Backgrounds/Aims: Accurate diagnosis of significant liver fibrosis in patients with chronic hepatitis B (CHB) is crucial when determining whether to initiate antiviral treatment (AVT). We conduct a meta-analysis to assess the diagnostic performance of vibration-controlled transient elastography (VCTE) for significant liver fibrosis in AVT-naïve CHB patients with serum alanine transaminase (ALT) levels within 5-fold the upper limit of normal (ULN). Methods: The Ovid-Medline, EMBASE, Cochrane, and KoreaMed databases were searched to identify studies that compared the performance of VCTE and liver biopsy (reference standard) when diagnosing significant liver fibrosis (≥F2) in AVT-naïve CHB patients with ALT within 5-fold the ULN. A hierarchical summary receiver operating characteristic curve (HSROC) and bivariate model were performed to evaluate the diagnostic performance of VCTE in the meta-analysis. Results: Eight studies (2,003 patients) were included. The summary sensitivity and specificity for diagnosis of significant liver fibrosis were 0.78 (95% confidence interval [CI], 0.66–0.86) and 0.72 (95% CI, 0.60–0.82), respectively. The HSROC for the diagnosis of significant liver fibrosis was 0.81 (95% CI, 0.72–0.86). The optimal cutoff value of VCTE for diagnosis of significant liver fibrosis was 7.7 kPa with a sensitivity of 0.64 (95% CI, 0.50–0.76) and specificity of 0.83 (95% CI, 0.72–0.90). Conclusions: Our study demonstrated that VCTE has an acceptable diagnostic performance for significant liver fibrosis in AVT-naïve CHB patients with ALT within 5-fold the ULN.

본 연구는 논에서 조사료 생산을 위한 작부형태별 우분액비의 시용효과를 규명하기 위하여 총체 벼 재배 후 우분액비 시용에 따른 총체보리 및 IRG의 생산성, 사료가치, 토양성분 및 용탈수중의 무기물 농도를 조사하였다. 본 연구는 2006년 5월부터 2009년 4월까지 3년 동안 전라북도 김제군 백산면 시험포장에서 완전임의배치 3반복으로 수행되었다. 총체보리의 2년간 평균수량은 7,520 kg/ha, 그리고 IRG는 평균 10,320 kg/ha으로 IRG가 총체보리 보다 현저하게 높은 수량을 보였다 (p<0.05). 이탈리안 라이그라스는 1번초가 2번초 보다 약 2배 이상의 수량을 보였다. IRG의 조단백질 함량은 총체보리보다 높았고 NDF, ADF 및 TDN 함량은 작물간에 큰 차이는 보이지 않았다. 경작형태별 우분액비 시용에 따른 토양내 pH, $P_2O_5$, T-N, OM 농도는 시험전에 비해 시험종료 후에 현저하게 증가하는 경향을 나타냈다 (p<0.05). 경작형태별 우분액비 시용에 따른 토양내 Ca, Na, Mg 및 K 농도는 시험전에 비해 시험종료 후에 현저하게 증가하는 경향을 나타냈다 (P<0.05). 경작형태별 우분액비 시용에 따른 용탈수중 $NH_4$-N, $NO_3$-N, $PO_4$-P, Cl, Ca, K, Mg, Na의 농도는 경작형태에 따른 확실한 차이를 보여주지 않았다. 따라서 논에서 우분액비를 이용하여 동계작물을 재배할 경우에는 수량도 중요하지만 환경적인 부분도 고려하여 우분액비를 이용해야 한다. This study was conducted to investigate the effects of cattle slurry on productivity of whole crop barley and Italian ryegrass and environmental pollution in paddy land. Cropping systems used in this study were consisted of two designs, such as whole crop barley applied with cattle slurry (WCB) and Italian ryegrass applied with cattle slurry (IRG). The field experiments were conducted on the clay loam at Backsanmyun, Kimje, Chunlabukdo province in Korea for three years (May 2006 to Apr. 2009). This study was arranged in completely randomized design with three replicates. The yields of WCB and IRG were 7,520 kg/ha and 10,320 kg/ha, respectively. The yields of IRG significantly increased as compared with that of WCB (p<0.05). The yield of Italian ryegrass in 1st cutting time was about 2-fold higher than that of 2nd cutting time. The contents of crude protein of IRG were higher than that of WCB. However, the contents of NDF, ADF and TDN were no difference between WCB and IRG treatments. The pH, and contents of T-N, $P_2O_5$ and organic matter (OM) in soil samples collected at the end of the experiment increased as compared with those at the beginning of the experiment (p<0.05). After the end of experiment, the concentrations of CEC (Ca, Na, Mg and K) in soil samples collected at the end of the experiment were remarkably higher than those at the beginning of the experiment (p<0.05). The concentrations of $NH_4$-N, $NO_3$-N, $PO_4$-P, Cl, Ca, K, Mg and Na in leaching water in paddy land cultivated with WCB and IRG were no difference between WCB and IRG treatments.

Patients with chronic renal failure (CRF) are known to be more susceptible to tuberculosis infection due to impairment of the host defense mechanism. Although extrapulmonary tuberculosis is more prevalent in those subjects and it may induce dismal outcome, its diagnosis has been challenging since there is no specific symptoms of the disease and the clinical course is usually atypical. Herein, We report a case of disseminated tuberculosis diagnosed by ultrasound-guided liver biopsy in a 31-year-old CRF patient presenting sustained fever despite broad-spectrum antimicrobial therapy and progressive cholestatic jaundice.

The definition of traditional 2-DS power factor current and co-ordinate did not represent clearly relation to power facter in circuit with nonlinear thyristor loads. A new simulator for performance evaluation of Active Power Filter (APF) by 3-D space current co-ordinate applicale to general power system have not been developed, because reserchers were merely interested in improvement of source current waveforms by using APF or its simulator. Moreover, There was not APF simulator with power analysis capability which was developed for the better. Current was decomposed by active, reactive, distortion, by digital correlation signal proccessing techique based on Nowomiejski theory. Therefore, A new 3-D space current co-ordinate composited by 3 current components was defined in this paper. A new 3-D space current co-ordinate was shown state and location of current vector by trajectories with 3-D co-ordinate rotation. It evidently has excellent performance not only performsance evaluation method of APF but alse evaluation method of general power system, not conventional domain but other domain. By proposed 3-D soace co-ordinate, space power factor is derived, the true state of physical meaning is proved, new distortion factor was proposed, this study was proved true state of appropriation throughout development of simulator for APF performance evaluation with power analysis capability.
인체 자궁암 세포에서 천연 성분이 P-당단백질의 활성에 미치는 영향

Multidrug resistance (MDR) of cancer cells is, at least in part, associated with the overexpression of P-gly-coprotein (P-gp). Many studies hve demonstrated that natural compounds obtained from fruits, vegetables, teas and medicinal plants may modulate P-gp activity. The objective of the present investigation was to examine the effect of seven natural compounds on the P-gp activity in human uterine sarcoma cell line, MES-SA/DX5. Daunomycin uptake was significantly increased by biochanin A and silymarin (p<0.0001) whereas it was reduced by morin (p<0.01). The efflux of daunomycin from the cells was significantly inhibited by biochanin A, morin, cephalotaxine, berberine (p<0.05) and silymarin (p<0.0001). Biochanin A, berberine and silymarin significantly decreased IC_(50) value of daunomycin (p<0.05) while morin increased it (p<0.05). These results suggest that some natural compounds such as biochanin A and silymarin may inhibit P-gp funciton and can by developed as MDR reversing agents to improve the efficacy of chemotherapeutic drugs when administered concomitantly.

취업장애인의 저임금은 장애로 인한 높은 추가비용과 공적급여의 미비로 인해 빈곤과 직결되어 있으며, 사회의 양극화 현상을 심화시키는 심각한 문제이다. 따라서 취업 장애인의 임금을 결정하는 요인을 포괄적이고 다각적으로 파악하고, 향후 발전적 방향에 대한제시가 필요한 시점이다. 본 연구는 취업장애인의 임금에 영향을 미치는 요인을 인구학적,장애관련, 인적자본, 노동시장 요인에 따라 분석하고 근로소득을 높이기 위한 정책적, 실천적 함의를 제안하고자 하였다. 이를 위해 제1차년도 장애인고용패널 데이터의 취업장애인 중 자영업자와 고용주를 제외한 임금근로자 1,212명을 대상으로 분석하였다. 연구결과, 인구학적 요인에서 성별, 결혼상태, 연령이, 장애관련 요인에서 장애유형, 장애등급, 장애발생 연령이, 인적자본 요인에서 고용서비스 이용여부, 직업교육 경험여부, 자격증 유무, 직급, 교육연한, 근속연수가, 노동시장 요인에서는 노동조합 유무, 복리후생 유무, 종사상 지위, 일자리 산업내용, 사업체 규모, 월근무시간이 월임금수준에 유의한 영향을 미치는 것으로 나타났다. 이를 통해 취업장애인의 임금을 향상하기 위한 방안을 인적자본 요인과 노동시장 요인을 중심으로 제언하였다. For workers with disabilities in Korea earned income accounts for the major proportion of their total income. This is due to a lack of public assistance for workers with disabilities, which means working is the major source of income. However, despite the importance of employment, their earned income is very low, so their economic situation is precarious. Nevertheless, there has been little research done on the determinants of earned income of workers with disabilities. The purpose of this study is to examine how demographic, disability, human capital, and employment characteristics affect earned income. Throughout this research, this study aims to propose implications for social work policy and practice to increase earned income. Utilizing data from ‘Panel Survey of Employment for the Disabled,’ 1,212 samples were analyzed. Results found that the type of disabilities, gender, vocational training, occupational type, and employment status influence the earned income of workers with disabilities. Based on the findings of this study, this paper argues that the capability of workers with disabilities should be improved by individualized vocational training. This paper also recommends the expansion of the employment market in the public sector and enterprise sector.
Characterization of Peripapillary Atrophy Using Spectral Domain Optical Coherence Tomography

Purpose: To characterize the features of peripapillary atrophy (PPA), as imaged by spectral-domain optical coherence tomography (SD-OCT). Methods: SD-OCT imaging of the optic disc was performed on healthy eyes, eyes suspected of having glaucoma,and eyes diagnosed with glaucoma. From the peripheral β-zone, the retinal nerve fiber layer (RNFL), the junction of the inner and outer segments (IS/OS) of the photoreceptor layer, and the Bruch’s membrane/retinal pigment epithelium complex layer (BRL) were visualized. Results: Nineteen consecutive eyes of 10 subjects were imaged. The RNFL was observed in the PPA β-zone of all eyes, and no eye showed an IS/OS complex in the β-zone. The BRL was absent in the β-zone of two eyes. The BRL was incomplete or showed posterior bowing in the β-zone of five eyes. Conclusions: The common findings in the PPA β-zone were that the RNFL was present, but the photoreceptor layer was absent. Presence of the BRL was variable in the β-zone areas.
