        Comparison of Computed Tomography-based Abdominal Adiposity Indexes as Predictors of Non-alcoholic Fatty Liver Disease Among Middle-aged Korean Men and Women

        Objectives: We compared the associations of 3 computed tomography (CT)-based abdominal adiposity indexes with non-alcoholic fatty liver disease (NAFLD) among middle-aged Korean men and women. Methods: The participants were 1366 men and 2480 women community-dwellers aged 30-64 years. Three abdominal adiposity indexes—visceral fat area (VFA), subcutaneous fat area (SFA), and visceral-to-subcutaneous fat ratio (VSR)—were calculated from abdominal CT scans. NAFLD was determined by calculating the Liver Fat Score from comorbidities and blood tests. An NAFLD prediction model that included waist circumference (WC) as a measure of abdominal adiposity was designated as the base model, to which VFA, SFA, and VSR were added in turn. The area under the receiver operating characteristic curve (AUC), integrated discrimination improvement (IDI), and net reclassification improvement (NRI) were calculated to quantify the additional predictive value of VFA, SFA, and VSR relative to WC. Results: VFA and VSR were positively associated with NAFLD in both genders. SFA was not significantly associated with NAFLD in men, but it was negatively associated in women. When VFA, SFA, and VSR were added to the WC-based NAFLD prediction model, the AUC improved by 0.013 (p<0.001), 0.001 (p=0.434), and 0.009 (p=0.007) in men and by 0.044 (p<0.001), 0.017 (p<0.001), and 0.046 (p<0.001) in women, respectively.         Claudin 11 regulates bone homeostasis via bidirectional EphB4-EphrinB2 signaling

        Claudins (Cldns) are well-established components of tight junctions (TJs) that play a pivotal role in the modulation of paracellular permeability. Several studies have explored the physiologic aspects of Cldn family members in bone metabolism. However, the effect of Cldn11, a major component of central nervous system myelin, on bone homeostasis has not been reported. In this study, we demonstrate that Cldn11 is a potential target for bone disease therapeutics as a dual modulator of osteogenesis enhancement and osteoclastogenesis inhibition. We found that Cldn11 played a negative role in the receptor activator of nuclear factor kappa B ligand-induced osteoclast (OC) differentiation and function by downregulating the phosphorylated form of extracellular signal-regulated kinase (ERK), Bruton’s tyrosine kinase, and phospholipase C gamma 2, in turn impeding c-Fos and nuclear factor in activated T cell c1 expression. The enhancement of osteoblast (OB) differentiation by positive feedback of Cldn11 was achieved through the phosphorylation of Smad1/5/8, ERK, and c-Jun amino-terminal kinase. Importantly, this Cldn11-dependent dual event in bone metabolism arose from targeting EphrinB2 ligand reverse signaling in OC and EphB4 receptor forward signaling in OB. In agreement with these in vitro effects, subcutaneous injection of Cldn11 recombinant protein exerted anti-resorbing effects in a lipopolysaccharide-induced calvarial bone loss mouse model and increased osteogenic activity in a calvarial bone formation model. These findings suggest that Cldn11 is a novel regulator in bone homeostasis.

        PDA를 위한 스크롤 및 정보 제시 방법의 인간공학적 설계

        Mobile internet access using such devices as PDAs and cellular phones becomes popular as mobile technologies grow. However, the characteristics of small screen devices such as small screen size and pen-based input cause many usability problems. In this study, a human factors experiment was conducted to identify the factors affecting the usability of information search on a PDA. Factors manipulated in the experiment included use of wheel equipment, scroll dimension, and screen orientation. Task completion time, error frequency, and subjective satisfaction level were measured. In addition, various users' behavioral patterns such as scanning routes and mainly used scrolling methods were analyzed. Scroll dimension has a significant effect on task completion time. Scroll equipment and screen orientation affect subjective satisfaction level. The results could be applied to designing information structure of web sites for mobile use by providing vertical scroll and using external scroll equipments.

        콘크리트 표면차수벽형 석괴댐의 지진 취약도 평가

        The fragility curves for CFRD dams are derived in this study for probabilistic damage estimation as a function of a ground motion intensity. The dam crest settlement, which is a widely used damage index, is used for minor, moderate, and extensive damage states. The settlement is calculated from nonlinear dynamic numerical simulations. The accuracy of the numerical model is validated through comparison with a centrifuge test. The fragility curve is represented as a log normal distribution function and presented as a function of the peak ground acceleration. The fragility curves developed in this study can be utilized for real time assessment of the damage of dams. 본 연구에서는 지반진동 강도에 따른 콘크리트 표면차수벽 석괴댐(CFRD)의 확률론적 피해예측을 위하여 취약도 함수를 개발하였다. 댐의 손상평가에 널리 사용되는 지표인 댐마루 침하량을 손상지수로 사용하여 경미(Minor), 중간(Moderate), 심각(Severe) 손상수준을 정의하였다. 침하량은 댐 표준단면에 대한 비선형 동적 수치해석을 통하여 계산하였다. 수치해석 모델은원심모형시험결과와 비교하여 정확성을 검증하였다. 취약도 곡선은 대수정규분포함수로 나타내어 최대지반가속도를 기준으로 도출하였다. 본 연구에서 도출된 취약도 곡선은 댐의 실시간 피해예측에 활용될 수 있을 것으로 판단된다.

        24시간 복제 음식물 중의 Fe, Cu, Zn 함량: 유도결합플라즈마-원자발광분광법에 의한 정량분석

        The present study was initiated to examine the measures of dietary intake of Fe, Cu and Zn. The food duplicate samples were collected in Busan and its neighboring area, from the 69 middle-aged women (healthy non-smoking, mostly house wives), who provided informed consent. The samples were wet ashed by being heated in the presence of mineral acids, and Fe, Cu, Zn in the wet-ashed samples were analyzed by inductively coupled plasma - atomic emission spectrometry(ICP-AES). Dietary intake of Fe, Cu and Zn were 10.4 mg/day in Fe, 1.2 mg/day in Cu, 7.4 mg/day as arithmetic mean. The values for dietary Fe and Zn were lower, and the values for dietary Cu were higher than the recommended daily intake from Korean Nutrition Society. Further studies of Korean foods are needed to clarify the representative values for daily dietary Fe, Cu and Zn intake in the Korean population.

        The Inhibitory Effects of Citrus unshiu Markovich Extracts on the Receptor Activator of Nuclear Factor-Kappa-B Ligand-Mediated Osteoclast Differentiation and Function

        Ingestion of Citrus unshiu Markovich (CUM) has been widely used as an alternative therapy for thousands years in Korean traditional medicine. Both anti-osteoclastic actions of CUM extracts (CUME) and the possibility for development as a healthy food product were investigated. CUME significantly (p<0.05) and dose-dependently diminished the receptor activator of nuclear factor kappa- B ligand (RANKL)-induced formation of tartrate resistant acid phosphatase (TRAP)-positive osteoclasts. At the molecular level, CUME interrupted osteoclastogenesis based on selective stimulation of ERK and Akt activation. CUME caused down-regulation of osteoclast-specific marker gene expression derived from impairment of c-Fos and nuclear factor of activated T cells c1 (NFATc1) activation. Treatment with CUME caused localization failure of filamentous actin (F-actin) rings during cellularization and, subsequently, mature osteoclasts suffered malfunction of osteolytic activity. CUME has potential for prevention of osteoporosis.

        Standardized Seed Extract of Quisqualis indica (HU-033) Attenuates Testosterone Propionate-Induced Benign Prostatic Hyperplasia via α1-Adrenergic Receptors and Androgen/Estrogen Signaling.

        Benign prostatic hyperplasia (BPH) is an age-related disease characterized by prostatic enlargement and is the most common urologic symptoms in elderly men 60 years of age and older. Previously, we documented that 70% ethanol (EtOH) seed extract of Quisqualis indica (QI) attenuates pathological condition of testosterone propionate (TP)-induced BPH rat model via modulation of proliferation and apoptosis of prostate cells. Based on this potential of QI, we produced standardized seed extract of QI (HU-033) in order to prove further mechanisms. In this study, we aimed to suggest further mechanisms underlying the relationship between BPH and HU-033. Through not only cellular and nuclear receptor functional assays, but TP-mediated BPH rat model treated with HU-033, we demonstrated that HU-033 exerted antagonist effect on α1A- and α1D-adrenergic receptors in vitro and inhibitory effect on protein expression of androgen receptor and estrogen receptor alpha in vivo. Taken together, these results suggest that HU-033 is a novel candidate for the management of BPH.

        Chylous Leakage: A Rare Complication after Axillary Lymph Node Dissection in Breast Cancer and Surgical Management

        Chylous leakage is an extremely rare complication of surgery for breast cancer. We experienced a case of chylous leakage after axillary lymph node dissection. A 38-year-old woman with invasive ductal carcinoma in the left breast underwent a modified radical mastectomy after four cycles of neoadjuvant chemotherapy. The postoperative serosanguinous drainage fluid became “milky” on the fourth postoperative day. After trying conservative management, we re-explored the axilla and ligated the lymphatic trunk. Although the success of many cases supports conservative management, timely surgical intervention represents an alternative in cases where leakage persists or where the output is high.

        고령취업자의 작업환경과 건강진단 결과에 관한 실태조사

        고령근로자의 작업환경 실태와 건강검진 자료를 분석하여 그들의 건강한 생활과 삶의 질을 향상 시킬 수 있는 정책의 기초자료를 마련하고자 시도되었다. 대상자는 부산, 경남지역에 소재하는 41개 사업장에서 유해인자에 노출되어 작업하는 공정에 있는 50세 이상 근로자 112명(남성91명, 여성 21명)을 대상으로 소음, 분진, 유기용제, 중금속, 작업형태 및 작업강도 등을 측정, 분석하였으며, 건강검진 자료는 일반건강 진단 표를 이용하여 다음과 같은 결과를 얻었다. 1. 대상자의 평균연령은 54.9(남성 55세, 여성 54.7세)세였으며, 평균근무기간은 8.7년이 였으며, 조사대상 사업장중 71.5%가 50인 이하의 근로자를 고용하고 있는 사업장이 였다. 2. 소음에 폭로 대상자 중 31.3%가 90dB(A)인 노출 기준치을 초과하였으며, 그 외의 분진, 유기용제, 중금속 등의 노출기준치를 초과한 대상자는 없었다. 3. 조사대상자 중9%는 청력이 비정상이였으며, 78.6%의 대상자는 경작업과 중등작업을 하였으며, 52.7%는 전신을 이용하여 작업하는 형태였다.
