상호운용성 수준에 독립적인 한국형 상호운용성 관리 모델 연구

One of the interoperability management activity in Korean military is interoperability level assessment. The concept of interoperability levels causes the assessed result to be uncertain and unreliable, despite the activity contributes to secure interoperability requirements of systems consistently. In this paper, I suggest Korea Interoperability Management(KIM) model which is independent of interoperability levels especially specified in LISI model, in order to support more practical and reliable interoperability management.

사회서비스형 노인일자리 지속가능성 제고를 위한 정책 과제

사회서비스 품질 측정모형 개발 : 공익형 노인일자리를 중심으로

본 연구는 보편적 사회서비스로서 공익형 노인일자리 서비스 품질 측정모형을 개발하기 위하여 수행되었다. 조사대상자는 공익형 노인일자리 사업 참여자 632명이며 우편설문조사를 통해 자료를 수집하였다. 주요 연구 결과를 살펴보면, 우선 공익형 노인일자리 서비스 품질은 결과품질, 상호작용품질, 환경품질, 사회품질의 총 네 개의 차원으로 도출되었다. 결과품질은 욕구충족성 요인과 호감성 요인으로 나타났고, 상호작용품질은 신속성 요인, 전문성 요인, 신뢰성 요인, 환경품질은 편리성 요인, 쾌적성 요인, 사회품질은 보편성 요인, 권익보호 요인, 가치인정 요인으로 나타났다. 탐색적 요인분석과 확인적 요인분석을 통해 구축한 사회서비스 품질 측정모형은 네 개의 품질차원과 열 개의 구성요인, 서른여섯 개의 측정문항으로 확정되었다. 본 연구는 공공부문 서비스로서 사회서비스의 공익적 특성을 규명하고, 실제 활용 가능한 서비스 품질 측정모형을 개발하였다는 것에 의의가 있다.

Tubed-Groin Flap for Coverage of Deep Burn Injuries in Two Adjacent Fingers: A Case Report

The reconstruction of two adjacent fingers is challenging for reconstructive surgeons, especially if there is a circumferential skin and soft-tissue loss along with bone exposure. Herein, we present the case of a 22-year-old male with a circumferential defect in two adjacent fingers due to an electrical burn. The patient had a circumferential soft-tissue defect with middle phalangeal bone exposure in the left index and middle fingers, revealed after serial debridement and ostectomy. After failure of free flap reconstruction, other options were considered, and the defects were repaired using a newly designed tubed-groin flap. Afterwards, flap detachment was performed on postoperative day 21 and provisional syndactyly separation without a skin graft was performed 2 weeks after flap detachment surgery. The patient was satisfied with the well-reconstructed fingers, which demonstrated adequate length and function. A full range of motion of the metacarpophalangeal joint was restored. The newly designed tubed-groin flap can be an efficient method to easily and effectively reconstruct a circumferential defect in two adjacent fingers.

Interdigitating Dendritic Cell Sarcoma in Upper Arm Misdiagnosed as Ruptured Epidermal Inclusion Cyst

Interdigitating dendritic cell sarcoma (IDCS) is an extremely rare neoplasm that usually arises in lymph nodes, but also can exist in extranodal sites. This report is about a case study of IDCS in the upper arm skin with axillary lymph node metastasis. A 66-year old woman had a slowly growing mass with tenderness sensation on her right upper arm that was being injected triamcinolone acetonide at local clinic. The presumptive diagnosis was a ruptured epidermal inclusion cyst, and empiric antibiotic therapy was done; however, she had poor respondence to antibiotic therapy. Therefore, magnetic resonance imaging, incisional biopsy, positron emission tomography-computed tomography, and bone scan were performed and a malignant tumor was diagnosed. She received surgical resection and lymph node dissection of the right axilla. No adjuvant chemotherapy was done. Although extremely rare, this case suggests that extranodal IDCS should be considered in differential diagnosis of untreated atypical skin mass and early biopsy should be performed.

타이어 공기압 감지 시스템 기술 동향

차량 운전자의 안전을 보장하고 운전의 편의를 돕기 위한 기술로는 차량용 레이더, 타이어 공기압 감지 장치, 원격시동 장치, 지리정보시스템 등이 있다. 그중 타이어는 차량의 수많은 부품들 중 도로와 직접 접촉하는 부품으로 타이어의 상태는 운전자의 안전에 직접적인 관련이 있다. 과거 포드 자동차에서는 파이어스톤에서 납품한 타이어 전량을 리콜하는 대규모 교체사례가 있었다. 초기 타이어 공기압 감지 장치는 일부 고급차종에만 적용되었는데 최근 압력 및 온도 측정용 센서의 높은 신뢰성과 양산비용의절감 및 국가의 정책적 요소가 복합적으로 작용하여 일반 차량에도 보급화가 진행중이다. 본 논문에서는 오늘날 타이어 공기압 감지 장치를 양산하는 업체들의 기술 동향을 분석하여 타이어 공기압 감지 시스템의 도입 배경, 국내외 기술기준 현황, 타이어 공기압 감지 시스템을 형성하는 기본 구조, 높은 정밀도를 가지는 반도체 센서 및 관련 기술에 대해 살펴본다.

Noninvasive Testing for Colorectal Cancer Screening: Where Are We Now?

Colorectal cancer (CRC) is one of the most prevalent cancers and is the leading cause of cancerrelated mortality worldwide. Based on the current screening guidelines by the American Cancer Society and Korean multi-society expert committee, CRC screening is recommended in asymptomatic adults starting at the age of 45 years. Fecal immunochemical test-based screening programs reduce the development of CRC and related mortality in the general population. However, this most popular CRC screening strategy demonstrates a crucial limitation due to modest diagnostic accuracy. Colonoscopy may be considered as an alternative primary method for CRC screening; however, its implementation can still be challenging due to concerns regarding invasiveness, low adherence, cost-effectiveness, and quality assurance. To overcome the limitations of current screening tests, innovative noninvasive tests for CRC screening have been developed with advances in molecular biology, genetics, epigenetics, and microbiomics for detecting CRC, which may enhance the approach to CRC screening and diagnosis in clinical practice in the near future. This review explores the emerging screening methods and discusses their potential for integration into current practice.

장기간 면역억제제를 사용하는 경우 감염 등의 합병증뿐만 아니 장기간의 면역억제로 인한 림프종의 발생위험도 고려해야 한다. 저자들은 류마티스관절염으로 MTX를 15년간복용한 병력이 있는 환자에서 간과 비장을 침범한 광범위큰 B세포림프종이 발생한 증례를 경험하였기에 문헌고찰과 함께 보고하는 바이다. Prolonged inflammation or immunosuppression may cause lymphoproliferative disorders (LPD). Rheumatoid arthritis (RA) itself causes LPD. The immunosuppressive drug methotrexate (MTX) is used widely to treat RA, and is also related to the development of lymphoma. Although the detailed pathogenesis and association are still controversial, care is needed when managing patients with RA treated with MTX. Many reports indicate that discontinuing MTX can induce the regression of LPD. Here, we describe a case of diffuse large B-cell lymphoma that presented with liver involvement in a 67-year-old woman who had used MTX for RA for 15 years. (Korean J Med 2015;88:715-718)

사회서비스의 사회품질 탐색을 위한 질적 연구

본 연구는 공공부문 서비스로서 사회서비스 이용자가 지각하고 있는 사회품질을 탐색하기 위한 목적으로 수행되었다. 문헌연구를 통해 개념적 틀을 구축한 뒤, 공익형 노인일자리 서비스 이용자와 서비스 제공자를 대상으로 표적집단 면접조사(Focus Group Interview: FGI)를 진행하였으며, 주요 연구결과는 다음과 같다. 공익형 노인일자리 서비스의 사회품질은 보편성, 권익보호, 가치인정의 세 가지 요인으로 나타났다. 보편성 요인은 공공부문 서비스로서 이용자가 국가에게 기대하는 공적 책임성이며, 참여를 원하는 사람 누구나 참여할 수 있는 권리에 대한 기대를 가리킨다. 권익보호 요인은 사회서비스로서 이용자가 서비스 제공주체에게 기대하는 공적 책임성이며, 서비스 이용자의 권익보호에 대한 기대를 의미한다. 가치인정 요인은 공익형 노인일자리 서비스 이용자가 서비스 제공주체에게 기대하는 공적 책임성으로 서비스 이용자들의 활동이 공공의 이익과 지역사회 발전에 기여하고 있다는 가치인정에 대한 기대를 나타낸다. 본 연구는 사회서비스의 사회품질 탐색 과정을 실증적으로 제시한 것에 함의가 있으며, 후속 연구의 기초 자료로 활용될 수 있을 것이다. The purpose of this study is to searching social quality perceived by social service users as public service and to provide high quality service that social service users can more satisfied with. Results of the study are as follows. Upon examining what ‘public responsibility’ is expected by the service provider to improve the quality of life of social service users in order to prescribe the social quality unique to public benefit-type senior job matching service, it was found that social quality had the three properties of universality, protection of rights, and value recognition. The universality is a common denominator of public sector services and jobs for senior citizens. The universality factor represents inclusiveness and social rights of the range of service beneficiaries. The protection of rights factor protects the rights and interests of social service users who are relatively more vulnerable than the beneficiaries in order to realize social integration and the value of solidarity. Lastly, the value recognition factor was analyzed to represent feeling a sense of benefiting rewards and receiving respect as the activities of the public benefit-type senior job matching service participants are volunteer activities to improve the public benefits of the community. This study has implications for the empirical presentation of the social quality search process of social services, and it can be used as the basic data of the follow-up study.
