증례 : 피하낭종과 관절염의 형태로 나타난 Sweet 증후군 1예

Sweet 증후군은 급성 열성 호중구성 피부질환으로 다양한 임상양상을 보일 수 있다. 피하 낭종을 포함하는 피부 병변, 관절염 및 구강내 궤양을 동반한 Sweet 증후군은 국내보고가 드물며 저자들은 초음파와 조직 생검을 통해 진단 후 스테로이드 전신 투여로 호전된 환자 1예를 경험하였기에 문헌고찰과 함께 보고하는 바이다. Sweet`s syndrome is characterized by a combination of clinical and pathologic findings including fever, neutrophilia, tender erythematous skin lesions, and a diffuse infiltration of mature neutrophils in the upper dermis. Numerous diseases and clinical manifestations have been associated with the disease; however, Sweet`s syndrome associated with subdermal cystic skin lesions and arthritis is rare. A 71-year-old female patient presented with fever, erythematous plaques, multiple hypoglossal ulcers, and arthritis in both ankles. The skin lesions were variously sized areas of erythematous swelling on the forehead, back, and left shoulder. Musculoskeletal sonography revealed hypervascularity and a subdermal cyst in the erythematous plaque on her back. The results of a skin biopsy indicated the presence of mature neutrophilic infiltration in the dermis and thus led to the diagnosis of Sweet`s syndrome. We herein present an unusual case of Sweet`s syndrome presenting as erythematous subdermal cystic lesions, multiple hypoglossal ulcers, and bilateral ankle arthritis with a literature review.

본 연구는 상아질에 대한 3종의 단일 단계 접착제의 미세인장 결합강도를 상호 비교하였다. 15개의 발거된 상·하악 대구치의 교하면측에 있는 표충 상아질을 노출시킨 후, 사용된 접착제에 따라 3개의 군 (Xeno 군-Xeno Ⅲ, Prompt군-Adper Prompt L-Pop, AQ군-AQ Bond)으로 분류하여 복합레진을 축조하였다. 각 시편을 막대모양으로 제작하였고, 각 군 당 20개의 시편을 이용하여 미세인장 결합강도를 측정하여 다음과 같은 곁과를 얻었다. 1. 상아질에 대한 각 관의 미세인장 결합강도는 Xeno 군에서 48.78±9.83 ㎫, Prompt 군에서 30.22±4.52 ㎫, AQ군에서 26.31±7.07 ㎫로 나타났다. 2. Xeno 군은 Prompt 군과 AQ 군보다 통계학적으로 높은 미세인장 결합강도를 나타냈다 (p<0.05). 3. Prompt 군과 AQ 군의 미세인장 결합강도 간에는 통계학적으로 유의차가 없었다 (p>0.05). This study compared the microtensile bond strength (μTBS) of three single step adhesives to dentin. Occlusal superficial dentin was exposed in fifteen human molars. They were assigned to three groups by used adhesives: Xeno group (Xeno Ⅲ), Prompt group (Adper Prompt L-Pop), AQ group (AQ Bond). Each adhesive was applied to dentin surface, and composite of same manufacturer was constructed. The bonded specimens were sectioned into sticks with an interface area approximately 1 ㎟, and subjected to μTBS testing with a crosshead speed of 1 mm/minute. The results of this study were as follows: 1. The μTBS to dentin was 48.78±9.83 ㎫ for Xeno Ⅲ, 30.22±4.52 ㎫ for Adper Prompt L-Pop, and 26.31±7.07 ㎫ for AQ Bond. 2. The mean μTBS of Xeno group was significantly higher than that of Prompt group and AQ group (p<0.05), 3. The abnormal change of vascular smooth muscle cell (VSMC) behavior is an important cellular event leading to neointimal hyperplasia in atherosclerosis and restenosis. Plantamajoside (PMS), a phenylethanoid glycoside compound of the Plantago asiatica, has been reported to have anti-inflammatory, antioxidative, and anticancer activities. In this study, the protective effects of PMS against intimal hyperplasia and the mechanisms underlying the regulation of VSMC behavior were investigated. MTT and BrdU assays were performed to evaluate the cytotoxicity and cell proliferative activity of PMS, respectively. Rat aortic VSMC migrations after treatment with the determined concentration of PMS (50 and 150 μM) were evaluated using wound healing and Boyden chamber assays. The inhibitory effects of PMS on intimal hyperplasia were evaluated in balloon-injured (BI) rat carotid artery. PMS suppressed the proliferation in platelet-derived growth factor-BB-induced VSMC, as confirmed from the decrease in cyclin-dependent kinase (CDK)-2, CDK-4, cyclin D1, and proliferating cell nuclear antigen levels. PMS also inhibited VSMC migration, consistent with the downregulated expression and zymolytic activities of matrix metalloproteinase (MMP)2, MMP9, and MMP13. PMS specifically regulated MMP expression through p38 mitogen-activated protein kinase and focal adhesion kinase pathways. Tissue inhibitor of metalloproteinase (TIMP)1 and TIMP2 levels were upregulated via Smad1. TIMPs inhibited the conversion of pro-MMPs to active MMPs. PMS significantly inhibited neointimal formation in BI rat carotid arteries. In conclusion, PMS inhibits VSMC proliferation and migration by upregulating TIMP1 and TIMP2 expression. Therefore, PMS could be a potential therapeutic agent for vascular atherosclerosis and restenosis treatment.

Stent thrombosis (ST) is the sudden occlusion of a stented coronary artery because of thrombus formation. ST is a rare, but can result in life-threatening complications. Early stent thrombosis occurs frequently after primary percutaneous coronary intervention (PCI) for acute ST-segment elevation myocardial infarction (STEMI). Here, we report a case of a 59-year-old man with acute stent thrombosis immediately after primary PCI with drug-eluting stent for acute STEMI.

This study evaluated the marginal microleakage of five single step adhesives. Class V cavity preparations with occlusal margins in enamel and gingival margins in dentin were prepared on both buccal and lingual surfaces of extracted human molar teeth. Prepared teeth were randomly divided into five groups and restored using one of the single step adhesives and composite resins: Prompt L-Pop/Filtek Z-250 (Group 1), AQ Bond/Metafil CX (Group 2), One-Up Bond F/Palfique Toughwell (Group 3), Futurabond/Admira (Group 4), Xeno III/Spectrum TPH (Group 5). The restored teeth were thermocycled. Microleakage was assessed by dye penetration using 2% methylene blue dye solution. The teeth were bisected buccolingually and evaluated for microleakage under steromicroscope. The data were statistically analysed by Kruskal-Wallis test and Mann-Whitney tests. The results of this study were as follows; 1. Microleakage of enamel margins in group 3 was statistically higher than that in groups 1, 2, 4, 5 (p < 0.05) 2. Microleakage of dentin margins in group 1 was statistically higher than that in groups 2, 5, and that in group 3 was statistically higher than that in groups 2, 4, 5 (p < 0.05). 3. Dentin marginal microleakage was higher than enamel marginal microleakage in all experimental groups. In conclusion, Prompt L-Pop showed the least leakage at enamel margin, and AQ Bond showed at dentin margin in this study. Marginal miroleakage in dentin was higher than that in enamel.
