Rectal tenesmus is a persistent, painful and ineffectual sensation of straining at stool or opening of the bowels. The pain is usually spasmodic in nature and most commonly en- countered in patients with carcinoma of the rectum or other pelvic organs. In 1988, Bristowand Foster reported that patients with severe spasmodic painful tenesmus were relieved with chemical sympathectomy. In 1990, Plancarte introduced block of Gangli- on impar. This technique is proposed as an alternative means of managin#g localized peri- neal pain of sympathetic origin. Ganglion impar block was performed on a 54-year-old female patient when analgesic or psychotropic drugs failed to control the symptoms of post-traumatic severe spasmodic pain- ful tenesmus. Postoperatively, patient was free of tenesmoid pain for only 7 days. We then performed neurotomy by RF lesion generator which provided complete pain relief.

The purpose of this study was to investigate the distribution of stress within the regenerated bone surrounding the implant using three dimensional finite element stress analysis method. Using ANSYS software revision 6.0 (IronCAD LLC, USA), a program was written to generate a model simulating a cylindrical block section of the mandible 20 mm in height and 10 mm in diameter. The 5.0 × 11.5-mm screw implant (3i, USA) was used for this study, and was assumed to be 100% osseointegrated. And it was restored with gold crown with resin filling at the central fossa area. The implant was surrounded by the regenerated type IV bone, with 4 mm in width and 7 mm apical to the platform of implant in length. And the regenerated bone was surrounded by type I, type II, and type III bone, respectively. The present study used a fine grid model incorporating elements between 250,820 and 352,494 and nodal points between 47,978 and 67,471. A load of 200N was applied at the 3 points on occlusal surfaces of the restoration, the central fossa, outside point of the central fossa with resin filling into screw hole, and the functional cusp, at a 0 degree angle to the vertical axis of the implant, respectively. The results were as follows: 1. The stress distribution in the regenerated bone-implant interface was highly dependent on both the density of the native bone surrounding the regenerated bone and the loading point. 2. A load of 200N at the buccal cusp produced 5-fold increase in the stress concentration at the neck of the implant and apex of regenerated bone irrespective of surrounding bone density compared to a load of 200N at the central fossa. 3. It was found that stress was more homogeneously distributed along the side of implant when the implant was surrounded by both regenerated bone and native type III bone. In summary, these data indicate that concentration of stress on the implant - regenerated bone interface depends on both the native bone quality surrounding the regenerated bone adjacent to implant and the load direction applied on the prosthesis.

This study was conceived by the desperate need for specific records considering the life and living of fishermen on the Gogunsan Archipelago. Gogunsan Archipelago had to go through ongoing and rapid changes under the influence of Saemangeum Reclamation Project that brought about huge transformation in the west coast during the 21st century. This study sorted out the oral contents of elderly fishermen that earned a living through fishing activities around the archipelago throughout their lives. The investigators visited Jangja, Seonyu, Munyeo, Bangchuk, Myeong and Mal Islands, collecting oral records or so-called life history and put them together. Other records regarding witnesses of elderly fishermen`s fishing styles and distribution were also recorded and put together. The study presented the story of elderly fishermen`s lives and living in order to communicate with the future generations through storytelling. It tried to recreate the traditional culture passed down from many years ago in today`s language, and offer an opportunity for people to recall the preciousness of traditional values. The purpose was to use storytelling elements in order to understand the present and prepare for the future. By observing the past, the study hoped to help the archipelago meet the demands of the times involving changes in the modern civilization. In this sense, many stories regarding the life and living of fishermen around the archipelago needed to be collected. The present study translated the stories into oral narratives and contemplated upon how to utilize storytelling and translate them into cultural content business. Based on the research experience on Festival of the Islands` Shamanist Customs, which grouped four islands (Seonyu, Munyeo, Jangja, and Daejang Island) of the archipelago, that enjoyed a high popularity and a large group of tourists, into a single traditional cultural content, the investigators thought over on how to highlight the individuality of the four islands and further on their neighboring ones, while at the same time grouping the advantages of a good cultural archetype around a single theme. The study explored one-source and multi-use approaches in order to analyze the development of traditional cultural content, and made use of those contents by introducing various storytelling elements.
A CLINICOSTATISTICAL STUDY OF NON-ODONTOGENIC CYSTS : THE EXPERIENCE OVER 10 YEARS

본 연구는 1987년 1월부터 1997년 9월까지 본과에 내원하여 조직검사 시행결과 비치성낭종으로 진단된 90명의 환자에서 각 비치성낭종의 연령분포, 성별비, 발생분포, 임상증상, 치료법을 조사하여 다음과 같은 결과를 얻었다. 1. 비치성낭종 환자의 빈도는 총 402명의 낭종환자중 90명 (22.4%)으로 나타났다. 2. 총 비치성낭종 환자의 연령분포는 7-77세이고, 평균연령은 36세였다. 3. 가장 흔한 증상은 무통성의 종창 (62.2%)이었다. 4. 호발부위는 하순 (17.8%), 협점막 (15.6%), 상악동 (15.6%)의 순이었다. 5. 모든 치료는 외과적 절제술을 시행하였다.

The purpose of this study was to analyze the stress pattern in different bone densities surrounding fin-type implant fixtures under the vertical and inclined loads (30) of 200N. Von-Mises stress, the pricipal stress, and the displacement on the implant fixtures under the loads were calculated by using the finite element method. Four different types of bicon implant fixture were used for this study. The geometries of implant fixtures to develop the model were used by a sales brochure and profile project. Three-dimensional finite element model of the mandible was developed with 6.0 mm implant in diameter wurrounded by approximately 2.5 mm of bone. Bone densities were classified according to the elastic modulus of the tree. The finite element program MSC PATRAN (MSC, Software Corp., USA) were used for analysis of stress distribution. The value of the Von-Mises stress, the pricipal stress, and the displacement on the implant fixtures under the vertical and inclined loads were decreased when the diameter of implant fixture was increased, and increased when the elastic modulus was decreased. The stress on implant fixture under the vertical and inclined loads was distributed through the length of implant fixtures in D3 and D4. The distribution of stress was influenced by the direction of loads. In the wide diameter of implants, the stress was developed at outer surface of bone. In conclusion, this study suggest that stress developing on the peri-implant tissues might be influenced by the dimension of implant, elastic modulus of bone, and direction of loads.

Facial nerve paralysis following the administration of a local anaesthetic can be alarming. By reading reports of such incidents, dentists who find themselves in similar situations will be able to reassure their patients and act accordingly. This article reviews the classifications of anesthetic complication, local complications, etiology, prevention, treatment of facial nerve paralysis fellowing the administration of a local anaesthetic. A thorough knowledge of the relevant anatomy pertinent to the various injections used in dental surgery is essential.

A new field in dental implantology is developing with the goal of finding new ways to improve the osteoconductivity of bone substitutes and to study new molecules able to dictate cellular differentiation and improve bone regeneration. The real future in bone regeneration seems to be in connection with the rhBMP 2s, currently obtained by synthesis using recombinant DNA. Since the first rhBMP 2 studies in humans by Boyne, There are many studies for bone regeneration at oral and maxillofacial area. The rhBMP 2 is widely used at sinus augmentation, alveolar bone defect, and socket preservation.
