      • Effectiveness of a low-frequency sports-specific resistance and plyometric training programme

        This intervention aimed to improve Badminton-specific functional performance of an elite under-19 player (18 years) via a single-weekly resistance and plyometric training (RPT) session for 8-weeks. The athlete’s ‘response-to’ and ‘withdrawal-from’ the RPT were assessed. The athlete (stature: 1.77 m, mass: 81.5 kg) completed the RPT intervention throughout a competitive season. Performance testing was conducted at; baseline, post-intervention (8-weeks) and after withdrawal-from the RPT (16-weeks). Jump height and throwing distance were assessed via countermovement jump (CMJ), standing long-jump (SLJ) and one-arm (1-MBT) and two-arm (2-MBT) medicine-ball throws respectively. Speed and agility were assessed by 5 m and 10 m sprints and ‘sideways’ and ‘four-corner’ tests respectively. A positive change from baseline to post-intervention (8-weeks) was observed in: CMJ (+0.07 m), SLJ (+0.13 m), 1-MBT (+2.25 m) and 2-MBT (+0.26 m). Observed changes in speed and agility were minimal. At the 16-weeks, small declines in CMJ, SLJ, 1-MBT and 2-MBT (0.04 m, 0.04 m, 0.52 m and 0.05 respectively) existed. Small improvements in 5 and 10 m sprint times (-0.12 s to -0.09 s) and side-ways agility were observed (-0.11 s to -0.39 s), possibly explained by conditioning effects of training and competition. An 8-week Badminton-specific RPT programme provided favourable increases in jumps and MB throws. However, such positive changes were not sustained following the withdrawal period.

        Prevalence and Predictors of Complementary Feeding Practices Among Children Aged 6-23 Months in Indonesia

        Objectives: Poor complementary feeding practices have consistently contributed to the burden of child undernutrition in Indonesia. This study aimed to estimate the prevalence and predictors of the time of the introduction of solid, semi-solid, and soft foods (ISSSF), minimum dietary diversity (MDD), minimum meal frequency (MMF), and minimum acceptable diet (MAD). Methods: We analyzed 4804 last-born infants aged 6-23 months from the 2017 Indonesia Demographic and Health Survey, which employed multistage cluster random sampling. The outcomes were calculated based on the 2021 World Health Organization/United Nations Children’s Fund guidelines. The predictors of the 4 complementary feeding indicators were assessed using multivariate Poisson regression with robust variance adjusting for potential confounders and study design. Results: The prevalence of ISSSF, MDD, MMF, and MAD was 86.1%, 54.3%, 71.8%, and 37.6%, respectively, with younger children less likely to meet 3 out of the 4 outcomes. Parental education, the presence of a birth attendant, and maternal media consumption were among the predictors of MDD and MAD. Children from families with higher income were more likely to meet MDD than those from low-income households (adjusted prevalence ratio [aPR], 1.16; 95% confidence interval [CI], 1.05 to 1.28). Living in an urban area was positively associated with MMF (aPR, 1.09; 95% CI, 1.04 to 1.15) and MAD (aPR, 1.12; 95% CI 1.02 to 1.24). In eastern regions, the prevalence of children achieving MDD and MAD was lower than in those living in Java and Bali. Conclusions: It is crucial that more attention and efforts are made to improve the recommended practices throughout Indonesia, since the prevalence of adequate complementary feeding practices remains low.

      • Dynamics of Precise Ethylene Ionomers Containing Ionic Liquid Functionality

        <P>This paper presents the first findings on the molecular dynamics of the remarkable new class of linear and precisely functionalized ethylene copolymers. Specifically, we utilize broadband dielectric relaxation spectroscopy to investigate the molecular dynamics of linear polyethylene (PE)-based ionomers containing 1-methylimidazolium bromide (<B>ImBr</B>) pendants on exactly every 9th, 15th, or 21st carbon atom, along with one pseudorandom analogue. We also employed FTIR spectroscopy to provide insight into local ionic interactions and the nature of the ordering of the ethylene spacers between pendants. Prior X-ray scattering experiments revealed that the polar ionic groups in these ionomers self-assemble into microphase-separated aggregates dispersed throughout the nonpolar PE matrix. We focus primarily on the dynamics of the segmental relaxations, which are significantly slowed down compared to linear PE due to ion aggregation. Relaxation times depend on composition, the presence of crystallinity, and microphase-separated morphologies. Segmental relaxation strengths are much lower than predicted by the Onsager theory for mobile isolated dipoles but much higher than linear PE, demonstrating that at least some <B>ImBr</B> pendants participate in the segmental process. Analysis of the relaxation strengths using the Kirkwood <I>g</I> correlation factor demonstrates that ca. 10–40% of the <B>ImBr</B> ion dipoles (depending on copolymer composition and temperature) participate in the segmental motions of the precise ionomers under study, with the remainder immobilized or having net antiparallel arrangements in ion aggregates.</P><P><B>Graphic Abstract</B> <IMG SRC='http://pubs.acs.org/appl/literatum/publisher/achs/journals/content/mamobx/2015/mamobx.2015.48.issue-2/ma502168e/production/images/medium/ma-2014-02168e_0011.gif'></P>

        A comparison of leak pressures between esophageal to esophageal anastomosis and esophageal to jejunal anastomosis

        The goal of this study was to determine if there was a difference in leak pressure between esophageal-esophageal anastomosis and esophageal-jejunal anastomosis when using cadaveric porcine tissue. Leak pressures were recorded for esophageal-esophageal anastomosis (Group 1 [control group], n = 7), cranial esophageal-jejunal anastomosis (Group 2, n = 7), and jejunal-caudal esophageal anastomosis (Group 3, n = 6). Each anastomosis was performed using polydioxanone sutures in a simple interrupted pattern. Results were analyzed using one-way analysis of variance. Mean ± SD of the leak pressures for groups 1, 2, and 3 were 46.1 ± 15.9, 36.5 ± 13.6, and 50.9 ± 11.1 mmHg, respectively (p = 0.18). When the results from groups 2 and 3 were combined and compared to that for Group 1, the mean ± SD leak pressures were 46.1± 15.9 and 43.1± 14.2 mmHg, respectively (p = 0.67). These results provide preliminary evidence that the jejunum may be a suitable option for use in esophageal replacement surgery; however, future studies of in vivo factors influencing the integrity of esophageal-jejunal anastomoses, including histologic evaluation of esophageal-jejunal anastomosis healing, are needed.

        Radiofrequency ablation (Rafaelo Procedure) for the treatment of hemorrhoids: a case series in the United Kingdom

        Purpose: Hemorrhoidal disease remains a common condition that can have a significant effect on a patient’s quality of life. Various methods have been introduced over the years; however, their overall success rates remain low. Although the traditional Milligan Morgan technique is effective, the associated pain level prevents it from being an attractive form of treatment. This study was devised to assess the safety and efficacy associated with a novel minimally invasive approach, radiofrequency ablation (RFA). Methods: Forty-two patients underwent RFA at a single center, by 1 of 2 surgeons. This was performed under local anesthetic and sedation. Outcomes including postoperative pain levels, recurrence rates, and patient satisfaction scores were recorded and analyzed using medians and interquartile ranges. Results: The median postoperative pain score was 2.5/10 (interquartile range [IQR], 0–4.5) and the overall patient satisfaction score was 9/10 (IQR, 6.5–10). Recurrence rates (6–12 months following the procedure) were low at 12% and all patients reported milder symptoms at recurrence. There were no serious adverse complications. Conclusion: The results from this case series supports other limited data in concluding that RFA is a safe and effective method in the treatment of hemorrhoids and patients report a high level of satisfaction following.

        Wireless structural health monitoring of bridges: present and future

        Internationally the load carrying capacity of bridges is decreasing due to material deterioration while at the same time increasing live loads mean that they are often exposed to stresses for which they were not designed. However there are limited resources available to ensure that these bridges are fit for purpose, meaning that new approaches to bridge maintenance are required that optimize both their service lives as well as maintenance costs. Wireless sensor networks (WSNs) provide a tool that could support such an optimized maintenance program. In many situations WSNs have advantages over conventional wired monitoring systems in terms of installation time and cost. In order to evaluate the potential of these systems two WSNs were installed starting in July 2007 on the Humber Bridge and on a nearby approach bridge. As part of a corrosion prevention strategy, a relative humidity and temperature monitoring system was installed in the north anchorage chambers of the main suspension bridge where the main cables of the bridge are anchored into the foundation. This system allows the Bridgemaster to check whether the maximum relative humidity threshold, above which corrosion of the steel wires might occur, is not crossed. A second WSN which monitors aspects of deterioration on a reinforced concrete bridge located on the approach to the main suspension bridge was also installed. Though both systems have provided useful data to the owners, there are still challenges that must be overcome in terms of monitoring corrosion of steel, measuring live loading and data management before WSNs can become an effective tool for bridge managers.

        Wireless operational modal analysis of a multi-span prestressed concrete bridge for structural identification

        Low-power radio frequency (RF) chip transceiver technology and the associated structural health monitoring platforms have matured recently to enable high-rate, lossless transmission of measurement data across large-scale sensor networks. The intrinsic value of these advanced capabilities is the allowance for high-quality, rapid operational modal analysis of in-service structures using distributed accelerometers to experimentally characterize the dynamic response. From the analysis afforded through these dynamic data sets, structural identification techniques can then be utilized to develop a well calibrated finite element (FE) model of the structure for baseline development, extended analytical structural evaluation, and load response assessment. This paper presents a case study in which operational modal analysis is performed on a three-span prestressed reinforced concrete bridge using a wireless sensor network. The low-power wireless platform deployed supported a high-rate, lossless transmission protocol enabling real-time remote acquisition of the vibration response as recorded by twenty-nine accelerometers at a 256 Sps sampling rate. Several instrumentation layouts were utilized to assess the global multi-span response using a stationary sensor array as well as the spatially refined response of a single span using roving sensors and reference-based techniques. Subsequent structural identification using FE modeling and iterative updating through comparison with the experimental analysis is then documented to demonstrate the inherent value in dynamic response measurement across structural systems using high-rate wireless sensor networks.
