        만성폐쇄성폐질환 환자에서 호흡곤란과 영상학적 정량과의 상관관계

        연구배경: COPD에서 보이는 호흡곤란의 주된 원인이 폐의 과팽창이고 이 과팽창을 치료하는 것이 호흡곤란의 향상에 중요한 것으로 밝혀지고 있다. 그러나 FEV1, FVC만으로는 호흡곤란의 정도와 과팽창을 정확히 측정할 수 없다. 따라서 단순 흉부 방사선에서 관찰되는 폐의 과팽창의 정도와 고해상도흉부 단층촬영에서 폐기종의 정도가 호흡곤란을 객관적으로 알 수 있는 지표가 되는 지를 알아보고자 하였다. 방법: COPD로 진단 받은 50명(평균 연령 69세, 남자 47명, 여자 3명)을 대상으로 하였다. 폐의 종축, 흉골 뒤 공간, 횡격막의 높이를 각각 측정하여 총 0에서부터 3까지로 폐의 과팽창 정도(chest score)를 측정하였고, 고해상도 흉부단층 촬영에서 폐기종의 정도(HRCT score)를 구하여 이를 환자의 호흡곤란의 정도 및 폐용적 검사를 비롯한 폐기능 검사와의 상관관계를 구하였다. 결과: 호흡곤란의 지수(Borg scale)는 폐기능 검사의 지표인 FVC, FEV1, FEV1/FVC, RV, RV/TLC, DLCO와 유의한 상관관계가 관찰되었다. Borg scale은 HRCT score와 상관관계가 있었으나, chest score와는 상관관계가 없었다. 그리고 Borg scale은 체질량 지수와 역 상관관계가 있었다. 결론: 고해상도 흉부 단층촬영의 폐기종의 정도는 COPD 환자에서 호흡곤란의 정도를 알 수 있는 객관적 지표가 될 수 있다. Background: A lung hyperinflation, or air trapping, caused by expiratory flow-limitation contributes to dyspnea in patients with chronic obstructive pulmonary disease (COPD). Forced expiratory volume in 1 second (FEV1) has served as an important diagnostic measurement of COPD, but does not correlate with patient-centered outcomes such as dyspnea. Therefore, this study was performed to investigate the role of radiologic quantity in evaluating the dyspnea in patients with COPD by measuring lung hyperinflation in chest x-ray and high resolution chest tomography (HRCT). Methods: Fifty patients with COPD were enrolled in this study. Their subjective dyspnea score (modified Borg scale dyspnea index), spirometry, and lung volume were measured. Simultaneous hyperinflations of chest x-ray score (chest score) and degree of emphysema of HRCT (HRCT score) were measured. The chest score were composed of lung length, retrosternal space width, and height of the arc of the diaphragm and HRCT score were composed of severity and extent of emphysema. Results: The mean age of patients was 69 years old and their mean FEV1 was 51.7%. The Borg score significantly correlated with parameters of spirometry and lung volume, including FVC, FEV1, FEV1/FVC, RV, RV/TLC, and DLCO. The Borg score correlated well with HRCT score, but did not correlate with chest score. Also, the Borg scale correlates inversely with body mass index. Conclusion: The quantity of emphysema on chest HRCT may serve as an objective marker of dyspnea in patients with COPD.

        Introduction: Placenta previa is a condition in which the placenta implants in the poorly vascularized lower uterine segment, which may result in inadequate uteroplacental perfusion, in turn, adversely affect the neonatal outcome. Abnormal placentation may also lead to severe postpartum hemorrhage as placenta separation proceeds. We aimed to evaluate the differences in placental histopathology and perinatal outcomes in pregnancies complicated with placenta previa and controls. Methods: We undertook a retrospective case-control study of 93 pregnancies with placenta previa and 81 controls between 2011 and 2017. Results: Gross findings of the placenta showed that the placentas in placenta previa had significantly higher mean large chorionic plate diameters (18.5 ± 3.2 vs 17.5 ± 2.6 cm, P = .0298), chorionic plate areas (218.4 ± 62.9 cm2 vs 198.7 ± 56.0 cm2, P = .0344), and marginal cord insertion (19.8% vs 8.6%, P = .0411) than control groups. Placental histopathological findings showed that placentas in placenta previa was significantly associated with maternal underperfusion, including villous infarction (50.5% vs 25.9%, P = .0009) and increased intervillous fibrin deposition (38.7% vs 7.4%, P < .0001). Also, women in the placenta previa group had a higher rate of abnormally invasive placenta and severe postpartum hemorrhage. However, placenta previa was not associated with the increased risk of neonatal mortality and morbidity. Conclusion: Abnormal placentation into the poorly vascularized lower uterine segment induces compensatory placental growth and increased surface area in response to reduced placental perfusion, which was consistent with the histopathological findings of coagulative necrosis of chorionic villi and fibrin deposition in the intervillous space. The morphological changes occurring in placenta previa may have important roles in maintaining adequate uteroplacental-fetal perfusion, which may prevent adverse neonatal outcomes.

        단청 문양을 활용한 3D 디지털 메이크업 작품 연구

        A new trend in the post COVID-19 ear is an uncontacted culture that connects without contact. As digital technology is used in various fields and social distancing is implemented, many changes in culture, education, and consumption patterns have also appeared. Also, contemporary society can easily access various cultures around the world with the development of social media. The design trends are recreated by combining originality and tradition and creating a new and colorful culture. Reflecting on this changing trend in the digital culture era, 3D digital makeup works were produced based on the characteristics of the basic patterns of Korean traditional Dancheong. This study reinterpreted the concept, types, and features of Dancheong and the meaning of the basic patterns and features of Dancheong. Besides, this study investigated the concept of 3D digital makeup and the functions and features of 3D face modeling. To express the characteristics of Dancheong, a 3D model using a tablet and a styler pen was used to express the Dancheong style with digital drawing and painting techniques using the Z brush and Adobe Photoshop CC 2020 program. Three 3D digital makeup designs were produced with a Dancheong pattern, peony, and butterfly based on the considerations. Through the above research, makeup artists" creative design presentation can maximize the possibility of expanding the makeup design part. This will be the starting point for creative and individual design production methods and the expansion of design areas.

        어린이집 평가인증 제도에 대한 보육교사의 인식과 실태 분석

        본 연구의 목적은 어린이집 평가인증 제도에 대한 보육교사의 인식과 실태를 분석하여 평가인증 제도의 효율적인 수행과 성공적인 정착을 위한 방안을 모색하는 데 있다. 어린이집 평가인증 제도에 대한 보육교사의 인식과 실태를 분석하기 위한 설문지를 개발한 후 연구자료를 수집하였다. 개발된 설문지는 평가인증 제도에 대한 보육교사의 인식, 평가인증 준비과정에 대한 보육교사의 인식 및 실태, 평가인증 지표(7개 영역)에 대한 보육교사의 인식, 평가인증 사후관리에 대한 보육교사의 인식의 4개 영역으로 구분하여 총 36문항으로 구성하였다. 연구대상은 S시에 소재한 어린이집 중 2005년 3월부터 2011년 6월까지 평가인증을 통과한 어린이집의 보육교사 154명이었으며, 수집된 자료는 χ2-검증을 실시하여 분석하였다. 연구결과, 평가인증 제도에 대한 인식, 평가인증 준비과정에 대한 인식 및 실태, 평가인증 지표(7개 영역)에 대한 인식, 평가인증 사후관리에 대한 인식은 보육교사가 재직하고 있는 어린이집 유형에 따라 부분적으로 유의한 차이가 있는 것으로 나타났다. The purpose of this study was to analyze the status and childcare teachers' awareness of the Childcare Center Accreditation System to implement and settle down the system successfully. A questionnaire was developed based on prior literature, research and arrangements with professors and childcare teachers. The questionnaire is composed of 4 items of 36 questions related to 'general awareness of the Childcare Center Accreditation System', 'the preparatory process of the Childcare Center Accreditation System','7 indicators of the Childcare Center Accreditation System', and 'post management of the Childcare Center Accreditation System'. The Subjects used in this study were 154 childcare teachers who work for childcare center passed the Accreditation System from March, 2005 to June, 2011 in S province. Data analysis was conducted by χ2-test. The results of this study indicated that there were partially significant differences among childcare teachers according to the types of childcare center.

        주거지 정비과정에서 활용된 주민참여기법 사례 연구

        The calls for qualitative improvements in the existing urban living environment are becoming more urgent due to the expansion of residents' participatory awareness regarding administrative procedures and the enhancement of their interest in the local community and of quality of life, and the importance of resident participation is becoming more widely recognized. To actively push forward resident participation in terms of policy, various techniques are stipulated in the law, such as public hearings, committees, public exhibits and announcements under urban planning laws. However, the general perception is that these measures are not being sufficiently utilized, and research is needed to discover how much resident participation techniques are being applied in reality and whether they can overcome their practical limitations. Such research can be carried out by studying examples where the residents' quality of life was increased from an overall perspective, but few relevant case studies have been carried out in Korea until now, and it was practically difficult to prove their effectiveness. The recent Daejeon Metropolitan City's Rain Project, however, is a resident-oriented development project that gathers and reflects the various opinions of community residents in the planning and promotional stages, and it is a good example of a project increasing the residents' quality of life from an overall perspective. Accordingly, the purpose of this paper is to systematically examine the types and characteristics of residents' participation techniques through an advanced case study. The results of this paper can be used to discover various residents' participation techniques that are needed for residential improvements in Korea.

        HRD 담당자의 전략적 인적자원개발 역할이 직무성과에 미치는 영향

        본 연구는 HRD 담당자의 전략적 인적자원개발 역할이 직무성과에 미치는 영향을 규명하는데 목적이 있다. 본 연구의 목적을 달성하기 위하여 HRD 담당자의 SHRD 역할을 측정하기 위한 측정도구를 개발하고, 직무성과와의 관계를 분석하였다. HRD 담당자의 SHRD 역할 측정도구 개발을 위하여 혼합연구방법을 적용하여 다음과 같이 진행하였다. 문헌연구 결과를 토대로 HRD 담당자의 SHRD 역할로 HRD 전문가, 전략적 파트너, 변화촉진자를 도출하였고, 초점집단면담을 통해 측정문항을 개발하였다. 전문가 검토를 진행하여 문항에 대한 내용타당도를 검증하였고, 설문조사를 실시하여 측정도구에 대한 구성타당도를 검증하였다. 본 연구에서 개발한 측정도구를 사용하여 HRD 담당자의 전략적 인적자원개발 역할이 과제성과 및 맥락성과에 미치는 영향을 조사하였다. 그 결과 HRD 전문가와 변화촉진자 역할이 과제성과 및 맥락성과에 유의미한 정적인 영향을 미치는 것으로 분석되었다. 전략적 파트너 역할은 과제성과 및 맥락성과에 유의미한 영향을 미치지 못하는 것으로 파악되었다. The purpose of this study is to empirically explore whether strategic human resource development roles of HRD practitioners have positive impact on job performance. To do this, mixed methodology is applied to develop the scale of strategic human resource development roles of HRD practitioners and measure the effect on job performance. Firstly, HRD professional, strategic partner, change facilitatior roles are identified as the strategic human resource development roles of HRD practitioners through the review of related literature. Secondly, the items of the scale are developed as the result of focus group interview. Thirdly, content validation is progressed by the review of HRD professionals. Fourthly, construct validation is examined as the survey result. Finally, the impact of strategic human resource development roles of HRD practitioners on job performance is investigated. HRD professional and change facilitator roles have positive relationship with task performance and contextual performance. However, strategic partner role is not related to task performance and contextual performance.

        경영전략 실행 관점에서 전략적 인적자원개발의 역할 탐색

        전략적 인적자원개발은 인적자원개발이 전량경영 차원에서 실행됨으로써 조직의 전략적 목표 달성에 기여할 것으로 기대되어 왔다. 그러나 전략적 인적자원개발이 어떠한 구체적 역할을 통해 전략경영에 기여할지에 대한 논의는 미흡한 현실이다. 이에 본 연구는 경영전략 실행 관점에서 전략적 인적자원개발의 역할을 탐색하는 데 그 목적이 있다. 이를 위하여 문헌연구를 토대로 전략적 인적자원개발의 역할을 규명하고 각 역할별 하위 역할을 탐색하였다. 먼저 경영전략 실행에서 인적자원의 질이 핵심 성공 요인으로 간주된다는 점에서 경영전략 실행의 개념과 구성요소를 분석하여 시사점을 분석하였다. 다음으로 전략적 인적자원개발의 개념을 고찰하여 전략적 인적자원개발의 주요 역할을 전략적 역량관리·성과관리·변화관리로 상정하였다. 전략실행 및 인적자원개발의 추가적인 문헌분석을 통해 경영전략 실행 관점에서 각 역할별 하위 역할을 도출하고 이를 제시하였다. 더불어 이들 역할에 기반한 전략적 인적자원개발의 개념적 모형을 제안하였다. 이를 토대로 전략적 인적자원개발의 역할에 대한 학문적·실천적 시사점을 논의하였다. Although strategic human resource development has been cited as the important function of human resource development, the results of researches of strategic human resource development have not been developed in its full extent. This article diagnoses one of the reasons of this problem as not being explored the concrete role of strategic human resource development in strategic management. Because strategic human resource development is a stressed concept linking human resource development with strategic management. Accordingly, this article explores the role of strategic human resource development in strategic management, specifically, strategic implementation. The process of strategic management includes strategic planning and strategic implementation. Human resource is regarded as a key success factor of strategic implementation. In this point of view, exploring the role of strategic human resource development in strategic implementation is regarded as suitable way to suggest the definite role of strategic human resource development. To achieve this, literature of strategic implementation and strategic human resource development are analyzed. As the result of analysis of literature, three roles of strategic human resource development - strategic competency management, strategic change management, strategic performance management - and sub roles are suggested. Academic and practical suggestions are made based on the result of this article.
