        간내 담석과 동반된 간내 담도암에서의 p53과 Ki-ras의 발현

        Background/Aims: Hepatolithiasis or primary intrahepatic stones may be accompanied later by intrahepatic cholangiocarcinoma. This cancerous lesion is frequently associated with atypical hyperplastic epithelium that was suspected of being precancerous. To investigate the Ki-ras or p53 mutation may play a role in carcinogenesis, and to determine whether atypical hyperplastic epithelium may be precancerous, this study was performed in intrahepatic cholangiocarcinomas associated with hepatolithiasis. Methods: We examined 12 patients with intrahepatic cholangiocarcinomas associated with hepatolithiasis. Ki-ras point mutations were assessed by direct DNA sequencing. Expressions of p53 protein were immunohistochemically assessed. The overexpression of p53 and point mutation of Ki-ras were examined in normal, hyperplastic, atypical hyperplastic, and can- cerous tissues separately. Results: The expressions of p53 protein were detected in 4 (33%) of 12 cholangiocarcinomas and Ki-ras point mutations at codon 12 were found in 2 (17%) of 12 cholangiocarcinomas. In those two carcinoma cases which contained the mutant sequence of Ki-ras, the same point mutation from wild type GGT (glycine) to GAT (aspartic acid) was. found in the associated atypical hyperplastic epithelium. However, none of the normal and hyperplastic epithelium harbored Ki-ras or p53 mutations. Conclusions: The overexpression of p53 may play a part in the carcinogenesis of some intrahepatic cholangiocarcinomas associated with hepatolithiasis, whereas the role of Ki-ras mutations in those cases is dubious. However, Ki-ras point mutation at codon 12 may be responsible for either cancer or atypical hyperplastic epithelium associated with hepatolithiasis in certain cases, suggesting atypical hyperplastic epithelium may give rise to carcinoma.

        Background : In the military, ingrown nails are common occurences. There are currently many viable methods of treatment available, but there is a possibility of recurrence, and there are instances where the infection has developed to a point where treatment becomes difficult. This report intends to introduce the possibility of using an electrocauter machine available at 16th Fighter Wing, where the author is posted, to treat ingrown nails. Method : From April 28 to September 2 2012, 8 cases from 7 patients suffering from ingrown nails were seen. The surgical procedures for those patients, were as follows: 1. After local anesthesia, removal of ingrown section of nail from within granulated tissue using a freer. 2. Approximate longitudinal incision of 1cm at the proximal nail fold, using a mass. 3. The removal of ingrown nail by using a pair of scissors by cutting up to the previous incision at the proximal nail fold. 4. Electrodessication of the nail matrix, after lifting incised nail fold using a pair of forceps 5. One stitch using nylon 3-0 Result : The stitch out period was on average, 18.4 days, and there were no recurrences. Conclusion : The author believes that the usage of the electrocauter machine is a relatively simple and better surgical procedure, with a low rate of recurrence, for the treatment of ingrown nails.

      • RESEARCH ARTICLE : A novel surface-attached carvedilol solid dispersion with enhangced solubility and dissolution

        A novel surface-attached, spray-dried solid dispersion containing poorly water-soluble carvedilol (CV) without any change in the crystallinity was prepared using water, polyvinylpyrrolidone (PVP K30) and Tween 80. The solid dispersion was optimized by investigating the effects of the weight ratios of Tween 80/PVP K30 and carrier/drug on the aqueous solubility of CV. The optimum solid dispersion consisted of a relatively low carrier to drug weight ratio: the weight ratio of CV/PVP K30/Tween 80 was 12/4/2. Unlike conventional methods of solid dispersion preparation, this method yielded CV-loaded solid dispersion with no change in the crystallinity of the drug as was evident from SEM, DSC and XRD. It was demonstrated that the solid dispersions prepared had hydrophilic carriers attached to the surface of the drug, thus changing it from a hydrophobic to a hydrophilic form without changing the crystalline form. The optimized solid dispersion improved the drug solubility and dissolution rate by about 11,500-fold and twofold, respectively. It was further suggested that this method of solid dispersion preparation is better than conventional methods in terms of environmental and industrial standpoints. Thus, it was concluded that CV-loaded solid dispersion prepared using this method would be of use for delivering poorly water-soluble CV with enhanced solubility and dissolution, but without crystalline changes.

        울산지역 췌담관질환 환자의 간흡충 감염 실태 : 담즙검사를 기준으로 based on bile examination

        목적 : 간흡충증의 진단은 분변 또는 담즙에서 간흡충의 성충 또는 충란을 발견하는 것이다. 담즙을 통한 간흡충 진단은 간흡충이 존재하는 담관에서 검체를 직접 채취한다는 장점과 이 검체가 분변이나 체액에 의해 희석되지 않으며, 특히 담관 폐색이 있을 경우 담즙배설 장애로 인한 분변검사의 위음성을 줄일 수 있는 가장 정확한 간흡충의 기생충학적 검사로 생각된다. 이에 저자들은 울산지역 췌담관 질환 환자에서 담즙검사를 기준으로 한 간흡충의 감염 실태를 알아보고자 하였다. 방법 : 내시경적 또는 경피경간 담관배액술을 통해 담즙을 채취하여 간흡충 충란 유무를 확인한 309명의 췌담과 질환 환자에서 간흡충 감염 실태를 조사하였다. 결과 : 대상 환자 전체의 충란 양성률은 27.5%였고, 남녀의 충란 양성률은 각각 35.%와 17.6%로 남자에서 더 높았다. 연령별 충란 양성률은 30대 20.0%, 40대 26.7%, 50대 24.2%, 60대 29.9%, 70대 36.2% 그리고 80대 이상이 16.7%로 연량에 따른 통계적 차이는 없었다. 질병별 충란 양성률은 담관암 32.65, 담낭암 38.5%, 파터팽대부암 11.1%, 췌장암 24.0%, 담석질환 26.4%,간흡충담관염 61.5% 그리고 이외 질환이 12.5%로 간흡충 담관염ㅇ르 제외한 질병간 의미있는 차이는 없었으며 결석의 위치, 간흡충 연관질환인지 아닌지가 충란 양성률에 영향을 미치지 못했다. 담관 조영술을 포함한 방사선 검사에서 정상 담관을 보인 환자에서 충란 양성률은 17.0%였다. 결론 : 본 연구 결과 울산지역 췌담관 질환 환자에서 간흡충 감염률은 연령, 성별, 질병의 종류에 관계없이 전반적으로 매우 높았다. 따라서 이러한 자료를 근거로 생각해 볼 때 우리 나라가 아직도 심각한 간흡충 감염의 유행지임을 추론할 수 있겠다. Background : Bile examination is believed to be the most precise method for detecting Clonorchis sinesis (CS) eggs. We carried out bile examination to evaluate infestation state of CS in patients with pancreatobiliary diseases in Ulsan, known as an endemic area of CS infestation. Methods : We examined CS eggs in bile in three hundreds and nine patients with pancreatobiliary diseases. The bile was obtained from endoscopic nasobiliary or percutaneous transhepatic biliary drainage tubes. Results : The overall egg positive rate was 27.5% (35.3% in male,17.6% in female). The egg positive rate was not significantly different according to the age group: 20.0% in thirties, 26.7% in forties, 24.2% in fifties, 29.9% in sixties, 36.2% in seventies and 16.7% in eighties or more. The egg positive rate according to the disease, except CS cholangitis, was not also statistically different: 32.6% in bile duct cancer, 38.5% in gallbladder cancer, 11.1% in ampulla of Vater cancer, 24.0% in pancreatic cancer, 26.4% in gallstone diseases and 12.5% in the reminder. The location of gallstone and whether CS related diseases or CS unrelated diseases did not affect the egg positive rates. The egg positive rate in patients with normal radiological findings including cholangiography was 17.0%. Conclusion : This result shows that regardless of age, sex and sorts of diseases, the infestation rate of CS was very high. On the basis of our results, it if therefore presumes that clonorchiasis is still endemic disease in Korea.

        서울지역 길고양이의 Toxoplasma 감염증 실태조사

        Toxoplasma gondii is one of the most common protozoa parasites of human and other warm-blooded animals. Cats and wild felidae play crucial roles in the epidemiology of toxoplasmosis. This study was performed to survey the prevalence of T. gondii infection among stray cats in the Seoul. A total of 422 stray cat blood samples were collected from Seoul area. Positive sera for T. gondii were identified in 56 samples (13.3%) exclusively via the latex agglutination test and the detection no antigen particles among seropositive samples by PCR. The overall infection rate of male stray cats (14.4%) presented as higher than that of female cats (10.7%). This study suggested that T. gondii is widespread in stray cats of Seoul area. It is needed to control urban stray cat population and to reduced the risk of zoonotic transmission of toxolasmosis to other animals and humans.

        Isolation of Anticancer Compounds from Peucedanum japonicum Thunb. Roots

        This study was conducted to isolate a compound with anticancer properties from the roots of Peucedanum japonicum Thunb. (Umbelliferae), and to evaluate the efficacy of that compound's anticancer activity. The $CHCl_3$ layer was purified via repeated column chromatography and recrystallization. The two compounds isolated from $CHCl_3$ layer were identified via NMR spectroscopic analysis as (10E) 1,10-heptadecadiene-4,6-diyne-3,8,9-triol (Comp. I) and anomalin (Comp. II). (10E) 1,10-heptadecadiene-4,6-diyne-3,8,9-triol was the first report from the roots of P. japonicum. MTT assays were conducted to evaluate the in vitro cytotoxic activities of Compounds I and II against the following human cancer cell lines: HeLa, HepG2, SNU-16, and AGS. Comp. I evidenced the most profound cytotoxic activity against HepG2 cells ($IC_{50}=6.04{\mu}g/mL$), and Comp. II exhibited the most profound cytotoxic activity against SNU-16 cells ($IC_{50}=18.24{\mu}g/mL$) among the human cancer cell lines tested in this study. However, no significant cell death was observed in the CCD-25Lu human normal lung fibroblast cells. Quantitative analysis using UPLC (Ultra Performance Liquid Chromatography) showed that the roots of P. japonicum contained 0.015 (Comp. I) and 1.69 mg/g (Comp. II) of these compounds.

        서울지역의 개와 고양이에서 분리한 Staphylococcus aureus의 항생제 감수성 및 multiplex PCR을 이용한 장독소의 검출

        The antimicrobial susceptibility of 73 strains of Staphylococcus aureus isolated from dogs and cats was determined against 16 antimicrobial agents. All isolates were susceptible to cephalothin (CF), vancomycin (VA), rifampin (RA), quinupristin-dalfopristin (SYN) and linezolid (LZD), whereas more than 86.3% of the isolates were resistant to penicillin (P). In the case of enterotoxin production, 8.2% of 73 strains produced enterotoxin A and 1 strain produced enterotoxin C. Appropriate protocols for antimicrobial agents and strategies to reduce antimicrobial resistance will need to be investigated in the future.

        The objective of this study was to develop a polymer mortar protection block with high strength and durability using unsaturated polyester resin to complement defects of conventional cement mortar protection block. Physical and mechanical properties of the polymer mortar protection block were also investigated. Low absorptivity, high impact strength, and great bending strength of the polymer mortar protection block was compared with those of the conventional cement protection block. In conclusion, the polymer mortar protection block is excellent and useful as industrial products for erosion control works.
