        빠른 종양 성장을 보인 청신경초종 환자: 증례보고

        Vestibular schwannoma (VS) is a benign tumor which tends to grow slowly. In general, the growth rate of VS is about 1mm/year. We treated an unusual case of rapid growing VS associated with multiple cystic changes. Also, we review the natural history and factors influencing tumor growth of VS. A 75-year old female patient presented with gait disturbance and hearing loss. Brain magnetic resonance imaging (MRI) showed a mass of 4.0×4.0×3.5cm size with multiple cystic changes and heterogeneous enhancement in right cerebellopontine angle (CPA). Twelve six months ago, the patient had a brain MRI which revealed CPA mass less than 1.0cm in diameter. Small sized VSs should be followed up with MRI at least every two years and the VSs consisted with cystic component need to be more closed observation than the solid type.

        In the present paper, a new concept called conditional are is added to tranditional GSPN(Generalized Stochastic Petri net) of which effectiveness is proved through an application to an instruction pipeline of general-purpose CPU. The problem in applying Petri nets including GSPN to performance evaluation of computer systems is that the number of nodes of reachability graph increases exponentially with the number of places in the model. Conditional ares proposed in this paper are shown to be a good tool not only for reducing the number of places in the model, but making the task of model construction easy.

        Acetylcholinesterase activity in the brain of wild birds in Korea—2014 to 2016

        Acetylcholinesterase (AChE) activity level can be used as a diagnostic marker for anticholinesterase pesticide poisoning. In this study, we aimed to establish a baseline level of normal brain AChE activity in wild birds. AChE activity was measured in the brains of 87dead wild birds (26 species). The level of AChE activity ranged from 6.40 to 15.9 µmol/min/g of brain tissue in normal wild birds. However, the brain tissue AChE activity level in wild birds exposed to organophosphate (OP) pesticide was 48.0%–96.3% of that in the normal birds. These results may serve as reference values to facilitate routine diagnosis and monitoring of OP-poisoned wild birds.

        Arthroscopic Assessment of Intra-Articular Lesion after Surgery for Rotational Ankle Fracture

        Background: The purpose of this study was to report findings of exploratory arthroscopic assessment performed in conjunction with removal of internal fixation device placed in the initial surgery for rotational ankle fracture. Methods: A total of 53 patients (33 male, 20 female) who underwent surgery for rotational ankle fracture between November 2002 and February 2008 were retrospectively reviewed. All patients gave consent to the exploratory arthroscopic surgery for the removal of internal fixation devices placed in the initial surgery. Lauge-Hansen classification system of ankle fractures was assessed for all patients. Intra-articular lesions (osteochondral lesion, loose body, and fibrosis) were evaluated via ankle arthroscopy. Comparative analysis was then performed between radiological classification of ankle fracture/patient’s symptoms and arthroscopic findings. Results: Lauge-Hansen classification system of ankle fractures included supination-external rotation type (n = 35), pronationexternal rotation type (n = 9), and pronation-abduction type (n = 9). A total of 33 patients exhibited symptoms of pain or discomfort while walking whereas 20 exhibited no symptoms. Arthroscopic findings included abnormal findings around the syndesmosis area (n = 35), intra-articular fibrosis (n = 51), osteochondral lesions of the talus (n = 33), loose bodies (n = 6), synovitis (n = 13), and anterior bony impingement syndrome (n = 3). Intra-articular fibrosis was seen in 31 of symptomatic patients (93.9%). Pain or discomfort with activity caused by soft tissue impingement with meniscus-like intra-articular fibrosis were found in 19 patients. There was statistical significance (p = 0.02) between symptoms (pain and discomfort) and the findings of meniscus-like fibrosis compared to the group without any symptom. Conclusions: Arthroscopic examination combined with treatment of intra-articular fibrosis arising from ankle fracture surgery may help improve surgical outcomes.
