OBJECTIVES: Self-rated health is an instrumental variable to assess the overall health status of a population. However, it remains questionable whether it is still useful for cognitively impaired individuals. Therefore, this study aims to analyze whether self-rated health by the cognitively impaired predicts mortality reliably. METHODS: This study used 7,881 community-dwelling individuals, aged 45 and above, from the Korean Longitudinal Study of Aging (2006-2016). It used the Cox proportional hazard models for analysis. Cognitive status was classified based on the Korean Mini Mental State Examination score and a stratified analysis was used to determine whether the predictability of self-rated health varies according to cognitive status. RESULTS: For cognitively intact individuals, the adjusted hazard ratios (aHR) of mortality were 2.00 (95% confidence interval [CI], 1.18 to 3.41, model 4) for those with ‘bad’ self-rated health and 2.40 (95% CI, 1.35 to 4.25, model 4) for those with ‘very bad’ self-rated heath, respectively, compared with those with ‘very good’ health. The results remain statistically significant even after adjusting for socio-demographic factors, health status, and health-related behaviors. For cognitively impaired individuals, the aHR of mortality was statistically significant for those with ‘very bad’ self-rated health, compared with those with ‘very good’ health, when socio-demographic factors were accounted for (aHR, 3.03; 95% CI, 1.11 to 8.28, model 2). CONCLUSIONS: Self-rated health by cognitively impaired individuals remains useful in predicting mortality. It appears to be a valid and reliable health indicator for the rising population with cognitive impairment, especially caused by aging population.

Short-chain acyl-CoA dehydrogenase (SCAD) deficiency is an autosomal recessive mitochondrial disorder of fatty acid oxidation associated with mutations in the ACADS gene. While patients diagnosed clinically have a variable clinical presentation, patients diagnosed by newborn screening are largely asymptomatic. We describe here the case of a 1-year-old male patient who was detected by newborn screening and diagnosed as SCAD deficiency. Spectrometric screening for inborn errors of metabolism at 72hrs after birth showed elevated butyrylcarnitine (C4) level of 1.69 mol/L (normal, <0.83 mol/L), C4/C2 ration of 0.26 (normal, <0.09), C5DC+C60H level of 39 mol/L (normal, <0.28 mol/L), and C5DC/C8 ration of 7.36 (normal, <4.45). The follow-up testing at 18 days of age were performed: liquid chromatography tandem mass spectrometry (LC-MS/MS), urine organic acids, and quantitative acylcarnitine profile. C4 carnitine was elevated as 0.91; urine organic acid analysis showed elevated ethylmalonic acid as 62.87 nmol/molCr (normal, <6.5), methylsuccinate 6.81 nmol/molCr (normal, not detected). Sequence analysis of ACADS revealed a homozygous missense mutation, c.164C>T (p.Pro55Leu). He is growing well and no episodes of seizures or growth retardation had occurred.

<P>This study optimizes the synthesis process of pellet-type adsorbents using alum sludge. The effect of the binder and heat treatment temperature on the nanopore formation in the adsorbent is investigated. The pellet-type adsorbent prepared using the powder-type sludge from water treatment is determined to be a material that contains nanopores. The specific surface area is increased significantly after the calcination process in the range of 132-172 m2/g. With the calcination treatment, the breakthrough time in the formaldehyde adsorption increases remarkably with an optimum calcination temperature of 400 °C. The breakthrough capacity of the formaldehyde increases to a maximum 2.96 mg/g at this temperature.</P>

Trajectory generation is important because it decides the walking stability, continuity and performance. Generally, LIPM is used for it, however it results in a trajectory that cannot have vertical motion because it's linearization. This paper proposes a new trajectory generation method called Extended Linear Inverted Pendulum Mode(ELIPM). It is possible to generate vertical motion and even revolute motion. It can change the performance of locomotion by controlling the stride and locomotion frequency.

Laparoscopy has been one of the most effective modalities in various surgical situations, although its use in trauma patients has some limitations. The benefits of laparoscopy include cost-effectiveness, shorter length of hospital stay, and less postoperative pain. This report describes diagnostic laparoscopy and laparoscopic diverting sigmoid loop colostomy in penetrating extraperitoneal rectal injury. A 41-year-old male presented with perineal pain following penetrating trauma caused by a tree limb. Computed tomography showed air density in the perirectal space and retroperitoneum. As his vital signs were stable, we performed diagnostic laparoscopy and confirmed no intraperitoneal perforation. Therefore, laparoscopic diverting sigmoid loop colostomy was performed. He was discharged without any complications despite underlying hepatitis C-related cirrhosis. Colostomy closure was performed 3 months later.

This study was carried out about the system engineering, the design, and Mock-up for the development of passenger car of Korean High Speed Train of maximum operating speed of 350㎞/h. The design was studied to carbody section & lay out, ring, fitting, carbody, electrical system based on Korean-TGV. The design of aluminium alloy carbody which was enabled to reduce the weight of carbody structure and was studied with construction of air-tightness. Air pressure controlled system provided to comfort passenger due to a reduction of difference pressure between inside and outside of passenger room on running of the tunnel. The interior design was performed in order to satisfy high speed and comfort to study by the modern design. The electrical system was designed two parts of logic and network for high reliability of train.

궤적생성은 로봇보행의 안정성, 연속성 그리고 보행특성을 결정하는 가장 중요한 일이다. 보통 궤적생성법으로 LIPM을 많이 쓰지만 그것은 그것의 선형화 기법 때문에 수직방향으로의 동작생성을 할 수 없다는 단점이 있다. 이 논문에서는 ELIPM이라는 새로운 궤적생성법을 제안한다. 이 궤적생성법은 전진방향의 동작은 물론이고 수직방향의 동작을 간단하게 생성할 수 있다. Generally, the Linear Inverted Pendulum Mode is used for trajectory generation; however, for the sake of simplicity, the trajectory in this mode does not allow vertical motions and pitching motions of the body. This paper proposes a new trajectory generation method called Extended Linear Inverted Pendulum Mode (ELIPM) that allows vertical motion as well as pitching motion. This method can also improve the performance of locomotion by controlling the stride and locomotion frequency of a body.
Isolated Common Hepatic Duct Injury after Blunt Abdominal Trauma

Extrahepatic bile duct injury is commonly associated with hepatic, duodenal, or pancreatic injuries, and isolated extrahepatic bile duct injury is rare. We report a patient who presented with an isolated extrahepatic bile duct injury after blunt trauma. A 50-year-old man was referred to our hospital after having suffered a fall down injury. His laboratory findings showed hyperbiliribinemia with elevated aspartate aminotransferase and alanine aminotransferase level. Initial abdominal computed tomography (CT) showed a mild degree of hemoperitoneum without evidence of abdominal solid organ injury. On the 3rd day of hospitalization, the patient complained of dyspnea and severe abdominal discomfort. Follow-up abdominal CT showed no significant interval change. Owing to the patient’s condition, Emergency laparotomy revealed a large amount of bile-containing fluid collection and about 1 cm in size laceration on the left lateral side of the common hepatic duct. Primary repair of the injured bile duct with T-tube insertion was performed On postoperative day (POD) 30, endoscopic retrograde cholangiopancreatography showed minimal bile leakage and endoscopic sphincteroplasty and endoscopic retrograde biliary drainage were performed. On POD 61, the T-tube was removed and the patient was discharged.
