        Vibration-controlled transient elastography for significant fibrosis in treatment- naïve chronic hepatitis B patients: A systematic review and meta-analysis

        Backgrounds/Aims: Accurate diagnosis of significant liver fibrosis in patients with chronic hepatitis B (CHB) is crucial when determining whether to initiate antiviral treatment (AVT). We conduct a meta-analysis to assess the diagnostic performance of vibration-controlled transient elastography (VCTE) for significant liver fibrosis in AVT-naïve CHB patients with serum alanine transaminase (ALT) levels within 5-fold the upper limit of normal (ULN). Methods: The Ovid-Medline, EMBASE, Cochrane, and KoreaMed databases were searched to identify studies that compared the performance of VCTE and liver biopsy (reference standard) when diagnosing significant liver fibrosis (≥F2) in AVT-naïve CHB patients with ALT within 5-fold the ULN. A hierarchical summary receiver operating characteristic curve (HSROC) and bivariate model were performed to evaluate the diagnostic performance of VCTE in the meta-analysis. Results: Eight studies (2,003 patients) were included. The summary sensitivity and specificity for diagnosis of significant liver fibrosis were 0.78 (95% confidence interval [CI], 0.66–0.86) and 0.72 (95% CI, 0.60–0.82), respectively. The HSROC for the diagnosis of significant liver fibrosis was 0.81 (95% CI, 0.72–0.86). The optimal cutoff value of VCTE for diagnosis of significant liver fibrosis was 7.7 kPa with a sensitivity of 0.64 (95% CI, 0.50–0.76) and specificity of 0.83 (95% CI, 0.72–0.90). Conclusions: Our study demonstrated that VCTE has an acceptable diagnostic performance for significant liver fibrosis in AVT-naïve CHB patients with ALT within 5-fold the ULN.

        악골 골수염의 조기 진단

        Early diagnosis and treatment of osteomyelitis provide good prognosis and prevent severe complications. Therefore, it is important to early diagnose and treat before the bony changes are observed in conventional radiograms. Authors experienced three cases of early osteomyelitis, and scintigrams were useful to differentiate them from other diseases. The purpose of this report was to aid in the early diagnosis and treatment of osteomyelitis occurred in the jaws. The characteristic features were as follows: 1. In clinical examination, the patients complained mild pain and localized swelling in the jaws. 2. In radiographic findings, the conventional radiograms showed relatively mild bony changes in the jaws. 3. The more severe periosteal reactions were observed in radiograms in children than in adult patient. 4. It showed marked increased uptake of radioisotopes in all scintigrams. 5. The three phase bone scannings were helpful to differentiate osteomyelitis from soft tissue diseases.

      • 梓木이 腦血流 및 血壓에 미치는 影響

        The purpose of this study is to measure the changes of regional cerebral blood flow (rCBF) and blood pressure (BP) in rats, following the intravenous injection of Catalpa Ovata Water Extract (COWE). The measurement was continually monitored by laser-doppler flowmeter (Transonic Instrument, USA) and pressure tranducer(Grass, USA) in anesthetized adult Sprague-Dawley rats through the data acquisition system composed of MacLab and Macintosh computer. The result of this experiment was as following : COWE increased the changes of rCBF and BP significantly. After pretreatment with propranolol and indomethacin, the changes of rCBF were inhibited significantly, but not by L-NNA and methylene blue. Also, after pretreatment with propranolol, the changes of BP were inhibited significantly, but not by L-NNA, methylene blue and indomethacin. According to above result, it is considered that Catalpa Ovata increased the rCBF and BP, related to adrenergic β-receptor.

        대동맥 판막 치환술 후 반복되는 Acinetobacter baumannii 균혈증의 원인이 된 대동맥염 1예

        감염성 대동맥염은 흔하지 않지만 매우 위중한 질병으로 Salmonella so.와 S. aureus가 주된 원인균이다. 아직까지 병원내 감염의 주요 원인균인 Acinetobacter baumannii에 의한 감염성 대동맥염은 보고된 바가 없었다. 저자들은 대동맥 판막 치환술을 시행받은 76세 남자 환자에서 적절한 항생제 투여에도 반복되는 A. baumannii 균혈증의 원인이 감염성 대동맥염으로 진단되었던 1예를 경험하였기에 보고하는 바이다. Infectious aortitis is an uncommon yet, life threatening disease. Early surgical treatment and prolonged antibiotic therapy is crucial to survival. Salmonella sp. and Staphylococcus aureus are the most common organisms isolated. There is no case report of infectious aortitis caused by Acinetobacter baumannii, which has recently emerged as a major cause of health care-associated infections. Here, we describe a 76-year-old male with infectious aortitis who experienced recurrent bacteremia due to A. baumannii in spite of adequate antimicrobial therapy after aortic valve replacement.

        Central pontine myelinolysis (CPM) is a neurological disease characterized by destruction of myelin sheaths of brain cells in the pons, usually caused by rapid correction of chronic hyponatremia. Although previous reports on CPM described very poor outcome, recent some reported that relowering of serum sodium within 24hr after symptom onset showed a favor recovery from CPM. However, it is unusual spontaneous full recovery from CPM without relowing of Na. We report a case of 59-year-old woman diagnosed with CPM, and recovered spontaneously. A 59-year-old woman presented to our hospital with scalp laceration. On admission she was alcohol ordor state, but she was alert. She complained nausea and vomiting which had evolved 2 days. Her serum biochemistry showed sodium of 111.1 mEq/L. She was treated with 3% saline. And six hours later, when serum sodium was 119.9 mEq/L, 3% saline was stopped and we started 0.9% saline. At that night, she showed severe emotional lability and disorientation. She didn’t sleep all night. On 2nd admission day, serum sodium was 130.4 mEq/L. We consult to psychiatry about anxiety. She was diagnosed with delirium tremens and started on chlordiazepoxide. On 3rd admission day, serum sodium reached 137.8 mEq/L. On 5th admission day, patient became drowsy. Neurological examination revealed only general weakness. Focal neurologic deficits were not observed. So we suspected drug side effect due to chlordiazepoxide. We decided to reduce chlordiazepoxide. On 9th admission day, her drowsy mental state wasn’t improved. We stopped chlordiazepoxide. Brain MRI was performed, and MRI showed high signal intensity in the central pons and bilateral basal ganglia and thalamus on T2-weighted image and diffusion weighted imaging. She was diagnosed with CPM and EPM. We continued supportive care since then, but she couldn’t speak and couldn’t move her extremities herself for a long time. While continuing supportive care, her drowsy mental state gradually improved. On 22th admission day, she became alert and could handshake. On 26th admission day, she was discharged to convalescent hospital. Three weeks after discharge, when she came to out-patient clinic, she showed full recovered neurologic problem.

        교과과정의 조직은 체계적이고 뜻이 있는 순서로 학생들을 가르치기 위해 지식을 어떻게 조직화할 것인지와, 과목들 사이에서 공통된 내용은 통합시켜 중첩되는 일이 없도록 조정할 필요가 있다. 이에 현재 본 대학 간호과에서 운영되었던 97학년도의 17개 전공과목들의 교과과정의 내용을 비교, 분석하여 중복을 줄이며 순차적 지식의 습득을 도모하는 새로운 통합교과과정의 틀을 개발, 제시함으로써 보다 나은 전문직 간호교육이 이루어지도록 하고자 하였다. Most curricula are considered how to teach the student in systematic and meaningful orders. And it is necessary to coordinate the duplicate contents in major subjects in nursing. So this study was designed to set up a new comprehensive curriculum through comparing and ananlyzing each seventeen major subjects in Nursing Department of Kyung-Bok College. Therefore, the purpose of this study is to present the new comprehensive curricula for promoting the efficiency and effectiveness in diploma degree course of nursing education.

      • Clearly Canadaian: How Canada’s Unique Diversity Education Structure can Represent an Ideal Learning Environment for South Korean Students

        Globalization has made the national borders less significant than in the past. Continuous efforts to enhance diversity education are necessary to prepare for the future. Korea, which has experienced rapid and compressed industrialization, attracts tourists from all over the world and many migrant workers from developing countries. However, it seems that Koreans are not yet prepared to welcome these people due to the low levels of English proficiencyprejudice towards foreigners. Canada is known to have a well-established diversity education system. Korean students who are in need of English education can greatly benefit from Canada’s learning environment. This paper explores issues in Korean and Canadian education in order to explore whether Canada can offer Korean students a suitable venue for study abroad opportunities. Considering Canada’s many advantages, including its multicultural atmosphere, low cost of education, and multilingual educational history, this paper found that Korean students Clearly Canadian: How Canada’s Unique Diversity Education Structure can Represent an Ideal Learning Environment for South Korean Students 81 would greatly benefit from choosing to study abroad in Canada. Canada could also take advantage from accepting Korean students to study overseas by further diversifying their classrooms with Korean students, and boosting its economy through an increase in Korean investment into Canadian education. Therefore, Canada’s efforts to enhance exchange programs are needed. These can be accomplished by lowering costs, improving its transitional programs, and more advertising on Korean websites.

        Analysis of porcine macrophage immune response to antigenic molecules and short chain fatty acids

        Macrophages play an important role in both the innate and adaptive immune responses. These include phagocytosis, killing of microorganisms, antigen presentation, and induction of immune cytokines and antimicrobial genes. Macrophage activity is reported to be controlled by diverse exogenous antigenic or endogenous metabolic molecules, and the underlying mechanisms are well documented in human and mouse macrophage cells. Bacterial lipopolysaccharide (LPS) is known to be one of the most potent stimuli activating macrophages through the toll like receptor 4 (TLR4) signaling pathway. There are other antigenic molecules, such as muramyl dipeptide (MDP) and outer membrane protein A (OmpA), that are also known to activate immune cells. On the other hand, short chain fatty acids (SCFAs) such as acetate and butyrate are produced by gut microbiota and control host energy metabolism and signal transduction through GPR receptors. However, there are few studies demonstrating the effects of these molecules in macrophages from domestic animals, including domestic pigs. In this study, we attempted to characterize gene expression regulation in porcine macrophages (PoM2, Pig Monocytes clone 2) following treatment with LPS, MDP, OmpA, and two short chain fatty acids using porcine genome microarray and RT-PCR techniques. A number of novel porcine genes, including anti-microbial peptides and others, appeared to be regulated at the transcriptional level. Our study reports novel biomarkers such as SLC37A2, TMEN184C, and LEAP2 that are involved in the porcine immune response to bacterial antigen LPS and two short chain fatty acids.

        Pediatric Extracorporeal Membrane Oxygenation in Korea: A Multicenter Retrospective Study on Utilization and Outcomes Spanning Over a Decade

        Background: Over the last decade, extracorporeal membrane oxygenation (ECMO) use in critically ill children has increased and is associated with favorable outcomes. Our study aims to evaluate the current status of pediatric ECMO in Korea, with a specific focus on its volume and changes in survival rates based on diagnostic indications. Methods: This multicenter study retrospectively analyzed the indications and outcomes of pediatric ECMO over 10 years in patients at 14 hospitals in Korea from January 2012 to December 2021. Four diagnostic categories (neonatal respiratory, pediatric respiratory, postcardiotomy, and cardiac-medical) and trends were compared between periods 1 (2012–2016) and 2 (2017–2021). Results: Overall, 1065 ECMO runs were performed on 1032 patients, with the annual number of cases remaining unchanged over the 10 years. ECMO was most frequently used for post-cardiotomy (42.4%), cardiac-medical (31.8%), pediatric respiratory (17.5%), and neonatal respiratory (8.2%) cases. A 3.7% increase and 6.1% decrease in pediatric respiratory and post-cardiotomy cases, respectively, were noted between periods 1 and 2. Among the four groups, the cardiac-medical group had the highest survival rate (51.2%), followed by the pediatric respiratory (46.4%), post-cardiotomy (36.5%), and neonatal respiratory (29.4%) groups. A consistent improvement was noted in patient survival over the 10 years, with a significant increase between the two periods from 38.2% to 47.1% (P = 0.004). Improvement in survival was evident in post-cardiotomy cases (30–45%, P = 0.002). Significant associations with mortality were observed in neonates, patients requiring dialysis, and those treated with extracorporeal cardiopulmonary resuscitation (P < 0.001). In pediatric respiratory ECMO, immunocompromised patients also showed a significant correlation with mortality (P < 0.001). Conclusion: Pediatric ECMO demonstrated a steady increase in overall survival in Korea; however, further efforts are needed since the outcomes remain suboptimal compared with global outcomes.
