Implantable middle ear hearing devices (IMEHDs) have been widely studied as an alternative hearing aids to solve the problems of conventional hearing aids. Vibration transducer of middle ear hearing aids is a key component because vibration characteristics of transducer is directly involved performance of hearing aids. So, the study about middle ear hearing aids concentrate on the transducers. A floating mass type transducer is most efficient. In this paper, we suggest a lumped mechanical model of electromagnetic floating mass transducer implanted on human middle ear. The proposed model enables analysis of the vibration characteristics of a floating mass transducer and prediction of the variation after implant on ossicle that offers a simple and easy to analyze. The parameters was drawn based on the components and the structures of transducer. The Lumped mechanical model was converted by the electrical-mechanical equivalent model, and simulated using PSpice. So, we investigated vibration characteristics of transducer influenced it's components. And we predict vibration characteristics of stapes footplate due to implanted transducer's vibration using combining model of transducer and human ear. To prove the feasibility of the suggested model, we fabricated a differential floating mass transducer (DFMT) as one of floating mass transducers and performed experiments using the human temporal bones.

Nowadays, implantable hearing aids have been developed to solve the problems of conventional hearing aids. In case of fully implantable hearing aids, an implantable microphone is necessary to receive sound signal beneath the skin. Normally, an implantable microphone has poor frequency response characteristics in high frequency bands of acoustic signal due to the high frequency attenuation effect of skin after implantation to human body. In this paper, the implantable microphone is designed to reduce the high frequency attenuation effect of a skin by putting its resonance frequency at the attenuated range through a finite element analysis (FEA) simulation. The designed implantable microphone through the simulated results has been fabricated by manufacturing process using bio-compatible materials. By the several in-vitro experiments with pig skin, it has been verified that the designed implantable microphone has a resonance frequency around the starting part of the attenuated range and reduces the attenuation effect.
소아암 환자에서 2009 대유행 인플루엔자 A(H1N1) 감염의 임상적 고찰 및 계절 인플루엔자와의 비교 분석

목적: 본 연구는 단일 기관에서 소아암 환자의 2009 인플루엔자 A(H1N1) [A(H1N1)pdm09] 감염 양상을 조사하고 이전의 계절 인플루엔자 감염과 비교 분석하고자 하였다. 방법: 삼성서울병원에서 2009년 8월부터 2010년 2월까지 A(H1N1)pdm09 감염, 2000년 1월부터 2009년 5월까지 계절 인플루엔자 A 감염이 확진된 소아암 환자의 의무기록을 후향적으로 분석하였다. 결과: 82명의 소아암 환자에서 A(H1N1)pdm09 감염이 확진되었다. 10명(12.2%)의 환자에서 하기도 감염증 또는 호흡기 외 감염이 발생하였다. 3명(3.7%)의 환자가 사망하였고 그 중 2명은 A(H1N1)pdm09 기여 사망이었다. 합병된 감염증과 관련하여 유의성을 가지는 위험인자는 감염의 시점(2009년 44-45주)과 병원 내 감염이었다. 이전의 계절 인플루엔자 A 감염과 비교하였을 때 임상적 특성에는 유의한 차이가 없었으나 A(H1N1) pdm09 감염에서 적극적인 항바이러스제 치료가 시행되었다. 결론: 소아암 환자에서 A(H1N1)pdm 감염은 경증부터 중증까지 다양한 임상 경과를 보였으며 이전의 계절 인플루엔자와 비교했을 때 임상 양상의 유의한 차이는 없었다. Purpose: This study was performed to compare the clinical characteristics of 2009 pandemic influenza A(H1N1) [A(H1N1) pdm09] and seasonal influenza A infection in the pediatric cancer patients. Methods: A retrospective review was performed in the pediatric cancer patients who had confirmed A(H1N1)pdm09 infection at Samsung Medical Center from August 2009 to February 2010. For the comparison, the medical records of pediatric cancer patients with seasonal influenza A from January 2000 to May 2009 were reviewed retrospectively. Results: Eighty-two A(H1N1)pdm09 infections were confirmed in the pediatric cancer patients. Ten patients (12.2%) developed complicated clinical course by lower respiratory infections or extrapulmonary infections; 4 pneumonia, 1 bronchitis, 1 pericarditis with pneumonia, 1 encephalitis with pneumonia, 2 meningitis and 1 pericarditis. Three patients received mechanical ventilator and ICU care. Three pediatric cancer patients (3.7%) died. The risk factors related to complicated A(H1N1)pdm09 infections were date of infection (44-45th week 2009) and nosocomial infection. When comparing with previous seasonal influenza A infections, more prompt and aggressive antiviral therapy was given in A(H1N1)pdm09 infections. Conclusion: The A(H1N1)pdm09 infections caused a various clinical manifestations including fatal cases in pediatric cancer patient during pandemic season. There was no significant difference in clinical course between influenza A(H1N1)pdm09 and seasonal influenza A infections except the antiviral treatment strategy.
소아 암 환자에서 발생한 Bacillus cereus 균혈증의 분자역학 분석에 관한 연구

목적: Bacillus cereus 는 암환자들에서 기회감염을 일으킬 수 있다. 2013년에서 2014년 기간 동안 삼 성서울병원 소아 암 병동에서 B. cereus 균혈증의 급격한 증가가 관찰되었다. 이에 증가된 B. cereus 균혈증에 대해 분자역학적 연구를 시행하였다. 방법: 2001년 1월부터 2014년 6월까지의 기간 동안 B. cereus 균혈증이 발생한 소아 암 환자들을 확 인하였다. 이번 연구에서 B. cereus 균혈증은, 오염여부와는 상관없이, 혈액배양검사에서 적어도 한 번 이상 B. cereus 가 확인된 경우로 정의하였다. 획득 가능한 균주들에 대해 multilocus sequence typing (MLST) 분석을 시행하였고, 후향적 챠트 리뷰를 실시하였다. 결과: 연구 기간 동안 총 19명의 B. cereus 균혈증 환자가 확인되었다. 그러나, 2013년도에는 B. cereus 균혈증 환자가 급격하게 증가하였다. 또한, 응급실 공사 중이던 2013년 7월의 1주, 2013년 10월의 한 주 동안 각각 3명의 환자가 발생하였다. 그러나 MLST 분석상 일정한 패턴이 없는, 다양한 sequence types (STs)들로 확인되었다. 2013년 이전의 5개의 균주들의 ST는 ST18, ST26, ST177, ST147-like type, ST219-like type이었고, 2013년도의 균주들의 ST는 ST18, ST73, ST90, ST427, ST784, ST34-like type, ST130-like type으로 확인되었다. 고찰: MLST 분석상 B. cereus 균주들의 다양한 ST 분포가 확인되었다. 이번 연구에서 단일 ST의 B. cereus 에 의한 균혈증 발생의 가능성은 낮아보인다. Purpose: Bacillus cereus has been reported as the cause of nosocomial infections in cancer patients. In our pediatric cancer ward, a sudden rise in the number of patients with B. cereus bacteremia was observed in 2013 to 2014. This study was performed to investigate the molecular epidemiology of increased B. cereus bacteremia cases in our center. Methods: Pediatric cancer patients who developed B. cereus bacteremia were identified from January 2001 to June 2014. The B. cereus bacteremia in this study was defined as a case in which at least one B. cereus identified in blood cultures, regardless of true bacteremia. Available isolates were further tested by multilocus sequence typing (MLST) analysis. A retrospective chart review was performed. Results: Nineteen patients developed B. cereus bacteremia during the study period. However, in 2013, a sudden increase in the number of patients with B. cereus bacteremia was observed. In addition, three patients developed B. cereus bacteremia within 1 week in July and the other three patients within 1 week in October, respectively, during emergency room renovation. However, MLST analysis revealed different sequence types without consistent patterns. Before 2013, five tested isolates were ST18, ST26, ST177, and ST147-like type, and ST219-like type. Isolates from 2013 were ST18, ST73, ST90, ST427, ST784, ST34-like type, and ST130-like type. Conclusions: MLST analyses showed variable ST distribution of B. cereus isolates. Based on this study, there was no significant evidence suggesting a true outbreak caused by a single ST among patients who developed B. cereus bacteremia.

저자들은 수모세포종이 있는 24세 여자 환자에서 가족성 선종성 용종증이 동반된 Tucot 증후군 1예를 경험하였으며 문헌고찰과 함께 보고하는 바이다. Turcot syndrome is characterized by the concurrence of a primary neuroepithelial brain tumor and multiple colorectal polyposis. We report a case of a 24-year-old woman diagnosed with Turcot syndrome. At first, the patient was diagnosed as having a medulloblastoma after a tumorectomy of the 4th ventricle mass. The patient underwent radiotherapy and chemotherapy. After high-dose chemotherapy, neutropenic fever and severe mucositis developed. For an evaluation of the persistent hematochezia and diarrhea, a colonoscopy was performed. It revealed pseudomembranous colitis and multiple polyps in the entire colon. According to the family history, her father had undergone a total colectomy due to colon cancer and polyposis of the entire colon. Her brother also was found to have multiple polyps in the colon by a colonoscopy. The patient was diagnosed with Turcot syndrome.

A microphone for fully implantable hearing device was generally implanted under the skin of the temporal bone. So, the implanted microphone`s characteristics can be affected by the accompanying noise due to masticatory movement. In this paper, the implantable microphone with 2-channels structure was designed for reduction of the generated noise signal by masticatory movement. And an experimental model for generation of the noise by masticatory movement was developed with considering the characteristics of human temporal bone and skin. Using the model, the speech signal by a speaker and the artificial noise by a vibrator were supplied simultaneously into the experimental model, the electrical signals were measured at the proposed microphone. The collected signals were processed using a general adaptive filter with least mean square(LMS) algorithm. To confirm performance of the proposed methods, the correlation coefficient and the signal to noise ratio(SNR) before and after the signal processing were calculated. Finally, the results were compared each other.
