case of appendiceal mucocele in a 60 year-old woman who had symptoms of lower abdominal discomfort and right lower quadrant abdominal pain, and it was diagnosed preoperatively by colonoscopy, abdominal US and CT. So we present this case with review of literature.

통풍은 Purine 대사의 이상으로 발생하는 질환으로, 고뇨산혈증인 사람의 전부가 통풍으로 발현되지 않고 증상의 출현 양상이 다양하여 진단과 치료에 주의가 필요하며, 조기에 적절한 조치를 한다면 충분히 조절이 가능한 질환이다. 본대학 내과학교실에서는 통풍으로 치료한 32명의 환자에서 임상양상, 병력과 검사소견을 분석하여 다음과 같음 결과를 얻었다. 1. 32명 모두 남자이며, 최초 발병 시기는 24세에서 72세로 평균 43.8±11.9세이었으며, 30대에서 40대까지가 19명(59.4%)으로 대부분을 차지하였다. 내원 당시 나이는 27세에서 75세까지로 평균 52.3±10.4세였으며, 내원시 까지 평균 유병기간은 8.5±6.8년으로 나타났다. 2. 동반 질환으로는 고지혈증 12례, 신장질환 10례, 고혈압 12례, 비만 8례, 당뇨 2례 등이었다. 3. 이환된 관절은 단관절 침범이 19례(59.4%), 다관절 침범이 13례 이었으며, 최초 이한된 관절은 족무지 중족골지골 관절로 19례(59.4%)로 가장 많았고, 통풍 결절은 20례(62.5%)에서 관찰되었으며, 유병기간이 10년 이상된 12례중 11례 (91.7%)에서 결절이 관찰되었다. 4. 평균 혈중 요산치는 9.17±1.75 ㎎/dl이었으며, 8.0 ㎎/dl에서 9.9 ㎎/dl 사이가 19명으로 전체의 59.4%를 차지하였다. Objective: The gout is a heterogeneous group of diseases resulting from tissue deposition of monosodium urate or uric acid crystals from extracellular fluids supersaturated with respect to this end product of human purine metabolism. The clinical manifestations are such as hyperuricemia, gouty arthritis, gouty nephropathy, uric acid nephrolithiasis. We analyze of clinical manifestations and associated factors in gout. Method: We have reviewed the medical records, radiologic findings and clinical results of thirty-two patients admitted at our department from April 1996 to July 1997. Result: 1) All patients were male. The mean age at initial attack was 43.8 years old, ranging from 24 to 72 years old. 2) The mean level of serum uric acid was 9.17mg/dl on admission. 3) The first metatarsopharyngeal joint was involved in 19 cases (59.4%). Tophus was observed in 20 cases (62.5%). 4) Hyperuricemia was associated with hypertension, obesity, nephrolithiasis and hyperlipidemia.

Background/Aims: In a gastric fundic strip, electrical field stimulation (EFS) evokes TTX-sensitive biphasic responses, consisting firstly of cholinergic contraction followed by a transient relaxation. It is well known that nonadrenergic noncholinergic (NANC) inhibitory nerve mediates a transient relaxation. This study was performed to investigate the characterization of relaxation and its modulation by substance P. Methods: Using Guinea-pig gastric fundic smooth muscle tissues, we recorded mechanical contractions induced by EFS in the organ bath with platinum. Results: NG-nitro-L-arginine methyl ester, an inhibitor of nitric oxide synthase, reduced EFS-induced relaxation and these effects were reversed by L-arginine, a precursor of nitric oxide (NO). Sodium nitroprusside, a NO-donor, suppressed the fundic basal tension. Cell permeable 8-bromo-cGMP inhibited noradrenaline-induced contraction. The application of substance P increased basal tension and EFS-induced contraction and relaxation. NK-1 receptor antagonist ([D-Pro9-(spiro--lactam)9,10-Trp11]-Substance P) inhibited substance P-induced effects. TEA and apamin, K+ channel blockers, increased basal tension and EFS-induced relaxation. Conclusions: These results indicate that NANC inhibitory responses are mainly mediated by NO in the guinea-pig fundus and the release of NO is modulated by substance P through NK-1 receptor and by prejunctional K+ channels. 본 연구에서는 변압기 내진성능을 확보하기 위해 면진장치에 따른 지진 저감성능과 특성을 평가하기 위한 수치해석을 수행하였다. 해석에 적용한 면진장치는 마찰진자면진장치(FIP)와 납-고무면진장치(LRB)이며 각 면진장치를 유한요소해석 프로그램을 사용하여 수치해석 모델화하였다. 또한, 변압기의 형상적 특성을 고려하기 위해 수치해석 모델링한 변압기의 형상과 하중이 실제 변압기와 유사하도록 모델링을 수행하였다. 변압기 해석모델에 마찰진자면진장치와 납-고무면진장치를 적용하여 동적해석을 수행하기 위해 내진설계기준 공통적용사항과 ICC-ES AC156의 응답스펙트럼을 참고하여 생성한 인공지진파를 해석에 적용하였다. 해석결과 면진장치가 적용되지 않은 해석과 비교하였을 때 마찰진자면진장치를 적용한 변압기에서 발생한 가속도 감쇠율은 75%로 나타났으며 납-고무면진장치를 적용한 변압기의 응답가속도 감쇠율은 최소 42%로 나타나 마찰진자면진장치가 납-고무면진장치에 비해 가속도 응답면에서 우수한 결과를 보였다. 또한, 면진장치를 적용하였을 때 발생하는 변압기의 회전을 검토하기 위해 가속도가 작용할 때 변압기 좌우측의 상대변위를 측정하였다. 이때 납-고무면진장치가 마찰진자면진장치에 비해 변압기의 회전이 큰 것으로 나타나 납-고무면진장치의 설치를 고려할 때 면진 대상구조물의 회전에 대한 적절한 대응책의 마련이 필요할 것으로 보인다. In this study, a numerical analysis was performed to evaluate the seismic reduction performance and characteristics of a seismic isolator used to ensure the performance of the transformer under seismic conditions. The seismic isolators used in this analysis were the Friction Isolation Pendulum(FIP) and the Lead-Rubber Bearing (LRB). Each seismic isolator was modeled using a numerical analysis program. To consider the characteristics of the transformer, the model in the simulation was made similar to the actual shape of the transformer. To perform dynamic analysis, artificial seismic waves were generated and targeted onto the transformer by referring to the response spectrums for common applications from the seismic design standards and the ICC-ES AC156. In the dynamic analysis of the model, the FIP showed better results in terms of acceleration response than the LRB. Also, in the analysis, the LRB showed bigger rotational deformation as compared to the FIP. Therefore, appropriate countermeasures against the rotational deformation of the structure are required while installing the LRB.

Fast and accurate diagnosis is needed to eradicate and manage economically important and invasive diseases like fire blight. Loop-mediated isothermal amplification (LAMP) is known as the best on-site diagnostic, because it is fast, highly specific to a target, and less sensitive to inhibitors in samples. In this study, LAMP assay that gives more consistent results for on-site diagnosis of fire blight than the previous developed LAMP assays was developed. Primers for new LAMP assay (named as DS-LAMP) were designed from a histidine-tRNA ligase gene (EAMY_RS32025) of E. amylovora CFBP1430 genome. The DS-LAMP amplified DNA (positive detection) only from genomic DNA of E. amylovora strains, not from either E. pyrifoliae (causing black shoot blight) or from Pseudomonas syringae pv. syringae (causing shoot blight on apple trees). The detection limit of DS-LAMP was 10 cells per LAMP reaction, equivalent to $10^4$ cells per ml of the sample extract. DS-LAMP successfully diagnosed the pathogens on four fire-blight infected apple and pear orchards. In addition, it could distinguish black shoot blight from fire blight. The $B{\ddot{u}}hlmann$-LAMP, developed previously for on-site diagnosis of fire blight, did not give consistent results for specificity to E. amylovora and on-site diagnosis; it gave positive reactions to three strains of E. pyrifoliae and two strains of P. syringae pv. syringae. It also, gave positive reactions to some healthy sample extracts. DS-LAMP, developed in this study, would give more accurate on-site diagnosis of fire blight, especially in the Republic of Korea, where fire blight and black shoot blight coexist.
폐쇄성 황달에서 담도 배액 전후의 Cytokine과 Endotoxin에 대한 연구

Background/Aims: Malignant biliary obstruction are associated with high postoperative morbidity and mortality. This study was performed to determine the effect of external and internal biliary drainage on plasma concentrations of cytokines and endotoxin. Methods: Twenly-one patients with malignant biliary obstruction were enrolled in this study. They were devided into two groups, PTBD (percutaneous transhepatic biliary drainage) and ERBD (endoscopic retrograde biliary drainage)groups. Then, plasma concentrations of TNF-α, sTNFr, interleukin 1α (IL-1α), interleukin 6 (IL-6), and endotoxin were measured before and after 1 week in PTBD and ERBD groups. Results: Total bilirubin decreased significantly from 13.3±8.2 to 6.2±4.4 mg/dL after PTBD and 7.0±11.2 to 5.3±4.0 mg/dL after ERBD (p=0.005, 0.007, respectively). Plasma sTNFr in PTBD group was 5.1±1.4 before drainage and remained unchanged 5.5±1.5 ng/mL at one week after drainage (p=0.155). Plasma sTNFr in the ERBD group was 5.9±1.5 and 5.6±1.0 ng/mL before and after biliary drainage, respectivedly (p=0.333). There was no significant difference of sTNFr changes between the PTBD and ERBD groups (p=0.113). Moreover, there was no significant difference in endotoxin changes between the PTBD and ERBD groups (p=0.477). Other cytokines changed insignificantly. Conclusions: Both PTBD and ERBD did not affect significantly plasma concentrations of sTNFr and endotoxin. Although ERBD provides a better quality of life and physiologic benefit, it dose not decrease cytokines and endotoxin associated with high morbidity and mortality in patients with malignant biliary obstruction.
