        Experience of endotracheal intubation using video styletscope in a patient with mucopolysaccharidoses -A case report-

        Mucopolysaccharidoses (MPSs) are a group of inherited heterogenous metabolic disorders, caused by deficiency of an enzyme involved in the degradation of mucopolysaccharides, and thus deposition of mucopolysaccharides in all connective tissue. MPS type III is heparan sulphate storage disorder, especially marked central nervous system involvement. As with all the MPS disorders,this patient with MPS III presents coarse outlook features, such as macrocephaly, macroglossia, prominent mandible, short neck, and unstable atlantoaxial joints, as well as tracheal abnormality. These clinical findings give significant challenges to the anesthesiologist for adequate airway management and tracheal intubation. Recently,a number of new devices have been developed to facilitate intubation. We report an experience of facilitated tracheal intubation using video styletscope (OptiScope PM201Ⓡ, Pacific Medical, Seoul,Korea) in a 16 year old male patient with MPS III, who was undergoing inguinal hernia repair.

        가속도 센서를 이용한 사격 훈련 시스템 개발

        거총부터 격발까지 총구 움직임의 궤적을 추적하여, 사격의 정확성을 향상시킬 목적으로 광전자 기반 사격 훈련 시스템이 사격 훈련 현장에서 활용되어 왔다. 광전자 기반 시스템은 설치가 복잡하고, 표적의 파손 위험이 존재하며, 고가의 장비로 선수들의 접근성이 떨어진다는 단점이 있다. 이에 본 연구는 저비용의 가속도 센서 모듈을 이용하여 체위 떨림을 측정하고 피드백할 수 있는 사격 훈련 시스템을 개발하고, 이의 활용성을 검증하였다. 가속도 센서 모듈은 총기의 에어 실린더에 부착할 수 있도록 제작되었다. 체위 떨림은 가속도 센서 데이터를 이용하여 진폭, 주파수, 공간적 패턴 지표로서 분석되었다. 가속도 센서와 기존의 광전자 기반 시스템에서 측정된 사격시 체위 떨림 진폭 지표 간에는 높은 상관관계(좌우 방향: r=0.76; 상하 방향: r=0.70)가 나타났다. 또한, 사격 선수를 대상으로 진행한 시스템의 유효성 평가에서는 선수의 사격 점수(최상, 최하 격발)에 따라 계산된 체위 떨림 지표가 유의한 차이(p<0.05)가 있음을 독립 표본 t-검증을 이용하여 검증하였다. Optoelectronic shooting training systems are used in shooting training sites to improve the accuracy of shooting by tracking the trajectories of gun movements. However, optoelectronic-based systems have limitations in terms of cost, complexity of installation, and the risk that electronic targets may be broken. In this study, we developed and verified a shooting training system that measures postural tremors using a low-cost accelerometer. The acceleration sensor module was designed to be attached to the air cylinder of a gun. Postural tremors were evaluated based on amplitude, frequency, and spatial pattern index, which were computed using acceleration data. The postural tremor indices between the accelerometer and optoelectronic-based system were highly correlated (left-right and up-down directions: r = 0.76 and r = 0.70, respectively). We validated the developed shooting training system using an independent two-sample t-test, which identified a significant difference (p < 0.05) in the calculated postural tremor index according to the athlete's shooting score (i.e., best and worst shots).

        관성 센서기반 스쿼트 각도 측정 융합 시스템 개발

        스쿼트는 장소에 제약 없이 효과적으로 하체 근력을 증진시킬 수 있는 운동이다. 스쿼트 시 잘못된 자세로 인한 부상이 빈번하게 발생하고 있으며, 부상을 예방하기 위해서는 적절한 각도를 유지하며 운동하는 것이 중요하다. 본 연구 에서는 허벅지에 착용된 단일 관성 센서 모듈을 이용하여 스쿼트 각도를 측정할 수 있는 시스템을 개발하고 검증하였다. 스쿼트 각도를 측정하기 위해 각 관성 센서(가속도 센서, 자이로 센서)의 장단점을 상호 보완 하는 상보필터 알고리즘을 사용하였고, 임상 실험을 통해 각 상보필터 계수에 따른 스쿼트 각도의 정확도를 평가하였다. 분석 결과, 최적의 상보필 터 계수를 기반으로 계산된 스쿼트 각도는 측각기에서 측정한 각도와 상관 계수 0.623와 편향 오차 –5.6°로 유의한 상관관계를 보였다. The squat is an exercise that can effectively improve the muscle strength of the lower body, which can be performed in a variety of places without restrictions on places including homes. However, injuries due to incorrect motion or excessive angles are frequently occurring. In this study, we developed a single sensor-based squat angle measurement system that can inform the squat angle according to the correct motion during the squat exercise. The sensor module, including the acceleration sensor and the gyro sensor, is attached to the user's thigh. The squat angle was calculated using the complementary filter complementing the pros and cons of acceleration and gyro sensor. It was found that the calculated squat angle showed the proper correlation compared to the squat angle measured by a goniometer, and the influence of the coefficient of the complementary filter on the accuracy was evaluated.

        Gender effect on the preoperative cardiovascular responses in pediatric patients undergoing two-stage ear reconstruction surgery for microtia

        Background: The hemodynamic reactions to psychological stress were different between adult genders. Our aim was to investigate the hypothesis that preoperative cardiovascular responses in children undergoing ear reconstruction surgeries will be different according to gender. Methods: We obtained data from children, aged 7−16 years,undergoing reconstruction surgeries for microtia during 2005−2009. Number of boys receiving primarily rib cartilage graft (RCG) surgery and secondarily ear elevation procedure was 121 and 83, respectively,and girls 48 and 28, respectively. Preoperative systolic (S),diastolic blood pressure (DBP) and heart rate (HR) on each surgery were compared between genders, and within genders. Results: Boys were younger than girls by one year at first and second admission. Boys showed significantly lower preoperative DBP than girls (mean difference [MD]: −3.7 mmHg, 95%CI: −6.1to −1.3, P = 0.032) before RCG, whereas significantly higher SBP (MD: 21.5 mmHg, 95%CI: 16.6 to 26.4, P < 0.001), and DBP (MD:10.2 mmHg, 95%CI: 6.0 to 14.4, P < 0.001) before ear elevation. Comparison between the two surgeries within the same gender indicated that boys demonstrated significantly higher SBP (P <0.001) and DBP (P < 0.001) before ear elevation than before RCG;however SBP was comparable and DBP was significantly lower (P = 0.035) before ear elevation than before RCG in girls. Conclusions: Gender impact on the preoperative cardiovascular reactions was evident in children undergoing microtia surgery.

        The effect of midazolam dose and age on the paradoxical midazolam reaction in Korean pediatric patients

        Background: Although midazolam administration may occasionally induce a paradoxical episode, such as threatened crying and violent behavior in children, systematic studies on the causes of paradoxical reaction are limited. We investigated the effect of children’s age and a dose of midazolam on the paradoxical reaction. Methods: A total of one hundred sixty four children of 1-3 years and 3-5 years, were enrolled in this study. Each age group randomly received 0.05 mg/kg or 0.1 mg/kg of intravenous midazolam (41 patients/group). Results: The incidence of paradoxical midazolam reaction in the study groups, 1-3 years with 0.1 mg/kg of intravenous midazolam, 1-3 years with 0.05 mg/kg, 3-5 years with 0.1 mg/kg, and 3-5 years with 0.05 mg/kg were as follows: 29.3%, 12.2%, 7.3% and 2.4%, respectively. The incidence among the 4 groups was significantly different (P = 0.002), highest in the 1-3 years receiving 0.1 mg/kg of midazolam (29.3%). Both age (P = 0.004, OR [95%CI] = 5.3 [1.7-16.8]) and dose of midazolam (P = 0.036, OR [95%CI] = 3.0 [1.1-8.4]) were risk factors. Perioperative clinical data including anxiety scales of children were not associated with the paradoxical midazolam reaction. Conclusions: In conclusion, we suggest that children less than 3 years old receiving higher dose of intravenous midazolam are at risk for the paradoxical midazolam reaction.
