      • 수학 학습 부진아의 인식 및 학습 실태 연구

        본 연구는 수학교과의 학습에 방해가 되고 있는 요인을 조사·분석함으로써 학생들의 수학 교과에 대한 부적응 상황을 진단하여 학생들의 수학 부진을 줄이고 나아가 교수-학습 개선에 도움을 주고자 하는데 목적을 둔다. 이를 위해 초등학교 5학년을 대상으로 우선 수학 학습 부진 학생들의 인식을 조사, 분석하였다. 또한 수학 학습 부진아를 지도할 때 학생들이 어려워하는 학습 내용을 사전에 파악하는 것이 학습부진을 줄일 수 있는 중요한 수단으로 보고 4학년 수학 단원 평가를 실시하여 이들이 어떤 내용을 잘 이해하지 못하는지를 분석하였다. 그 결과 수학 학습 부진 학생들은 수학 교과에 대해 부정적으로 생각하고 있었으며, 이들이 어려워하는 수학 내용은 다단계 사고를 요하는 개념, 논리적 연역적으로 추론을 요하는 개념, 문제해결이었다. 그 중에서도 특히 사고를 요하는 문제해결을 어려워하는 것으로 나타났다. This study sets out to investigate underachieved fifth graders in mathematics in terms of the understanding and learning state of mathematics and to help mathematics underachievers by providing make-up class. Thus we need to investigate the following: 1. An understanding of mathematics for underachieved students. 2. A research of the learning contents that the students have difficulties to deal with. For this study we first select 20 underachieved students(12 boys and 8 girls) in mathematics attending an elementary school located in Busan and then a questionnaire is distributed to see their understanding of mathematics. In addition, their performances are evaluated by using a checklist. The questionnaire contains total 30 items and each frequency and percentage are analyzed. The checklist contains 10 items of the contents of 4-Ga and 4-Na respectively. textbooks, The frequency and percentage are also analyzed. We have the following conclusion on mathematics underachievers. First, since they have a negative opinion of the subject, we need to increase the chances of their successful experiences so that they have their confidence and positive attitudes. Second, we should try to teach basic learning contents at the beginning of each unit thoroughly and focus on advanced learning contents gradually by intensifying and reinforcing the contents. Third, it is more desirable to give them suitable assignments they can handle rather than to force to teach advanced contents. An evaluation should be given every time after finishing a unit, and the results should be informed to students. And make-up class should be given if necessary.

      • 히스토아크릴을 이용한 십이지장 궤양 출혈의 지혈 후 발생한 급성췌장염

        Upper gastrointestinal bleeding is common and potentially life threatening medical emergency. Emergency endoscopy is the first choice of diagnostic and treatment measure for patients with active upper gastrointestinal bleeding. Histoacryl should be considered a second-line treatment modality when conventional treatments were failed. However, it might cause several complications. Herein, we report a case of acute pancreatitis developed after histoacryl injection therapy for active duodenal ulcer bleeding. A 71-year-old man was admitted with melena and hematemesis. On emergency endoscopy, a 2 cm sized active ulcer with bleeding from an exposedvessel was seen at the duodenal bulb. Attempts to arrest the bleeding with hemoclipping and submucosal epinephrine injection were tried, but failed. We changed the method to endoscopic histoacryl injection, and obtained hemostasis immediately. A few hourslater, after successful hemostasis, patient complained diffuse abdominal pain. Ultrasonography revealed hyperechoic heterogenous diffuse pancreatic enlargement and right pararenal space fluid collection, this ultrasonographic findings and elevated serum pancreatic enzymes are compatible with acute pancreatitis.

        Effect of Di-(2-ethylhexyl)-phthalate on Sphingolipid Metabolic Enzymes in Rat Liver

        Di-(2-ethylhexyl)-phthalate (DEHP), the most widely utilized industrial plastizer and a ubiquitous environmental contaminant, can act on peroxisome proliferators-activated nuclear hormone receptor family (PPAR) isoforms. To understand the contribution of sphingolipid metabolism to DEHP-induced hepatotoxicity, effect of DEHP exposure on activities of sphingolipid metabolic enzymes in rat liver was investigated. DEHP (250, 500 or 750 ㎎/㎏) was administered to the rats through oral gavage daily for 28 days. The activities of acidic and alkaline ceramidases were slightly increased in 250 ㎎/㎏ DEHP-administered rat livers and significantly elevated in 500 ㎎/㎏ DEHP-administered ones, although the level of 750 ㎎/㎏ DEHP-administered ones was not increased. Neutral ceramidase, acidic and neutral sphingomyelinases, sphingomyeline synthase and ceramide syhthase were not changed at all by DEHP exposure. Therefore, acidic and alkaline ceramidases might play important roles in DEHP-induced hepatotoxicity.

        저 골대사 질환의 위험성이 있는 환자에서 저칼슘 복막투석액의 임상적 유용성

        고칼슘혈증은 투석액내 칼슘농도가 3.5 mEq/L인 통상적인 복막투석액을 사용하면서 인흡수 억제제로 칼슘을 복용하는 지속성 외래 복막투석 환자에게서 흔히 관찰된다. 혈청 칼슘농도의 증가는 부갑상선 호르몬(iPTH)을 억제하여 저 골대사 질환(low-turnover bone disease)의 빈도를 높일 수 있다. 저자들은 저칼슘 복막투석액(Ca^(++) : 2.5 mEq/L)의 사용이 혈청 칼슘, iPTH및 기타 혈중 생화학적 표지자에 미치는 영향에 대해 알아보고자 본 연구를 시행하였다. 1996년 1월부터 1999년 12월말까지 본원에서 지속성 외래 복막투석을 시행 받은 환자 386명중 혈청 교정칼슘농도(혈청 알부민으로 교정한 칼슘농도)가 10.0 mg/dL을 초과하거나 iPTH가150 pg/mL 미만인 환자에게 8주간의 예비기간 이후 저칼슘 복막투석액을 사용하였다. 저칼슘 복막투석액을 사용하는 12개월의 관찰 기간동안 혈청 칼슘, 인 및 골표지자(iPTH, ALP)를 측정하였고 인흡수 억제제와 calcitriol 복용량의 변화를 조사하였다. 전체 386명 중 저칼슘 복막투석액을 사용한 환자는 46명(11.9%)이었으며 그 중 22명(5.7%)은 저칼슘 복막투석 시작 당시 고칼슘혈증(교정칼슘농도 >10.5 mg/dL)으로 관찰되었다. 관찰 기간동안 혈청 인농도는 차이가 없었으나 혈청 칼슘농도는 유의하게 감소하였다(10.5±1.4 vs 9.4±1.3 mg/dL, p<0.05). 혈청ALP농도(203.0±92.9 vs 257.2±103.4 U/L, p<0.05)와 iPTH 농도(92.7±128.8 vs 225.3±237.3 pg/mL, p<0.05)는 유의하게 증가되었다. 저칼슘 복막투석액을 사용하는 동안 인흡수 억제제 복용량의 유의한 차이는 없었으나 calcitriol 복용량은 유의하게 증가하였다(0.038±0.087vs 0.158±0.288 tablets/day, p<0.05). 저칼슘 복막투석액을 사용한 46명 중 6명이 표준 복막투석액으로 다시 전환하였으며, 그 이유는 iPTH의 과도한 증가가 3예, 저칼슘혈증이 2예, 부종이 1예였다. 결론적으로 저칼슘 복막투석액의 사용은 고칼슘혈증과 부갑상선 호르몬억제 소견이 있는 환자에서 혈청 칼슘은 감소시키고, 부갑상선 호르몬 농도와 ALP 농도는 증가시켜 고칼슘혈증과 저골대사 질환이 의심되는 지속성 외래 복막투석 환자에서 유용할 것으로 생각된다. Hypercalcemia is a common complication in CAPD patients treated with calcium-containing phosphate binders and using the standard dialysate(Ca^(++) : 3.5 mEq/L). Furthermore, the high calcium concentration in standard dialysate may have a suppressive effect on parathyroid hormone(iPTH) level, contributing to the high prevalence of low-turnover bone disease. We studied the effect of low calcium dialysate(Ca^(++) : 2.5 mEq/L) for those patients with high risk of lowturnover bone disease. Among 386 patients(1996. 1.- 1999. 12.) who had been stable on CAPD for at least 3 months, 46 patients were included in this study. The patients were divided into 3 groups on the basis of the iPTH levels(<150 pg/mL) and/or corrected serum calcium levels(>10 mg/dL) before the conversion to low calcium dialysate. Group 1(n=29), iPTH <150 pg/mL and Ca^(++) >10 mg/dL; Group 2 (n=14), iPTH <150 pg/mL and Ca^(++) <10 mg/dL; Group 3(n=3), iPTH >150 pg/mL and Ca^(++) >10 mg/ dL. During a 2-month run-in period, those patients were treated with standard dialysate. After that, a 12-month therapy with low calcium dialysate was followed. Biochemical data including calcium, phosphorus, iPTH and alkaline phosphatase were measured regularly and daily phosphate binder and calcitriol intake(pill counting) were assessed during the run-in and therapy period. We obtained the following results : the prevalence of hypercalcemia(Ca^(++) >10.5 mg/dL) was 5.7%(22/ 386 patients). Serum calcium levels decreased during the therapy period(12 months)(10.5±1.4 vs 9.4±1.3 mg/dL, p<0.05). Serum phosphorus levels remained unchanged. Mean serum alkaline phosphatase level increased(203.0±92.9 vs 257.2±103.4 U/L, p<0.05). Serum iPTH levels increased (92.7±128.8 vs 225.3± 237.3 pg/mL, p<0.05). The mean intake of oral phosphate binders was not significantly different between run-in period and therapy period. But calcitriol doses increased 0.038±0.087 at run-in period to 0.158± 0.288 tablets/person/day at therapy period(p<0.05). In the six patients, low calcium dialysate was converted to standard dialysate due to high iPTH level (n=3), symptomatic hypo calcemia(n=2), and uncontrolled edema(n=1). In conclusion, in the study of 46 patients over 12 month period, the usage of 2.5 mEq/L calcium dialysate resulted in a significant decrement in calcium levels and increased iPTH levels. Therefore, we propose that dialysis with a low calcium dialysate is an acceptable form of therapy for the patients with high risk of low-turnover bone disease showing hypercalcemia and low iPTH level. However, further study will be needed for evaluating the effect of low calcium dialysate in low-turnover bone disease.

        UPLC-PDA를 이용한 창포류의 분류 및 함량 분석

        A quantitative method using ultra performance liquid chromatography with a photodiode array detector (UPLCPDA) was established for the analysis of 2 major plant metabolites: β-asarone and α-asarone from Acorus gramineus, A. tatarinowii, A. calamus and Anemone altaica, and their contents are compared with other herbs of Acorus species. The method was validated according to the International Conference on harmonization (ICH) guideline for validation of analytical procedures with respect to precision, accuracy, and linearity. The average content of β-asarone in Acorus gramineus was significantly higher than that in others, with the second highest concentration observed in A. tatarinowii, and only a trace amounts found in A. calamus and Anemone altaica. In contrast, the average content of α-asarone in A. calamus was the highest, followed by that in Acorus gramineus and A. tatarinowii. principle component analysis (PCA) confirmed that β-asarone and α-asarone content differed among the species. These results suggest that this UPLC-PDA method can be considered as good quality control criteria for Acorus gramineus.
