Removal of Esophageal Variceal Bands to Salvage Complete Esophageal Obstruction

Esophageal varices develop in almost half of the patients with cirrhosis, and variceal hemorrhage constitutes an ominous sign with anincreased risk of mortality. Variceal banding is considered an effective and mostly safe measure for primary and secondary prophylaxis. Although adverse events related to banding including dysphagia, stricture formation, bleeding, and ligation-induced ulcers have beendescribed, complete esophageal obstruction is rare, with only 10 reported cases in the literature. Among those cases, 6 were managedconservatively; 1 patient had esophageal intraluminal dissection from an attempt to remove the bands using biopsy forceps butultimately recovered with conservative management. Three patients developed strictures following removal of the bands, requiringrepeated sessions of dilation therapy. We report on a patient who developed absolute dysphagia and complete esophageal obstructionafter variceal banding. We successfully used the endoloop cutter hook to release the bands intact and restore luminal integrity.
Impedance/Modulus Analysis of Sol-gel Bax Sr1−x TiO3 Thin Films

Perovskite-structure-type sol-gel barium-strontium-titanate (BST) thin films with molar formula Bax Sr1−x TiO3 (x = 0.5, 0.6, 0.7, and 0.8) have been successfully fabricated as Al/BST/Pt capacitors. In order to study the role of the grain size in the conduction mechanisms, we present atomic force microscopy (AFM) results. The impedance and the electric modulus of Bax Sr1−x TiO3 thin films have been analyzed in order to correlate the effect of the film regional processes on the conduction mechanism. The impedance and the electric modulus complex planes show three overlapping regions as response for the bulk, the grain boundary and the film/electrode interface mechanisms, and an equivalent circuit has been proposed for each mechanism. The conduction mechanism resulting from the bulk and the grain boundaries is discussed; the results show that the grain boundaries have less effect on the conduction mechanism than the bulk.

Objective To examine the efficacy of MSH6 and PMS2 immunohistochemistry (IHC) as a screening method for Lynch syndrome in endometrial cancer patients. Methods Through multidisciplinary discussions, an institutional MSH6 and PMS2 IHC-initiated cascade test (MSH6, PMS2 IHC→microsatellite instability [MSI] assay→germline mismatch repair [MMR] gene sequencing) was developed to screen for Lynch syndrome in endometrial cancer patients. Testing was performed on a consecutive cohort of 218 newly diagnosed endometrial cancer patients who underwent surgery at a tertiary hospital in the Republic of Korea between August 2018 and December 2020. The number of MMR deficiencies (MSH6 or PMS2 loss in IHC) and results of subsequent tests (MSI assay and germline MMR gene sequencing) were examined. Results MMR deficiency was detected in 52 of the 218 patients (24.0%). Among these 52 patients, 34 (65.0%) underwent MSI testing, of which 31 (91.0%) exhibited high MSI. Of the 31 patients with MSI-high status, 15 (48.0%) underwent germline MMR gene sequencing. Subsequently, Lynch syndrome was diagnosed in five patients (33.0%). Conclusion Lynch syndrome screening using MSH6 and PMS2 IHC-initiated cascade testing is a viable strategy in the management of endometrial cancer. A simplified strategy (MSH6 and PMS2 IHC→germline MMR gene sequencing) was proposed because most women with MMR deficiencies exhibited high MSI.

There is paucity in the literature on the use of endoscopic ultrasound (EUS) for evaluating the thyroid gland. We report the first case of primary papillary thyroid cancer diagnosed by using EUS and fine needle aspiration (FNA). A 66-year-old man underwent EUS for the evaluation of mediastinal lymphadenopathy. FNA of the lymph nodes showed benign findings. A hypoechoic mass was noted in the right lobe of the thyroid gland. Therefore, FNA was performed. The cytological results were consistent with primary papillary thyroid cancer.

This paper presents a three dimensional (3D) grasp synthesis algorithm to achieve distinguished grasps supporting both stability and human-like grasping. The algorithm, which is based on the concepts of a graspable boundary and convex segments, was developed to enable a two-fingered gripper to grasp any unknown object, regardless of its shape, texture, or concavity, given a single 3D image data from depth sensors. The proposed algorithm provides ways to grasp any object using boundary, envelope, and functional grasps. The algorithm is based on identifying graspable segments, analyzing them geometrically, and incorporating the memory of grasping experience. Unlike most grasp synthesis research that focuses on complete 3D contours, our algorithm concentrates only on the graspable boundary and convex segments and thereby achieves stable grasps with less computational complexity. The experimental results show that the proposed algorithm provides distinguished and stable grasps for various objects in various environments, and is suitable for robots to grasp the objects successfully.
