        주물공에서 발생한 Goodpasture증후군 1예

        Background: Smoking, upper respiratory tract infection, genetic factors and hydrocarbons are known as risk factors of Goodpasture's syndrome. We studied a patient with Goodpasture's syndrome who had worked for 27 years in a foundry company. Based on a study on the work-relatedness of the syndrome, we describe and discuss our study results. Case: A 46-year-o1d man, who had worked as a foundry worker for 27 years and had a 12 1/2 pack-year history of smoking cigarettes, was admitted into a hospital on 15th February 2006 with coughing, chest pain and dyspnea. On admission, he had hematuria, proteinuria, severe restrictive pulmonary function disorder and rapid elevation of blood urea nitrogen/creatinine. Immunological examination showed ANA (+), ANCA (-) and Anti-GBM Ab (+). Kidney biopsy showed pauci-immune crescentic glomeru-lonephritis. Mild bleeding was revealed through bronchoscopy and no vasculitis and granuloma were present on at lung biopsy. Finally, we diagnosed the worker's illness as Goodpasture's syndrome and carried out hemodialysis and plasmapheresis. In the workplace survey, the exposure level of respirable crystalline silica exceeded the TLV-TWA(0.0106 mg/m³), which was calibrated for overtime. Conclusion: Based on both the clinical test and industrial hygiene examination, we concluded that the Goodpasture's syndrome in this case was caused by long-term silica exposure. 배경: 유리규산에 노출되면 말기 신질환의 주요원인인 사구체신염이 증가할뿐만 아니라. 자가면역질환인 Goodpasture 증후군도 발생하는 것으로 보고되어 있다. 저자들은 27년간 주물공으로 근무한 근로자에서 발생한 Goodpasture 증후군이 작업관련성 조사를 통해 유리규산에 의해 발생하였을 가능성이 높다고 판단하였기에 이를 고찰과 함께 보고하는 바이다. 증례: 25년간 반갑씩 흡연을 하였고. 주물공장에서 수동 및 자동조형작업을 27년간 하였다. 내원 2주전부터 발생한 흉통과 기침으로 입원하였으며, 내원 당시 혈뇨, 단백뇨 및 중증의 제한성 폐기능 장애를 보였다. 이후 갑작스럽게 혈액요소질소와 크레아티닌이 증가하였다. 면역검사에서 ANA (+), ANCA (-), Anti-GBM Ab가 36.8 EU/mL(참고치 0∼10 EU/mL)로 양성이었으며, 신조직검사에서 무면역 사구체신염, 기관지내시경에서 출혈이 보여 Goodpasture 증후군으로 진단하고 투석 및 혈장분리 반출술을 시행하였다. 노출평가에서 호흡성분진 중 석영이 0.0793 mg/m³로, 근무시간으로 보정한 노출기준인 0.0106 mg/m³를 초과하였다. 결론: 본 증례는 노출평가와 작업력조사를 통해 유리규산 노출이 확인되었고 다른 전신질환으로 인한 신질환이 아니며 신세뇨관의 기능 이상이 사구체신염보다 먼저 나타난 이유로 유리규산에 노출되어 발생한 Goodpasture 증후군으로 판단된다.

        In this report we developed a high voltage alarm for the safety of workers who check and repair the 154kV power line. This high voltage alarm we developed is attached on workers' helmet and alarms automatically in order to let workers know when workers work over the safety distance near 154kV power line. Until now the exact safety distance hasn't been regulated and high voltage alarm we've been using was for the range of 6∼22.9kV power line. We fabricated this high voltage alarm on the basis of electric density and actual measurement of power line. The experimental result was that high voltage alarm at 66kV power line alarmed near 90cm, at 154kV power line alarmed near 160cm from the line. Accordingly, we developed the new high voltage alarm which is operated in the range of 66∼154kV power line.

        Dynamic Reconstruction of Anal Sphincter with Camera Shutter Style Double-Opposing Gracilis Flaps

        Fournier's gangrene is a life-threatening infection which requires prompt recognition, early surgical debridement of unhealthy tissue, and initiation of broad-spectrum antibiotics. Relook debridement are usually performed until all the devitalized tissue has been removed. Involvement of the anal sphincter may result in significant morbidity such as permanent incontinence. Dynamic reconstruction of the anal sphincter has always been one of the holy grails in the field of pelvic reconstruction.We demonstrate a new method of camera shutter style double-opposing gracilis muscle flaps that allows dynamic sphincteric function without the need for electrostimulation. The bilateral gracilis muscles are inset in a fashion that allows orthograde contraction of the muscle to narrow and collapse the neoanal opening. With biofeedback training, the patient is able to regain dynamic continence and return to function without a stoma. There was also no need for neurotization or microsurgery techniques to restore sphincteric function to the anus. The patient was able to reverse his stoma 14 months after the initial insult and reconstruction with biofeedback training without the use of electrostimulation.

      • Sonocatalytic reduction of nitrate using magnetic layered double hydroxide: Implications for removal mechanism

        <P><B>Abstract</B></P> <P>In this study, magnetic layered double hydroxides (mag-LDHs) were synthesized through compositing magnetite with three different metals (Mg, Cu and Al) under ultrasound (US, 100 kHz frequency and 50 W power). For the first time, mag-LDHs were applied to sonocatalytic reduction of nitrate (NO<SUB>3</SUB> <SUP>−</SUP>) and the reduction mechanism were determined by conducting kinetic tests and various spectroscopic analyses. Based on the kinetic data, NO<SUB>3</SUB> <SUP>−</SUP> reduction and the selectivity for N<SUB>2</SUB> highly depends on the ratio between Mg/Al, solution pH and sonication frequency. The best condition for sonocatalytic denitrification was found to be pH 7 operated under 100 kHz (50% power) using the catalyst with lowest amount of Al (mag-LDH-Al<SUB>0.3</SUB>Mg<SUB>1.5</SUB>). As a proposed mechanism, NO<SUB>3</SUB> <SUP>−</SUP> is initially reduced to NO<SUB>2</SUB> <SUP>−</SUP> by Cu<SUP>0</SUP>, and then further reduced to N<SUB>2</SUB>/NH<SUB>4</SUB> <SUP>+</SUP> by Mg<SUP>0</SUP>. Hypothetically Al<SUP>0</SUP> could provide sorption sites for hydrogen radicals (·H) dissociated from ultrasound, hence served as reducing sites in denitrification process. The XPS analysis showed an increased peak of Cu<SUP>0</SUP> after the sonocatalytic reduction when catalyst has lower amount of Al. The excessive hydrogen adsorbed on Al<SUP>0</SUP> might spill-over to the adjacent Cu, thus reducing the CuO into Cu<SUP>0</SUP> at high temperature created by the implosion of the microbubbles. Without the use of consumable reducing agents (i.e. H<SUB>2</SUB> gas), sonocatalytic reduction could be a potential candidate of remediation method to treat NO<SUB>3</SUB> <SUP>−</SUP> polluted water with high N<SUB>2</SUB> selectivity and easy magnetic recovery.</P> <P><B>Highlights</B></P> <P> <UL> <LI> Sonocatalytic reduction of NO<SUB>3</SUB> <SUP>−</SUP> was studied for the first time. </LI> <LI> Magnetically separable Mag-LDH was synthesized via ultrasound irradiation. </LI> <LI> Denitrification occurs without reducing chemicals. </LI> <LI> High selectivity for N<SUB>2</SUB> was achieved by mag-LDH under sonication. </LI> <LI> Solution pH and percentage of Mg play an important role in denitrification. </LI> </UL> </P> <P><B>Graphical abstract</B></P> <P>[DISPLAY OMISSION]</P>

        Characterization of an H4N2 influenza virus from Quails with a multibasic motif in the hemagglutinin cleavage site

        <P><B>Abstract</B></P> <P>The cleavage motif in the hemagglutinin (HA) protein of highly pathogenic H5 and H7 subtypes of avian influenza viruses is characterized by a peptide insertion or a multibasic cleavage site (MBCS). Here, we isolated an H4N2 virus from quails (Quail/CA12) with two additional arginines in the HA cleavage site, PEK<B>RR</B>TR/G, forming an MBCS-like motif. Quail/CA12 is a reassortant virus with the HA and neuraminidase (NA) gene most similar to a duck-isolated H4N2 virus, PD/CA06 with a monobasic HA cleavage site. Quail/CA12 required exogenous trypsin for efficient growth in culture and caused no clinical illness in infected chickens. Quail/CA12 had high binding preference for α2,6-linked sialic acids and showed higher replication and transmission ability in chickens and quails than PD/CA06. Although the H4N2 virus remained low pathogenic, these data suggests that the acquisition of MBCS in the field is not restricted to H5 or H7 subtypes.</P> <P><B>Highlights</B></P> <P> <UL> <LI> An H4N2 influenza virus with a multibasic cleavage site in the hemagglutinin protein was isolated from quails. </LI> <LI> This virus remained lowly pathogenic in chickens and required trypsin for <I>in vitro</I> growth. </LI> <LI> This virus showed higher preference for mammalian-type sialic acid receptors. </LI> <LI> This virus transmitted better in chicken than a duck-origin H4N2 virus. </LI> </UL> </P>

        Improvement of the Thermal Latency for Epoxy-Phenolic Resins by Novel Amphiphatic Imidazole Catalysts

        Novel amphiphatic imidazole compounds were evaluated as thermal latent catalysts for the polymerization of diglycidyl ether of bisphenol A (DGEBA). Amphiphatic compounds 5-9, two commercially available catalysts 1 and 2, and compounds 3 and 4 were used to cure epoxy resin systems for an investigation of their thermal latency and storage stability. The results from the cure activation energy and viscosity-storage time of the catalysts, the order of thermally latent activity was 3-phenylpropanoic acid (4) > 2-amino-3-phenylpropinoic acid (5) > 2-amino-3-(imidazole-4-yl)-propionic acid (H-His-OH, 8) > N-tert-butoxycarbonyl-histidine (9) > imidazole-4-acrylic acid (6) >3-(imidazole-4-yl)propionic acid (7) > 1-cyanoethyl-2-ethyl-4-methyl-imidazole (2) > 2-ethyl-4-methylimidazole (1) > histamine (3). From the results, the amphiphatic imidazole catalysts 5-9 showed better thermal latency than commercialized catalysts 1 and 2, basic catalyst 3 and acidic catalyst 4. Concerning the glass transition temperature (Tg), the use of amphiphatic imidazole catalysts 5-9 provided complete or near complete curing systems at temperatures ranging from 152-163 oC, which were similar to two commercially available catalysts (151-152 oC, 1 and 2)and histamine (159 oC, 3). On the other hand, the Tg for compounds 4 and 5 could not be detected at 30-300 oC from the temperature scans because of their weak nucleophilicity and low cross-linking reactivity.
