        외과적 보조의 급속상악확장술 : 치험 5예 REPORT OF 5 CASES

        Five adults (3 cases of bilateral posterior crossbite, 2 cases of unilateral posterior crossbite) with intermaxillary discrepancy of over 5㎜ due to maxillary transverse deficiency were treated by surgically assisted rapid maxillary expansion (SARME). Bilateral maxillary horizontal osteotomies, osteotomies of pterygomaxillary junction and anterior lateral nasal wall, ans anterior palatal osteotomy were performed in 4 cases, and unilateral osteotomies in 1case, followed by palatal expansion with tooth-borne orthopedic applicance. No significant complication was observed. The mean palatal expansion was 7.2㎜ (5.8∼10.0㎜) in the maxillary first molar region. During the mean follow-up period of 4 years (5 months∼8 years 7 months), no recurrence of crossbite was observed in all cases. The SARME seemed to be a simple, stable, and reliable procedure for achieving a permanent increase of over 5㎜ in skeletal maxillary transverse width of adults.

        투상도법을 이용한 건축디자인의 형태생성에 관한 연구 : Preston Scott Cohen과 Peter Eisenman의 작품을 중심으로

        This study proposes a digital design method using a projective perspective, one of classical architectural principles, for architectural design profession. After observing fundamental principles of architectural design in projective geometry and its usage in design process, a digital method is proposed as an application to generate and develop architectural objects. The usage case of projective perspective method in two architects' design works was analyzed. As a bridge from the fundamental principles of architecture to contemporary practice, this study aims to relate the projective perspective to new generative approaches with the help of digital technology and make a contribution to design strategies of form-making in architecture. Previous research on projective perspective method with theory and practical issue was studied to find an adequate digital form-making process. Extensions of theory and design method related to projective geometry was sought to propose design possibilities utilizing digital technology. Also, method of using digital tool is presented in order to demonstrate the findings and issues to give directions in developing an expanded digital process.

        만곡 근관에서 삼각 혹은 장방형 단면 구조의 니켈-티타늄 파일 응력 분포에 관한 3차원 유한요소 연구

        이 연구의 목적은 3차원 유한요소 모형 분석을 이용하여 삼각 대칭형과 S형태의 단면 및 변형된 사각 형태인 장방형 단면으로 분류되는 네 종류의 니켈티타늄 파일이 만곡 근관 적용 시의 응력 분포를 비교하는 것이다. 삼각 대칭형 단면 구조의 ProFile #30 / .06과 HeroShaper #30 / .06,장방형 단면구조의 Mtwo #30 / .05와 NRT #30 / .06 파일을 마이크로컴퓨터 단층촬영을 하고 reverse engineering을 통하여 파일의 구조를 얻고 삼차원 유한요소모형을 제작하였다. 모형 근관 내에서 파일이 근관장 끝까지 진입하여 회전할 때 발생하는 von Mises 응력 분포 및 파일의 제거후의 잔류 응력의 분포양상을 ABAQUS 프로그램을 이용하여 비교하였다. 근관 내 회전 시에 발생하는 응력을 관찰하였을 때 NRT 파일에서 가장 큰 응력을 나타냈으며 , 각 파일에서의 최고 응력과 최저 응력을 비교하였을 때 Mtwo파일에서 가장 큰 차이가 나타났다. 응력의 내부 분포 경향을 보았을때 장방형 구조의 단면을 가진 Mtwo 및 NRT 파일에서 불연속적인 응력의 집중 부위가 관찰되었으며 , 근관 외부로 파일을 제거하여 탄성 회복이 일어난 후의 잔류 응력도 NRT 파일에서 가장 높게 나타났다. 이상의 결과로 유추할 때, 삼각대칭 단면 구조를 가진 파일보다 S-형태의 단면과 변형된 사각 형태의 단면 등의 장방형 단면 구조의 파일이 파절의 위험성이 더 클 것으로 사료된다. The purpose of this study was to compare the stress distributions of NiTi rotary instruments bascd on their cross-sectional geometries of triangular shape-based cross-sectional design. S-shaped cross-sectional design and modified rectangular shape-based one using 3D FE models. NiTi rotary files of S-shaped and modified rectangular design of cross-section such as Mtwo or NRT showed larger stress change while file rotation during simulated shaping. The stress of files with rectangular cross-section design such as Mtwo, NRT was distributed as an intermittent pattern along the long axis of file. On the other hand, the stress of files with triangular cross-section design was distributed continuously. When the residual stresses which could increase the risk of file fatigue fracture were analyzed after their withdrawal, the NRT and Mtwo model also presented higher residual stresses. From this result, it can be inferred that S-shaped and modified rectangular shape-based files were more susceptible to file fracture than the files having triangular shape-based one.

      • 유리단 국소의치 장착 후 M.K.G.에 의한 하악운동 형태 및 속도의 변화

        The purpose of this study is to evaluate the effects of distal extension partial denture on the mandibular movement patterns and velocity. The subjects are 10 patients who have unilateral or bilateral distal extension edentulous area in maxillary and/or mandibular arches. Their mandibular movement patterns and velocity had been measured by Mandibular Kinesiograph(M.K.G.-K6 diagnostic system, Myo-tronics Inc. Seattle. WA. U.S.A.) once just before the treatment and 8 times every week since the dentures were placed. The data have been analyzed statistically by means of ANOVA and Tukey test. The results have been obtained as follows : 1. The placement of the dentures has reduced the amount of vertical and antero-posterior displacement in the habitual mandibular movement significantly(p<0.01). 2. The maximum opening and closing velocity and the mean opening and closing velocity showed no significant change with the placement of the partial dentures(p>0.01). 3. The velocity trace of maximum opening-closing movement has been more regularized and well-coordinated, and the number of clicks decreased continuously after placement of the dentures and showed stable patterns since the 5th week(p<0.01). 4. The left deviation has been decreased significantly after the placement of the dentures and showed no significant change since the 4th week. The right deviation has also been reduced, but there was shown no difference after the 1st week(p<0.01). 5. The crossover pattern has been most frequently observed in the frontal plane and the sagittal plane. It has not been changed with the denture placement and stabilized uniformly.

        항암 화학요법을 받는 유방암 환자의 삶의 질에 관한 연구

        Purpose: This study is a descriptive research study to measure the quality of life of those who suffer from breast cancer and take the chemotherapy. Method: The Subjects were 70 breast cancer patients who took the chemotherapy from September 2 to October 31, 2003. Quality of life was measured by Ferrell's measurements. Result: Quality of life indicators were spiritual domain=6.44, physical domain=5.45, social domain=4.15, and mental domain=3.95. Whole quality of life was 4.68 out of 10 points. The quality of life of those with a practicing religion was significantly higher than those without(F=3.88, P=0.026). Subjects who were less than 2 months post-operation had higher points in the physical domain than those who were more than 2 months post-operation (t=2.76, p=0.007). Subjects who had less than 4 treatments of chemotherapy had higher points in the physical domain than those who had more than 4 treatments of chemotherapy (t=2.03, p=0.046). Conclusion: The results of this study serve as a meaningful source to promote quality of life of breast cancer patients who undergo chemotherapy. The results can also be applied to the development of education programs and counseling materials for chemotherapy patients. Health care strategy can also raise the quality of life of brest cancer patients.

      • 호박(Cucurbita pepo L.) 종자의 자엽조직을 이용한 체세포배형성

        호박종자의 자엽절편체를 20μM 2,4-dichlorophenoxy acetic acid (2,4-D)가 첨가된 Murashige와 Skoog(MS) 배지에 치상한 후, 24시간 낮은 광조건(3000Lux)에서 배양하여 callus와 체세포배형성을 유도하였다. 2,4-D가 첨가된 MS배지에서 유도된 캘러스를 동일 배지에 계대배양하는 동안 치밀하게 생장하는 embryogenic callus는 계대배양 횟수의 증가와 함께 더 높은 빈도로 출현하였다. 이러한 callus로부터 somatic embryos의 생성은 3차 계대배양 후, 생장조절제가 첨가되지 않은 MS 배지에 2주 배양 후에 embryo의 형성이 관찰되었으며, 계속되는 배양에서 embryo 형성이 증가되었다. Somatic embryogensis was induced from seed cotyledons of pumpkin (Cucurbita pepo L.). Seed cotyledonary explants were cultured on MS medium supplemented with 20μM 2,4-dichlorophenoxey acetic acid(2,4-D) by incubating at 25 ± 2℃ under 24 hrs photoperiod (3000 Lux) for 4 weeks. The explants were transferred (subcultured) to same medium and to growth regulator-free MS medium for 4 weeks. Three times subculture on the same medium containing 20μM 2,4-D was required for somatic embryogenesis. Somatic embryos was only observed on growth regulator-free MS medium after 2 week cultures in the 4th subculture.

      • 간접부착술식시 치면과 레진 베이스의 간격 및 접착제에 따른 결합강도

        The purpose of this study was to evaluate the feasibility of the combined use of paste resin with sealant as an adhesive of the indirect bonding. The resin bases made of lighcured resin were bonded to the bovine incisors at a distance of 0.0, 0.2 or 0.4mm using the sealant only or the mixture of sealant and paste resin. Bond strength of each group was measured using universal testing machine. The results were as follows: 1. The bond strength was reduced in both groups as the distance between enamel surface and resin base increased. 2. The bond strength showed no statistical difference between two groups in case the distance between enamel surface and resin base was 0.0mm. 3. In case the distance between enamel surface and resin base was 0.2 or 0.4mm, the sealant with paste resin group showed significantly higher bond strength than the sealant only group. The results of the present study indicate that the use of paste resin with sealant may be desirable as an adhesive in indirect bonding, particularly in case a gap is suspected between enamel surface and resin base.

        푸드뱅크 기탁식품의 안전성 확보를 위한 HACCP 제도 적용

        The purpose of this study was to analyze the problems arising from the actual conditions of the Foodbank, and to implement the HACCP system as a solution in terms of increasing the safety of donated food within the Foodbank. In order to apply HACCP system, the entire Foodbank working process such as preparation, collection, transportation, division, and distribution was considered and analyzed to decide the application point for CCPs. Donated foods mainly consisted of processed foods, raw materials, lunch boxes, and cooked foods from mass catering establishments, which dominated over the others in terms of quantity. Cooked foods were divided into three groups based on menu-types and processing methods. Temperature, pH, and aw were measured on cooked foods, and Total Plate Count, Coliforms, E. coli, Salmonella spp., Staphylococcus aureus, Listeria monocytogenes, and E. coli O157:H7 were conducted in order to apply a HACCP plan. Form these experiments, temperature, pH, and aw of donated food were likely contributed to microbial growth. Donated foods before HACCP implementation showed high numbers in terms of total plate count and Coliforms, both well over the acceptable standard levels. By setting the CCPs on maintenance of donated food below 10℃ and using a 75 ℃ reheating method, microbiological hazard levels were able to be controlled and lowered. From these results, it is concluded that in order to guarantee food safety, foods donated to the Foodbank must not only maintain a reasonable level of initial microbiological growth, but also must be handled properly through time and temperature controls within the Foodbank system. Furthermore, in terms of implementing the HACCP plan within the Foodbank management structure, basic food safety and sanitation measures, such as reheating facilities and various cold chain systmes such as refrigerated vehicle for food transportation are importantly needed. The training and education of Foodbank personnel and management in areas such as awareness of hygiene and safe food handling and practice are also required and necessary.
