연구배경: 가정의학 전공의 수련 적절성 판단 기준의 하나는 수련 후 업무 수행이다. 전공의 수련에서 보완해야 할 점을 찾기 위하여 수련을 마친 가정의학 전문의가 진료에서 어려움을 느낀 문제를 조사하였다. 방법: 일개 대학병원에서 1993년 2월~1999년 2월에 수련을 마친 20명의 가정의를 대상으로 진료에서 어려움을 느낀 환자를 선택하여 미리 작성 배포한 설문지를 작성하게 하였다. 자료 수집기간은 1999년 3월~1999년 12월 중 매달 첫째 월요일에서 토요일 사이로 하였고, 가능한 1주에 2명 이상씩을 작성하도록 권유하였다. 대상 가정의에게는 매달 연구시작일 전에 설문지 작성기간을 전화로 알려주었다. 결과: 설문을 제출한 가정의는 개원 9명, 중소병원 근무 3명, 건강검진센터 근무가 1명이었으며, 회수된 설문지는 총 179건이었다. 진료에 어려움을 느꼈던 문제를 분야별로 감별진단, 치료계획 및 처방, 검사 및 해석, 교육, 신체검사 순서였다. 어렵다고 느끼는 이유로는, 증상지속, 지식부족, 술기 및 장비 부족, 불확실한 증상, 다양한 증상, 낮은 순응도, 부정적 감정, 의사소통 장애, 환자의 경제적 문제 순서였다. 해결방법에는 경과관찰, 타병원 전원, 문헌을 찾아봄, 지인에게 자문, 다른 자원 물색의 순서였다. 179건 중 어려움을 겪었던 상위 10가지 문제는 복통 15예, 저배통 14예, 기침 14예, 관절통 및 근육통 12예, 발진 11예, 호흡곤란 8예, 다발성 신체화 증상 8예, 급성 및 중이염 7예, 코막힘 6예, 어지러움 6예 등이었다. 빈도가 한 번뿐인 다양한 문제도 41예로 22.2%를 차지하였다. 결론: 본 전공의 수련 프로그램 수련 후 진료에서 어려움을 느낀 대표적인 문제는 치료에도 불구하고 증상이 지속되는 만성질환, 지식이나 술기 부족으로 감별진단이 어려운 급성질환 및 지역사회에서 드물게 발생하는 문제들이었다. Background: A previous Korean study recommended minimal 50 cases of Esophgogastroduodenoscopy (EGD) to achieve technical competency, based on director’s opinion. Therefore, this study was conducted to evaluate the achievement of EGD’s technical skills during the first 50 EGD cases. Methods: The data were collected from the first 50 EGDs consecutively performed by six family medicine residents trained the EGD procedure in one residency program from 2004 to 2006. Technical competency of the EGD procedure was evaluated in 4 steps divided with anatomical landmarks (the gastroesophageal junction, the antrum, the fundus of the stomach, and the second portion of the duodenum). Authors measured the procedure time of EGDs performed by residents in 4 steps and calculated the rate of success cases satisfied with author’s own criteria made by procedure time. Results: The success rate of EGD procedure in all steps was 26.7% at 10 EGDs, but sharply increased to 80.0% at 30 EGDs,reached 88.0% at 50 EGDs. Fail to perform esophageal intubation and retroversion to the fundus of the stomach was none after 30 EGDs. However, in steps from gastroesphageal junction to the antrum of the stomach and from the antrum of the stomach to the second portion of the duodenum, the failure rate kept up less than 10% during the first 50 EGDs. A statistically significant reduction of the procedure time in success cases was observed between 20 and 30 EGDs in all steps. Conclusion: The technical competency of EGD was achieved approximately 90% during the first consecutive 50 EGDs.
