Purpose: The aim of this study was to present specific criteria for setting goals for hand rehabilitation by comparing the degree of difference in grip force, coordination, and dexterity between the dominant and non-dominant hand according to gender. Methods: We recruited 100 healthy adults in their 20s and 30s. A handheld digital dynamometer was used to evaluate the grip force of each of the dominant and non-dominant hand, a chopsticks manipulation test was used to evaluate coordination, and the Purdue Pegboard test was used to evaluate agility. Results: In all subjects, the grip force, coordination, and dexterity showed statistically significant difference (p <0.01) between the dominant and non-dominant hand. In the comparison according to gender, both male and female dominant and non-dominant hands showed statistically significant differences in grip force, coordination, and dexterity (p <0.01). In the comparison according to grip force, there was a statistically significant difference between the dominant and non-dominant hand, and men showed stronger result values in both hands compared to women (p <0.01). In the comparison according to coordination, there was no statistically significant difference between the dominant and non-dominant hand in men and women (p >0.05). In the comparison according to dexterity, there was a statistically significant difference between the dominant and non-dominant hand, and women were shown to be faster in performance time with both hands, compared to men (p <0.01). Conclusion: Differences according to gender exist in grip force and dexterity but not coordination, and differences between dominant and non-dominant hand exists across all measurements. The results suggest setting a recovery goal according to dominance and gender during rehabilitation of hand function.
warfarin 과량 사용후 발생한 Purple Toe 증후군과 동반된 급성 신부전 1예

Classically, purple toe syndrome is known to be an adverse effect of warfarin anticoagulant and it is due to direct cytotoxic effect of warfarin derivatives to capillary wall. But purple toe syndrome is recently said to be associated with cholesterol ernbolism which rr.ay be due to warfarinization and/or thrombolytic therapy and/or intraarterial procedure. In case of cholesterol embolism, acute renal failure is an omnious complication and usually irreversible. An 82-year old man visited us for painful both lower extremities and abdominal pain with dark colored stool. He had been on warfarinization for a month since the recent attack of cerebellar infarction. Inadvertently, he took his medicine excessively to relieve recently developed common cold. He developed a refractory renal failure associated with purple toe syndrome. Thirty nine days later, he died of renal failure. Although it was not confirmed by skin biopsy, the excessive warfarinization seems to be causally related to this purple toe syndrome.
FRC-포스트의 표면처리가 레진시멘트와의 접착력에 미치는 영향

연구목적: Silane 처리가 FRC-포스트와 레진 시멘트 간의 결합강도에 미치는 영향을 알아보고자 하였다. 연구 재료 및 방법: FRC-포스트로 LuxaPost (DMG)와 Rely-X Fiber Post (3M ESPE)를 사용하였으며 자가 접착 레진 시멘트인 Rely-X Unicem (3M ESPE)과 전통형 레진시멘트인 Rely-X ARC (3M ESPE)와의 Rely-X Ceramic Primer (3M ESPE)를 이용한 표면처리 유무에 따른 결합강도를 push-out test를 실시하여 측정하고 independent samples t-test 및 one-way ANOVA와 Scheffe 사후검정을 95% 유의수준에서 실시하였다. 결과: 실험 결과 Rely-X Fiber Post를 사용한 실험군에서 silane 처리와 관계없이 Rely-X ARC를 적용하였을 때, 다른 실험군들에 비해 높은 결합 강도가 나타났으며 LuxaPost를 사용한 실험군에서 각각의 레진 시멘트에 대해 silane 처리를 하였을 때 시행하지 않은 실험군에 비해 높은 결합 강도가 나타났다. 결론: 이상의 실험 결과로 볼 때 미리 silane 처리된 LuxaPost를 사용하더라도 silane 처리를 시행하는 것이 좋은 것으로 생각된다. Objectives: The purpose of this study was to evaluate the effect of surface treatment of FRC-Post on bonding strength to resin cements. Materials and Methods: Pre-surface treated LuxaPost (DMG), Rely-X Fiber Post (3M ESPE) and self adhesive resin cement Rely-X Unicem (3M ESPE), conventional resin cement Rely-X ARC (3M ESPE), and Rely-X Ceramic Primer (3M ESPE) were used. After completing the surface treatments of the posts, posts and resin cement were placed in clear molds and photo-activation was performed. The specimens were sectioned perpendicular to the FRC-Post into 2 mm-thick segments, and push-out strength were measured. The results of bond strength value were statistically analyzed using independent samples t-test and oneway ANOVA with multiple comparisons using Scheffe's test. Results: Silanization of posts affect to the bond strength in LuxaPost, and did not affect in Rely-X Fiber Post. Rely-X ARC showed higher value than Rely-X Unicem. Conclusions: Silanization is needed to enhance the bond strength between LuxaPost and resin cements.
불안장애 환자에서의 치료약제 중복사용에 대한 후향적 의약품사용평가 연구

The present study was aimed to examine the occurrence and influencing factors therapeutic duplication (TD) of medications for anxiety disorders by analyzing the relevant prescription data. In this study, the prescription data issued on March 19, 2008 in domestic medical institutes were utilized. TD was defined as more than two medications under the same therapeutic classification per prescription based on the Anatomical Therapeutic Classification (ATC) code. The assessment of TD was performed based on the number of cases and on the ratio determined. To identify the influencing factor of TD, the variables related to the differences in the TD ratio were analyzed based on the results of the Chi-Square test conducted with the variables; patients, medical institutes, diseases, and treatments. The number and ratio of TD were determined to be 1,333 out of the total of 19,219 anxiety disorder cases, and 6.94%, most cases involving benzodiazepine derivatives, respectively. The TD ratio was found to be higher in relation to males than to females. Patients with national health insurance benefits have a higher TD ratio compared to the medical-aid beneficiaries. The TD ratios were highest in clinics, psychiatry divisions, and Gyeongsang district. The TD ratio of the cases with more than two anxiety disorders was found to be higher than that of the cases with only one anxiety disorder. As the number of medications per prescription increased, the TD ratio was shown to have become gradually higher. In conclusion,in order to prevent TD, the concurrent DUR system should be implemented. The prescribers and pharmacists must be educated regarding duplicated medications to promote the safe and effective use of medicines, without unnecessary TD.
