        Successfully treated isolated renal artery pseudoaneurysm in a patient with Behcet`s disease

        Renal artery aneurysms and pseudoaneurysms are an uncommon clinical problem with a low incidence rate. They are abnormal dilatations of the vessel lumen with some different natures. However, the rupture of an aneurysm and pseudoaneurysm is the most dreaded complication because it causes death of the patient. There are many causes of renal artery aneurysm and pseudoaneurysm, including Behcet``s disease; however, renal involvement in Behcet``s disease is less frequent.We report a case of renal artery pseudoaneurysm induced by Behcet``s disease and treated successfully with coil embolization. A 56-year-old woman with Behcet``s disease presented with an incidental left renal artery pseudoaneurysm measuring 18 mm. We successfully performed endovascular treatment with coil embolization instead of surgical treatment.

        Hemorrhagic cystitis is defined as diffuse mucosal inflammation of the urinary bladder that presents with gross hematuria. A variety of factors, including chemotherapeutic agents, radiation therapy and infection, can cause hemorrhagic cystitis. Among them, BK virus-associated hemorrhagic cystitis is common in patients who have undergone stem cell transplantation but relatively rare in kidney transplantation patients. Here, we present the case of a kidney-transplant patient with BK virus-associated hemorrhagic cystitis and interstitial nephritis who was successfully treated with leflunomide and ciprofloxacin.         CLIPick: a sensitive peak caller for expression-based deconvolution of HITS-CLIP signals

        <P><B>Abstract</B></P><P>High-throughput sequencing of RNAs isolated by crosslinking immunoprecipitation (HITS-CLIP, also called CLIP-Seq) has been used to map global RNA–protein interactions. However, a critical caveat of HITS-CLIP results is that they contain non-linear background noise—different extent of non-specific interactions caused by individual transcript abundance—that has been inconsiderately normalized, resulting in sacrifice of sensitivity. To properly deconvolute RNA–protein interactions, we have implemented CLIPick, a flexible peak calling pipeline for analyzing HITS-CLIP data, which statistically determines the signal-to-noise ratio for each transcript based on the expression-dependent background simulation. Comprising of streamlined Python modules with an easy-to-use standalone graphical user interface, CLIPick robustly identifies significant peaks and quantitatively defines footprint regions within which RNA–protein interactions were occurred. CLIPick outperforms other peak callers in accuracy and sensitivity, selecting the largest number of peaks particularly in lowly expressed transcripts where such marginal signals are hard to discriminate. Specifically, the application of CLIPick to Argonaute (Ago) HITS-CLIP data were sensitive enough to uncover extended features of microRNA target sites, and these sites were experimentally validated. CLIPick enables to resolve critical interactions in a wide spectrum of transcript levels and extends the scope of HITS-CLIP analysis. CLIPick is available at: http://clip.korea.ac.kr/clipick/</P>

        Case Report : Cavitary Lung Lesion in a Patient with Systemic Lupus Erythematosus: An Unusual Manifestation of Cytomegalovirus Pneumonia

        Cytomegalovirus (CMV), a member of the human herpesvirus group, causes severe disease in immunocompromised patients. In particular, CMV pneumonia can be a life-threatening disease to patients taking immunosuppressive drugs. The radiographic manifestations of CMV are variable and may consist of reticular or reticulonodular patterns, ground-glass opacities, air-space consolidations, or mixed patterns. A cavitary lesion in pneumonia associated with CMV infection is extremely rare. Herein we report on a case of CMV pneumonia which presented with a cavitary lesion and was treated successfully in a systemic lupus erythematosus patient who was taking immunosuppressive drugs.         Analysis of the risk factors of acute kidney injury after total hip or knee replacement surgery

        Background: Postoperative acute kidney injury (AKI), which increases the risk of postoperative morbidity and mortality, poses a major concern to surgeons. We conducted this study to analyze the risk factors associated with the occurrence of AKI after orthopedic surgery. Methods: This was a retrospective study that included 351 patients who underwent total hip or knee replacement surgery at Inje University Haeundae Paik Hospital between January 2012 and December 2016. Results: AKI occurred in 13 (3.7%) of the 351 patients. The patients’ preoperative estimated glomerular filtration rate (eGFR) was 66.66±34.02 mL/min/1.73 ㎡ in the AKI group and 78.07±21.23 mL/min/1.73 ㎡ in the non-AKI group. The hemoglobin levels were 11.21±1.65 g/dL in the AKI group and 12.39±1.52 g/dL in the non-AKI group. Hemoglobin level was related to increased risk of AKI (odds ratio [OR], 0.13; 95% confidence interval [CI], 0.02-0.68; p=0.016). Administration of crystalloid or colloid fluid alone and the perioperative amount of fluid did not show any significant relationship with AKI. Further analysis of the changes in eGFR was performed using a cutoff value of 7.54. The changes in eGFR were significantly related to decreased risk of AKI (OR, 0.74; 95% CI, 0.61-0.89; p=0.002). Conclusion: Renal function should be monitored closely after orthopedic surgery if patients have chronic kidney disease and low hemoglobin level. Predicting the likelihood of AKI occurrence, early treatment of high-risk patients, and monitoring perioperative laboratory test results, including eGFR, will help improve patient prognosis.

        활성건조효모를 이용한 오미자 발효주의 이화학 및 관능 특성

        오미자주 양조에 있어서 중요한 효모 발효의 최적화를 위하여 시중에서 판매되고 있는 다양한 활성건조효모 8종(Lalvin 1116, Lalvin 1118, Lalvin D-47, Lalvin Bourgovin, Parisienne, Fermivin, Red Star Monrachet, Red Star Premier Cuvee)을 선정하여 오미자주의 이화학 및 관능특성을 연구한 결과, pH 및 산도는 각각 3.0-3.3, 1.8-2.4%였으며, 당도는 발효초기 24°Bx에서 24일 후 8.4-10.2°Bx이었다. 활성건조효모 8종을 이용하여 제조한 오미자주의 알코올 함량은 10.8-13.0%로 나타났으며 Lalvin D-47과 Red Star Premier Cuvee로 제조한 오미자 발효주의 알코올 함량(13.0%)이 가장 높게 나타났다. 효모수는 Lalvin 1118로 제조한 오미자 발효주가 5.8 log CFU/mL로 가장 낮게 나타났고, 7종의 효모수는 7.0-7.7 log CFU/mL로 비슷하게 나타났다. 색도는 효모의 종류에 따라 L, a, b 값에 모두 변화가 있는 것으로 나타났다. Lalvin 1118을 사용한 오미자 발효주의 맛에 대한 기호도는 6.75로 가장 높았으며, 활성건조효모 8종을 이용한 오미자 발효주의 향과 색의 기호도는 각각 4.90-5.75, 5.25-6.05로 5% 수준에서 유의적 차이를 나타내지 않았고, Lalvin 1118을 사용한 오미자 발효주의 목넘김은 6.65로 가장 높게 나타났다. 위의 결과를 바탕으로, Lalvin 1118을 사용한 오미자 발효주는 알코올 함량(12.0%), 맛(6.75±1.68), 목넘김(6.65±1.50), 종합적 기호도(6.70±1.34)에서 우수한 관능적 특성과 과실주로서의 적절한 알코올 함량을 나타냈으므로 Lalvin 1118은 오미자주 발효에 가장 적합한 것으로 사료된다. In order to select a superior yeast, 8 kinds of commercial active dry yeasts (Lalvin 1116, Lalvin 1118, Lalvin D-47, Lalvin Bourgovin, Parisienne, Fermivin, Red Star Monrachet, and Red Star Premier Cuvee) were utilized for omija wine fermentation. During fermentation, the physicochemical characteristics and sensory properties of the various omija wines were evaluated. According to the results, pH and titratable acidity were in ranges of 3.0-3.3% and 1.8-2.4%, respectively. Sugar content was 24°Bx at early fermentation and changed to 8.4-10.2°Bx at 24 days of fermentation. While the omija wines fermented by Lalvin D-47 and Red Star Premier Cuvee showed the highest alcohol contents (13.0%), the omija wine fermented by Parisienne showed the lowest alcohol content (10.8%). The omija wine fermented by Lalvin 1118 had an alcohol content of 12.0% and showed the lowest yeast count of 5.8 log CFU/mL. Hunter's values (L, a, and b) were all different among the 8 omija wines. Moreover, the omija wine fermented by Lalvin 1118 showed the highest scores for taste (6.75±1.68), swallowing (6.65±1.50), and overall acceptability (6.70±1.34). It is concluded that Lalvin 1118 was the best yeast among 8 tested commercial active dry yeasts, having a high potential for omija wine fermentation.

      • 갑상선에서 발생한 원발성 편평상피세포암 1예

        Primary squamous cell carcinoma of thyroid is a very rare malignant tumor with poor prognosis. It is usually diagnosed as an advanced disease infiltrating adjacent organs, and characterized by aggressive clinical course with an average postoperative survival time of less than 1 year. Recently, we had a 79- year-old woman with a painful neck mass who was diagnosed as primary squamous cell carcinoma of thyroid gland. She underwent total thyroidectomy and selective neck dissection(level Ⅲ, Ⅵ) with no further postoperative managements such as radiation therapy or chemotherapy; she died of poor general condition and pneumonia resulting from rapid progression of the lesion on the 38th day after surgery. We report this case with a review of relevant literatures.

        야생효모의 분리, 동정과 이를 이용한 오미자 발효주의 이화학 및 관능 특성의 비교

        오미자로부터 야생효모 Saccharomyces cerevisiae(S. cerevisiae) SH8094와 S. cerevisiae SH2855를 분리, 동정하였으며, 이 효모를 이용하여 오미자 발효주를 제조하였다. 야생효모 S. cerevisiae SH8094와 S. cerevisiae SH2855를 이용한 오미자 발효주와 시판효모 Lalvin 1118을 이용한 오미자 발효주의 이화학 특징과 관능 특성을 비교 분석하였다. 그 결과, 야생효모 S. cerevisiae SH8094로 제조한 오미자 발효주가 pH 2.8, 산도 1.1%, 당도 14.5°Brix, 알코올 함량 9%를 나타냈으며, 목넘김, 종합적 기호도에 있어서 높은 점수를 얻었다. 또한 오미자로부터 분리된 야생효모 S. cerevisiae SH8094와 S. cerevisiae SH2855를 이용하여 만든 오미자 발효주와 상업적으로 판매되는 오미자 발효주의 관능 특성을 비교한 결과, 생오미자에서 분리된 야생효모 S. cerevisiae SH8094로 제조한 오미자 발효주가 상업적으로 판매되는 오미자주보다 향, 색, 종합적 기호도에 있어서 우수하게 나타났다. 이로써 오미자로부터 분리한 효모 S. cerevisiae SH8094를 이용한 우수한 오미자 발효주 제조의 가능성을 확인하였다. The purpose of this research was to characterize physicochemical properties and sensory evaluation of schizandra wines fermented by the yeasts, Sacchromyces cerevisiae SH8094 (S. cerevisiae SH8094) and Sacchromyces cerevisiae SH2855 (S. cerevisiae SH2855) isolated from schizandra fruits and stems and compare these results with the results from commercial activated yeast (Lalvin 1118) and a commercial schizandra wine. Three different schizandra wines fermented by S. cerevisiae SH8094, S. cerevisiae SH2855, and Lalvin 1118 showed similar results in pH and titratable acidity. On the other hand, the schizandra wines fermented by S. cerevisiae SH8094 and S. cerevisiae SH2855 showed high brix (14°brix), low alcohol content (9%), and low yeasts count (4.1 log CFU/mL), compared with the schizandra wine fermented by Lalvin 1118. Both schizandra wines made with S. cerevisiae SH8094 and S. cerevisiae SH2855 showed higher scores in swallowing and overall acceptability than the schizandra wine made with Lalvin 1118. When compared with a commercial schizandra wine, the schizandra wine fermented with S. cerevisiae SH8094 showed better qualities in aroma (6.65±1.47), color (7.53±1.14), and overall acceptability (6.76±1.03). In conclusion, S. cerevisiae SH8094 which was isolated from schizandra fruits and stems has a high potential in schizandra wine fermentation.
