        출생체중 6.14 kg의 초거대아 1예 보고

        Infants, born with a birth weight above 4.0 kg, are categorized as high birth weight infant (HBWI). The term HBWI is often used in similar context with macrosomia. Macrosomia is associated with many complications, and is considered to be a high risk group that requires an intensive care in most cases. This report is presenting an extreme macrosomia born at a gestational age of 38+5weeks, with a body weight of 6.14 kg. The infant was born by a cesarean section from a mother with diabetes, and was admitted into an intensive care unit with tachypnea, which had occurred soon after birth. There were other complications, such as hypoglycemia,hypocalcemia, secondary atrial septal defect, patent ductus arteriosus, pulmonary hypertestion, and etc. With conservative management, the symptoms improved over a 10 day course and the patient was discharged from the hospital. To this day, the child has not presented with further health problems during the 6 months of follow up period. We reviewed the frequency and trend of the births of HBWI, through the raw data from the Statistics Korea on births between 2000 and 2010. With additional analysis of the cases of macrosomia, through the years of 1964 to 2011, we were able to find 7 reports, including this current case of infants born with a body weight above 6.0 kg. This case was the fifth heaviest infant among these 7 cases. We are reporting this case with the hope that it may contribute to the future care of high risk infants in a neonatal intensive care unit. 신생아에서 출생체중별 구분에서 4.0 kg 이상을 과체중출생아(high birth weight infant, HBWI) 라고 한다. HBWI는 거대아112 KA Jeong, et al. • A Case of Macrosomia of Birth Weight 6.14 kg (macrosomi)와 같이 사용되는데, macrosom는 출생체중 4.0 혹은4.5 kg 이상을 칭한다. macrosomia에서는 여러 질환이 이환 될 수있기 때문에 고위험신생아군에 속하며, 집중치료가 필요한 경우가많다. 저자들은 임신나이 38주 5일에 출생체중 6.14 kg로 태어난 초거대아 1례를 경험하였기에 최근 보고하는 바이다. 환아는 임신성당뇨 모체로부터 제왕절개로 출생한 여아로 출생 후부터 시작된 빈호흡으로 입원하였다. 동반된 질환은 저혈당, 저칼슘혈증, 이차심방결손, 동맥관개존, 폐동맥고혈압 등 이었다. 대증요법으로 호전되어입원 10일에 증상 호전되어 퇴원하였고, 출생 후 6개월까지 외래 추적 관찰 중에서 특이 문제는 없다. 저자들은 이번 초거대아 증례의경험을 기회로, 한국통계청의 2000-2010년 출생 통계의 원시자료를통해서 총 출생아 중에서 HBWI의 빈도와 추이를 살펴보았다. 또한1964-2011년까지 한국에서 보고된 각종 macrosomia에 대한 보고들을 분석해본 결과, 동기간에 출생체중이 6.0 kg이 상인 경우는 본증례를 포함해서 7례였는데, 이중에서 본 증례는 5번째로 큰 증례이었다. 저자들은 향후 신생아분야의 고위험군 중의 하나인 거대라 관리에 도움을 주고자 관련 문헌고찰과 함께 본 증례를 보고하는 바이다.

        녹차가 납중독된 흰쥐의 신장에 미치는 조직학적 및 생화학적 연구

        To investigate the defensive effect of green tea against the lead toxicity, Sprague-Dewley rats (150 gm) were divided into 5 groups; the control group (A), the group treated with lead for 4 weeks (Group B-1), the group treated with lead and green tea for 4 weeks (Group B-2), the group treated with lead for 8 weeks (Group C-1), and the group treated with lead and green tea for 8 weeks (Group C-2). The lead acetate (500 ppm) was injected two times for one week into the abdomen and green tea solution (3 g/100 ml distilled water) offered freely. The results of histological and biocheical study are as follows; 1. Blood Urea Nitrogen (BUN) were increased in all the tested groups. The Group B-1 was more increased than the Group B-2, and the Group C-1 more than the Group C-2. The values of Alkaline phosphatase (ALP) were also decreased in all the tested groups, as such the former phenomenon. 2 In the Group B-1, some microvilli, mitochondria and rER were modificated on epithelial cell of proximal renal tubules. The cristae of mitochondria were enlarged, microvilli and nucleus were observed normally on the Group B-2. The number of Microvilli, mitochodria and rER were decreased, many lysosomes and irregular nucleus observed in the Group C-1. In the Group C-2, microvilli were modificated slightly and other organelles were observed similary with the Group B-2. 흰쥐에서 납 독성에 대한 녹차의 방어효과를 알아보기 위하여 150g 내외의 흰쥐를 대조군, 납 단독투여군, 납-녹차 투여군으로 나누어 4주, 8주간 lead acetate (500 ppm)를 1주에 2번씩 복강 투여하였고, 녹차침수액(3g of tea/100m1 of water)은 식수로 공급하였다. 생화학적 및 형태학적 실험을 통해 다음과 같은 결론을 얻었다. 1. ALP값은 대조군에 비해 실험군 모두 감소하였고, 납-녹차투여군은 납 단독투여군보다 감소가 완화되었다. BUN는 대조군에 비해 실험군 모두 증가하였고, 납 녹차투여군은 납 단독투여군보다 증가가 완화되었다. 2. 신장의 전자현미경적 결과 B-1군은 미세융모, 사립체가 변형되었다. 다수의 리소좀이 관찰되었다. 핵의 모양은 불규칙하게 나타났다. B-2군은 미세융모가 잘 발달되었다. 다수의 사립체와 리소좀이 관찰되었다. rER의 수조는 팽창되고 수가 감소하였고 핵은 정상적으로 관찰되었다. C-1군은 대부분의 미세융모가 탈락, 변성되었고, 다수의 리소좀을 관찰할 수 있었다. 사립체는 감소, 변형되었다. 핵은 함입되어 불규칙하였다. C-2군은 미세융모의 일부가 팽창되거나 변화되었고, 다른 세포소기관들은 B-2군과 비슷한 양상을 나타냈다.

        영상의 재생 스타일에 따른 감성적 효과와 감성 평가 도구의 개발

        This study investigated how video playback styles affect viewers’ emotional responses to a video and then suggested emotion assessment tool for playback-edited videos. The study involved two in-lab experiments. In the first experiment, observers were asked to express their feelings while watching videos in both original playback and articulated playback simultaneously. By controlling the speed, direction, and continuity, total of twelve playback styles were created. Each of the twelve playback styles were applied to five kinds of original videos that contains happy, anger, sad, relaxed, and neutral emotion. Thirty college students participated and more than 3,800 words were collected. The collected words were comprised of 899 kinds of emotion terms, and these emotion terms were classified into 52 emotion categories. The second experiment was conducted to develop proper emotion assessment tool for playback-edited video. Total of 38 emotion terms, which were extracted from 899 emotion terms, were employed from the first experiment and used as a scales (given in Korean and scored on a 5-point Likert scale) to assess the affective quality of pre-made video materials. The total of eleven pre-made commercial videos which applied different playback styles were collected. The videos were transformed to initial (un-edited) condition, and participants were evaluated pre-made videos by comparing initial condition videos simultaneously. Thirty college students evaluated playback-edited video in the second study. Based on the judgements, four factors were extracted through the factor analysis, and they were labelled “Happy”, “Sad”, “Reflective” and “Weird (funny and at the same time weird).” Differently from conventional emotion framework, the positivity and negativity of the valence dimension were independently treated, while the arousal aspect was marginally recognized. With four factors from the second experiment, finally emotion assessment tool for playback-edited video was proposed. The practical value and application of emotion assessment tool were also discussed.

        녹차가 납 중독된 흰쥐의 간장에 미치는 형태학적 연구

        흰쥐의 간장에서 납 독성에 대한 녹차의 방어효과를 알아보기 위하여 150g 내외의 흰쥐를 대조군, 납 단독 투여군, 납-녹차투여군으로 나누어 4주, 8주간 lead acetate (500 ppm)를 1주에 2번씩 복강 투여하였고, 녹차침수액 (3g of tea/100 ml of water)은 식수로 공급하였다. A-1군은 사립체와 rER의 수조가 팽대되고, 유리 리보솜의 수가 증가되었다. 다수의 리소좀이 관찰되었다. A-2군은 사립체가 팽대되었고, 리소좀이 관찰되고 핵은 정상으로 나타딘다. B-1군은 rER의 수조가 심하게 팽대, 감소되었고 다수의 리소좀이 관찰되었다. 사립체의 수는 감소하고 사립체능이 손실, 절단되고 모양은 변형되었다. 핵이 약간 함입되어 불규칙하였다. B-2군은 B-l군보다 rER과 사립체의 수가 증가하고, 핵은 정상으로 관찰되었다. In the liver cell, The cristae of mitochondria were enlarged, the rough endoplasmic reticulum (rER) extended and many lysosome observed on the Group A- 1. The cristae of mitochondria were enlarged and many lysosome observed, but nucleus was normal on the Croup A-2. All of observed parts on the Group B-1 deeply wounded by lead than the group A-1. In the Group B-2, the mitochondria and rER were increased in number than the Group B-1 and the nucleus was observed normally.

        간세포암의 십이지장 직접전이에 의한 상부 위장관 출혈 1예

        간세포암의 위장관 전이는 드물게 나타나며, 주로 혈행성 전파 또는 림프절 전이에 의해 발생하지만, 드물게는 직접 침범에 의해 일어난다. 위장관 전이의 증상으로는 대부분이 복부 통증이며, 위장관 출혈이 동반될 수도 있다. 위장관 전이가 동반된 간세포암의 경우 대개 예후는 불량하다. 저자들은 위장관 출혈로 내원한 원발성 간암 환자에서 십이지장으로 직접 전이된 간세포암 1예를 경험하였기에 보고하는 바이다. Although a metastasis to the gastrointestinal tract (GI) is rare in patients with hepatocellular carcinoma (HCC), it can occur by hematogenous or lymphatic spread, or by direct invasion of a tumor. A 61-year old woman who had a progressing large primary liver cancer presented with upper gastrointestinal (UGI) bleeding. UGI endoscopy showed a large duodenal submucosal tumor-like mass with a central ulcer and adherent blood clots. Endoscopic biopsy and coagulation using argon plasma probe were performed. The microscopic examination revealed a HCC. We report this unusual case of HCC with direct invasion of the duodenum. (Korean J Gastrointest Endosc 2006;33:298-302)

        Hot Wall Epitaxy(HWE)법에 의한 BaIn<sub>2</sub>S<sub>4</sub> 단결정 박막 성장과 광전도 특성

        A stoichiometric mixture of evaporating materials for $BaIn_2S_4$ single crystal thin films was prepared from horizontal electric furnace. To obtain the single crystal thin films, $BaIn_2S_4$ mixed crystal was deposited on thoroughly etched semi-insulating GaAs(100) substrate by the Hot Wall Epitaxy (HWE) system. The source and substrate temperatures were $620^{\circ}C$ and $420^{\circ}C$, respectively. The crystalline structure of the single crystal thin films was investigated by double crystal X-ray diffraction (DCXD). The carrier density and mobility of $BaIn_2S_4$ single crystal thin films measured from Hall effect by van der Pauw method are $6.13{\times}10^{17}cm^{-3}$ and $222cm^2/v{\cdot}s$ at 293 K, respectively. The temperature dependence of the energy band gap of the $BaIn_2S_4$ obtained from the absorption spectra was well described by the Varshni's relation, $E_g(T)=3.0581eV-(3.9511{\times}10^{-3}eV/K)T^2/(T+536K)$. The crystal field and the spin-orbit splitting energies for the valence band of the $BaIn_2S_4$ have been estimated to be 182.7 meV and 42.6 meV, respectively, by means of the photocurrent spectra and the Hopfield quasicubic model. These results indicate that the splitting of the ${\Delta}so$ definitely exists in the ${\Gamma}_5$ states of the valence band of the $BaIn_2S_4/GaAs$ epilayer. The three photocurrent peaks observed at 10 K are ascribed to the $A_1$-, $B_1$-exciton for n = 1 and $C_{24}$-exciton peaks for n = 24.         호흡곤란 증후군 미숙아에서 혈중 L-arginine 농도와 Oxygenation Index 및 폐동맥압과의 관계

        Purpose : This study was performed to observe the relationship of plasma L-arginine level and the severity of disease and pulmonary artery pressure in respiratory distress syndrome of premature infants. Methods : Peripheral blood samples were obtained at 1st, 3rd and 7th day from 21 premature infants with respiratory distress syndrome to analyze the L-arginine concentration. Oxygenation index (OI), an indicator of the severity of the disease, was calculated at the same time of the blood sampling. And pulmonary artery pressure was measured by Doppler echocardiography at each period. Plasma L-arginine level, OI and right ventricular systolic time interval(RVSTI) were analyzed. Results : Plasma L-arginine concentration of the 1st day was lower than 3rd and 7th day. OI and RVSTI were significantly correlated with each other(r=0.772, P<0.01). Plasma L- arginine level was correlated with oxygenation index(r=-0.346, P<0.01) and RVSTI(r=-0.416, P<0.01). Conclusion : Plasma L-arginine level was lowest in the 1st day during the study period. Plasma L-arginine concentration correlated significantly with the severity of respiratory distress syndrome and pulmonary artery pressure in premature infants. 목 적 : RDS가 있는 미숙아에서 질병의 심한 정도와 폐동맥압 및 혈중 L-arginine 농도와는 어떤 연관이 있는지 알아보고자 하였다. 방 법 : RDS로 인공 환기요법을 시행 받은 재태기간 37주 미만 미숙아 21명을 대상으로 하여 제 1병일, 제 3병일, 제 7병일에 혈액을 채취하여 혈중 L-arginine 농도를 측정하였다. 혈액채취와 동일한 시기에 호흡곤란 증후군의 중등도를 나타내는 지수로서 OI를 산출하였고, 도플러 심초음파를 이용하여 RVSTI를 측정하여 폐동맥압의 지표로 삼았다. OI와 혈중 L-arginine 농도 및 RVSTI 간의 상관관계를 알아보았다. 결 과 : 1) OI가 클수록 RDS 미숙아의 혈중 L-arginine 농도는 낮았다(r=-0.346, P<0.01). 2) OI가 클수록 RVSTI도 증가되었다(r=0.772, P<0.01). 3) RVSTI가 커지면 RDS 증후군 미숙아의 혈중 L-arginine 농도는 낮았다(r=-0.416, P<0.01). 결 론 : RDS가 있는 미숙아에서 질병이 심할수록 혈중 L-arginine 농도는 낮고, 폐동맥압은 높았으며, 폐동맥압이 높을수록 혈중 L-arginine 농도는 낮은 관계를 보였다.
