        뇌신경계 중환자실 전담 약사의 활동에 따른 약물 조정 효과 및 회피비용 분석

        Background: The role of clinical pharmacists in medication therapy to improve clinical and economic outcomes has been reported in the literature. This study was conducted to analyze the changes in details of medication interventions before and after the introduction of clinical pharmacists into the care of neurocritical care unit (NCU) patients, and to evaluate the economic effects of clinical pharmacists by calculating the avoidance cost. Methods: A retrospective study was conducted reviewing the electronic medical records from June 2013 to May 2014 (before), and from June 2016 to May 2017 (after). We calculated the number and rates of intervention, the acceptance rates of it, and also reviewed the list of interventions. We calculated avoidance cost if there was no intervention. Results: The monthly mean number of interventions increased from 8.0 (±5.7) to 31.7 (±12.8) (P <0.001) and the frequency of intervention also increased from 0.8% to 1.6% (P =0.003). The most frequently provided pharmacist intervention was nutritional support before introduction of clinical pharmacists and discussions on the medication plan after. The number of classified interventions was 14 before introduction of clinical pharmacist services and 33 after. The calculated cost avoidance associated with a clinical pharmacists’ integration was 77,990,615 won per year. Conclusions: Introduction of clinicals pharmacist into the NCU was associated with increased intervention rates and expanded types of clinical interventions. The cost avoidance achieved by the pharmacists’ interventions can be further explored to evaluate if similar expansions of pharmacists’ services achieve similar results in other settings.

        류마티스관절염 치료에서 오메가-3 다가불포화지방산의 역할

        Rheumatoid arthritis (RA) is a chronic inflammatory disease of unknown cause. It is characterized by polyarthritis. Currently, treatments for RA include the use of non-steroidal antiinflammatory drugs (NSAIDs), steroid, disease-modifying antirheumatic drugs (DMARDs), corticosteroid, and so on. New treatment options for RA have been actively developed. In this study, we determined pharmacological advantages of using omega n-3 polyunsaturated fatty acids (PUFAs) as adjunctive therapy in RA based on articles demonstrating the role of omega n-3 in the prevention and treatment of RA. Omega n-3 PUFAs can partially suppress a number of inflammatory reactions such as leukocyte chemotaxis and adhesion molecule expression. Mechanisms fundamental to the anti-inflammatory actions of omega n-3 PUFAs involve changing the compositions of cell membrane's phospholipid fatty acids. Omega n-3 PUFAs with inflammation control action may also result in the reduction of pain in rheumatoid arthritis. The benefit of n-3 PUFAs in RA has also been suggested in some clinical studies. In conclusion, omega n-3 PUFAs might be useful as an adjuvant therapy for the treatment of RA.

        Zunongwangia flava sp. nov., belonging to the family Flavobacteriaceae, isolated from Salicornia europaea

        A yellow pigmented bacterium designated strain MBLN094T within the family Flavobacteriaceae was isolated from a halophyte Salicornia europaea on the coast of the Yellow Sea. This strain was a Gram-stain negative, aerobic, non-spore forming, rod-shaped bacterium. Phylogenetic analysis of the 16S rRNA gene sequence of strain MBLN094T was found to be related to the genus Zunongwangia, exhibiting 16S rRNA gene sequence similarity values of 97.0, 96.8, 96.4, and 96.3% to Zunongwangia mangrovi P2E16T, Z. profunda SM-A87T, Z. atlantica 22II14-10F7T, and Z. endophytica CPA58T, respectively. Strain MBLN094T grew at 20‒37°C (optimum, 25‒ 30°C), at pH 6.0‒10.0 (optimum, 7.0‒8.0), and with 0.5‒15.0% (w/v) NaCl (optimum, 2.0‒5.0%). Menaquinone MK-6 was the sole respiratory quinone. The polar lipids were phosphatidylethanolamine, two unidentified aminolipids, and four unidentified lipids. Major fatty acids were iso-C17:0 3-OH, summed feature 3 (C16:1 ω6c and/or C16:1 ω7c), and iso-C15:0. The genomic DNA G + C content was 37.4 mol%. Based on these polyphasic taxonomic data, strain MBLN094T is considered to represent a novel species of the genus Zunongwangia, for which the name Zunongwangia flava sp. nov. is proposed. The type strain is MBLN094T (= KCTC 62279T = JCM 32262T).

        Power system stabilizer act efficiently to damp the electromechanical oscillations in interconnected power systems. This paper presents an algorithm for the optimal parameter selection of a power system stabilizer in two-area power systems with a series HVDC link. This method is one of the classical techniques by allocating properly pole-zero positions to fit as closely as desired the ideal phase lead between the voltage reference and the generator electrical power and by changing the gain to produce a necessary damping torque over the matched frequency range. Control of HVDC converter and inverter are used a constant current loop. Proper parameters of PI controllers are obtain based on the Root-locus technique in other to have sufficient speed and stability margin to cope with charging reference values and disturbance. The small signal stability and transient stability studies using the PSS parameters obtained from this method show that a natural oscillation frequency of the studycase system is adequately damped. Also the simulation results using the HVDC converter and inverter parameters obtained from this proposed method show proper current control characteristics. The simulation used in the paper was performed by the Power System Toolbox software program based on MATLAB.

        When we construct a railroad tunnel excavation, the quality control points of the reinforced-rod auto-grouting method grouting pressure(p), flow quantity(q) and injection radius(r). To record and manage pressure(p) and speed(q) of grouting autographic devices such as integration flow-meter usually record data in a roll of paper. Integration flow-meter can record grouting flow quantity exactly, but the recorded pressures differ from the any basis such as initial, intermediate and final point. Therefore, it has been argued that is a need of reliable method to describe the connection between the pressure recorded by an integration flow-meter and the special properties of the grouting target ground. Auto-grouting method can describe the reliable connection between the grouting pressure and the special properties of the grouting target ground. So, in this paper by using auto-grouting method, we are going to secure basis of quality control techniques during railroad tunnel excavation.

        Probiotic and antioxidant properties of C30 carotenoid-producing Lactiplantibacillus plantarum isolated from kimchi

        This study focuses on the development of functional probiotics using caroteonid-producing lactic acid bacteria (LAB) with antioxidant properties. Thirty LAB strains were evaluated for their probiotic properties. Carotenoid biosynthesis gene cluster (crtMN operon) was detected using polymer chain reaction (PCR). The carotenoid identified as 4,4′-diaponeurosporene was analyzed via UV visible absorption spectra and HPLC. Five carotenoid-producing strains showed antioxidant activities. Lactiplantibacillus plantarum MGB0112, which showed the highest carotenoid production measuring at 470 nm of absorbance per ml of culture broth (0.014 A470nm/ml), showed low pH (56.5%) and bile salt (97.8%) tolerance with high adhesion properties (55.1% for toluene). Furthermore, this strain and 4,4′-diaponeurosporene extract exhibited antioxidant activity (99.5 and 40.1%, respectively) against DPPH free radicals in vitro. Their antioxidant properties were confirmed in vivo (45.6 and 55.2% survival rates in Caenorhabditis elegans). Therefore, C30 carotenoid-producing strain MGB0112 demonstrates outstanding antioxidant effects and can be a potential functional probiotics.

        Cultivable Microbial Diversity in Domestic Bentonites and Their Hydrolytic Enzyme Production

        We have isolated and identified 72 bacterial strains from four bentonite samples collected at the mining areas located in Gyeongsangbuk-do, Republic of Korea, and measured their hydrolytic enzyme (α-amylase, protease, and cellulase) activities to identify the isolates with industrial-use potential. Most of the isolates belonged to the Bacillaceae, with minor portions being from the Paenibacillaceae, Micrococcaceae, and Bacillales Family XII at the family level. Of the strains isolated, 33 had extracellular α-amylase activity, 30 strains produced cellulase, and 35 strains produced protease. Strain MBLB1268, having the highest α- amylase activity, was identified as Bacillus siamensis (0.38 ± 0.06 U/ml). Bacillus tequilensis MBLB1223, isolated from Byi33-b, showed the highest cellulase activity (0.26 ± 0.04 U/ml), whereas Bacillus wiedmannii MBLB1197, isolated from Zdb130-b, exhibited the highest protease activity (54.99 ± 0.78 U/ml). These findings show that diverse bacteria of the Bacillaceae family adhere to and exist in bentonite and are potential sources of industrially useful hydrolytic enzymes.

        A report of seven unrecorded bacterial species in Korea, isolated from marine sediment

        In March 2021, marine sediment from East Sea samples were suspended in a 2% NaCl solution, and serial dilution was performed in fresh marine and Reasoner’s 2A agar. Isolated bacterial strains were identified based on 16S rRNA gene sequences, and showed at least 98.7% sequence similarity with previously reported bacterial species. Finally, seven bacterial strains which were validly published but not reported in Korea, were obtained. These isolates were allocated to the orders Bacillales and Flavobacteriales. The three Flavobacteriales strains are classified into the family Flavobacteriaceae. The other four Bacillales belong to the families Bacillaceae and Paenibacillaceae. The seven unrecorded bacterial strains in this study are classified into seven different genera, which are assigned to Mesobacillus, Paenibacillus, Gramella, Gillisia, Arenibacter, Fictibacillus, and Brevibacillus. During the investigation, the possibility of excavation of various unrecorded species in domestic marine sediment was confirmed. Gram-staining, cell morphology, physiological and basic biochemical characteristics, and phylogenetic analysis were performed in this study and provided in the description of each strain.

        The paper considers the theoretical problem of eliminating harmonics in inverter waveforms. The problems associated with the harmonics distortion of PWM inverter devices were analysed. Voltage control methods are variable parameter each harmonics magnitude improve the voltage wareform.
