For prevention of bacteremia and surgical field contamination, various concentration are used in preoperative skin preparation. Of these, povidone(PVP)-iodine has been known as one having believable effect. But, application of PVP-Ⅰon pral and maxillofacial field has some problems-skin discoloration, hypersensitivity, corneal irritation,etc. Therefore chlorhexidine has been tried to replace PVP-Ⅰ. More recent studies of antibacterial effect of chlorhexidine and PVP-Ⅰ has been achieved on the other field. The purpose of this study was to compare the antibactreial effect of two kinds of skin disinfectants used in oral and maxillofacial surgical field.-chlorhexidine and PVP-Ⅰ. We studied the 40 patients who have taken operation of orthognathic surgery in Dept. of oral & Maxillofacial Surgery, Kangnam General Hospital Public Corp..Results of bacterial culture taken from facial skin and intraoral mucosa after application 10% PVP-Ⅰ and 0.5% chlorhexidine were compared.
신호교차로 지체시간을 고려한 버스도착시간 예측 신뢰성 향상 연구

본 연구는 현재 적용되고 있는 버스 도착 예측정보 생성 알고리즘에서 반영할 수 없었던 요소인 신호 지체시간을 고려한 버스 도착시간 예측 방법론을 제안하였다. 신호지체시간을 반영하기 위해 정류장간 통행시간을 서비스시간, 순통행시간, 신호지체시간으로 분할하였고, 신호지체시간은 교차로 도착시간과 신호운영계획(TOD)를 이용하여 추정하였다. 본 연구에서 제안한 방법론으로 도착시간을 예측할 때에 발생하는 오차 대부분이 약 ±30초 이내로 나타 났으나 일부 다소 큰 값의 오차를 발견할 수 있었는데 이러한 오차의 발생 원인은 예측 신호 주기보다 앞 신호주기에 통과했기 때문인 것으로 분석되었다. 교차로 도착시각을 관측값이 아닌 추정값을 사용하는 본 방법론의 특성상 광역버스 등의 도착시각을 예측할 수 없는 한 계가 존재한다. 실시간 위치정보를 통해 이를 개선하는 등의 향후 연구를 통해 본 방법론의 정확도를 크게 향상시킬 수 있을 것이다. This study propose a method to predict the bus arrival time by considering the signal delay time which is an element which can not be considered in the current bus arrival prediction information generation algorithm. In order to consider the signal delay time, travel time is divided into three components: service time, cruising travel time, and signal delay time. Signal delay time was estimated using intersection arrival time and TOD. The results show that most of the errors that occurred in predicting the arrival time are within about 30 seconds. Some of the estimates have large errors due to the nature of this methodology that uses the estimated value of the intersection arrival time rather than the observation value. It is also difficult to predict the arrival time of the express buses using this method. Future studies such as improving this through real-time location information will greatly improve the accuracy of the methodology.
악교정 수술후 자가통증조절장치(Patient-Controlled Analgesia)의 사용 효과에 관한 연구

The concept of PCA(Patient Controlled Analgesia) was first described in 1968, by Sechzer. The earliest descriptions of actual self-administered PCA machines were by Forrest et al. In gastric bypass surgery, cesarian section, orthopedic surgery etc, PCA is widely used in the control of postoperative pain. Previous Studies have shown that PCA provides effective pain-control for the postoperative patient. The postoperative pain-control is a problem that should be solved in surgery. Especially in orthognathic surgery, it is not same as in the case of maxillofacial trauma surgery or of tumor surgery : most orthognathic surgery patients are under operation not accustomed to pains, and difficulties in pain-complaint due to IMF(Intermaxillary fixation), postoperative nausea, and vomiting are additional problem. In this study, we have compared PCA and IM analgesics with respect not only to time request but also to the quality of postoperative pain control.

The nasal and zygoma bone are parts of frequently fracturing of facial bones because of anatomically protrusion. Of facial bone fractures, nasal bone is at the high level of incidence rate. But zygoma fracture that anatomical characteristics increase the incidence rate also is occupied considerable part of the facial bone fracture. The outline of face is decided by form of underlying skeletal structure, of that, zygoma plays an important role in. Zygoma is closely attached to surrounding anatomic structure as orbit, maxillary sinus. Aesthetic and functional disturbance are developed by zygoma fracture from trauma, complications, as facial asymmetry, trismus, sensory disturbance, epistaxis, periorbital hemorrhage, diplopia etc, are developed. The patterns of complications following displacement of fractured fragment of zygoma by trauma are slightly different.
Trypan blue 도포를 사용한 누드 마우스와 흰쥐에서 경혈자리 찾기와 DiI 추적 법

Objectives: To find the potential acupuncture points by using Trypan blue staining on the skin of rat and Nude mouse. Methods: 0.4% Trypan blue was applied to the skin of rat or Nude mouse previously treated by surfactant. Washing by warm saline was followed after enough application of trypan blue and surfactant. Frequency of Trypan blue application should be varied to the experimental animals' condition for visualizing significant spots. Results: Blue spots appeared roughly in symmetry along kidney meridian or stomach meridian. Several spots outside of kidney or stomach meridian were also observed; however, the detail stereoscopic images of those blue spots were slightly different according to the position blue-colored. DiI signals were visualized along blood vessel after DiI injection into the Trypan blue-visualized blue spots. Conclusion: Our method to visualize the potential acupuncture points as a blue spot on rat and Nude mouse skins may contribute to the next step for finding specific flowing channels among blue spots.
고압산소치료의 구강악안면영역에서의 임상적 이용에 관한 연구

Hyperbaric oxygen therapy(HBO) is defined as breathing 100% oxygen while in an enclosed system pressurized to greater than on atmosphere(sea level). This increased oxygen delivery furthers your body's ability to kill germs and to increase healing. HBO is a supplemental therapy to be used in addition to the current medical and surgical therapy you are receiving. HBO typically is used to complement treatments of medical problems such as bone infections, complication of radiotherapy, and certain chronic, non-healing wounds. On an emergency basis, the chamber also is used to treat problems such as carbon monoxide poisoning and the decompression sickness. We analysed stastically cases which are treated by hyperbaric oxygen therapy in point of oral and maxillofacial region on the Kangnam General Hospital for aid in comprehension and application of this therapy. Total 760 patients were treated at Gangnam General Hospital from July 1996 to September 1999. They were classified by region to Decompression sickness(DCS), Carbon monoxide poisoning(CO), General surgery(GS), Orthopedics(OS), Oral and Maxillofacial surgery(OMFS), others. Patients of Oral and Maxillofacial surgery were divided by diseses to Osteomyelitis, Osteoradionecrosis, Reconstruction, Bone graft, Difficult wounds, others. The results were as follows. 1. This institute conducts HBO therapy for DCS which takes up 62% and 10.5% for OMF region. 2. In OMF region, Osteomyelitis is 40%, Osteoradionecrosis is next, Bone graft, and Reconstruction is a row. 3. According to our precious study, HBO has been frequently conducted in OMF region compared to past, however, it is less actively conducted in this area for research than other conturies. Therefore, We need further application to the clinical use.
