        한약과 양약 병용 투여시의 부작용

        근래 동서양의학의 협진에 의해서 치료 효과를 높이고 환자의 삶의 질을 높이려는 시도가 증가하고 있으나, 양약과 한방 약물을 동일 환자에 병용 투여하였을 때 발생할 수 있는 약물상호작용에 대해서 아직 연구도 미진할 뿐 아니라, 각각의의학적 이론상, 판이한 작용용량 곡선의 이론적 바탕을 가지기 때문에, 병용 투여 시, 투여 안전역과혈중 약물농도의 모니터링 및 병용 투여로 발생하는 부작용 등을 예상할 수 없기 때문에 아직까지는 실제적인 병용 요법이 시행되기 어려운 실정이다. 차후로 병원에서 동서협진의 시행폭이 넓어지게 되고, 병용투여요법이 증가하게 된다면, 약물 상호작용에 대한 새로운 가이드 라인이 마련되어야 할 것이고, 병원 약사의 전문적 임상 업무의 한 분야로 그 방법적 기초를 마련하는 것이 필요하다고 할 수 있다. 병용요법의 요점은 일단 한약에 무엇을 기대할 것인가를 명확히 하여야 한다. 한약에 비하여 양약의 효과가 확실하고 안전한 경우에는 양약을 먼저 투여하며, 이렇게 하더라도 증상의 개선이 충분히 이루러지지 않으면 한약을 추가하는 것을 기본으로 한다. 결국 병용요법이 목표로 하는 것은 동서양의 의학의 장점과 결점을 충분히 인식한 다음에 서로 보완하고 새로운 임상의학을 목표로 하는 것이다. 본 발표에서는 병용요법의 장점 및 기본 사항, 그리고 병용 시 발생할 수 있는 약물상호 작용 중 부작용에 대해서 고찰함으로써, 병용요법의 medication oeeor를 방지하는데 도움이 되고자 하였다.

      • 순무의 d-galactosamine 유발 간장해 보호효과

        Brassica rapa L. (Turnip) which is one of the specialized crops in Ganghwa island, has been used for diuretic, digestive, and curative for jaundice, etc. In this study, the antioxidative effects and hepatoprotective effects of turnip in vitro and in vivo were investigated in order to evaluate the possibility as hepatoprotective agents. Ethanol extract of turnip potently showed the scavenging effect on DPPH and inhibitory effect on lipid peroxidation. Oral administration of turnip extract to d-galactosamine-induced experimental liver injured rats was significantly reduced the serum AST, ALT and LDH enzyme activities. And the decrease of catalase and SOD activities in liver microsolmal cytosol was significantly improved by the treatment of turnip. Based on these findings, it is presumed that ethanol extract of turnip may have the hepatoprotective effect on d-galactosamine-induced hepatotoxicity rat.

        Variations in gait features in elderly adults during walking considering their balance

        The aim of this study was to evaluate the influenceof balance on the spatiotemporal features, lower-limbkinematics, and center of mass (COM) of the non-fallerelderly during walking. In this study, 20 healthy elderlywomen (age, 76.2 ± 5.6 years; height, 150.1 ± 3.2 cm;weight, 55.8 ± 9.0 kg) were enrolled. Based on the Bergbalance scale (BBS), the elderly were classified into twogroups: poor balance (PB; BBS scores \46; n = 10;43.8 ± 1.8) and good balance (GB; BBS scores C46;n = 10; 50.4 ± 2.5). The two groups had no differences interms of the spatiotemporal features and range of motion(ROM) of the vertical COM (all p[0.05). The ROM ofthe mediolateral COM was greater in PB than in GB. Hiptransversal movements in the two groups were different. The impairment of the lateral balance function mightcontribute to an increase in the incidence of fall events inthe elderly with poor balance.

      • 발효강화쑥의 간장해 보호효과

        Artemisia princeps PAMPANINI has been used in traditional medicine of the treatment of inflammatory, liver dysfunction and order disorder in the far east countries including Korea. The present study was carried out to investigate the hepatoprotective etfects of ethanol extract of Artemisia princeps (AP) and its fermented agents (AP-F) by lactic acid bacteria derived from human intestinal bacteria on liver injured rat induced by CCl_(4) and d-galactosamine. Hepatoprotective activity was monitored by estimating serum ALT, AST, ALP, LDH, superoxide dismutase (SOD), glutathione redeuctase (GR) and glutathione peroxidase (Gpx) activities in the liver injured by hepatotoxin. Pretreating rats with AP or AP-F at the same dosage regimen significantly suppressed the acute elevation of serum transaminase, ALP, LDH and GR activities, and significantly increased the lowering of blood SOD and GR activites induced by hepatoxin. Based on these findings, it is presumed that AP and APF may have the hepatoprotective effect on CCI_(4) and d-galactosamine-induced hepatotoxicity rat.

      • 석이중 오르시놀 정량 및 급성독

        In previous study, Gyrophora esculenta showed significant inhibitory effect on α-glucosidases in vitro and blood glucose elevation in vivo In the isolating process of active substance, orcinol was separated from Gyrophora esculenta. Orcinol is known to be toxic, therefore, in this study, it was analyzed by the TLC densitometry method for quantitative determination from Gyrophora esculenta. The average amount of orcinol of Gyrophora esculenta was 0.2%. For the purpose of removing orcinol, the water extract of Gyrophora esculenta was sequentially fractionated by organic solvents, and the acute toxicity of each fraction was assessed in mice. Among them, the LD50 of butanol fraction was 1.19 g/kg(p.o.) and the weight increase of mice in that group was somewhat retarded.

      • 한방방제의 제제학적 연구(Ⅰ) : 추출방법에 따른 녹용사군자탕의 비교

        Decoction of oriental medicinal preparation is prepared in various mariners, and changes of chemical constituents might be occurred depending on the processing techniques. The present study was undertaken to investigate the physio-chemical and pharmacological equivalence between two extraction methods of Nokyong-Sagunja-Tang. Samples were prepared as follows ; Sample-I was prepared by simultaneously extracting Sagunja-Tang and velvet antler in one vessel. Sample-II was prepared by adding velvet antler extract to the water extract of Sagunja-Tang. Both samples showed similar results of physparameters such as pH, yield, TLC and HPLC chromatogram, and contents of ginsenoside Rb1 and glycyrrhizin. Also, there were little different between two samples in pharmacological effects such as DPPH free radical scavenging effect, and inhibitory effects on xanthine oxidase, hyaluronidase, trypsin, TBA-Rs formation and hemolysis in vitro. And both samples showed no significant difference in antifatigue activities in mice. These results suggest that there might be little difference between two extraction process when velvet antler added to Sagunja-Tang.

      • 석이에서 분리한 GE974의 α-Glucosidase 저해효과

        This study was conducted to search for the α-glucosidase inhibitor from the natural products. In the previous study, the water extract of Gyrophora esculenta exhibited a potent inhibitory effect on α-glucosidase activities. Then, by bioassay-guided fractionation followed by chromatographic seperation of the water extract of Gyrophora esculenta, α-glucosidase inhibitor was isolated as GE974. GE974 showed significant inhibitory activities on some kinds of α-glucosidases in vitro. Its inhibitory mechanism seemed to be competitive for disaccharides. Also, it markedly inhibited α-glucosidases of intestine separated from both nondiabetics and diabetics.
