        高麗本 『孝行錄』과 中國의 「二十四孝」

        The ≪Hyohaengnok(孝行録, Record of Filial Piety)≫ was completed by Kwon Bo's adding 38 pieces of Chinese Folklore of Filial Piety besides the Painting of the Twenty‐Four Filial Exemplars to the original version dedicated to himself, who is Kwon Jun's father.  The original version was composed of the Painting of the Twenty‐Four Filial Exemplars, which Kwon Jun had ordered up, and the comments added by Yi Chehyon. However, there exists distinction in the characters between the stories selected in the Painting of the Twenty‐Four Filial Exemplars for Record of Filial Piety and the contemporary Twenty‐Four Filial Exemplars circulated in China.   Archeologists excavated a large number of Painting of the Twenty‐four Filial Sons(二十四孝圖) in Song(宋), Jin(金), Yuan(元) tombs throughout Henan and Shandong Provinces. All of these Painting of the Twenty‐four Filial Sons are identical to the Twenty‐Four Filial Exemplars of the Record of Filial Piety. Given this, how can we explain the change in characters that had been consistent up to Yuan(元) Dynasty? Presumably it is due to the substitution of parts involving brutality or immorality such as cutting off of limbs or abandonment of a parent that occurred after Yuan(元) period. Therefore, the Twenty‐Four Filial Exemplars in the current Record of Filial Piety are the original Twenty‐Four Filial Exemplars that had been popular in the northern parts of China in Song(宋), Jin(金) and Yuan(元) periods. However, in Korea the Twenty‐Four Filial Exemplars developed into a moral and instructive text of Three Fundamental Principles and the Five Moral Disciplines in Human Relations with the addition of moral texts by Gwon Geun(權近). By contrast those transmitted to Japan, China and Vietnam became a major source of recreational literature with moral lesson. In this regard the Twenty‐Four Filial Exemplars can be seen as a valuable material for studying the commonness and difference within the East Asian culture as well as the cultural exchanges.

        수혈의학 검사실의 환자안전사고 보고서 분석 및 개선 방안 적용 경험

        Background: Blood transfusion poses high risks and has a high probability of error because of the complexity and involvement of several people in the process. The purpose of this study was to share our experience in classifying reports related to blood transfusions. We included patient safety reports that were prepared over a 10-year period that began from the opening of the hospital. We then analyzed the causes and the corrective actions. Methods: We analyzed 125 reports related to blood transfusions, and these reports were included in the patient safety reports received from November 2008 to December 2018. The events were categorized as sampling error, inspection error, testing error, issue error, disposal error, transfusing blood components error, or others error, depending on the stage of the blood transfusion process. Regardless of the cause, the event that led to an inappropriate transfusion was classified as a transfusion incident. Results: The number of blood transfusions per year increased, and the rate of blood transfusion accidents ranged from 0.00% to 0.05% per year. A total of 125 reports were prepared over a 10-year period, and these included 8 blood sampling errors, 11 testing errors, 2 issuing errors, 94 disposal errors, 3 others errors, and 7 errors associated with the transfusing of blood components. After the transfusion incident, PDA was applied as a solution. Transfusing the wrong blood components did not occur, and the incidence of taking blood from the wrong patients was decreased. Conclusion: We applied corrective actions according to the cause of the event and we confirmed that the blood transfusion incidents decreased. (Korean J Blood Transfus 2019;30:212-218)

        Metastatic Skin Lesions on Lower Extremities in a Patient with Recurrent Serous Papillary Ovarian Carcinoma: A Case Report and Literature Review

        Clinical observation of skin metastasis in ovarian cancer cases is relatively uncommon. And distant metastatic skin lesions including the extremities are much rarer still as most metastatic skin lesions are located in the skin in the abdominal wall adjacent to where the primary ovarian tumors exist. We report the case of a 60-year-old woman who presented skin lesions on both lower extremities as a consequence of the metastasis of serous papillary adenocarcinoma of the ovary. She presented with erythematous and painful cutaneous nodules on both upper legs and in the inguinal area 42 months after initial diagnosis of ovarian cancer. Skin biopsy revealed metastasis of adenocarcinoma in the dermis. She was treated with surgical excision and systemic chemotherapy. Literature review has suggested that a combined modality approach including surgical excision and chemotherapy may be useful in the management of skin metastases due to ovarian cancer.

        義湘의 입당항로와 義天의 여송길

        The Korean envoy’s route to China was likely to be influenced by the contemporary international situation. There were three sea routes in Tang and Silla Dynasty;the north sea route, the sea route across Yellow sea and the south sea route. The Dengzhou was the central harbor in China of the north sea route. The sea route was safe relatively because it was not only along the coast but capitals of China of many generation located in North. The sea route across Yellow sea was from Gyeongju to DengZhou through the southern coast of Korea and Heuksando Island. In the neighborhood of Heuksando Island, if they sailed to south, they could reach to the downstream of Yangzi River and Mingzhou and Gwangzhou, if more sailing to south. There were materials about the south sea route to assume that there was excahnge between Korea and south·west world earlier on. The historian recorded the content of exchange with southern dynasties in China as the legend of Heo family did. As mentioned earlier, there were group residence of Silla people and Goryeogwan in Ningbo wich was one of central ports and harbors in southern China. Uisang succeed to reach to Tang Dynasty in 661, after his first failure to arrival in 650. He landed to Ma harber, Maqiaocun, Yangzhou. And using the Grand Canal, he moved northward, and reached to Zhixiang Temple in Mt. Zhongnan. He studied in 7 years with Tripitaka Ziyan and written famous book, Ilseungbeongyedoki. He is though to return to his county from Dengzhou, because in 671 when he returned to Korea, the international situation around Yellow sea was relatively stable, and many merchant vessels could come and go. He established Buseoksa(Temple) in Mt. Taebaeksan in 676 and became the founder of Huayan school. As the relation between Korea and Song Dynasty was broken, when Uicheon reached to Song Dynasty, Song Dynasty was glad to his appearance. At first, he had purpose to examine and study Buddhism in Song Dynasty, and discourse with high priest of Buddhism. He discussed the religious viewpoint of suspension and Chentae with Rabbi Youcheng and studied the Buddhism from Tianjixiang. After one month, he departed to Hangzhou, the center of Buddhism of southern China, because Rabbi Jingyuan was waiting him. On his way along the Grand Canal, he met Chan master Liaoyuan and discussed with him to the Chan in Jinshansi Temple. And then, he met Rabbi Jingyuan and talked about Avatamsa, Surangama, Mahavaipulya, Mahayana. In February, 1086, Uicheon moved to Bianjing and bid farewell to Zhezong, and came back to Hangzhou, took his leave of Jingyuan and then leaved to Mt. Tiantai. He swore under the pagoda Liushen of Rabbi Zizhe, and made a trip back to his homeland in 20 May, and arrived in 29. When he stayed in Song, he discussed with 50 high priests of Buddhism regardless denomination. Uicheon, who passed away enter Nirvana in 47, became the founder of Tiantai Buddhism and was granted to Daegakguksa. 우리나라의 대중국 사신길은 당시의 국제정세에 좌우되는 경우가 많았다. 당과 신라 때의 해상교통로는 대개 세 길이 있었다. 북방해로와 황해횡단항로 그리고 남방해로였다. 북방횡단해로는 등주가 중국의 중심항이었다. 연해안을 따라 항해하는 해로라 비교적 안전하였을 뿐만 아니라, 역대 중국의 수도가 북쪽에 위치하고 있었기 때문이기도 했다. 황해횡단해로는 신라 경주의 인근 항구에서 남해안을 거쳐 흑산도 부근에서 뱃길을 서북쪽으로 돌려 등주 관내의 항포에 도착한다. 흑산도 인근에서 남쪽으로 항해하면 양자강 하구 인근이나 더 남쪽으로 가면 명주, 광주 방면으로 갈 수 있다. 조선술과 항해술의 발달 결과라 하겠다. 남방해로는 일찍부터 우리나라와 남·서쪽 세계와 교류가 있었다고 짐작되는 기록들이 있다. 1세기 중반, 김해 ‘가야’ 수로왕비 허씨의 전설도 그러하지만 사서는 중국 남조국가들과의 교류 내용들을 남기고 있다. 앞에서 논급한 바와 같이 남중국 항포의 중심항의 하나인 명주(영파)에는 신라인의 집단 거주지와 고려관이 있었다. 의상은 650년 첫 번째 입당에 실패하고 11년이 지난 661년 남방해로로 도당에 성공한다. 의상은 양주땅 여고진 마교촌 마항에 상륙했다. 대운하를 이용하여 북상, 종남산 지상사에 도착했다. 그곳 지엄삼장 회하에서 7년간 수학하면서 명저 『일승법계도기』를 찬술했다. 의상의 귀국항은 등주였을 것으로 짐작하고 있다. 귀국한 671년은 황해 정세는 비교적 안정되었고 많은 무역선들이 왕래하고 있었기 때문이다. 676년 태백산에 부석사를 창건하고 해동 화엄종의 개산조가 되었다. 702년 78세가 되던 해에 입적하였다. 의천이 입송한 때는 여·송관계가 단절되고 있었던 때라 송은 의천의 출현을 몹시 반가워했다. 원래 의천은 송나라 불교계를 두루 살펴보고 배우며 고승과 담론할 목적을 가지고 있었다. 의천은 천거해 준 유성법사와 현수·천태의 교관을 논하고, 천길상으로부터 범학을 배웠다. 한 달이 지난 뒤 중국 남쪽 불교의 중심지 항주로 떠났다. 전부터 알고 지낸 정원법사가 기다리고 있었던 때문이었다. 운하길을 따라 가는 길에 금산사에서 불인 요원선사를 만나 선을 논했다. 그 뒤 정원법사를 만나 화엄, 능엄, 원각, 기신 등을 담론하였다. 1086년 2월 의천은 변경으로 가서 철종에게 하직하고 항주로 돌아와 4월 정원과 작별하고 천태산으로 떠났다. 지자대사의 육신탑에 ‘탑하의 서’를 올리고, 5월 20일 귀국길에 올라, 29일에 돌아왔다. 의천은 송에 머무는 동안 종파를 가릴 것 없이 50여 명의 고승을 만나 담론했다. 세수 47세가 되던 해 입적한 그는 천태종의 비조가 되었고 대각국사의 시호가 내려졌다.

      • Changing etiologies of early-onset neonatal sepsis in preterm premature rupture of membrane and associated factors

        Our aim is to investigate the etiological changes in early-onset neonatal sepsis (EONS) in preterm premature rupture of membrane (pPROM) over the past 16 years and to analyze the associated factors. We included 1,050 neonates who were born before 34 weeks of gestation between January 1996 and June 2012 at our institution. EONS was defined when the micro-organisms were isolated from the blood culture within 3 days of birth. Micro-organisms were groups as gram positive, gram negative, and yeasts species. Comparisons of etiological micro-organisms were carried out between earlier period (January 1996 to December 2004) and later period (January 2005 to June 2012). Perinatal course including the use of antibiotics (beta-lactam alone vs beta-lactam plus macrolide vs not used), interval between rupture of membrane and delivery (longer than 7 days), and the incidence of neonates less than 1kg were also analyzed and logistic regression was performed to see which is the most associated factor with the changing etiologies. Out of 1,050 neonates who were born before 34 weeks of gestation after maternal pPROM, 34 (3.16%) were confirmed as EONS, and 38 causative organisms were identified on blood culture. EONS rate was 2.4% (11/449) in the earlier period and 4.5% (27/601) in the later period. There was no change in EONS rate caused by gram positive bacteria between the two periods (1.8% vs 1.6%). However, EONS rate caused by gram negative bacteria was significantly increased over the time period (0.7% vs 2.5%, p=0.020). In contrast that there was no significant difference in EONS rate caused by gram negative bacteria according to the use of antibiotics, longer ROM to delivery interval and neonates with less than 1kg showed higher EONS rates caused by gram negative bacteria.The etiological pattern of EONS has changed over the past several years, increasing rate by gram negative micro-organism. The longer ROM to delivery interval and increased neonates with less than 1kg were associated with this change. Further surveillance of organisms causing EONS and antibiotic susceptibility is warranted.
