        The First Case of Multisystem Inflammatory Syndrome in Adult after COVID-19 in Korea

        Multisystem inflammatory disease in children is a Kawasaki disease like illness occurring after severe acute respiratory syndrome coronavirus 2 infection in children. As the pandemic progresses, similar syndromes were also reported in adult with a decreased incidence. Multisystem inflammatory syndrome in adults (MIS-A) can be characterized with shock, heart failure, and gastrointestinal symptoms with elevated inflammatory markers after coronavirus disease 2019 (COVID-19) infection. Herein, we describe the first case of MIS-A in South Korea. A 38-year-old man presented to our hospital with a 5-day history of abdominal pain and fever. He had been treated with antibiotics for 5 days at the previous hospital, but symptoms had worsened and he had developed orthopnea on the day of presentation. He suffered COVID-19 six weeks ago. Laboratory data revealed elevated white blood cell counts with neutrophil dominance, C-reactive protein, and B-type natriuretic peptide. Chest X-ray showed normal lung parenchyme and echocardiography showed severe biventricular failure with normal chamber size. We diagnosed him as MIS-A and treated with intravenous immunoglobulin and steroid.

        Causes of a Low Measles Seroprevalence among Young Healthcare Workers in Korea

        Background: Sporadic measles outbreaks have continued to occur in Korea, mainly in adults in their 20s and 30s, most notably in 2014 and 2019. We here evaluated the possible causes of a low seroprevalance of measles by testing young healthcare workers (HCWs). Materials and Methods: This study was conducted in a 2,743-bed tertiary-care hospital in Seoul between 2020 and 2021. We performed a measles antibody test (chemiluminescence immunoassay), measured the IgM/IgG index ratio, and conducted an avidity test at 1-month after Measles-Mumps-Rubella (MMR) vaccination in HCWs who had been seronegative for measles. Measles vaccination histories were obtained from the national vaccine registry. Results: Of the 3,173 HCWs newly employed in our hospital during the study period, 54 with a negative measles IgG at commencement were enrolled. Thirty six (67%) of these subjects were female, and the median age was 25 years (interquartile range [IQR]: 24 - 27). Fourty nine (91%) showed seroconversion at 1 month after the first vaccination. Of these individuals, 38 received both measles IgM and IgG test, and all had an IgM/IgG index <1. Of the 49 seroconverters, all HCWs showed a high avidity index. According to the national immunization registry, 45 (83%) received at least 2 doses of an MMR vaccination. Conclusion: Secondary vaccine failure may underlie vaccine failure in young Korean adults. HCWs born after 1985 with a negative measles antibody may need only a single dose booster vaccination rather than a 2-dose vaccination regimen.

      • Proposal of Smart Cart Design to Improve User Experience

        Objective: The aim of this study is to improve users’ shopping experience through smart shopping cart design, which complements the inconvenience of existing shopping carts. Background: Consumers experience several difficulties in supermarkets. Common problems are spending time searching for products and waiting in line at the counter. For the convenience of customers, unmanned technology is in progress, and cases of using smart shopping carts are increasing. Method: To figure the focus points for design, we first divided the research field into three; general, unmanned, and smart carts. We then identified each category’s (1) shopping path, (2) how to find the goods, and (3) how users pay. The wasted time while shopping was discovered through this comparative analysis, and we looked into factors that could reduce shopping time. Through the prototype testing, we figured out that it is necessary to optimize the speed of product recognition. This is because lower barcode recognition functions also affect user shopping experiences. Results: The smart shopping cart noticeably reduced shopping times and routes through installed payment function and touchscreen. There is no need to take out and put the goods back in for payment, since the search for product’s location information on the touchscreen can be obtained immediately without having to wander or look for staff. The app linkage also increased the user’s shopping quality in the supermarket. Conclusion: Nowadays the users are oriented towards convenience. In line with the user trends, supermarkets are becoming unmanned using self-counters. The smart shopping cart we designed is also one of the ways to improve the user shopping experience. Smart shopping carts compensate for consumers’ inconveniences that could not be met only by self-checkout counters. Application: This study will provide smart shopping carts that can easily be used to help consumers save time and improve user’s shopping experience.

        Prediction for risk of ischemic heart disease (IHD) in aged 30 or older Korean using national health insurance service-national sample cohort (NHIS-NSC)

        The mortality rate of ischemic heart disease (IHD) was the highest rate of any heart diseases in South Korea. In this study, we predict the absolute risk factors of IHD according to individual characteristics or health status. Cox proportional hazards regression models were fitted to estimate hazard ratio of risk. Stepwise method was used to select a final model. After predictors were selected, the results were used to estimate the absolute 10-year IHD risk using Framingham risk score function and to calculated a simple point system. The total observation period for 106,167 subjects was 1,071,584 person-years. During the observation period, 4,627 subjects (4.4%) developed IHD with incidence rate of 431.79 per 100,000 person-year. Sex, age, smoking, drink, blood pressure, fasting blood sugar and total cholesterol were included in the final model. A Harrell?s c-index was 0.732l as a measure of discrimination and the overall O/E ratio was 0.972 (95% CI 0.944-1,000). We have predicted a model for 10-year risk of IHD related to risk factors using NHIS-NSC. Using these results, if you have your health information, you will be able to predict your risk of IHD.

        Postoperative Prognostic Predictors of Bile Duct Cancers: Clinical Analysis and Immunoassays of Tissue Microarrays

        Background/Aims: Cholangiocarcinoma frequently recurs even after curative resection. Expression levels of proteins such as epidermal growth factor receptor (EGFR), Snail, epithelial cadherin (E-cadherin), and interleukin-6 (IL-6) examined by immunohistochemistry have been studied as potential prognostic factors for cholangiocarcinoma. The aim of this study was to investigate significant factors affecting the prognosis of resectable cholangiocarcinoma. Methods: Ninety-one patients who underwent surgical resection at Samsung Medical Center for cholangiocarcinoma from 1995 to 2013 were included in this study. Expression levels of E-cadherin, Snail, IL-6, membranous EGFR, and cytoplasmic EGFR were analyzed by immunohistochemistry using tissue microarray blocks made from surgical specimens. Results: Patients with high levels of membranous EGFR in tissue microarrays had significantly shorter overall survival (OS) and disease-free survival (DFS): high membranous EGFR (score 0–2) 38.0 months versus low membranous EGFR (score 3) 14.4 months (p=0.008) and high membranous EGFR (score 0–2) 23.2 months versus low membranous EGFR (score 3) 6.1 months (p=0.004), respectively. On the other hand, E-cadherin, Snail, cytoplasmic EGFR, and IL-6 did not show significant association with OS or DFS. Patients with distant metastasis had significantly higher IL-6 levels than those with locoregional recurrence (p=0.01). Conclusions: This study showed that overexpression of membranous EGFR was significantly associated with shorter OS and DFS in surgically resected bile duct cancer patients. In addition, higher IL-6 expression was a predictive marker for recurrence in cholangiocarcinoma patients with distant organ metastasis after surgical resection.

        <P><B>Abstract</B></P> <P>It is thought that vitamin C has protective roles on stress-induced heart damage and the development of cardiovascular diseases, but its precise role and mechanisms are unclear. In the present study, we investigated the specific mechanisms by which vitamin C leads to protecting the heart from stress-induced damage in the <I>Gulo(</I>−/−<I>)</I> mice which cannot synthesize vitamin C like humans. By exposure to stress (1h/day), the heartbeat and cardiac output in vitamin C-insufficient <I>Gulo(</I>−/−<I>)</I> mice were definitely decreased, despite a significant increase of adrenaline (ADR) and noradrenaline (NA) production. A change of cardiac structure caused by the death of cardiomyocytes and an increased expression of matrix metalloprotease (MMP)-2 and -9 were also found. Moreover, lipid peroxidation and the production of tumor necrosis factor-alpha (TNF-α) in the heart were increased. Finally, all vitamin C-insufficient <I>Gulo(</I>−/−<I>)</I> mice were expired within 2 weeks. Interestingly, all of the findings in vitamin C-insufficient <I>Gulo(</I>−/−<I>)</I> mice were completely prevented by the supplementation of a sufficient amount of vitamin C. Taken together, vitamin C insufficiency increases the risk of stress-induced cardiac damage with structural and functional changes arising from the apoptosis of cardiomyocytes.</P> <P><B>Highlights</B></P> <P> <UL> <LI> Vitamin C insufficiency accelerates the stress-induced death of <I>Gulo(</I>−/−<I>)</I> mice. </LI> <LI> Oxidative stress in the heart is increased by stress and vitamin C insufficiency. </LI> <LI> TNF-α, ADR, and NA are excessively produced by stress and vitamin C insufficiency. </LI> <LI> Structural/functional cardiac changes are induced by stress and vitamin C insufficiency. </LI> <LI> Vitamin C supplementation prevents cardiac changes and death of <I>Gulo(</I>−/−<I>)</I> mice. </LI> </UL> </P>

        논문 : 가리봉동의 스크린 재현 경관 속 타자화된 장소성

        본고는 영화 속 재현 경관에 특정 시대의 사회문화적 특성이 투영되어있다는 점에 주목하여 극영화인 <장미빛 인생>, <가리베가스>, 그리고 다큐멘터리 <가리봉>의 세 작품에 재현된, 사회적 방외인으로 인식되어 타자화된 노동자층과 조선족 이주자들의 공간으로서 가리봉동의 장소성을 살펴보았다. 영화 텍스트의 독해를 위한 틀로서 크로노토프의 의미를 차용하였는데, 영화 텍스트 속 가리봉동의 이미지에는 노동자, 조선족 등 소외 집단의 사회적 배제와 타자화가 투영되어 있음을 확인하였다. <장미빛 인생>에서 1987년 봄, 민주화 항쟁기 동안의 벌집촌 건물 지하의 만화방, <가리베가스>에서 뉴밀레니엄의 시작과 신산업단지로의 변화를 꾀하던 시기 가리봉동 벌집 옥상에서 바라본, 건설 중인 아파트형 공장 건물군, <가리봉>에서 2010년대 경계인의 도시로 변한 옌벤거리와 가리봉동 내 직업소개소, 중국동포교회, 여인숙, 노래방 등은 소외와 배제의 주요 장소 기표라고 할 수 있다. 이와 같은 장소의 기표들은 물리적인 외피로만 존재하는 것이 아니라 개개인의 삶의 경험과 공간적 실천이 누적된 다층적 의미체로서의 역할을 한다. 영화를 포함한 대중매체를 통해 전달된 가리봉동에 대한 장소 이미지가 피상적일 수 있기 때문에 일반 대중에게 선입관념을 형성시킬 수 있다는 문제점과 장소 재현 상의 엄연한 한계가 존재한다는 것을 인정할 필요가 있으나 스크린의 영상으로 재현된 장소는 시공간의 유기적 연결성과 맥락을 통해 장소의 변화를 동시적으로, 그리고 맥락적으로 파악할 수 있는 기회를 제공한다. This paper explores Garibondong`s place identity as a residential area for manual workers and Chinese Koreans (Joseonjok) who have been marginalized by society as reflected in the narrative films of Rosy Life and GariVegas and the documentary of Garibong. The term “chronotope” is adopted as a framework to analyze how configurations of time and space are represented in cinematic texts. While the film Rosy Life displays a comic book store in the basement of a building during the spring of 1987, the period of democratic uprisings, the film GariVegas exhibits a structural change of the Guro district from the industrial complex to the digital one with the start of the new millennium. In the film Garibong, a commercial district of Yanbian Street and various establishments including an employment agency, a church and a karaoke representing the community of Joseonjok diaspora demonstrate the city of outsiders in the early 2010`s. The signifiers of places of Garibong-dong are not just physical entities but they transmit the multilayered meanings of individuals` life experiences and spatial practices. Although it is necessary to recognize that fragmentary images and information derived from mass media including films may trigger negative perceptions of specific places, pictures projected on a screen can provide opportunities to fathom the meanings of the changes of places concurrent with actual events and contexts of the reality.

      • Structural and magnetic properties of TiZrNi thin films prepared by magnetron sputtering and thermal annealing.

        <P>Distinctive thin layers of TiZr and Ni were deposited by using a magnetron sputtering method and a thermal annealing was applied to discover metallic films of quasicrystals. After a heat treatment in vacuum, 70 nm thick deposited layers were well mixed with nominal compositions of 49.7, 29.3 and 21.0 for Ti, Zr and Ni, respectively, which is very close with the one forming a quasicrystalline phase. The magnetization values were significantly decreased from 0.286 to 0.142 emu/mm3 at 2000 Oe, after annealing, while a shape of magnetic hysteresis was maintained. It is believed that a different magnetic behavior after thermal annealing is due to the homogeneous mixing of atomic elements and possible existence of a metastable phase.</P>

        본 논문에서는 웹 및 문자 공지문을 자동으로 분류하고 추천함으로써 사용자가 원하 는 공지문만을 볼 수 있도록 하는 애플리케이션을 개발한다. 본 애플리케이션은 공지문을 여러 카테고리로 자동 분류하여 사용자가 원하는 카테고리에 속한 공지문만을 볼 수 있도록 하며,사용자가 선호할 만한 공지문을 추천하는 기능을 제공한다. 공지문 분류를 위해 다층 신경망 모델과 Naive Bayes 분류기를 사용하였으며,공지문 추천을 위해 키워드 기 반 자체 알고리즘을 사용하였다. 그 밖에 Word2Vec 을 활용한 검색어 추천 등 부가 기능을 제공하여 사용자가 쉽게 공지문을 찾을 수 있도록 하였다. 본 애플리케이션을 통해 사 용자는 수많은 공지문 중 관심 있는 공지문만을 효율적으로 확인할 수 있다.
