        전통시장의 디지털 전환에 관한 연구; 전통시장 배달 앱을 중심으로

        본 연구는 최근 전통시장의 디지털 전환의 시도로 진행되는 전통시장 배달 앱(application; app)의 성공요인을 파악하고자 시행되었다. 연구를 위해 전통시장 배달 앱의 성공요인을 앱의 시스템 특성, 배송 특성, 상품 특성, 고객 응대 특성의 4가지로 설정하고 운영성과와의 관계를 실증분석 하였다. 전통시장 배달 앱을 이용한 영업활동을 하고 있는 상인 148명을 대상으로 설문을 진행하였으며, 연구 결과 앱의 시스템 특성, 배송, 상품판매와 관련한 판촉활동은 운영성과에 정(+)의 영향을 미치는 것으로 나타났으며, 고객 응대와 관련해서는 유의한 결과를 얻지 못하였다. 이는 일부 배달 앱의 경우 고객의 리뷰를 확인하고 대응할 수 있는 메뉴가 탑재되어있지 않아 나타난 결과로 해석할 수 있다. 전통시장 배달앱이 시행의 초기 단계이고 점진적으로 확대되어가는 현 상황에서 전통시장 배달 앱의 특성을 파악하고 운영성과에 미치는 영향을 분석하여 전통시장 배달 앱의 개발 지표를 만들고 디지털 전환을 통한 전통시장 활성화에 기여하는 데 본 연구의 시사점을 두고 있다. This study was undertaken to identify success factors of traditional market delivery apps and lead digital transformation of traditional markets. The success factors of traditional market delivery apps are represented by four variables: system characteristics of the app, delivery, products, and responsibility towards customer responses. These factors were analyzed to examine their impact on the operations performance. Data was collected by surveying 148 traditional market business owners. Results of the empirical analysis revealed that the app system characteristics, delivery characteristics, and product sales promotion, impart a significant effect on the operations performance. Conversely, the customer response characteristics showed no significant effects. This could be because some apps lack the function to check customer reviews and respond appropriately. Currently, where traditional market delivery apps are in the early stages of implementation and are gradually expanding, data obtained from this study helps to identify characteristics of traditional market delivery apps, and offers suggestions to create development indicators of the traditional market delivery apps by analyzing their effects on operational performance, thereby contributing to the revival of traditional markets through digital transformation.

        누적이론을 적용한 디지털전환과 경쟁우선순위에 관한 연구

        본 연구는 누적이론을 적용하여 디지털전환을 포함한 기업의 경쟁우선순위 간의 관계를 규명하고자 하였다. 경쟁우선순위간의 관계를 파악하는 일은 운영전략을 수립하는 데 매우 중요한 역할을 한다. 또한, 4차산업혁명 시대에 디지털전환을성공적으로 달성하는 것은 경쟁우위를 확보하는데 중요한 요소가 될 것이다. 본 연구는 경쟁전략분야에서 꾸준히 논의되어 온 누적이론이 현재에도 유효하게 적용 가능한지를 검증하고, 품질, 신뢰성, 유연성, 디지털전환, 비용, 5가지의 경쟁우선순위 간의 관계를 누적이론을 토대로 실증 분석하였다. 연구 결과를 토대로 디지털전환의 측정에 대한 새로운 방향을제시하고 디지털전환과 관련한 운영전략의 수립에 실증적 토대를 마련하고자 한다. 153개 제조 기업을 대상으로 분석을 진행하였으며, 구조방정식 모형을 이용하여 분석을 진행하였다. 연구 결과, 품질-신뢰성-유연성-디지털전환-비용의 순으로 통계적으로 유의미한 인과관계가 있음을 검증하였다. 디지털전환의 경쟁우선순위간 순서에 대한 타당성을 높이고자 대안모형을 설정하여 본 연구에서 설정한 순서와 비교한 결과, 본 연구에서 설정한 모형의 적합도가 가장 우수한 것으로 나타났다. 본 연구는 오랜 시간에 걸쳐 연구되어 온 경쟁우선순위인 품질, 신뢰성, 유연성, 비용에 디지털전환을 추가하여 경쟁우선순위 간의 인과관계를 실증 분석함으로써 누적이론을 양적/질적으로 확대하였으며, 디지털전환을 하나의 새로운 경쟁우선순위로 제시하고자 한 탐색적 연구라는 점에서 높은 시사점을 지닌다. Understanding the relationship among competitive priorities is essential in establishing an operational strategy. In addition, successful digital transformation will be essential in securing competitive priorities in the fourth industrial revolution era. By applying the cumulative theory, this study investigates the relationship among competitive priorities (quality, dependability, flexibility and cost), including digital transformation. The analysis was conducted on 153 manufacturing companies using SEM(structural equation modeling). The result shows a statistically significant causal relationship in the order of quality - dependability - flexibility - digital transformation - cost. Alternative models were established to validate the order of competitive priorities in digital transformation and compare with the research model in this study. Consequently the sequence established in this study was the most appropriate one. This study has major implications as it is a preemptive empirical study on digital transformation, and quantitatively and qualitatively expanded on the well-studied Cumulative theory.

      • Establishment of Particulate Matter Induced Acute Lung Inflammation Model in Mice

        Background Particulate matter (PM) is one of the main causes of the development and progression of lung diseases. Animal experiments have been attempted to investigate the effect of PM on human lung diseases. However, various Methods of PM delivery into lungs as shown in published reports cannot reflect the actual exposure of PM in humans. In this study, we want to establish mouse model of PM exposure using a whole-body inhalation system that is similar to humans exposed to PM. Then, we evaluated the damage of lung following inhalation of various doses of PM. Methods Low grade (50), middle (150), and high grade (300 μg/m3) concentrations of road dust were inhaled to C57 BL/6J mice for 8h/ day during 7 days. At D8, the mice were sacrificed then bronchoalveolar lavage (BAL)Fluids and lung tissues were prepared for analyzing cell & diff count, H & E stain, and cytokine assay. Results Total cell count and macrophage in BAL fluid were significantly increased at 150 and 300 μg/m3 treated group than controls. The percentage of foamy macrophage was significantly increased by treatment of road dust in a dose-dependent manner. Histologic findings from 300 mg/m3 group mice revealed dust contained alveolar macrophages, peribranchial dust deposition with subtle fibrosis, and bronchial epithelial regenerative hyperplasia. The levels of TNF-α in BAL fluids were increased by treatment of road dust. Conclusions We established a PM-induced lung inflammation model that is close to human exposure. Further studies including longer exposure duration will be needed to establish a chronic lung injury model. We suggest that our exposure system is suitable to apply to lung fibrosis, asthma, and emphysema model for studying the effect of PM in the diseases.       • Exogenous Tryptophan Aggravates Experimental Lung Fibrosis Through Activation of AKT-mTORC1 Pathway

        Background Using the metabolomic approach, we previously reported that lung tryptophan levels were significantly increased in IPF. However, the role of tryptophan in IPF pathogenesis remains unclear. we investigated whether regulation of tryptophan modulates the severity of lung fibrosis and to find its possible mechanism. Methods We measured the expression of main extracellular matrix components in MRC-5 cells with or without tryptophan. IPF primary fibroblasts were used to perform proliferation assay and measurement of tryptophan hydroxylase (TPH-1). Exogenous tryptophan was administrated in the bleomycin (BLM) exposed mice. Results In IPF lung lysates and fibroblast, TPH-1 protein levels were significantly increased than controls. Treatment of tryptophan increased collagen, fibronectin, and a-SMA expressions in MRC5 cells. Tryptophan dramatically augments fibroblast proliferation. Treatment of tryptophan activates AKT-mTOR C1 pathway molecules in IPF fibroblast. Exogenous tryptophan promotes BLM-induced lung damage and fibrosis in mice. Conclusions These findings suggest that overproduction of tryptophan may contribute to the IPF pathogenesis and regulation of the tryptophan pathway may have therapeutic potential in preventing lung fibrosis.         Inhibition of trichostatin A-induced antiangiogenesis bysmall-interfering RNA for thrombospondin-1

        Expression of thrombospondin-1 (TSP-1), which is a known inhibitor of tumor growth and angiogenesis, is reciprocally regulated by positive regulators, such as VEGF. Additionally, trichostatin A (TSA) sup - presses tumor progression by altering VEGF levels and VEGF-mediated signaling. Thus, understanding TSA-regulated TSP-1 expression and the effects of altered TSP-1 levels might provide insights into the mechanism of action of TSA in anti-tumorigenesis, and provide an approach to cancer therapy. Here, we examined the effect of TSA on TSP-1 expression, and the effects of TSA-induced TSP-1 on cell motility and angiogenesis, in HeLa and bovine aortic endothelial cells. TSA remarkably increased TSP-1 expression at the mRNA and protein levels, by controlling the TSP-1 promoter activity. Both TSA and exogenous TSP-1 reduced cell migration and capillary-like tube formation and these activities were confirmed by blocking TSP-1 with its neutralizing antibody and small-interfering RNA. Our results suggest that TSP-1 is a potent mediator of TSA-induced antiangiogenesis.

        Immunogenicity of COVID-19 Vaccination in Patients With End-Stage Renal Disease Undergoing Maintenance Hemodialysis: The Efficacy of a Mix-and-Match Strategy

        Background: The objective of this study was to evaluate the immunogenicity of coronavirus disease 2019 (COVID-19) vaccination in patients with end-stage renal disease (ESRD) on hemodialysis. Methods: ESRD patients at the hemodialysis center of a tertiary-care university-affiliated hospital and healthy employees at the clinical laboratory center were prospectively recruited between March and June 2021. For severe acute respiratory syndrome coronavirus 2 (SARSCoV-2) antibody analysis, blood samples were collected serially on days 0, 14, 28, and 56 after the first vaccine dose, and on days 7 and 50 after the second dose. Antibodies against the SARS-CoV-2 spike protein were quantified, and SARS-CoV-2 neutralizing antibodies were measured in the serum and plasma. Results: Thirty-one ESRD patients and 55 healthy employees were regularly monitored. Twenty-five (80.6%) ESRD patients on hemodialysis received a mix-and-match strategy with ChAdOx1-BNT162b2 (AZ–Pf group) and six (19.4%) received two doses of ChAdOx1 (AZ–AZ group). ESRD patients on hemodialysis showed lower binding antibody titers and neutralizing antibody activities compared to healthy participants following the first vaccination with ChAdOx1. After the second dose, AZ-Pf group had higher immunogenicity than healthy people on days 7 and 50. The binding antibody titer and neutralizing antibody activities on days 7 and 50 were significantly higher in the AZ–Pf group than in the AZ–AZ group. Conclusion: ESRD patients on hemodialysis receiving the mix-and-match strategy (ChAdOx1–BNT162b2) have COVID-19 vaccine immunogenicity comparable to healthy individuals receiving two doses of ChAdOx1.

        Introduction: Hemobilia is a bleeding from the biliary tract and is relatively rare cause of bleeding in the digestive tract. Hemobilia is often developed due to trauma, surgery in biliary tract, gallstone, cholangitis and rarely by the tumors of the biliary tract. Biological behavior and clinical course of gallbladder adenosquamous carcinoma are not clear. Few cases of hemobilia caused by gallbladder adenosquamous carcinoma have been reported. Here, we describe a rare case of gallbladder carcinoma containing squamous component manifested by hemobilia. Case report: A 66-year-old female patient referred to our clinic due to acute cholecystitis. Patient`s symptoms are chill, fever, right upper quadrant abdominal pain for 7days. Computed tomography(CT) showed gallbladder distension, CBD dilatation and 39x25mm sized soft tissue mass with enhancement in the gallbladder. We performed ERCP for evaluation of biliary obstruction. Fresh blood was gushing out from major papilla on duodenoscopy. Cholangiography revealed amorphous fi lling defect in common bile duct with duct dilatation. We observed amorphous isoechoic lesion from proximal bile duct to distal duct in intraductal ultrasound (IDUS). We performed endoscopic sphincterotomy (EST) and removed blood coagula by basket and balloon sweeping. Biliary tract obstruction was not resolved due to persistent gallbladder bleeding. Repeated clot removal and cleansing of CBD releaved biliary obstruction and decreased liver function test. Seven days later, she underwent laparoscopic cholecystectomy. Pathologic fi nding of gallbladder was adenosquamous carcinoma. Conclusion: Hemobilia can occur with trauma, cholelithiasis, acalculous infi ammatory disease, medical intervention, neoplastic disease and surgury. Clinical feature and course of gallbladder cancer containing squamous component is not well known. We experienced a persistent hemobilia by bleeding from adenosquamous gallbladder cancer, and resolved the CBD obstruction by EST and washing the CBD following cholecystectomy.

        Introduction: Clinostomum complanatum species are fiuke mainly found a bird`s throat or esophagus which eats freshwater fi sh. Rarely the fi uke is attachced on the throat of human who eat raw fi sh and causes the symptom such as pharyngitis. A few cases of Clinostonum complanatum in human have been reported in Korea. We describe a case of human infection with Clinostomum complanatum which was extracted from the larynx endoscopically. Case report: A 46-year-old female patient presented at our clinic with a 2day history of throat discomfort. Throat discomfort developed after a week from eating sliced raw gray mullet. Endoscopic fi nding revealed about 0. 5cm long, fi at and transparent movable fi uke with inner black lines was found on the mucosa of left arytenoid cartilage. The fi uke was endoscopically removed successfully. Histopathological examination of the specimen revealed a parasite known as Clinostomum Complanatum. Conclusion: Clinostomum is usually attached to the membrane of the throat in human and causes acute pharyngitis, or laryngitis called halzoun. Though C. Complanatum rapidly grows after human infection, most case are not serious. Thus endoscopic removal of parasite is the fi rst choice of treatment in human infection with Clinostomum. This case emphasize the importance that pharyx and laryx should be carefully examined especially in patient showing throat discomfort.
