We present the dynamic contrast-enhanced computed tomography (CT) findings of atypical pulmonary hamartoma with a rare histologic pattern in a 20-year-old male. CT showed a 3.4 cm lobulated mass with a ‘tip of the iceberg’ appearance in the medium bronchus of the right lower lobe. Dynamic contrast-enhanced CT demonstrated a heterogeneously and persistently enhancing mass. The CT Hounsfield unit (HU) measurements were: 17 HU pre-contrast, 32 HU at 1 minute, 44 HU at 2 minutes, 51 HU at 4 minutes, and 64 HU at 15 minutes. Pathologic examination after video-assisted thoracoscopic surgery disclosed a pulmonary hamartoma with a predominant fibroblastic component.
Adrenal Castleman’s Disease Mimicking Other Adrenal Neoplasms: A Case Report

We present a rare case of adrenal Castleman’s disease with hyaline vascular type mimicking other adrenal neoplasms in a 65-year-old woman. Although rare, the hyaline vascular type of adrenal Castleman’s disease should be included in the differential diagnosis if an adrenal mass shows a well-defined, highly enhancing solid adrenal mass with peripheral rim enhancement, multiple satellite lymph nodes, and peritoneal thickening around the dominant mass on computed tomography as shown in this patient.
Seasonal Variation of Rubber Production in Russian Dandelion, Taraxacum kok-saghyz, Grown in Korea

Russian dandelion, Taraxacum kok-saghyz L.E.Rodin, is an important alternative source of natural rubber. Thus, it is critical to identify the best season for natural rubber production and harvesting for this species. In this study, T. kok-saghyz was grown in experimental fields in Korea to check the variation in rubber content with the change in the season. Studies using T. kok-saghyz plants indicate that spring and autumn are the best seasons for rubber accumulation. For several reasons, our study suggests that planting in autumn and harvesting in late spring is the best strategy for economic cultivation in Korea. Our data will be useful for future commercialization of rubber production from T. kok-saghyz because time will be saved if the plant is grown and harvested in the optimum season.

Percutaneous balloon dilation with or without placement of an external biliary drain is a nonoperative alternative method for treating benign bilioenteric anastomotic strictures. Although this procedure has a high technical success rate, outcomes are less optimal when attempting to dilate refractory tight strictures. For the stricture, cutting balloon can be an option. We present four patients with benign bilioenteric anastomotic strictures refractory to conventional balloon dilation. To the patients, a peripheral cutting balloon over-the-wire system was inflated, following subsequent conventional non-compliant balloon dilation. After the balloon dilation treatment, an external drainage catheter was placed through the stricture site and maintained for up to 30 days. Technical and end-treatment success was achieved in all four patients. In conclusion, the use of cutting balloon dilation may appear to be a safe and effective alternative method of treatment in patients with benign bilioenteric anastomotic strictures refractory to conventional balloon dilation.

Purpose: Our study aimed to evaluate the predictive value of multiparametric magnetic resonance imaging (mpMRI) to assess the capsular extension of prostate cancer. Risk factors for extraprostatic invasion or microscopic invasion of bladder neck (pT3a stage) in the cases showing localized disease findings on MRI scan were also identified. Materials and Methods: We identified 198 patients who underwent robot-assisted or conventional laparoscopic radical prostatectomy in our institute between June, 2016 and May, 2018. Altogether, 170 patients with preoperational mpMRI scans and complete pathologic data were included. Positive and negative predictive values of mpMRI to detect capsular extension were estimated. Chi-square test was performed for ratio variables. Logistic regression analysis was performed to identify capsular invasion risk factors. Results: Median age and prostate-specific antigen level were 68 years and 7.5 ng/mL, respectively. Based on findings, 16 patients (9.4%) were identified as cT3a stage (unilateral or bilateral extraprostatic extension), whereas 37 patients (21.8%) as pT3a stage. Positive and negative predictive values of mpMRI to detect capsular extension were 75.0% and 73.9%, respectively. In the logistic regression analysis, positive biopsy core rate was the significant predictor for pT3a disease in the patients with negative capsular invasion findings on mpMRI (p<0.001). According to receiver-operating characteristic curve (area under the curve=0.691, p=0.001), the positive biopsy core rate of 0.275 was the best threshold. Conclusions: Multiparametric MRI is an appropriate test to predict pT3a disease preoperatively. The patients with positive core rate over 0.275 may have pT3a diseases despite negative image findings.
