      • 고빌리루빈혈증을 동반한 자가면역성 간염 1례

        자가면역성 간염은 대개 만성 경과를 가지며, 혈중 자가면역항체와 혈청 글로불린치의 상승, 그리고 조직학적으로 괴사 염증성 변화를 특징으로 하는 질환으로 아직 정확한 병인이 밝혀져 있지 않은 상태이다. 발병연령은 대개 젊은 영자(15-25세)에서 호발한다. 이 질환은 급성 간염의 임상경과를 보일 수 있으나, 심한 급성 간염이나 전격성 간염으로도 나타날 수 있는데, 이 경우 아주 나쁜 예후를 보인다고 한다. 치료는 자가면역성 간염 임상 경과의 다양성이나 병인, 병리기전의 불확실성에도 불구하고 대개 steroid 치료에 반응하는 것으로 알려져 있으며 대개 80%의 관해율을 나타내며 궁극적으로 간경변으로의 진행을 막을 수 있는 것으로 보인다. 저자들은 58세 남자에서 발생한 급성의 경과를 가지고 심한 황달을 동반한 자가면역성 간염을 경험하였기에 문헌고찰과 함께 보고하는 바이다. Autoimmune hepatitis (AIH) is a chronic necroinflammatory liver disorder of unknown cause associated with circulating autoantibodies and a high serum globulin level. The age of onset of AIH show a peak between the age of 15 and 25 years. AIH can develop and be manifested as acute hepatitis, but severe form of acute hepatitis or fulminant hepatic failure has a poor prognosis. Although AIH is likely to progress from chronic active hepatitis to cirrhosis, steroid therapy can control the disease activity, prolong survival, improve the quality of life , and defer liver transplantation. In the present report we describe a 58-year-old man who admitted because of progressive jaundice and fatigue. He was diagnosed with AIH from laboratory test result showing positivity for antinuclear antibodies, anti-smooth muscle antibodies, and negativity for hepatitis viral markers and from liver biopsy. Steroid therapy, oral administration of prednisolone, was effective in improving the liver function test. Following liver biopsy 6 months after onset shows markedly improved necroinflammatory activity.

      • 경제적 부가가치의 유용성에 관한 연구

        Value of an enterprise can be represented by its market share, net sale and stock price etc. Among them stock price can be considered as representative index reflecting actual value of an enterprise. For this reason there should be adequate compensation to the investors for their investment. Under such consept EVA(Economic Value Add) is calculated by subtracting the cost of capital from the NOPAT(Net Operating After Tax). The aim of this study is to express effectiveness of EVA theoretically and indentify its effectiveness as an explanation for stock value (Cummulated Abnormal Return : CAR). This study show theoretically the greatest of utility of equity and harmony of executive and shareholders. the method of the empirical study is the regression analysis. therr are a dependent variable is CAR, independents variables are Price Earing Ration(PER), Return On Equity(ROE), Return on Asset(ROA), Return On Sales(ROS), EVA and Equity Ratio(ER). This study is to analysis of the study is that EVA is useful to information users. In order to test the above major hypothesis, the following minor hypothesis is divided. H₁ : EVA have information added for CAR. H₂ : EVA is no difference between brisk market and recessed market period. In conclusion, We have found the following though the empirical study. Result of analysis showed that EVA has overall effectiveness as an explanation on stock value (CAR) on stocks and it is identified that investors are making their investment after receiving advance information on difference between investment profitability and cost of capital. Analysis is made by dividing the period into brisk market and recessed market period. During the brisk market period EVA is appeared to have significant effectiveness as explanation on stock value(CAR), but during recessed market period it appeared to have no such significance as effective explanation. It is shown during brisk market period investment information on the difference between profitability rate and cost of capital rate is furnished in advance and being reflected to decision making by investors. And such result comes from the fact that during the period of recessed market due to presence of mixed information over-reaction of the investors and difference among enterprises with respect to its calculation of capital is deepened. For the purpose of increasing the value of an enterprise more effort shall be made to establish the composition ratio between own capital and other's capital properly and increase ratio of investment profitability to a level higher than cost of capital to bring about value of enterprise.

      • IgA 腎病症에 對한 病理學的 및 免疫病理學的 硏究

        A clinicopathologic analysis for 88 cases of IgA nephropathy has been carried cut from a series of 333 renal biopsy specimens which were studied at Department of Pathology, College of Medicine, Chungnam National University, during a period of 54 months from October 1983 to March 1988. The results were as follows: 1. IgA nephropathy was 26.4% of renal biopsy specimens and 30.4% of primary glomerulonephritis. It was common in young adults and 77.3% of patients were between 16 years and 39 years. Male to female ratio was 1.1 : 1. 2. Modified classification of Meadow et al. for Henoch-Schoenlein nephritis was adopted for the histologic grading of glomerular lesions. The result was as follows; Grade I 5.7%, Ⅱ 46.6%, M 45.5%, Ⅳ 1.1%, an V 1.1%. The clinical class was possible in 70 cases; Class B38.6 %, C 61.4%. 97.1% of these. 70 cases were within the histologic Grade Ⅲ. 3. The results of immunofluorescent study were as follows; All cases showed granular mesangial deposits of IgA. Complement 3 was found, in same manner as IgA, in 83 0%, fibrinogen 23.9%, IgM 18.23%, and IgG 3.4%. 10.2% of these cases showed clumps of fluorescent deposits involving the mesangium and continuous loops. 4. Among the chief complaints on admission, gross hematuria was observed in 59.1%, microscopic hematruia 39.8%, and proteinuria 69.3%, including nephrotic range protein uria 6.8%. Along the histologic grading, the frequency of gross hematuria was different, as follows; Grade I 100.0%, Grade Ⅱ 65.9%, and Grade Ⅲ 47.5%. Statistically, these differences were significant(P<0.01) On serologic study, hepatitis B surface antigen was positive in 6.7% and level of serum IgA was significantly increased in 27.6%.

      • Craves병 환자에서 백혈구와 갑상선 조직의 HLA-DR β 유전자 부위의 비교

        정상 갑상선세포에서는 HLA-DR 항원이 발현되지 않았으나, 그레이브스병 환자의 갑상선세포 표면에는 HLA-DR 항원이 발현됨이 보고되어, 이러한 현상은 이 질환의 자가면역 기전에 중요한 역할을 하고 있을 것으로 생각되고 있다. 한편 최근 DR이나 DQ 유전자의 상부 159kb 이내에는 이들의 발현을 조절하는 유전자가 존재함이 알려져 있고, 이 부위의 구조적인 변화로 인하여 DR DQ 유전자의 발현을 유도하는 여러 가지 핵내 인자들과 interferon-r 와 같은 외부인 자들이 보고되고 있다. 그러나 그레이브스병에서 이들 유전자 부위의 구조적인 변화에 관해서는 아직 보고가 없다. 이에 본 연구자 등은 이러한 가능성 여부를 규명하고자 2명의 전형적인 그레이브스병 환자의 수술 우 얻어낸 갑상선 조직에서 RNA를 분리한 우 DR β유전자를 소식자로 northem blotting을 하여 mRNA의 발현을 관찰하였으며, 말초혈액 백혈구와 갑상선 조직으로부터 분리된 DNA를 EcoRI BamHI. HindⅢ PvuⅡ TaqI, PstI등의 6가지 제한효소로 소화한 후 DR β유전자를 소식자로 하여 RFLP 양상을 비교한 결과, 환자 모두에서 mRNA의 발현이 관찰되었으나, 환자 모두에서 6가지 제한효소에 의한 RFLP 양상이 동일하였다. 이러한 결과는 Graves병 환자의 DR 유전자의 발현에 있어서 이 유전자 부위의 구조적인 변화가 관여할 가능성이 적음을 시사하는 사실이라고 사료되나 향후 더 많은 예와 더 많은 제한효소를 이용한 주시가 필요할 것이다. The requirement for major histocompatibility antigen class Ⅱ molecules in the recognition of antigen by helper T cells suggests that the expression of class Ⅱ antigen may be important in the initiation and prolongation of immunopathology. HLA class Ⅱ antigenes are expressed on the surface of thyrocytes of the patients with graves disease. The increased expression of class Ⅱgene can be induced by trans acting factor such as interferon However the possibility of rearrangement of their regulatory genes has not been explored so far. We studied the mRNA expression in the thyrocytes of 2 patients with Graves' disease and compared the restriction fragment length polymorphism (RFLP) between thyroid and peripheral leukocyte DNA. The prominent expression of mRNA was observed in the thyroid tissues of all the two patients. But we did not find any difference in RFLP pattern in both patients. These results suggest the possibility that the rearrangement of the regulatory gene located in the upstream of DR- β gene can be a role in expression of DR antigen is less likely.

      • 세포질내 정자주입법(ICSI)에 있어서 정자흡입 및 난자내 주입방법에 관한 연구

        Study on Method of Sperm Aspiration and Injection into an Oocyte in Intracytoplasmic Sperm Injection(ICSI) Immobilization of spermatozoa prior to intracytoplasmic sperm iniection(ICSI) sometimes results in crooked tail and this makes it difficult to aspirate sperm into an injection pipette tail first. Head-first sperm aspiration into an injection pipette avoid this problem due to the bigger size of the sperm head. The effect of head or tail-first sperm injection into an oocyte on fertilization cleavage, percentage of grade I embryos and development to blastocyst stage in ICSI program has been studied. A single living immobilized spermatozoa from oligoasthenozoospermic patient was injected into an oocyte head-first or tail-first according to the treatment. Eighteen hours after microinjection, oocytes ware inspected for survival and fertilization Fertilized oocytes with two pronuclei were cultured in 30μl drop of mHTF supplemented with 10% heat-inactivated follicular fluid(FF) at 37℃. On day 2. embryo transfer was performed with cleaved embryos. The remaining 2-8 cell stage embryos were co-cultured with BRL cells in mHTF + 10% FF for 72 hours and the developmental stage was observed. The data were analyzed by Analysis of Variance. A total of 164 oocytes from 36 cycles were assigned to earth treatment and ICSI was performed(88 head-first, tail-first). The rates of normal fertilization were 81.8% and 76.3% for head-first and tail-first, respectively. Of the fertilized oocytes, the percentage of cleaved embryos and the percentage of grade 1 embryo among cleaved embryos were 88.9% and 68.8%, 93.1% and 74.1% for head-first and tail-first, respectively. Of the 2-8 cell embryos cultured, 44.4%(16/36) and 50.0%(10/20) for head first and tail first, respectively developed to blastocyst stage. There were no differences in fertilization, cleavage, rates of grade 1 embryos, and development to blastocyst stage. In conclusion, head-first or tail-first sperm injection into an oocyte in ICSI program does not affect fertilization and subsequent embryo development to blastocyst stage in vitro.

      • Effect of Soybean Isoflavones on the Apoptosis and the Responsiveness of Local Factors Induced by TNF-alpha in Osteoblastic Cells

        We have investigated the effects of individual soybean isoflavones, genistein (4,5,7-trihydroxyisoflavone) and daidzein (4,7-dihydroxyisoflavone), on tumor necrosis factor-α (TNF-α)-induced apoptosis and the production of local factors in osteoblastic cells. Soybean isoflavones increased DNA synthesis and the number of viable cells. When the cells were treated with TNF-α (10^(-11)M∼10^(-9) M), the number of viable cells was dose-dependently decreased. The decrease in the cell number caused by TNF-α treatment was due to apoptosis, which was manifested by TUNEL and cell death ELISA method. Soybean isoflavones inhibited the apoptosis of osteoblastic cells subjected to TNF-α treatment. It can be suggested that these isoflavones may be involved in maintaining the viability and proliferation of osteoblastic cells. MC3T3-E1 osteoblastic cells secrete IL-6, IL-1ß, NO and PGE₂ constitutively, but at low levels. Soybean isoflavones have no effect on the constitutive production of these local factors. When the cells were treated with TNF-α (10^(-10)M), the production of IL-6 and PGE₂, but not that of IL-1ß and NO, was increased significantly. Treatment with soybean isoflavones (10^(-5)M) in the presence of TNF-α (10^(-10)M) for 48 hours inhibited the production of IL-6 and PGEz, suggesting the antiresorptive action of soy phytoestrogen may be mediated by decrease in these local factors. The findings of this study suggest that the function of osteoblastic cells is promoted by soybean isoflavones, thereby playing an important role in bone remodeling.

        Bergenin increases osteogenic differentiation and prevents methylglyoxal-induced cytotoxicity in MC3T3-E1 osteoblasts

        <P>Bergenin, an active component of plants in the genus Bergenia, has multiple biological activities, including anti-inflammatory and immunomodulatory properties. We investigated the effects of bergenin on MC3T3-E1 osteoblasts. Bergenin treatment significantly elevated collagen synthesis, alkaline phosphatase activity, osteocalcin synthesis, and mineralization in the cells (p < 0.05). Additionally, bergenin increased the ratio of osteoprotegerin to receptor activator of nuclear factor kappa-B ligand, and cyclophilin B release. Methylglyoxal (MG), a highly reactive dicarbonyl compound, is the major precursor in the formation of advanced glycation end products. Pretreatment of MC3T3-E1 cells with bergenin prevented MG-induced cell death. Furthermore, bergenin treatment significantly reduced the induction of activating transcription factor 6 and autophagy by MG. These results indicate that bergenin may have positive effects on critical osteoblastic cell functions.</P>

      • Effect of Scutellariae Radix Extract on the High Glucose-Induced Apoptosis in Cultured Vascular Endothelial Cells

        Endothelial cell apoptosis has been postulated as the initial trigger of the progression of progression disease in patients with diabetes mellitus. To investigate the role of Scutellariae radix extract, we examined its effect on the endothelial cell proliferation using the [(3)^H]-thymidine incorporation method. Scutellariae radix extract significantly stimulated endothelial cell proliferation in a dose-dependent manner. We focused on the protective action of Scutellariae radix extract on the endothelial tell apoptosis induced by high glucose concentrations. Determination of endothelial cell apoptosis was performed using DNA gel electrophoresis, terminal deoxynuclotidyl transferase-mediated dUTP nick end-labeling(TUNEL) assay, and an ELISA kit. Exposure of vascular endothelial cell to high glucose (16.7mM) for 72h resulted in a significant increase in apoptosis, compared with the normal glucose concentrations (5.5mM). Scutellariae radix extract inhibited high glucose-induced endothelial cell apoptosis. This result suggests that Scutellariae radix extract may contribute to antiapoptotic action against vascular endothelial cells, resulting in a beneficial effect in preventing diabetes-associated microvascular complications.
