        Ursodeoxycholic Acid Ameliorates Pain Severity and Cartilage Degeneration in Monosodium Iodoacetate-Induced Osteoarthritis in Rats

        Osteoarthritis (OA) is a degenerative joint disease characterized by a progressive loss of cartilage. And, increased oxidative stress plays a relevant role in the pathogenesis of OA. Ursodeoxycholic acid (UDCA) is a used drug for liver diseases known for its free radical-scavenging property. The objectives of this study were to investigate the in vivo effects of UDCA on pain severity and cartilage degeneration using an experimental OA model and to explore its mode of actions. OA was induced in rats by intra-articular injection of monosodium iodoacetate (MIA) to the knee. Oral administration UDCA was initiated on the day of MIA injection. Limb nociception was assessed by measuring the paw withdrawal latency and threshold. Samples were analyzed macroscopically and histologically. Immunohistochemistry was used to investigate the expression of interleukin-1β (IL-1β), IL-6, nitrotyrosine and inducible nitric oxide synthase (iNOS) in knee joints. UDCA showed an antinociceptive property and attenuated cartilage degeneration. OA rats given oral UDCA significantly exhibited a decreased number of osteoclasts in subchondral bone legion compared with the vehicle-treated OA group. UDCA reduced the expression of IL-1β, IL-6, nitrotyrosine and iNOS in articular cartilage. UDCA treatment significantly attenuated the mRNA expression of matrix metalloproteinase-3 (MMP-3), -13, and ADAMTS5 in IL-1β-stimulated human OA chondrocytes. These results show the inhibitory effects of UDCA on pain production and cartilage degeneration in experimentally induced OA. The chondroprotective properties of UDCA were achieved by suppressing oxidative damage and inhibiting catabolic factors that are implicated in the pathogenesis of cartilage damage in OA.

        GloSea5 모델의 자료처리 시스템 구축 및 시·공간적 재현성평가

        기상청에서 운영하고 제공하는 전지구 계절예측시스템 GloSea5 (Global Seasonal forecasting system version 5)자료를 활용하여 용담댐유역에 적용하고자 하였다. GloSea5는 예측자료(Forecast; 이하 FCST)와 과거재현자료(Hindcast; 이하 HCST)로 제공되며 공간 수평해상도는 N216 ($0.83^{\circ}{\times}0.56^{\circ}$)으로 중위도에서 약 60km이다. 이를 유역단위 물관리에 활용하기 위해서는 시 공간적인 상세화가 필요하므로 통계적 상세화 기법을 수행하여 변수가 갖는 계통적인 지역 오차를 보정함으로써 자료의 신뢰도를 향상시키고자 하였다. HCST자료는 앙상블 형태로 주어지며 용담댐 유역의 앙상블 평균에 대한 6번 격자의 통계적인 상관성($R^2=0.60$, RMSE=88.92, NSE=0.57)이 가장 높게 나타났다. 또한 계절분석시 여름철의 경우 원시 GloSea5 강우량이 600.1mm로 관측값인 816.1mm 대비 -26.5%로 가장 많은 차이를 보였으며 상세화 후 GloSea5 강우량은 -3.1%의 오차율을 보였다. 대부분의 과소 모의된 결과가 여름철 홍수기에 해당되는 강우로 상세화 이후 강우가 회복되는 매우 중요한 결과를 보였다. 계절별 Moran's I 지수를 이용한 공간적 자기상관분석 결과 역시 통계적으로 유의성 있는 공간적인 분포를 나타냄으로써 자료의 불확실성을 개선하고 시 공간적인 정확도와 타당성을 입증하였다. HCST기간에 대한 GloSea5의 앙상블 강우에 대한 신뢰도를 향상시킴으로써 수문학적인 영향을 평가하기 위한 자료로서의 충분한 가능성을 확보하였으며 이러한 시 공간적인 재현성에 대한 평가결과는 향후 유역단위 물관리를 위한 기초자료로서 매우 중요한 역할을 할 것이다. The GloSea5 (Global Seasonal forecasting system version 5) is provided and operated by the KMA (Korea Meteorological Administration). GloSea5 provides Forecast (FCST) and Hindcast (HCST) data and its horizontal resolution is about 60km ($0.83^{\circ}{\times}0.56^{\circ}$) in the mid-latitudes. In order to use this data in watershed-scale water management, GloSea5 needs spatial-temporal downscaling. As such, statistical downscaling was used to correct for systematic biases of variables and to improve data reliability. HCST data is provided in ensemble format, and the highest statistical correlation ($R^2=0.60$, RMSE = 88.92, NSE = 0.57) of ensemble precipitation was reported for the Yongdam Dam watershed on the #6 grid. Additionally, the original GloSea5 (600.1 mm) showed the greatest difference (-26.5%) compared to observations (816.1 mm) during the summer flood season. However, downscaled GloSea5 was shown to have only a -3.1% error rate. Most of the underestimated results corresponded to precipitation levels during the flood season and the downscaled GloSea5 showed important results of restoration in precipitation levels. Per the analysis results of spatial autocorrelation using seasonal Moran's I, the spatial distribution was shown to be statistically significant. These results can improve the uncertainty of original GloSea5 and substantiate its spatial-temporal accuracy and validity. The spatial-temporal reproducibility assessment will play a very important role as basic data for watershed-scale water management.

        o-Nitrobenzyl Ester기를 이용한 감광성 폴리이미드의 합성과 그 특성

        The photosensitive polyamide precursors were synthesized from pyromellitic acid dich-loride di(o-nitrobenzyl) ester with o-nitrobenzyl group and two amines such as m-phenylene diamine and m-diaminobenzophenone by interfacial polycondensation reaction. The polymers obtained were able to be converted into usual polyimide by curing The photoreaction mechanism of photosensitive polyimide precursors was investigated by UV and NMR spectroscopy. It was considered from the absorbance of UV at 23O~28Onm that characteristic rearrangement in the o-nitrobenzyl ester occurred m this system. Their thermal properties were studied using thermogravimetry, both polylmide precursors exhibited the weight loss in two stages. The second weight loss depends on the thermal degradation of polyimide. Noticeable weight loss was observed at about 500˚C. After preparing photoresist from these polyimide precursors synthesized, normalized thickness remaining were measured with varying exposure energy to determine whether the polymers can be used as a commercial photoresist.

        Oncologic Safety of Endoscopic Removal of Infiltrated Tumor Onto the Periorbita Using Bipolar Cauterization Technique in Sinonasal Malignancy

        The periorbita has been regarded as the crucial structure in decision of orbital exenteration in the patients with sinonasal malignancies. The purpose of this study is to evaluate the oncological safety of endoscopic removal using bipolar cauterization in tumor encroaching on the periorbita without orbital sacrifice through analysis of long-term follow-up results of 5 cases. Methods:Retrospective review including demographic data, followup results, and local recurrence were performed on the 5 patients of advanced sinonasal cancer who showed bony orbital wall destruction and infiltration onto the periorbita but not transgressing into the orbital fat. Partial or total maxillectomy with orbital preservation was conducted in each patient. The tumor was dissected along the periorbita using bipolar nasal coagulation forceps by one senior surgeon under the endoscope. Preoperative CT and MRI scan were performed in all cases and retrospectively compared with intraoperative and permanent pathologic reports. Results:The mean age of tumor onset was 51.8 (39-74) years. Histopathology included four squamous cell carcinomas and one adenoid cystic carcinoma. Follow-up period ranged from 31 to 219 months (mean 112.6months). All cases showed no local recurrence in the orbit but one patient had local recurrence in the pterygopalatine fossa and the other had local recurrence in the neck. Conclusions:Endoscopic removal of infiltrated tumor onto the periorbita using bipolar cauterization technique might be oncologically safe technique in advanced maxillary cancer infiltrated onto the periorbita which is not invading the orbital fat.

        비출혈을 주소로 내원한 환자에서 확인된 비부비동 IgG4-연관 질환 1예

        Immunoglobulin (Ig) G4-related disease is a systemic chronic inflammatory disease characterized by sclerosing lesions by IgG4 positive plasma cells and T lymphocytes. It is rarely described in the sinonasal cavity and the paranasal sinus. Here we report a case involving a 69-year-old woman whose chief complaint was recurrent nasal bleeding for 2 years. Her nasal cavity showed erosion of nasal septum as well as the synechia in between the nasal septum and the lateral nasal wall with severe edematous change in the left inferior turbinate. Imaging finding showed bilateral soft tissue density in the maxillary and ethmoid sinuses and mucosal swelling of left nasal cavity. Her IgG4 level was elevated and proteinase-3 Ab was positive. Histopathological examination from the left inferior turbinate showed dense lymphoplasma cell infiltration and storiform fibrosis. Immunostaining for IgG4 and IgG revealed numerous plasma cells positive for IgG4 (more than 90 cells per high power field) and IgG4/IgG ratio of 0.7. Otorhinolaryngologist should consider IgG4 related disease as one of the differential diagnosis in case of bloody rhinorrhea with mucosal erosion or synechia inside the nasal cavity.

        Periarticular Osteoporosis Is a Prominent Feature in Early Rheumatoid Arthritis: Estimation Using Shaft to Periarticular Bone Mineral Density Ratio

        We aimed to quantify periarticular osteoporosis and investigate its significance in 45patients with rheumatoid arthritis (RA) and 106 controls. Dual-energy X-ray absorptiometry (DXA) was used to determine the ratio of shaft to periarticular bone mineral density (BMD) as an index of periarticular demineralization. Periarticular osteoporosis was measured by conventional radiography. The BMDs of shaft and periarticular regions in eight designated areas on proximal phalanges were quantified. Clinical variables were examined to identify risk factors for periarticular osteoporosis. The assessment of periarticular osteoporosis on X-ray images reached a moderate degree of interobserver agreement among four physicians (κ = 0.47). For BMD quantification, we designed three types of mathematical formulae: the ratio of shaft to periarticular BMD,the mean of the ratios, and the ratio of the sums. These ratios were significantly higher in the patients with early RA (disease duration ≤ 3 yr) than in controls (P < 0.01). The findings were not as distinctive in patients with established RA. Body mass index,cumulative dose of corticosteroid, and C-terminal telopeptide were correlated with BMD ratios. Conclusively, DXA-assisted localized quantification and BMD ratio calculations are feasible for assessing periarticular demineralization. Periarticular osteoporosis is a relatively distinctive feature of early RA.

        만성신부전으로 혈액투석 중인 판상 건선 환자에서 발생한 저용량 Methotrexate 독성 1예

        Methotrexate is an effective treatment for psoriasis and psoriatic arthritis. However, as methotrexate is mainly eliminated by the kidneys, patients with chronic kidney disease may experience hematologic complications such as myelosuppression due to methotrexate toxicity. A 58-year-old male psoriasis patient with end-stage renal disease undergoing hemodialysis was treated with low-dose MTX (7.5 mg/week) and developed pancytopenia after 5 weeks of treatment. His serum MTX level was < 0.040 umol/L. While MTX toxicity typically occurs within 7~14 days, in this case, it occurred one month later. Despite the low levels of MTX in his blood, myelosuppression led to pancytopenia, which resulted in the development of pneumonia with Pseudomonas aeruginosa infection. Ultimately, this led to septic shock and the patient’s death. In conclusion, since low-dose MTX toxicity in CKD patients is challenging to predict, we should exercise extra caution.
