급속한 노령화추세는 핵가족화로 인한 가족구조와 기능 및 의식의 변화와 함께 노인수발을 가중시키고 있다. 또한 최근 치매노인인구의 급증은 수발부담이라고 하는 새로운 가족문제 및 사회문제로 부각되고 있다. 우리나라 노인수발형태는 전통적 가부장적 이데올로기와 효 사상에 근거하여 사회부양보다는 가족부양에 의존하고 있는 실정으로, 노인수발의 주책임자는 며느리나 부인 등 여성이 대부분을 이루고 있다. 그러나 최근 여성의 고학력화, 여성지위의 향상 등으로 여성의 취업이나 사회참여가 증가하고 있는 상황에서 장기적으로 보호를 요하는 의존적 노인의 간호와 수발에 있어서 많은 문제를 야기하고 있다. 특히, 사회생활을 하고 있는 기혼여성의 경우, 노인수발의 주수발자로서의 역할과 함께 또 다른 가족원으로서의 역할과 같은 다중적인 역할요구는 증가할 것으로 보여진다.

One of the most important steps of the design, capacity and operation analysis stapes in the two-lane two way highways is the effect of heavy vehicle to traffic flow quality. This heavy vehicle's effect on traffic flow can be represented as PCE, which is the number of passenger cars that are displaced by a single heavy vehicle of a particular type under prevailing roadway, traffic, and control conditions. In this paper, we focus on the heavy vehicles effect on volume, speed, delay, and the maneuver of freedom which are major MOE's in traffic operation analysis and PCE criterion which should be measurable, determinable and able to reflect the traffic flow characteristics. Therefore, the objective of the paper is to determine the PCE criterion and to develop a new PCE determination method. In this study, delay is adopted as PCE criterion and, for calculation of delay, the highway is divided into the passing zone and the no-passing zone. PCE is determined by comparing the delay due to total traffic flow interaction with the delay due to a single heavy vehicle, Also, this paper proposes a new method to determine the average PCE on the highway that has the passing zones and no-passing zones.
수장 잠김 금속판을 사용하여 치료한 AO C형 원위 요골 골절의 단기 추시 결과

Purpose: To evaluate clinical and radiological results of AO type C distal radius fractures treated with a volar locking plating system. Materials and Methods: We retrospectively reviewed 31 patients with AO type C distal radius fracture treated with a volar locking plating system. We evaluated the clinical results according to the Mayo wrist performance scoring system and disabilities of the arm, shoulder and hand (DASH) score and evaluated the radiographic results. Results: All cases had bony union. The mean Mayo wrist performance score was 81.9. The mean DASH score was 11.2. Between preoperative and immediate postoperative radiographic measurements, the mean radial length was improved from 8.8 mm to 13mm, radial inclination from 14.7° to 22.5°, volar tilt from -11.3° to 9.4° and intra-articular step-off from 2.2 mm to 0.3 mm (p<0.05). Between immediate postoperative and latest follow-up radiographic measurements,the mean loss of radial length measured under 1 mm, radial inclination under 1°, volar tilt under 1° and intra-articular step-off under 1 mm (p>0.05). Internal fixation using volar locking compression plate could allow early postoperative exercise and could result in low incidence of postoperative complication. All cases showed bone union showing no evidence of malunion, nounion, nor metal failure. Conclusion: Treatment of AO type C fractures using a volar locking plate showed satisfactory outcomes. It is a good option to obtain stable fixation without significant loss of reduction. 목적: AO C형 원위 요골 골절에 대한 전방 도달법을 이용한 수장 잠김 금속판 고정술의 임상적 및 방사선학적 결과를 분석하였다. 대상 및 방법: 술 후 6개월 이상의 추시 관찰이 가능하였던 31예를 대상으로 하였으며, 치료 결과의 판정을 위해 Mayo wrist scoring system과 disabilities of the arm, shoulder and hand (DASH) 점수를 이용한 임상적 평가 및 방사선학적 평가를 시행하였다. 결과: 전예에서 골유합을 얻었으며, 임상적 결과는 평균 81.9점으로 우수 13예, 양호11예, 보통 7예였고 DASH 점수는 평균 11.2이었다. 방사선학적으로 요골 길이는 수술 전 평균 8.8 mm에서 수술 후 평균 13 mm로, 요측 경사는 평균 14.7˚에서 평균22.5˚로, 수장측 경사는 평균 -11.3˚에서 평균 9.4˚로, 관절면의 층형성은 평균 2.2mm에서 평균 0.3 mm로 향상되었다(p<0.05). 수술 직후와 최종 추시 방사선 사진을 계측 비교하였을 때 평균 요골 길이, 관절면의 층형성은 1 mm 이하, 요골 경사 및수장측 경사 역시 1˚이하로 감소하여 현실적으로 차이를 관찰할 수 없었고 통계학적유의성도 없었다(p>0.05). 조기에 관절 운동을 가능하게 해주고 낮은 합병증의 빈도를 보였으며 술 후 불유합, 부정유합 등은 관찰되지 않았다. 결론: AO 분류 C3형 원위 요골 골절의 치료에 있어서 수장 잠김 금속판 고정술은 방사선학적, 임상적으로 만족할만한 치료 결과를 보였다.
Navigation-Assisted Total Knee Arthroplasty for Patients with Extra-Articular Deformity

Purpose: Since the existence of an extra-articular deformity seriously alters the normal geometry and kinetics around the knee joint, difficulties are often encountered in total knee arthroplasty (TKA) using a standard surgical technique. The purpose of this study was to evaluate the usefulness of surgical navigation system as a treatment option for osteoarthritic knees with extra-articular deformity.Materials and Methods: The authors retrospectively reviewed medical records of the patients who underwent primary TKA between 2007 and 2012. Knees with preoperative radiography showing an angular deformity within the region from the middle third of the femur to the middle third of the tibia in the ipsilateral limb of the arthritic knees were considered as cases having extra-articular deformity. Thirteen knees of the 13 patients were found to have undergone TKA using a navigation system for osteoarthritis with ipsilateral extra-articular deformity. The hip-knee-ankle angle, Knee Society score (KSS), and range of motion were measured before and after the operation to evaluate the improvement. Results: The mean hip-knee-ankle angle in the coronal plane was improved to 0.2o±4.5o in valgus alignment postoperatively. The KSS was improved to 89.6±4.6 points postoperatively at the last follow-up, with over 90% of good and excellent results. The range of motion was improved to 118.5o±10.5o postoperatively.Conclusions: Navigation-assisted TKA is a good treatment option of osteoarthritic knees with extra-articular deformity.

This paper analyzes the content of the generational argument written by Oe Kenzaburo in Warera no Jjidai(われらの時代). Oe, As a "Shokokumin少国民 generation" who did not experience the war, tries to make up for his lack of war experience through metaphysical abstraction. Methodologically introduced for this purpose is sexuality. Sexuality employed in this novel is heterosexuality and homosexuality. Heterosexuality in this novel represents misogyny. And even anal sex, which is supposed to cause disturbance while displacing heterosexual norms, is not given enough queerness. This is because homosexuality in this novel is still encompassed by heterosexual norms. The subject of the generational argument offered by Warera no Jidai is only men themselves, to the exclusion of women. 本稿は、大江健三郎『われらの時代』(1959)において展開されている世代論的な主張の内実を分析したものである。戦争体験を有しない「少国民世代」であるがゆえに、大江は自らの体験の欠如を「形而上学」的な抽象化によって充填しようとする。そのため、方法論的に導入されたのが、「性的なもの」である。本小説に採用された「性的なもの」は異性愛と同性愛の二つによっている。男性的な世界を目指す異性愛はミソジニーに支配されている。異性愛の規範を転位させつつ撹乱を巻き起こすはずの同性愛的なアナルセックスには、そのクィア性が十分付与されていない。それは、同性愛すら異性愛の規範に包摂されたままであるからだ。『われらの時代』が差し出す世代論的な主張の主語は、男性のみであり、そこに女性は排除されている。

For decades, many traffic flow studies on the analysis and determination of level of service (LOS) for the weaving sections have been made to Provide several regression equations. Weaving and non-weaving speeds were dependent variables for the equations, with independent variables being weaving length, number of lanes, and weaving ratios. One of the difficulties in developing the equations was that the weaving areas were rare in Korea, so the statistical analyses for calibrating the equation parameter could not be performed in a desirable manner. In this regard, a new and stochastic methodology for predicting the weaving and non-weaving speeds within the weaving sections was required. In this study the following design variables were developed; influence area of the weaving section. headway distribution within the weaving section, maximum weaving volume of the weaving section, length of the ideal weaving section, and speed estimations for the weaving and non-weaving flows. The evaluation of the new model was made comparing the delay in the weaving section with the one in the freeway basic section.

purpose: The purpose of this study is to compare and analyze the precision of optical and electromagnetic navigation systems in total knee arthroplasty (TKA). materials and methods: We retrospectively reviewed 60 patients who underwent TKA using an optical navigation system and 60 patients who underwent TKA using an electromagnetic navigation system from June 2010 to March 2012. The mechanical axis that was measured on preoperative radiographs and by the intraoperative navigation systems were compared between the groups. The postoperative positions of the femoral and tibial components in the sagittal and coronal plane were assessed. results: The difference of the mechanical axis measured on the preoperative radiograph and by the intraoperative navigation systems was 0.6 degrees more varus in the electromagnetic navigation system group than in the optical navigation system group, but showed no statistically significant difference between the two groups (p>0.05). The positions of the femoral and tibial components in the sagittal and coronal planes on the postoperative radiographs also showed no statistically significant difference between the two groups (p>0.05).conclusions: In TKA, both optical and electromagnetic navigation systems showed high accuracy and reproducibility, and the measurements from the postoperative radiographs showed no significant difference between the two groups.
고속도로 최대통과교통량 산정 및 서비스수준 평가 기법 개발

본 연구의 목적은 고속도로 교통운영상태 분석과 이로부터 도출된 하나의 평가척도(평균통행속도)에 의해 서비스수준 평가 방법론을 구축하는 것이다. 본 연구는 고속도로를 분석하기 위해서는 고속도로의 어느 구간에서 정체가 발생될 것이며, 발생된 정체의 영향은 어느 정도가 될 것인가를 파악하는 것이 우선되는 과제라는 관점에서 출발하였다. 따라서 고속도로 상에서 발생되는 정체를 유형화하였고(이는 곧 잠재적 병목구간 및 병목구간에서 발생되는 정체메커니즘의 유형화를 의미한다), 이를 통해 병목구간의 수요교통량에 따른 최대통과가능교통량 및 최대통과교통량의 산출을 통해 정체류 분석을 수행하였다. 본 연구가 기존의 연구들과 구별되는 가장 큰 특징들 중의 하나는 병목구간의 용량으로 볼 수 있는 최대통과가능교통량(정상류에서의 용량) 및 최대통과교통량(정체류에서의 용량)이 기존의 용량 개념과 같이 불변의 값이 아니고 수요교통량에 따라 변하는 가변적 용량이라는 점이다. 따라서 정체의 유형화와 가변적 용량 개념의 도입을 통해서 본 연구를 통해 구축된 고속도로 운영상태 분석 방법론은 고속도로 기하구조 조건에 의해 결정되는 물리적 병목구간에만 국한하지 않고 잠재적 병목구간(사고 및 공사 등의 돌발상황 발생구간 뿐만 아니라, 진출 연결로의 대기행렬이 본선으로 넘치는 분류부 등)에 대해서도 수요교통량에 따른 정체류 분석이 가능하였다. The objective of this thesis is to develop a capacity analysis and to develop a methodology to evaluate Level of Service over the entire freeway sections by single MOE(Measure of Effectiveness). This study set forth from a following viewpoint; to analyze entire freeway sections as freeway facility system, it is important to identify the exact point where congestion would occur and the extent of the congestion. Therefore, in this thesis, congestion mechanism on freeways was figured out and congestion analysis methodology was developed. Thereby maximum possible throughput rate and maximum throughput rate in bottleneck sections were calculated and a congestion analysis was carried out. The difference between the new method and existing procedures is that maximum possible throughput rate and maximum throughput rate, that can be considered as capacities of un-congested and congested flow in the bottleneck section, are variable capacities dependent on demand flow.

Hospital mode of death in therapeutic hypothermia-treated out-of-hospital cardiac arrest patients

Introduction: To determine the hospital mode of death in patients with therapeutic hypothermia (TH) after out-of-hospital cardiac arrest (OHCA) in South Korea where the withdrawal of life-sustaining treatment (WLST) is not applied to the patients due to cultural factors. Material & Methods: We performed a retrospective analysis of a multicenter registry including adult OHCA patients treated with TH in 24 hospitals from 2007 to 2012. The registry data and electronic medical records of the individual patients were carefully reviewed and categorized as brain injury, cardiovascular disorder or others.Descriptive analysis and comparison of time to death between the 3 categories were performed. Results: Among 930 patients included in the registry, 680 (73%) exhibited poor neurological outcome [CPC 3=55 (5.9%), CPC 4=251 (27.0%), and CPC 5=374 (40.2%)]. Among 373 patients with identified cause of death, 73 (19.6%) died due to cardiovascular disorder; 98 (26.3%) due to brain injury; and 202 (54.2%) due to other causes. Death due to cardiovascular disorder occurred significantly earlier (2 [1-2] days) than brain injury (5.5 [3-12] days, p<.001) and other causes (5.5 [3-14] days, p<.001). Conclusion: In our study including adult TH-treated OHCA patients who WLST were not applied, death due to brain injury was relatively low, whereas other causes, mainly multiple organ failure, were main cause of death.
