        Importance of Differential Diagnosis of a Possible Brain Tumor in Patients with Cervical Radiculopathy

        Lesions occurring simultaneously in the somatosensory or motor cortex of the brain and the cervical spine are rare. Brain tumors can cause similar symptoms to cervical lesions which can lead to confusion in treatment priorities. Moreover, if cervical disease is noticeably observed in radiologic findings of a patient complaining of cervical radiculopathy with non-specific electromyography results, it is common to no longer perform further evaluation. Here we introduce two cases where the cause of cervical radiculopathy was first considered to be the result of a degenerative cervical disease but was later discovered to be a result of a brain tumor.

        White Cord Syndrome: A Reperfusion Injury Following Spinal Decompression Surgery

        Reperfusion injury of the spinal cord has been scarcely reported. Herein, we present a case of white cord syndrome after spinal decompression. A 61-year-old male, who initially had ossification of the posterior longitudinal ligament at C3-6 level, was admitted to our hospital with a ruptured disc at the C6-7 level. The patient experienced radiating pain in both upper extremities. Anterior cervical discectomy and fusion was performed. However, the patient developed quadriplegia. Emergency magnetic resonance imaging revealed a new and enlarged signal change in the spinal cord at the C4-7 level. Additional posterior decompression surgery was performed. After intense rehabilitation, the patient’s motor function improved to grade 4. White cord syndrome is likely due to reperfusion injury following operative decompression of a compressed spinal cord segment. Although rare, spine surgeons should be aware of this complication and warn patients preoperatively.

        Segmental Zoster Paresis with Late-Onset Skin Rash Mimicking a Herniated Cervical Disc: A Case Report

        Segmental zoster paresis (SZP) of the limbs, characterized by focal weakness of the extremity, is recognized as a rare complication of herpes zoster that can lead to a misdiagnosis of other radiculopathies or musculoskeletal problems. Here, we present a rare case where the patient experienced severe pain and weakness of the right upper extremity caused by SZP. A 58-year-old man came to the hospital with severe right upper extremity radicular pain. Cervical magnetic resonance imaging of the spinal cord showed disc protrusion at the C5-6 level with a high signal change. Anterior cervical discectomy and fusion was performed. On the first and second postoperative days, the patient’s symptoms showed slight improvements. However, on the fourth postoperative day, the patient experienced a right shoulder drop along with exacerbated radicular pain and presented a herpetic rash on the right extremity. The patient showed a dramatic improvement in pain and weakness after taking antiviral medication and undergoing a fluoroscopic-guided right C6 selective nerve root block. He fully recovered from his symptoms after 12 weeks of rehabilitation and physical therapy. This rare case involved SZP which was misunderstood as a herniated cervical disc. If a patient complains of severe pain with motor weakness inconsistent with radiologic findings, SZP should be discriminated from other possible diagnosis.

        하안검의 종괴로 발현된 전이성 위암 1 예

        Eyelid tumors, including metastatic lid tumors, present clinically as recurrent chalazia, multicentric nodules, ulcerated lesions, diffuse swelling, or chronic blepharoconjuntivitis. Lid tumors consist of benign lesions(69%) and malignant lesions(31%). Basal cell carcinoma and squamous cell carcinoma account for 92% of the malignant lesions. Metastatic lid disease was found to be of rare occurence, accounting for 0.1% of all lid lesions and 0.3% of malignant lid lesions. We report a case of metastatic lid carcinoma of stomach cancer. A 60-year-old man developed a painless, slow-growing nodular lesion of his right lower eyelid. Excisional biopsy revealed metastatic undifferentiated adenocarcinoma. We believe this is the first case in Korean literature.

        저자들은 안면부 등의 경화를 주소로 내원한 28세 남자환자에서 당뇨병 등의 동반 질환이 없이 급성 상기도 감염에 속발하여 발생한 경화부종 1예를 경험하였기에 문헌고찰과 함께 보고하는 바이다. Scleredema is a rare connective tissue disease of unknown etiology and characterized by nonpitting thickening of subcutaneous tissue of the neck, face, head, upper trunk and arms. We present a 28 year-oldman with extensive skin changes following upper respiratory infection, who have not any concurrent disorders such as diabetes mellitus. Histopathological findings of skin showed normal epidermis with a widening of spaces between collagen bundles by infiltration of acid mucopolysaccharide in the dermis.

      • Could a Simple Nerve Block be Considered as a Treatment Option for Occipital Neuralgia with Both Short-Term and Long-Term Effects?

        Objective The study aims to show both the short- and long-term treatment outcome of occipital nerve block (ONB) patients with occipital neuralgia (ON). Methods Patients who visited our hospital between 2013 and 2020 were reviewed retrospectively. Patients were excluded if the medical records were incomplete, if they had received a cervical operation, if they had a traumatic event prior to the symptom onset, or if the follow-up period was less than 3 months. ONB targeted the greater occipital nerve, the lesser occipital nerve or both. Short term follow-up period was defined as 3 months and long term was defined as 12 months. Injection consisted of a mixture of triamcinolone acetonide, lidocaine, and normal saline. Visual analogue scale (VAS) was recorded and compared before and during the follow-up to period to assess treatment outcome. Results Clinical charts of 309 candidates were reviewed and 72 patients were excluded, making it 237 patients. VAS scores significantly decreased in both 3-month follow-up (from 7.20±0.94 to 3.48 ±1.66, p<0.05) and after 1 year follow-up (from 7.20±0.94 to 2.71±1.07, p<0.05). Only 8 patients (3.3%) were refractory to ONB and the procedure was found to be relatively safe since only 2 patients (0.8%) showed transient side effects. Conclusion There are many treatment options for ON. However, from the results of our study, conservative treatment via ONB may have sufficient effect in controlling symptoms of ON in both short and long term.

        Pharmacological Management of Germinal Matrix-Intraventricular Hemorrhage

        Germinal matrix-intraventricular hemorrhage (GM-IVH) is among the devastating neurological complications with mortality and neurodevelopmental disability rates ranging from 14.7% to 44.7% in preterm infants. The medical techniques have improved throughout the years, as the morbidity-free survival rate of very-low-birth-weight infants has increased; however, the neonatal and long-term morbidity rates have not significantly improved. To this date, there is no strong evidence on pharmacological management on GM-IVH, due to the limitation of well-designed randomized controlled studies. However, recombinant human erythropoietin administration in preterm infants seems to be the only effective pharmacological management in limited situations. Hence, further high-quality collaborative research studies are warranted in the future to ensure better outcomes among preterm infants with GM-IVH.

        간의 국소결절성과형성 1 예

        Focal nodular hyperplasia(FNH) is a rare benign hepatic tumor usually discovered serendipitous]y by an imaging procedure performed for some other reason. FNH is typically asymptomatic, and invariably follows a benign course. Accordingly, the preferred management is conservative, and excision is reserved for large symptomatic and complicated lesion, or when the diagnosis remains uncertain. Although many cases of FNH has been described to date in other countries, only two cases of FNH has been reported in Korean literature. But biopsy proven case have not been reported in Korea. In the present report we describe a 15 cm sized symptomatic lesion of FNH in a 29-year-old woman, that was disclosed by various kinds of imaging procedure. A left lateral segmentectomy was performed. The mass was firm and showed areas of localized growth of mature hepatocytes and septal fibrosis accompanied with marginal ductal proliferation, consistent with FNH. It also displayed an incomplete stellate architectural configuration consisted of a central fibrous scar. We report a case of FNH with a review of the literature.
