        혈액 투석중인 만성 신부전증 환자에서 혈청 Apolipoprotein A - I 과 B 의 변화

        N/A Cardiovascular complications are frequently observed in patients with chronic renal failure on hemodialysis. The role of the disorder of apolipoprotein composition has been emphasized. Serum concentrations of apolipoprotein A-I and apolipoprotein B were determined by radioimmunodiffusion in 44 patients with chronic renal failure on hemodialysis. The results were compared with those obtained in asymptomatic normolipidemic subjects and patients with chronic nephritis. Chronic renal failure patients on hemodialysis had reduced concentration of apolipoprotein A-I (107.1±25.54㎎/dl) compared with chronic nephritis (116.9±20.51㎎/dl) and the normal controls (137±25.54㎎/dl) as well as increased concentration of apolipoprotein B (123.5±20.54㎎/dl) compared with chronic nephritis (102.4±26.04㎎/dl) and the normal control (98.0±20.12㎎/dl), respectively (p<0.01). The ratio of apolipoprotein A-I to apolipoprotein B (0.87±0.514) was decreased compared with chronic nephritis (1.13±0.601) and the normal controls (1.39±0.513), respectively (p<0.01). These findings suggest that cardiovascular complications observed in chronic renal failure patients receiving hemodialysis may be related with the abnormalities in serum apolipoprotein A-I and apolipoprotein B level.

      • 1989년 가을 전남지역에서 발생한 쭈쭈가무시병 25예의 임상적 고찰

        We observed 25 cases of tsutsugamushi disease occuring in the Chulanam-do area from October to November 1989, which were diagnosed clinically and serologically by indirect immunofluorescent antibody test. In korea, there has been an increasing concern as a possible cause of unknown febrile disease, since tsutsugmushi disease were first reported in 1986. The clinical illness was characterized by fever, skin rash, eschar and lymphadenopathy. On laboratory findings, antibody titer to three strains of R. tsutsugamushi ranged negative to 1 : 2560. All of the patients showed milder clinical courses than those reported till now in Korea, and showed good response to antibiotic therapy with doxycycline or chloramphenicol.

        백서 폐 Submitochondrial Particles의 Superoxide 생성에 대한 Beomycin 효과

        Bleomycin is used clinically against various tumors, but diffuse interstitial pulmonary fibrosis has been recognized as a severe complication. Ferrous iron and bleomycin in the presence of reducing agents stimulate DNA degradation. This process causes the reduction of oxygen and the production of toxic oxygen metabolites, which may play a role in the ability of this drug to break down DNA. We studied the effect of bleomycin on superoxide production, as well as enzyme activities related to toxic oxygen metabolites in lung submitochondrial particles and the mitochondria of rats, respectively. The results were as follows; 1) In the saline-pretreated group, the amount of superoxide produced and the ratio of superoxide production to NADH consumption increased by 12.5% and 76.6% compared with basal levels at 2㎍/㎖ and 5㎍/㎖ of the added bleomycin concentration, respectively. 2) In the bleomycin-pretreated group, the amount of superoxide produced and the ratio of superoxide produaction to NAHD consumption increased by 18.2% and 238.7% compared with basal levels at 2㎍/㎖ and 10㎍/㎖ of the added bleomycin concentration, respectively. 3) Superoxide production in the bleomycin-pretreated group was elevated by 37.5% at basal level, and by 44. 4% at 2㎍/㎖ of the added bleomycin concentrations compared with the saline-pretreated group. 4) The cytosolic enzyme activities of superoxide dismutase, catalase, and peroxidase in the lung of bleomycin-pretreated rats increased by 125%(p<0.001), 54%(p<0.01) and 78%(p<0.01) compared with the saline-pretreated groups, respectively. Of the mitochondrial enzyme activities, superoxide dismutase increased by 203%(p<0.001), catalase decreased by 17%(p<0.01) and peroxidase increased by 7% compared with the saline-pretreated groups, respectively. The above findings suggest that superoxide production is stimulated by bleomycin administration in the mitochondria of rat lungs.
