신생아에서 출생체중별 구분에서 4.0 kg 이상을 과체중출생아(high birth weight infant, HBWI) 라고 한다. HBWI는 거대아112 KA Jeong, et al. • A Case of Macrosomia of Birth Weight 6.14 kg (macrosomi)와 같이 사용되는데, macrosom는 출생체중 4.0 혹은4.5 kg 이상을 칭한다. macrosomia에서는 여러 질환이 이환 될 수있기 때문에 고위험신생아군에 속하며, 집중치료가 필요한 경우가많다. 저자들은 임신나이 38주 5일에 출생체중 6.14 kg로 태어난 초거대아 1례를 경험하였기에 최근 보고하는 바이다. 환아는 임신성당뇨 모체로부터 제왕절개로 출생한 여아로 출생 후부터 시작된 빈호흡으로 입원하였다. 동반된 질환은 저혈당, 저칼슘혈증, 이차심방결손, 동맥관개존, 폐동맥고혈압 등 이었다. 대증요법으로 호전되어입원 10일에 증상 호전되어 퇴원하였고, 출생 후 6개월까지 외래 추적 관찰 중에서 특이 문제는 없다. 저자들은 이번 초거대아 증례의경험을 기회로, 한국통계청의 2000-2010년 출생 통계의 원시자료를통해서 총 출생아 중에서 HBWI의 빈도와 추이를 살펴보았다. 또한1964-2011년까지 한국에서 보고된 각종 macrosomia에 대한 보고들을 분석해본 결과, 동기간에 출생체중이 6.0 kg이 상인 경우는 본증례를 포함해서 7례였는데, 이중에서 본 증례는 5번째로 큰 증례이었다. 저자들은 향후 신생아분야의 고위험군 중의 하나인 거대라 관리에 도움을 주고자 관련 문헌고찰과 함께 본 증례를 보고하는 바이다.
젊은 여성의 부속기 수술에서 가임력 보존을 위한 로봇 단일공법의 이용

목적: Demonstrate a new instrument, technique or procedure. 방법: 젊은 여성의 부속기 수술에 주로 사용되는 기존의 단일공법 수술에 비해 로봇 단일공법을이용하여 보다 효과적으로 가임력 보존이 가능한지 알아보고자 하였다. 이화여대 목동병원 로봇센터에서 로봇 단일공법을 이용한 부인과 수술이 300례를 넘었고 이 중 100건 이상이 젊은 여성의 부속기 수술에 이용되었다. 기존의 단일공법에 비해 고가의 추가 비용이 발생하는 로봇 단일공법 수술을 향후 임신을 위한 난관 주변 유착 박리가 필요하거나 항뮬러관으로 측정한 난소예비력이 이미 감소되어 난소 기능 보존이 반드시 필요한 난소낭종절제술에 이용하였다. 로봇 단일공법 수술 후 임신의 결과를 추적 관찰하였다. 결과: 로봇 단일공법은 기존의 단일공법 수술에 비해 봉합이 정교하고 쉬우므로 특히 항뮬러관 호르몬으로 측정한 난소예비력이 감소되어 있는 난소낭종 환자에서 지혈을 위한 소작술을 최소화하고 혈관이 노출된 난소낭종 절제면을 빠르고 정확하게 봉합하여 수술 후 난소기능 감소를 최소화 할 수 있다. 또한 로봇 단일공법은 기존의 복강경을 이용한 단일공법 보다 난관 주변 유착을 박리하기가 쉽고 세심하여 향후 임신을 위한 수술에 적합하였다. 난소예비력 감소 및 유착을 동반한 여성에서 로봇 단일공법 수술 후 성공적으로 자연임신이 가능한 예가 있었다. 결론: 젊은 여성의 부속기 수술에서 향후 임신을 위한 가임력 보존은 중요하므로, 유착 박리와 봉합술기에 방법적 제한이 있을 수 있는 기존의 복강경 단일공법에 비해 로봇 단일공법 수술을 이용하여 임신을 위한 긍정적 영향이 기대된다면, 수술절개부위 최소화로 인한 미용효과 이상으로 고비용 지불의 이유가 될 것이다.

In this study, the environment of Baegun-myeon, Jinan, Jeonbuk was divided into hills, waterways, reservoirs, Paddy fields and fields, and Sherman traps were installed to capture Wild Rodents for 18 months from March to November 2019 and from March to November 2020. 134 Wild Rodents of 2 classes and 4 species were captured. 119 stripe Apodemus agrarius were dominant species by type. 59 mice were captured at hills by environment. To look at the characteristics of collection by period, there was not much difference during May, June, August, and September excluding March in 2019, and lots of Wild Rodents were captured in March 2020, and thus variation due to climate change in the winter of 2019 and 2020 was identified. As a result of PCR-based DNA test of Leptospira, 4 mice (2.99%) tested positive. As for the periods of their collection, 2 mice tested positive in March, 1 mouse in April, and 1 mouse in November, respectively. To classify the Leptospira-positive Wild Rodents, three mice (75%) were stripe Apodemus agrarius and one mouse (25%) was Apodemus peninsulae. In the future, it is necessary to investigate carriers of Leptospira such as rodents including Wild Rodents over the whole period, not a specific period, given climate change in the Jeonbuk area and the positive reaction to Leptospira found irrespective of seasons. Then, it is judged that prompt and efficient response will be possible in the event of change.
설탕 대체 Neohesperidin Dihydrochalcone를 첨가한 양갱의 품질 특성

The purpose of this study is to prepare low-calorie yanggaeng with reduced sugar by adding neohesperidin dihydrochalcone or NHDC (0.0125%, 0.025%, 0.05%, and 0.1%) and to compare its quality characteristics. NHDC is a high sweetener, 1,500-1,800 times sweeter than sucrose. In the sugar (o Bx) and water content of the NHDC-added yanggaeng, there was no significant difference except for the sugar sample. The brightness of the yanggaeng according to the amount of NHDC added was 37.7-38.7 in the NH group, which was brighter than sugar’s (35.5). The yellowness in the NH group was -0.50 to -0.54, which was higher than the sugar’s (-0.80). The total polyphenol content ranged from 16.0-16.8 mg/100g, while DPPH radical scavenging activity ranged from 63.8-65.2. In the overall acceptability, sugar and NH3 were in the order of 6.25 and 5.25, respectively, and it was found that adding 0.05% of NHDC was desirable in replacing 50% of sugar in the manufacturing of Yanngang. Accordingly, NHDC does not have sugar level sweetness, so it does not seem easy to replace sugar completely. Research on improving sweetness by using the structural modification of NHDC is essential in the future.
Hot Wall Epitaxy(HWE)법에 의한 BaIn<sub>2</sub>S<sub>4</sub> 단결정 박막 성장과 광전도 특성

A stoichiometric mixture of evaporating materials for $BaIn_2S_4$ single crystal thin films was prepared from horizontal electric furnace. To obtain the single crystal thin films, $BaIn_2S_4$ mixed crystal was deposited on thoroughly etched semi-insulating GaAs(100) substrate by the Hot Wall Epitaxy (HWE) system. The source and substrate temperatures were $620^{\circ}C$ and $420^{\circ}C$, respectively. The crystalline structure of the single crystal thin films was investigated by double crystal X-ray diffraction (DCXD). The carrier density and mobility of $BaIn_2S_4$ single crystal thin films measured from Hall effect by van der Pauw method are $6.13{\times}10^{17}cm^{-3}$ and $222cm^2/v{\cdot}s$ at 293 K, respectively. The temperature dependence of the energy band gap of the $BaIn_2S_4$ obtained from the absorption spectra was well described by the Varshni's relation, $E_g(T)=3.0581eV-(3.9511{\times}10^{-3}eV/K)T^2/(T+536K)$. The crystal field and the spin-orbit splitting energies for the valence band of the $BaIn_2S_4$ have been estimated to be 182.7 meV and 42.6 meV, respectively, by means of the photocurrent spectra and the Hopfield quasicubic model. These results indicate that the splitting of the ${\Delta}so$ definitely exists in the ${\Gamma}_5$ states of the valence band of the $BaIn_2S_4/GaAs$ epilayer. The three photocurrent peaks observed at 10 K are ascribed to the $A_1$-, $B_1$-exciton for n = 1 and $C_{24}$-exciton peaks for n = 24.

Objective: The purpose of this study was to investigate whether the intrinsic foot muscle training method can improve the medial longitudinal arch in patients with chronic ankle injury and with pronated feet, as well as to investigate for the most effective exercise method for these patients. Design: Randomized controlled trial. Methods: Thirty men and women with pronated feet had participated in this study and were allocated to either the short foot exercise group (SFEG) or the towel curl exercise group (TCEG) randomly. SFEG and TCEG underwent exercises three times a week for 8 weeks, with three sets per day, totalling up to 5 minutes per day. The navicular drop test (NDT) was used in order to assess for changes in the medial longitudinal arch and the Cumberland ankle instability tool (CAIT) was used to assess for ankle instability of the chronic ankle sprain patients. Results: There was a significant increase in CAIT scores in the SFEG (p<0.05) and a significant difference between groups was presented (p<0.05). The NDT scores were significantly decreased in both groups (p<0.05). In the SFEG, the NDT scores were more decreased than in the TCEG (p<0.05). Conclusions: These results suggest that short foot exercises are more effective in providing intrinsic foot muscle training for patients with pronated feet among chronic ankle sprain patients. Furthermore, short foot exercises may be used to provide ankle stability.
녹차가 납중독된 흰쥐의 신장에 미치는 조직학적 및 생화학적 연구

To investigate the defensive effect of green tea against the lead toxicity, Sprague-Dewley rats (150 gm) were divided into 5 groups; the control group (A), the group treated with lead for 4 weeks (Group B-1), the group treated with lead and green tea for 4 weeks (Group B-2), the group treated with lead for 8 weeks (Group C-1), and the group treated with lead and green tea for 8 weeks (Group C-2). The lead acetate (500 ppm) was injected two times for one week into the abdomen and green tea solution (3 g/100 ml distilled water) offered freely. The results of histological and biocheical study are as follows; 1. Blood Urea Nitrogen (BUN) were increased in all the tested groups. The Group B-1 was more increased than the Group B-2, and the Group C-1 more than the Group C-2. The values of Alkaline phosphatase (ALP) were also decreased in all the tested groups, as such the former phenomenon. 2 In the Group B-1, some microvilli, mitochondria and rER were modificated on epithelial cell of proximal renal tubules. The cristae of mitochondria were enlarged, microvilli and nucleus were observed normally on the Group B-2. The number of Microvilli, mitochodria and rER were decreased, many lysosomes and irregular nucleus observed in the Group C-1. In the Group C-2, microvilli were modificated slightly and other organelles were observed similary with the Group B-2.
다낭난소증후군 진단시 복식 초음파의 유용성에 관한 연구

목 적: 한국 가임기 여성의 다낭난소증후군 진단시에 복식 초음파를 이용하는 방법의 유용성과 정확도를 분석하여 국내 다낭난소증후군의 진단 기준 마련에 도움이 되고자 하였다. 연구방법: 이화여자대학교 목동병원에서 선별설문조사를 받은 15~44세 여성 8,793명에서 복식 초음파를 시행한 총 701명 중, 정상 월경을 하면서 안드로겐 과다 및 초음파의 다낭난소 소견이 없는 정상 대조군에서 복식 초음파를 시행한 185명과 초음파 검사 소견을 제외하고 NIH 진단 기준으로 다낭난소증후군을 진단 받은 248명의 환자에서 복식 초음파를 시행한 소견을 ROC (receiver operating characteristic) 곡선 분석하였다. 결 과: 정상 대조군의 평균 연령은 $23.64{\pm}4.26$세로 복식 초음파로 측정된 평균 난소 부피는 $6.03{\pm}1.89\;cm^3$, 평균 난포 개수는 $6.49{\pm}1.93$개였다. 복식 초음파를 진단에 사용할 경우 난소 부피를 이용하면 AURC (area under the ROC curve)가 0.761로 난소 부피>$9\;cm^3$ 기준일 때, 민감도 51.0%, 특이도 91.4%였다. 난포 개수에 대한 AURC는 0.733으로 ${\geq}9$개 기준일 때, 민감도 54.9%, 특이도 87.0%였으며, 난포 개수 ${\geq}10$개 기준일 때, 민감도 53.2%, 특이도 90.4%였다. 복식 초음파로 난소 부피와 난포 개수를 검사하여 다낭난소증후군의 선별 진단에 이용하기에는 부적합하였다. 결 론: 복식 초음파를 이용하여 다낭난소증후군을 진단하는 방법은 유용성과 정확도에서 신뢰할만하지 못하다. Objective: The objective of the study was to determine the diagnostic performance of transabdominal ultrasound by receiver operating characteristic (ROC) curve analysis, in order to evaluate the usefulness in establishing the diagnosis of polycystic ovary syndrome (PCOS). Methods: Questionnaires were given to 8,793 reproductive women reviewed at Ewha Womans University Mokdong hospital. Ultrasound examinations were performed in 701 women with a transabdominal transducer. Transabdominal ultrasounds were performed in 185 normal control women (normal menstruation without hyperandrogenism or PCO morphology) and 248 PCOS patients according to National Institutes of Health (NIH) PCOS diagnosis criteria. ROC curves were calculated for ovarian volume and follicle number. Results: In normal control group, the mean age were $23.64{\pm}4.26$ years old and the mean ovarian volume and follicle number were $6.03{\pm}1.89\;cm^3$ and $6.49{\pm}1.93$, respectively. The ovarian volume showed an area under the ROC curve (AURC) of 0.761. A ovarian volume decision threshold >$9\;cm^3$ had a sensitivity of 51.0% and a specificity of 91.4% for the diagnosis of PCOS. The follicle number showed an AURC of 0.733. A follicle number decision threshold ${\geq}9$ had a sensitivity of 54.9% and a specificity of 87.0% for the diagnosis of PCOS. A follicle number decision threshold ${\geq}10$ had a sensitivity of 53.2% and a specificity of 90.4%. A follicle number and a ovarian volume did not have a high diagnostic power for screening for PCOS. Conclusion: Our results suggest that transabdominal ultrasound assessment is not effective for the detection of PCOS in young women of reproductive age.
