Benzalkonium chloride is one of the most commonly used preservatives in eyedrops, artificial tears, and solution for contact lenses, which has occasionally been reported as a causative agent producing allergic contact dermatitis. We have experienced a case of benzalkonium chloride induced allergic contact dermatitis in a 66-year-old woman presented with edema and erythematous plaque of the left eyelid and itching. She had been using two kinds of eyedrops for uveitis and cataract of her left eye for seven years, when dermatitis developed one month ago. Patch test with the European standard series, her own eyedrops, and their individual components gave positive reactions to neomycin sulfate, nickel sulfate, potassium dichromate, benzalkonium chloride. All of her eyedrops were prohibited and after being treated with topical prednisolone, her eruption disappeared completely.
