      • 이중 디스크 상승작용검사를 이용한 extended-spectrum β-lactamase 생성 Enterobacter cloacae 위균혈증 조사

        Background : Enterobacter species are frequent nosocomial pathogens and the proportion of β-lactam resistant strains are on the increase. Extended-spectrum β-lactamases (ESBL) were mainly investigated in Klebsiella pneumoniae and Escherichia coli in Korea. Recently, there occurred Escherichia cloacae stains causing pseudobacteremia which shows multidrug-resistant by contaminated boric acid sponge in neonatal nursery. It is, however, difficult to detect expression of ESBL in ordinary clinical laboratories. And in case of outbreak, there is also limitation to molecular epidemiological study that is used to search for identical origin because of so much cost and effort. The aim of this study was to detect the ESBL-producing E. cloacae through the use of double disc synergy test and to examine effectiveness of outbreake of ESBL. Methods : We detected the 14 E. cloacae strains by ESBL double disc synergy test for strains which isolated from blood specimens of a neonate and from environment. Results : All of fourteen E. cloacae strains were synergy positive. The 13 E. cloacae strains of them showed positive reaction identically to aztreonam and cefotaxime discs and negative reaction to cefpodoxim and ceftazidime discs. Conclusion : According to the result of this study, we could find that the double disc synergy test is efficient in detecting ESBL-producing stains from E. cloacae strains causing pseudobacteremia. We should be aware of ESBL producing Enterobacteriaceae and the possible institutional spreal of resistance.

      • 최근 6년간 로타바이러스 항원의 검출 양상

        Rotavirus activity in Korea has been reported beginning in October, peak in November, continuing in winter and ending in spring. But the peak month and the incidence of rotavirus seems to be changed recently. So the author investigated the trends of rotavirus activity from 1993 through 1998 at Soonchunhyang Kumi hospital. Four hundred and eighty five stool specimens from pediatric patients with acute diarrhea were tested for rotavirus by latex agglutination test (Slidex Rota-kit 2, bioMerieux Vitek. France). The results were as follows: 1. The each annual incidence of rotavirus diarrhea from 1993 to 1998 was 15.0%, 28.6%, 5.6%, 31.8%, 9.4% and 24.6%, respectively. There was no change of the annual positive rate of rotavirus(p=0.162). 2. Total average positive rate was 19.0% during 6 years. 3. Positive rate was not significantly different between the first 3 year and the last 3 year(p=0.313). But Epidemic period was from March and April during the last 2 year The each annual incidence of rotavirus diarrhea was not changed during 6 years. But the peak month of rotavirus activity was March and April during the last 2 year.

      • MAST Chemiluminescent Assay (MAST CLA) 알레르기 검사의 평가

        Background : Multiple allergosorbent test chemiluminescent assay (MAST CLA) allergy system is a simple method for measuring total and allergen-specific IgE antibodies in human serum. Total IgE, however, was much frequently high, even if no allergen-specific IgE antibodies could be detected in serum. The aim of this study was to evaluate total IgE class of the MAST CLA and the allergen frequencies among child and adult patients respectively. Methods : Three hundred and ninety four patients requested for MAST CLA from June 1998 through May 1999 at Soonchunhyang Kumi hospital were evaluated. Positive rates of specific IgE and the total serum IgE level of the MAST CLA allergy system for inhalent panel were compared by child and adult patients. Results : Two hundred and sixty six adult patients and one hundred twenty nine child patients were included in this study. The overall positive rate was 37.7%; 23.3% for child patients, 44.7% for adult patients. The average 4.3 different allergens was simultaneously detected in a single positive serum. Commonly detected allergens were Dermatophagoides farinae, Dermatophagoides pteronyssinus and house dust. Serum total IgE level of allergen-specific IgE antibody positive and adult patients was significantly higher than that of negative results and child patients(p<0.000). The number of allergen-specific IgE antibody positive results was increased in serum total IgE positive results and adult patients(p<0.000). But total IgE positive rates who had allergen specific-antibody negative patients was 89.0%; 83.8% for child patients, 92.5% for adult patients. Conclusion : These results suggest that MAST CLA allergy system is a useful screening test to detect allergen-specific IgE antibodies. But another allergen screening tests should be used for detecting allergens, when MAST CLA total IgE class is increased over class 2 with no detectable specific IgE.

      • 멸균품의 오염 상태에 관한 연구

        Background: The shelf life policies for central supply department (CSD) sterilized items and other device should be determined by the healthcare facility's infection control program. We investigated effect of the sterile integrity by the CSD sterilized packs by wrapping-materials, storage period and environment to modify and extend current shelf-life. Methods: The first phase study was from September in 2002 and the second phase study was from June in 2003. Eight hundred and fifty four packs containing small guaze with five wrapping materials were stored on the shelves or closed cabinets and storage durations from 1 to 12 weeks. The test packs were collected weekly and cultured in the laboratory. The temperature and relative humidity was monitored whenever the pack was collected. Results: The guaze in the test packs were not contaminated until four weeks in the first study phase, and until three weeks in the second phase study, respectively. The temperature and relative humidity of storage locations were 21.6℃ and 40.3% int the first study and 27.9℃ and 53.3% in the second phase study, respectively. Conclusion: There was contamination in the sterility integrity of the test packs with different wrapping materials storage locations, environments and durations. It was carefully considered to extecd shelf-life from two weeks to four weeks in the study hospital.

      • 최근의 성분수혈 현황과 혈액반환 분석

        In recent transfusion practice, there has been great changes combined by various aspects. Blood utilization review has been used to minimize inappropriate transfusion and improve the quality of transfusion practice. The author observed the changes in component transfusion from 1990 through 1998 at Soonchunhyang Kumi hospital. The effectiveness of the transfusions were evaluated by the cause, the time of blood returned to the blood bank, and the blood discarded by clinical departments for the periode of past four years. The results were as follows : 1. The annual amount of blood transfusion for 9 years was 6,067units and the annual average rate of blood component transfusion was 98.4%. 2. The annual average rate of blood returning to the blood bank was 2.7%, and the highest rate were observed in department of general surgery in 1995 and department of internal medicine in 1998. 3. The most common cause of blood returning to the blood bank was improvement of the patients condition. 4. The average time of blood returning to the blood bank was seven hours and twelve minutes. In conclusion, a dialogue and studies between the transfusing physician and the transfusion medicines pecialist were desirable for more effective and appropriate utilization of blood components and improvement of transfusion practice.

      • 자동화학검사용 신양시약의 평가

        Background : Routine chemical tests are one of the most fundamental and informative laboratory tests in the clinical management. It is considered essential to choose and evaluate optimal reagents in order to get reliable results. Most of laboratories in Korea uses imported foreign chemistry autoanalyzers and reagents. The purpose of this study is to evaluate quality of domestic reagents in comparison with foreign imported reagents. Method: Evaluation of Shin-yang reagents were performed through precision and comparison study, the performance results were compared with those of existing reagents. Sixteen kinds of reagents were evaluated for AST, ALT, ALP, LDH, GGT, total protein, albumin, BUN, creatinine, glucose, total cholesterol, triglyceride, uric acid, Iron, UIBC and amylase. Patient sera and commercial quality control materials were used as specimen. Results: Coefficients of variance(CV) of within-run precision showed less than 5% in all items, comparable to the currently used reagents. Correlation analysis revealed good results in most items, except total protein and UIBC. Conclusions: Reagents manufactured by Shin-yang for chemistry autoanalyzer showed acceptable precision and correlation with imported reagents currently being used. Thus it may be acceptable for an alternative reagents for chemistry autoanalyzer.

      • 사람-사람 하이브리도마의 생산을 위한 fusion partner cell의 개발

        This study was performed to establish human hybridmyeloma cell lines as fusion partner cells for producing human-human hybridoma. Human myeloma, SKO-007(J3), and human cervix epitheloid carcinoma cell line, HeLa MR, were fused by using with 50% PEG, and cultured in RPMI 1640 medium containing 15% FBS. Hybrids of SKO-007(J3) and HeLa MR were selected in RPMI 1640 media containing 6-thioguanine(6-TG ; gradually increasing concentration from 10 ㎍/ml to 20 ㎍/ml, and then gradually decreasing to 10 ㎍/ml) for 6 weeks. 6-TG resistant cells were collected and cloned by limiting dilution method. The fused myelomas with HeLa MR grew more rapidly than the fused cells with a small number of HeLa MR. Their doubling time was 72 hours.

      • 이공산(異功散)의 in vitro에서 Cisplatin 유도 신장독성의 감소 효과

        In vitro model systems are increasingly used to investigate the mechanism of chemical-induced toxicity and for toxicological screening of new drug families. The effect of Ekongsan on the cytotoxicity induced by cisplatin on human and rabbit kidney cells was evaluated by tetrazolium-dye(MTT) assay. On the basis of Cl_50 value, cisplatin-induced cytotoxicity on human renal cortical cells was increased depend on concentration, whereas Ekongsan alone did not influence Simultaneous administration of cisplatin and Ekongsan, cytotoxicity of cisplain was reduced depend on the concentration of Ekongsan. With primary cultured proximal tubule cells results to reduce the cisplatin induced-cytotoxicity by Ekongsan was comparable to those with renal cortical cells. Based on these results, Ekongsan may has the potency to protect the nephrotoxicity induced by cisplatin.
