      • 초등학생 입학 후 발생한 두통의 유병률과 임상적 특성

        Purpose : The purpose of the study was to determine prevalence and clinical features of the occurrence of childhood headache after the start of elementary school. Method : The survey was performed by a special questionnaire on 552 children in grade 1 (195 children) and grade 6 (357 children) at the three elementary schools from 1 to 31 August, 2002 in Gwangju city. We have analyzed the prevalence and the clinical features of headache of them. Result: The prevalence of headache in grade 6(49.0%) was statistically higher than grade 1(22.6%). The onset time of headache within 6 months in grade 1 (88.2%) was significantly higher than grade 6 (64.1 %). The frequency of headache is more often in grade 6. As for the timing of occurrence of headache, grade 1 student suffered from it after school (36.4%), grade 6 student suffered from it at school (30.9%). Children who suffered from severe headache can't live in the routine life were grade 1 20.5 % and grade 6 21.1 %, respectively. According to the duration of headache, there were within 1 hour grade 1 90.9%, grade 6 81.8%, respectively. Risk factors of headache were, in grade 1, fatigue (34.0%), stress (25.0%), weather change (18.2%), anxiety (11.4%), and others (11.4%). In grade 6, there were fatigue(44.3%), stress (34.0%), weather change(10.9%), others (5.7%) and anxiety (5.1%). Conclusion : Starting school represents a remarkable life event for children. Starting school results in significantly increasing prevalence of overall headache. 목적 : 입학 전에 비해 입학 후 두통의 증가률을 알아보고, 1학년의 두통의 임상적 특징을 알아보고자 하였다. 방법 : 2002년 8월 1일부터 31일까지 광주시내 3개 초등학교 1학년과 6학년 학생 612명에게 설문지 (두통 에 대한 주요 설문지 내용 :부록)를 배부하여 그 중 작성이 불량한 60명을 제외한 552명 (1학년 195명, 6학년 357명)을 대상으로 두통의 유병률을 알아보았으며, 임상적 특징은 두통이 있는 학생 219명 (1학년 44명, 6학년 175명)을 대상으로 분석하였다. 결과 : 두통의 빈도는 1학년 44명 (22.6%), 6학년 175명 (49.0%)로 6학년에서 의의 있게 높았다(0.001). 두통의 초발 시기가 6개월 이상인 경우 1학년 11.8%, 6학년 35.9%이였으며, 6개월 미만인 경우는 1학년 88.2%, 6학년 64.1%로 입학 전보다 입학 후에 더 높았으며 6개월 미만의 초발 두통이 1학년에서 의의 있게 높았다(p=o. 03). 두통의 발생빈도는 6학년에서 통계학적으로 의의 있게 높았다(P=0. 03). 두통이 가장 자주 발생하는 시기는 1학년에서 학교 수업 후 (36.4%)이었고, 6학년에서는 학교 수업 중(30.9%)이었다. 학교나 일상 생활을 하기 힘들 정도의 두통은 각각 1학년에서 20.5%, 6학년에서 21.1%이었다. 두통의 지속시간은 1시간 이내인 경우가 각각 1학년 90.9%,6학년 81.8%이었다. 두통의 유별인자는 1학년에서 피로, 수면 부족이 34.0%로 가장 많았으며, 스트레스, 기후 변화, 불안, 기타 순이었고, 6학년은 피로 수면 부족 44.3%로 가장 많았으며, 스트레스 34.0%, 기후 변화, 기타, 불안 순이었다.

      • 소아 흉통의 원인질환에 따른 임상양상

        목적: 소아에서의 흉통은 흔히 접하게 되며 일반적으로 좋은 경과를 보인다. 가장 흔한 원인으로 특발성, 골격계이상, 위장관계 질환 등이 다양하게 언급되고 있으며 감별진단이 쉽지 않아 여러 검사를 시행하게 된다. 자세한 병력청취와 진찰이 진단에 가장 중요하다고 하지만 통계적 의의를 보고한 바는 드물다. 이에 흉통 원인을 분류하고 각각의 임상양상을 파악하여, 병력 청취와 진찰소견의 감별진단에 대한 유용성과 시행 검사들의 의의를 알아보고자 하였다. 방법: 2004년 3월 부터 2007년 8월 까지 조선대학교 병원 소아청소년과에 흉통을 주소로 방문한 환아 77명을 대상으로 하였다. 연령, 성별, 흉통의 위치, 지속시간, 양상, 빈도, 방문까지 걸린 시간, 동반증상, 유발인자의 각 항목과 원인 질환과의 관련성을 분석하였다. 흉부방사선, 심전도, 심초음파, 24시간 Holter, 운동부하 심전도, 상부 위장관 조영술, 24시간 pH 검사, 위내시경, 심근효소를 검사 하였다. 결과: 평균 나이는 10.2±2.8세로 6세에서 11세까지가 48명(62.3%)으로 가장 많은 분포를 보였고 남아 42명(54.5%), 여아 35명(45.5%)이었다. 흉통의 위치는 좌측이 32명(41.6%), 양상은 콕콕 찌르는 것이 27명(35.1%)으로 가장 많았고 지속시간은 1분미만이 36명(46.8%), 1-5분 사이가 28명(36.4%)으로 대부분 5분 미만이었다. 흉통 원인은 특발성이 27례(35.1%)로 가장 많았고, 위장관 질환 23례(29.9%), 늑골 연골염 12례(15.6%), 심혈관 질환 7례(9.1%), 정신신체화 질환 6례(7.8%), 폐질환 2례(2.6%)였다. 여아에서 연령이 높을수록 정신신체화 질환의 빈도가 높았다(P= 0.046). 흉골 하부 통증을 호소한 17명 중 13명(76.5%)이 위장관 질환으로 통계적으로 유의하였다(P= 0.023). 정신 신체화 질환은 6명에서, 위장관 질환도 20명(86.9%)에서 흉통지속시간이 5분 미만이었으나 통계적 의의는 없었다. 흉통 양상 중 타는 듯한 양상을 호소한 7명중 6명에서 위장관 질환이 원인이었으나 통계적 의의는 없었다. 선택 시행 검사 중 심초음파는 32명중 3례(9.4%), 24시간 Holter 검사는 13명중 5례(35.7%), 24시간 pH 검사는 28명 중 16례(57.1%)에서 이상소견을 보였다. 결론: 소아 흉통 환자 중 연령이 높은 여아에서는 정신 신체화 장애를, 흉골 하부 통증을 호소한 경우는 위장관 질환을 먼저 생각하는 것이 좋다고 판단된다. 흉통원인으로 위장관 질환의 빈도가 높고 시행검사들의 진단율을 볼때, 소아 흉통 환아 에서는 상부 위장관 조영술 등 위장관 질환의 검사를 기본적으로 시행하는 것이 좋을 것으로 생각된다. Purpose: Chest pain in children is usually benign. Chest pain in children results from a wide variety of causes. Differential diagnosis of causes is not easy and then many diagnostic approach is trying. Careful history and physical exams must guide the assessment of children with chest pain. The purpose of this study intends to evaluate children with chest pain and usefulness of careful history, physical examination and several diagnostic examinations. Methods: From March 2004 to August 2007, 77 patients presented with chest pain were enrolled. we evaluated age, sex, characteristics of chest pain (location, duration, description, frequency), accompanying symptoms, increasing reasons and relationship with cause of chest pain. Chest x-ray and ECG were checked in all patients. Cardiologic and gastrointestinal evaluations were checked when considered necessary. Results: The male and female ratio was 0.55:0.45, mean age 10.2 ± 2.8 year. Common ages of chest pain were 10 to 12 years old (38case, 49.3%). The location was on the left precordium in 32cases (41.6%) of the patients. The most common diagnostic findings of chest pain were idiopathic(27cases, 35.1%), gastrointestinal disorder (23cases, 29.9%), costo chondritis (12cases, 15.6%), cardiovascular disorder (7cases, 9.1%), psychosomatic disorder(6cases, 7.8%) and pulmonary disorder (2cases, 2.6%). Older children in female were more likely to have a psychosomatic cause (P= 0.046). The patients complained chest pain on substernal area were 17cases and 13cases (76.5%) were diagnosed as gastrointestinal disorder (P= 0.023). Positive findings of echocardio graphy, 24hr Holter ECG and 24hr pH monitoring were present in 3 (9.4%), 5 (35.7%) and 16 (57.1%) of the patients respectively. Conclusion: Gastrointestinal disorders are common causes and positive diagnostic rate of gastrointestinal evaluation is relatively high in children with chest pain. Old girls are more likely to have a psychosomatic disorder and substernal pain is the reliable sign to be considered as a gastrointestinal disease in children with chest pain. Hence, Diagnostic approach of gastrointestinal disease such as upper GI should be performed basically in mostly children with chest pain.

        수종의 근관세척액과 Listerine의 항균성 비교 연구

        본 연구는 여러 가지 근관세척액(NaOCl, CHX, EDTA)과 구강세정제로 사용되는 Listerine을 근관감염균주인 Porphyromonas gingivalis와 Enterococcus faecalis를 상대로 항균효과를 비교하고 Listerine이 근관세척액으로 사용가능한지를 확인하고자 시행하였다. 본 연구에서는 Porphyromonas gingivalis ATCC 3327과 Enterococcus faecalis ATCC 29212의 표준균주를 사용하였다. 실험을 위한 근관세척제로 0.1%, 0.2%, 1%, 2% Chlorhexidine(CHX)과 0.5%, 1%, 2.5%, 5.25% NaOCl, 0.5M EDTA(18.6% EDTA). 그리고 Listerine원액을 이용하였다. 항균효과의 비교는 액체배지상에서 균주의 혼탁도와 한천 확산법을 이용한 억제대 비교로 하였다. 모든 실험군은 대조군과 비교시 근관내 균주에 항균성을 나타냈다(p < 0.001). 모든 농도의 NaOCl, CHX, 그리고 EDTA는 실험균주에서 높은 항균성을 보였다. 모든 실험에서 Listerine은 다른 근관세척제에 비해 낮은 항균성을 보였다. 결론적으로, Listerine이 E. faecalis와 P. gingivalis에 대해 항균성을 보이나, 일반적으로 사용되는 근관세척 액에 비해서는 현저히 낮은 항균성을 나타냄으로 근관세척 액으로 사용은 적합하지 않음을 나타냈다. The purpose of this study is to compare the antibacterial effect of Listerine on two microorganisms (P. gingivalis and E. faecalis) with various root canal irrigants (NaOCl, CHX, EDTA) and to identify possibility of using Listerine as a root canal irrigant. Porphyromonas gingivalis ATCC 3327 and Enterococcus faecalis ATCC 29212 were used in this experiment. For the test irrigants, 0.5%, 1%, 2.5%, 5.25% NaOCl, 0.1%, 0.2%, 1%, 2% CHX, 0.5M EDTA (18.6% EDTA) and Listerine were prepared. Distiled water was used as control. Two methods-1) Comparison of turbidity in broth and 2) Agar diffusion test-were used to determine the extent of antibacterial effect of Listerine and to compare it with that of NaOCl, CHX, and EDTA. All solutions tested were effective against two bacterial strains compared with control (p < 0.001). Any concentration of NaOCl, CHX, and EDTA showed similarly high effectiveness against all bacterial strains. In all experiment, Listerine showed significantly low antibacterial effect compared with the other root canal irrigants (p < 0.05). In conclusion, the results reflect remarkably low antibacterial effect of Listerine as compared with root canal irrigants in general so it is not suitable for the root canal irrigant.

        대동맥 판막 치환술 후 반복되는 Acinetobacter baumannii 균혈증의 원인이 된 대동맥염 1예

        감염성 대동맥염은 흔하지 않지만 매우 위중한 질병으로 Salmonella so.와 S. aureus가 주된 원인균이다. 아직까지 병원내 감염의 주요 원인균인 Acinetobacter baumannii에 의한 감염성 대동맥염은 보고된 바가 없었다. 저자들은 대동맥 판막 치환술을 시행받은 76세 남자 환자에서 적절한 항생제 투여에도 반복되는 A. baumannii 균혈증의 원인이 감염성 대동맥염으로 진단되었던 1예를 경험하였기에 보고하는 바이다. Infectious aortitis is an uncommon yet, life threatening disease. Early surgical treatment and prolonged antibiotic therapy is crucial to survival. Salmonella sp. and Staphylococcus aureus are the most common organisms isolated. There is no case report of infectious aortitis caused by Acinetobacter baumannii, which has recently emerged as a major cause of health care-associated infections. Here, we describe a 76-year-old male with infectious aortitis who experienced recurrent bacteremia due to A. baumannii in spite of adequate antimicrobial therapy after aortic valve replacement.

      • Renal mRNA Expression of Renin, AT₁Receptor, TGF-β1 and Fibronectin in Obstructive Nephropathy

        The present study was designed to quantify the alterations of renal renin, angiotensin type I receptor (AT1), TGF-β1, and fibronectin gene expression in rats with unilateral ureteral obstruction (UUO). We also investigated the change of AT1 density during UUO. Reverse transcription-polymerase chain reaction (RT-PCR) technique and receptor binding assay were used to detect mRNA expression and receptor density, respectively. At one day after UUO, retnin mRNA level of the obstructed kidneys was decreased transiently and then subsequently increased to the level of sham kidneys. In the contralateral kidneys of the same rats, on the contrary, renin mRNA level was gradually decreased. Then, at 9 days after UUO, it was significantly lower than that of sham kidneys. The expressions of both AT1 subtypes, called AT1A and AT1B, mRNAs did not change at any time. UUO led to a significant decrease in AT1 density in the obstructed kidneys compared with the sham kidneys at 1 and 3 days (66 ± 11.6% (p<7.05) and 73 ± 4.0% (p<0.01), respectively). Thereafter, AT1 density was gradually increased and at 9 days it showed a marked elevation in the obstructed kidneys compared to the sham kidneys. In contrast, in the contralateral kidneys AT1 density was significantly reduced from 3 to 9 days after UUO. The TGF-β1 mRNA level of the obstructed kidneys was unexpectedly decreased at 6 days after UUO. Then, at 9 days it was followed by a significant increase in the obstructed kidneys, whereas it showed an obvious decrease in the contralateral kidneys. In addition, fibronectin mRNA leve1 was also significantly increased in the obstructed kidneys after UUO compared to the sham or the contralateral kidneys of the same rats. These results suggest a differential regulation of renal renin, AT1 receptor, TGF-β1 and fibronectin mRNA levels at different stages of UUO.

        Role of Angiotensin II and Nitric Oxide in the Rat Paraventricular Nucleus

        <P> To investigate the mutual relationship between angiotensin II (Ang II) and nitric oxide (NO) in paraventricular nucleus (PVN), Ang II receptor type Ia (AT<SUB>1A</SUB>), type Ib (AT<SUB>1B</SUB>), endothelial constitutive nitric oxide synthase (ecNOS), and neuronal constitutive nitric oxide synthase (ncNOS) mRNA levels of rat PVN were measured after unilateral carotid artery ligation. AT<SUB>1A</SUB> and AT<SUB>1B</SUB> mRNA levels were markedly elevated 6 hrs after unilateral carotid artery ligation. Losartan injection (10μg/0.3μl) into the PVN augmented of the increment of AT<SUB>1A</SUB> and AT<SUB>1B</SUB> mRNAs It also increased ecNOS gene expression. In addition, AT<SUB>1B</SUB> mRNA levels increased after N-nitro-L-arginine methyl ester (L-NAME) injection (50μg/0.3μl) into the PVN. These results suggest that Ang II and NO in the rat PVN may interplay, at least in part, through regulation of gene expression of ecNOS and AT<SUB>1B</SUB>, respectively.

        Pulse Exposure to Ethanol Augments Vascular Contractility Through Stress Response

        <P> Drinking excessive alcohol has been recognized as a risk factor for hypertension. However, the mechanism by which alcohol intake causes hypertension still remains elusive. We tested the hypothesis that ethanol itself acts as a stress factor on vasculature and indirectly modulates vascular contractility. After end of exposure to 1, 2.5 and 5% ethanol for 45 min, rat aortic strips were subjected to contractile responses, immunoblot for Hsp70 and the measurement of levels of myosin light chain phosphorylation. Exposure to 5% ethanol not only augmented contractions to KCl or phenylephrine, but also increased expression of Hsp70 and the levels of myosin light chain phosphorylation. There were no significant differences in contractions produced by 1μmol/L phorbol 12,13-dibutyrate, a protein kinase C activator, whether the tissues were exposed to 5% ethanol or not. This is the first report to show that even short exposure to ethanol has a delayed effect to increase vascular smooth muscle contractility through a modulation of thick filament regulation. It may be a mechanism by which ingestion of alcohol induces hypertension.

        What Is Not the Controversy on the Ontology of Space

        본 논문은 공간의 존재론에 대한 실체론과 관계론의 논쟁을 해석하는 여러 다른 관점들을 고찰한다. 이들 관점들은 공간에 대한 전통적인 형이상학, 과학적 실제론, 인과적 설명에 근거하고 있다. 본 논문은 시공간의 존재론에 대한 이 세 가지 해석이 공간에 대한 실체론과 관계론 논쟁의 핵심을 포착하는 데 실패함을 지적한다. This essay considers various perspectives that attempts to capture what is in fact at stake within the substantivalism vs. relationism controversy on the ontology of space. These attempts are based on perspectives involving traditional metaphysics, scientific realism, causal accounts with regard to space. This essay evaluates these three interpretations of the ontological status of space. It is argued that these characterizations of the ontology of space fail to capture what is at issue within the substance-relation debate about space.

        Gene Expression of Intrarenal Renin-angiotensin System in Streptozotocin-induced Diabetic Rats

        In humans and many animal models with chronic progressive renal diseases, angiotensin-converting enzyme (ACE) inhibitor markedly attenuates the progression of nephropathy. Several studies have reported augmented gene expression and redistribution of renal renin in partial nephrectomized rats. Although precise mechanism(s) is not known, the renin-angiotensin system (RAS) may play an important role in the progression of renal diseases. Thus, this study was undertaken to examine the gene expression of renal renin, angiotensinogen, and $AT_1$ subtypes ($AT_{1A}$ and $AT_{1B}$) in rats with diabetic nephropathy, and the influences of lipopolysaccharide (LPS)-induced septicemia on the gene expression. Four weeks after streptozotocin (STZ) treatment (55 mg/kg, i.p.), rats were randomly divided into LPS-treated (1.6 mg/kg, i.p.) and control rats. At 6 hours after LPS treatment, the rats were killed and the kidney was removed from each rat. Northern blot and reverse transcription-polymerase chain reaction (RT-PCR)techniques were used to detect mRNA expression. STZ treatment markedly attenuated body weight gain and significantly increased blood glucose level. Renal renin content (RRC) was significantly decreased in the STZ-treated rats compared to that in control rats. The renal ACE activity between STZ-treated and control rats was not significantly different. Renal renin mRNA level was prominently increased, while angiotensinogen and $AT_{1A}$ mRNA levels were slightly decreased in STZ-treated rats compared to those in controls. $AT_1$B mRNA level did not differ in both groups. Acute LPS treatment did not show any significant changes of mRNA levels of intrarenal RAS components in both groups. These results suggest that intrarenal RAS components were differentially regulated in STZ-treated diabetic rats. Further studies are required to evaluate the relationship between intrarenal RAS and other vasomodulatory systems.
