        Effects of Parity and Breastfeeding Duration on Bone Density in Postmenopausal Women

        Purpose: This study aimed to identify the effect of parity and breastfeeding duration and the occurrence of lumbar vertebral and femoral neck osteoporosis in Korean postmenopausal women. Methods: This study analyzed the data of 1,770 women based on the 2010-2011 results of the Korea National Health and Nutrition Examination Survey. Extracted data concerning bone density included variables known to be associated with osteoporosis. Complex sample multivariate logistic regression analysis was conducted to determine whether parity and breastfeeding duration were associated with osteoporosis in postmenopausal women. Results: Parity was not associated with postmenopausal osteoporosis in the femoral neck or lumbar vertebrae; however, the risk of femoral neck osteopenia was significantly higher in women with a history of 12-24 months of breastfeeding than in women who breastfed for less than 12 months (odds ratio = 2.12, 95% confidence interval = 1.07-4.21). In women who breastfed for 24 months or longer, the risk of lumbar vertebral osteoporosis was significantly higher than in those who breastfed for less than 12 months (odds ratio = 2.73, 95% confidence interval = 1.18-6.32). Conclusion: Breastfeeding duration may affect the occurrence of lumbar vertebral or femoral neck osteopenia or osteoporosis. Therefore, women who breastfeed for one year or more require education on the risk of bone loss and the need for preventive measures such as adequate calcium intake and physical exercise.

        능력증강전략을 활용한 자조관리과정이 류마티스관절염 환자의 능력증강과 신체적, 심리적 기능에 미치는 효과

        Purpose : The purpose of this study was to examine the effects of a six week self-help empowerment strategy program on the empowerment status, pain intensity, physical function, joint flexibility and depression status in rheumatoid arthritis patients. Method : The subjects of the study consisted of a convenience sample thirteen rheumatoid arthritis patients. Data was collected from July to November 2001. All patient received a self-help empowerment strategy program, once a week for six weeks. Result : After receiving the self-help empowerment strategy program, the empowerment status increased significantly, the pain intensity and the depression score decreased significantly and the level of physical function significantly improved but the flexibilities of shoulders, knees and ankles were not significantly change. Conclusion : Further studies need to be done to determine the feasibility of implementing empowerment strategy and its possibility as an effective intervention program for decreasing pain intensity and depression score and improving the physical function.

        응급실 간호사의 업무분석을 통한 경력등급별 실무교육안 개발

        Purpose: This study was done to provide basic data for developing an in-service education program to improve nurses' quality. First, frequency of nursing activities and competency levels of emergency nurses according to their career ladders were compared through job analysis and then practical education programs were presented on based of the results. Method: Data were collected from 335 nurses working in emergency rooms in 31 tertiary hospitals. Data collection was done from September to November 2005 using the job analysis questionnaire. Results: There were 41 nursing activities that showed differences in frequency and 78 activities that showed differences in perceived competency level. Irrespective of emergency nurses' careers, activities that show high frequency but low competency were sputum liquefying therapy, assessment of cranial nerve function, identification of diagnostic radiology, and communication with various departments. In-service education content according to nurse's career ladders was presented by adding high frequency nursing activities and activities with low competency level even though having high frequency. Conclusion: There is a need to develop and provide in-service education programs, which consider nurses' difference in frequency and competency level for their career ladders.

        Purpose: The purpose of this study is to investigate the effect of eye-shields and sleep-inducing music application on sleep quality and incidence of delirium for intensive care unit (ICU) patients. Methods: This study was a untreated control group design with pretest and posttest. The data was collected in the ICU of D University Hospital in Busan, from June to September 2011. Thirteen subjects were assigned to eye-shields and sleep-inducing music intervention and 15 subjects were in the control group. Results: Eye-shields and sleep-inducing music application was effective for improving the quality of sleep in ICU patients. There was no statistically significant difference in the incidence of delirium between the two groups. Conclusion: The sleep improvement intervention with eye-shields and sleep-inducing music would be a meaningful part of nursing intervention programs to enhance the quality of sleep in ICU patients.

        건강신념 및 효능기대증진 프로그램이 류마티스 관절염환자의 골다공증 예방행위에 미치는 영향

        Osteoporosis has been known as a common complication of rheumatoid arthritis and a major preventable health problem. Lots of studies have demonstrated that changes in life style can help delay or prevent osteoporosis. Therefore nursing intervention related osteoporosis prevention have consisted of education programs aimed at changing dietary and exercise habit. However knowledge gained from education haven't always leaded to behavior change. Therefore it is important to consider other psychological variables in effecting behavior change. Numerous research have found self efficacy and health belief to be an important factor in individual decision making behavior. The purpose of the study was to develop health belief and efficacy expectation promoting program based on Health Belief Model & Self Efficacy Model and to investigate its effects in women with rheumatoid arthritis. For this purpose, one group pretest-post design was used. The subject of the study were 16 women with rheumatoid arthritis in Pusan city and data collection was carried out from April, 1997 to May, 1998. The intervention program was consisted of educating on osteoporosis and enhancing and reinforcing self efficacy by verbal persuasion during the period of 4 weeks. The instruments were used to collect data in this study were Osteoporosis Health Belief Scale, Osteoporosis Self Efficacy Scale, and Osteoporosis Preventive Behavior Scale. Data was analyzed by Wilcoxon signed rank test using SPSS PC+ program. The results are as follows : 1) The behavior should be increased after intervention was supported(Z=-3.5162, p=.0004, diet : Z=-3.2942, p=.0010, exercise) 2) The sub-hypothesis that perceived sensitivity should be increased after intervention was supported(Z=-2.3854, p=.0171). 3) The sub-hypothesis that perceived severity should be increased after intervention was rejected(Z=-1.4327, p=.1520). 4) The sub-hypothesis that perceived benefit should be increased after intervention was supported(Z=-2.6410, p=.0083). 5) The sub-hypothesis that perceived barrier should be decreased after intervention was supported(Z=-2.4138, p=.0158). 6) The sub-hypothesis that efficacy should be increased after intervention was supported(Z=-3.5162, p=.0004). As a conclusion, it was found that health belief and self efficacy promoting program was an effective nursing intervention for preventing osteoporosis of rheumatoid arthritis.

Method: The Q-methodology which provides a method of analyzing the subjectivity of each item was used. Twenty-nine nurses classified the 50 selected Q-statements into a normal distribution using a 9 point scale. The collected data was analyzed using the Quanl PC program. Result: Four types of attitudes toward transsexuals were identified. The first type (humanitarian acceptance) showed an attitude of respecting transsexuals as human beings and understanding and accepting their desires and difficulties. The second type (superficial understanding) understood the psychological conflicts and suffering of transsexuals but could not accept them as members of families or society. The third type (insufficient understanding) did not feel a sense of rejection toward transsexuals but showed a lack of understanding of their desires and difficulties. The fourth type (rejection) failed to understand the desires and difficulties of transsexuals and showed a sense of rejection toward them, in addition to regarding them as sexually immoral people. Conclusion: The results of the study indicate that different approaches of educational programs based on the four types of attitudes toward transsexuals are recommended.

        8주간의 타이치 운동 프로그램이 골관절염 노인환자의 낙상 위험요인에 미치는 효과

        Purpose: The purpose of this study was to determine the effects of 8-week Tai Chi exercise program on improving the risk factors for falls in the older adults with chronic arthritis pain. Method: Fifteen subjects were assigned for experimental group and thirteen were assigned for control group. The subjects for the experimental group participated in a 8-week program of Tai Chi exercise. In order to evaluate the effects of Tai Chi program pain intensity postural stability balance gait and flexibility of knees and ankles were measured before the first and after the 8th session. Results: After a 8-week Tai Chi program there were significant improvement in single-leg stance time with eyes open(left: p=000, right: p=003) total scores of balance(p=004) total scores of gait(p=005) and the angle of ankle dorsiflexion (p=008) in the experimental group when compared to the control group. However there was no significant difference in pain intensity between Tai Chi group and control group(p=054) Conclusion: These results suggest that a 8-week Tai Chi exercise program can be utilized as a safe and effective nursing program to improve on improving the risk factors for falls in the older adults with osteoarthritis.<br/> <br/>

        류마티스 관절염 환자의 대체요법에 대한 이용율, 이용형태 및 지각된 효과에 대한 연구

        Alternative therapy for rheumatoid arthritis has become worldwide Phenomenon this decade without scientific validation. This study was undertaken to explore the Prevalence, patterns of use, and Perceived efficacy of alternative therapies by patients attending a rheumatology clinic. We conducted a crossectional survey of 142 RA patients with structured questionnaire at outpatient rheumatic clinic from July to August in 2001. Results of this study were summarized as follows : 1. 85.9% of subjects were woman. Mean age was 49.97years, duration of rheumatoid arthritis was 5.6years Mean Pain score was 5.29cm. 2. 74.6% of subjects perceived rheumatoid arthritis as a controllable disease. 31% of patients combined medical treatments with alternative therapies. 58.5% of patients perceived that combining medical treatments with alternative therapies is the most effective strategy in controlling disease. 3. 77.5% had used more than one alternative therapy since the diagnosis was made. The major reason of discontinuation of use was no effect for the treatment of their disease. 4. 34.5% were currently using an alternative therapies and 50.9% were going to use alternative therapies in future. 5. 42.7% reported that alternative therapies was effective and only 36% reported side effects. 6.The Perceived effect score was 3.14cm in general, expecially the physiological score 2.91cm, psychological score 4.16cm. In conclusion, our results demonstrate a high use of alternative therapies by rheumatoid arthritis patients. And the perceived effect score of physiological aspect was not higher than one of psychological aspect. Therefore it should be considered in education program for the use of alternative therapies.

        류마티스 관절염 환자의 재활동기에 대한 개념분석

        Purpose: This study was to identify the attributes, antecedents, their consequences, and empirical indicators of rehabilitation motivation in rheumatoid arthritis patients. Methods: Walker and Avant's method was used to analyze the concept. Articles published after 1990 were searched in Medline, CINAHL, NSDL, and RISS databases using “rehabilitation”, “motivation” and their combination as keywords. Results: The attributes of rehabilitation motivation are: 1) certitude and trust toward rehabilitation treatment; 2) confidence in the rehabilitation process; 3) efforts and commitments to achieve health goals; 4) psychological needs to act toward health recovery. Its antecedents include: 1) rights of selfdetermination; 2) goal setting and goal-oriented attitude; 3) personal needs; 4) getting rewards; 5) social and family support; 6) professional behavior of healthcare providers; and 7) least risks or costs for actions taken. Conclusion: The study results could be used as a conceptual framework for developing tools to measure the motivation of rheumatoid arthritis patients.
