The open Hospital Attending Physician System has been discussed and operated for developing the efficiency and the substitutes for an increase in medical expenses, for overlapping investment in medical resources, and for normalization of medical delivery system. This study assessed the related factors to the acceptance and introduction of Attending Physicians on ophthalmologist. Data was collected mail response to 179 ophthalmologists in 2005. Applying multilevel logistic regression, we examined the relationship between their acknowledgment, attitude and acceptance of the open Hospital Attending Physicians System. The percentage of participation in Hospital Attending was indicated to be 3.9%. The 44.7% of all participants agreed to adopting the Hospital Attending, 33.5% of doctors are willing to participate in the future. The positive and normal attitude of physician toward the Hospital Attending is 6.6 times (95%CI 1.315, 33.138) and 55.2 times(95%CI 11.352, 268.347) more than that of negative after adjustment for other variables. Attitude was found to be the important factor influencing physicians` participation in the Attending Hospital. Thus, it is need to development and implication for the strategies that lead the practical necessity and positive attitude toward the Attending Hospital.
탄소나노튜브의 합성수율 증대와 저온 합성에 미치는 기판 전처리의 영향

Carbon nanotubes (CNT) on metal substrates are definitely beneficial because they can maintain robust mechanical stability and high conductivity between CNT and metal interfaces. Here, we report direct growth of CNT on Ni-based superalloy, Inconel 600, using thermal chemical vapor deposition (CVD) with acetylene feedstock in the growth temperature range of 400-725°C. Furthermore, we studied the effect of substrate pretreatment on the growth yield enhancement and growth temperature decrease of CNT on Inconel 600. Activation energy (AE) for CNT growth was estimated from the CNT height change with respect to the growth temperature. The AE values significantly decreased from 205.03 to 24.35 kJ/mol by the pretreatment of thermal oxidation of Inconel substrate at 725°C under ambient. Higher oxidation temperature tends to have lower activation energy. The results have shown the importance of pretreatment temperature on CNT growth yield and growth temperature decrease.

전력계통의 부하역률을 산출하기 위하여 변전소운전실적시스템(SOMAS)을 이용하여 실계통에 근접한 최적부하역률을 부하대별, 지역별, 직거래고객 최적부하역률을 산출하였다. 최적부하역률 산출에 따른 효과를 검증하기 위하여 2009년 하계예상피크 데이터를 이용하여 조상설비(Sh.C) 소요량을 산출하였다. 또한 최적부하역률 산출에 따른 전력계통의 전압안정도를 분석하기 위하여 PSS/E 프로그램을 이용하여 Q-V, P-V 분석을 수행하였으며 765㎸ 송전선로 2회선 상정고장시 수도권 부하차단량에 대하여 검토하였다.
의료보험자료 상병기호의 정확도 추정 및 관련 특성 분석 -법정전염병을 중심으로-

This study was undertaken in order to estimate the accuracy of disease code of the Korean National Medical Insurance Data and disease the characteristics related to the accuracy. To accomplish these objectives, 2,431 cases coded as notifiable acute communicable diseases (NACD) were randomly selected from 1994 National Medical Insurance data file and family medicine specialists reviewed the medical records to confirm the diagnostic accuracy and investigate the related factors. Major findings obtained from this study are as follows : 1. The accuracy rate of disease code of NACD in National Medical Insurance data was very low, 10.1% (95% C.I. : 8.8-11.4). 2. The reasons of inaccuracy in disease code were 1) claiming process related administrative error by physician and non-physician personnel in medical institutions (41.0%), 2) input error of claims data by key punchers of National Medical Insurer (31.3%) and 3) diagnostic error by physicians (21.7%). 3. Characteristics significantly related with lowering the accuracy of disease code were location and level of the medical institutions in multiple logistic regression analysis. Medical institutions in Seoul showed lower accuracy than those in Kyonngi, and so did general hospitals, hospitals and clinics than tertiary hospitals. Physician related characteristics significantly lowering disease code accuracy of insurance data were sex, age group and specialty. Male physicians showed significantly lower accuracy than female physicians; thirties and fortieg age group also showed significantly lower accuracy than twenties, and so did general physicians and other specialists than internal medicine/pediatric specialists. This study strongly suggests that a series of policies like 1) establishment of peer review organization of National Medical Insurance data, 2) prompt nation-wide expansion of computerized claiming network of National Medical Insurance and 3) establishment and distribution of objective diagnostic criteria to physicians are necessary to set up a national disease surveillance system utilizing National Medical Insurance claims data.
다슬기에서 추출한 Lipoxygenase의 정제와 특성

양산의 내원사 계곡에 서식하는 다슬기를 채집하여 다슬기 내에 존재하는 lipoxygenase를 정제하고 특성을 관찰하였다. 다슬기에서 추출한 lipoxygenase는 40~60% ammonium sulfate 획분에서 최대의 활성을 보였으며, 20~40%에서도 약간의 활성을 볼 수 있었다. Sephacryl S-200 gel chromatography에서는 3개의 단백질 peak를 얻었고, 그 중 첫번째 peak에서 활성이 나타남을 확인하였다. DEAE-Sepharose ion exchange chromatography에서는 0M~0.5M범위의 NaCl용액의 농도구배로 용출시킨 결과 0.1M에서 lipoxygenase가<br/> 용출되는 것을 알 수 있었다. 각 단계의 정제도는 염석, Sephacryl S-200 gel chromatography, DEAE-Sepharose ion exchange chromatography 과정을 거치는 동안 specific activity가 20.8U/㎎, activity yield는 19.8%였다. 효소의 pH에 대한 영향을 보기 위해 pH 4.0~9.0의 범위에서 효소의 활성을 관찰해 보았다. pH 4.0에서부터 pH가 증가함에 따라 활성도 증가하다가 pH 6.0~8.0 사이에서는 최대의 활성을 나타내었고, pH 8.0 이후에서는 점차 감소하는 추세를 볼 수 있었다. 효소의 온도에 따른 영향을 관찰한 결과 최적 반응온도는 30℃에 서 나타남을 알 수 있었다. 7.5% acrylamide gel을 이용하여 전기영동을 실시한 결과 단일 밴드를 확인할 수 있었는데, 이 단일 밴드를 통해서 완전한 정제가 이루어진 것으로 판명할 수 있었고, 표준단백절의 밴드와 비교하여 분자량을 검정한 결과 약 35kDa으로 판단되었다. Melania snail(Semisulcopira bensoni) is used as an ingredient in Korean traditional soup and nutritional foods. Generally, lipoxygenase in several food products may produce off-flavors during their processing and storage. Therefore, the inactivation of lipoxygenase is required to make the better extracts from Melania snail. Also, the quality on freshness of Melania snail may be evaluated by lipoxygenase activity. The lipoxygenase activity was the highest at 40~60% saturation among several concentrations in salting-out saturated solution of ammonium sulfate. The partial purification of lipoxygenase was successfully obtained by Sephacryl S-200 gel chromatography. The first peak among three peaks for protein determination snowed the highest activity of lipoxygenase in 13~16 fractions among 100 fractions. The highest peak of lipoxygenase activity by ion exchange chromatography was shown at 0.1M NaCl. In the purification step, the specific activity was 20.8U/㎎ and activity yield was 19.8%. The optimum pH and temperature were pH6.0~8.0 and 30℃, respectively. Molecular weight of the lipoxygenase was estimated about 35kDa by SDS-PAGE.

Much of the working population in developing countries are engaged in shift work now and the number of shift workers is not expected to decrease in the future mostly because the need for continuity of production is increasing. Therefore, the possible effects of shift work on health are of particular interest, and in fact, there have been many epidemiological studies on shift work since the first world war. However, no studies on health effects of shift work have been reported in Korea, and the existing studies in western world have arrived at quite different conclusions mainly because the conditions of work other than shift work, such as age and selection of workers, work environment, and labor conditions also influence the health of workers. This study was firstly carried out in Korea to investigate the health risks related to shift work with 2,093 female workers randomly selected from three major manufacturing industries in proportion to total number of female workers in those industries. Differences of work conditions other than shift work in this study were adjusted by multivariate analysis. Ma findings obtained from this study are as follows : 1. There were significant differences between shift and day workers in the distribution of age, type of industry, condition of noise and dust, regularity of mealtime, working position, and working duration. Shift workers tended to be younger, to have shorter working duration, to have more irregular mealtime, to work in standing position, and to work under more noisy and dusty environment than day workers. 2. Univariate analysis showed that shift work increased the Todai Health Index (THI) scores of digestive tract, respiratory tract, and mental instability symptom categories. Shift work also increased days of sickness absence and number of industrial accident per 100 workers per month. 3. Multivariate analysis that adjusted the differences of demographic, occupational and non-occupational health-related working conditions showed that digestive tract symptom and mental instability symptom scores were significantly higher in shift workers than those in day workers. Based on these study results, it is concluded that the shift work has significant effects on some psychophysiological conditions of the workers and the effects are also influenced by several other personal and working conditions.
