        면역글로불린 G4 연관 자가 면역 질환의 폐 침범 2예

        Immunoglobulin G4 (IgG4) related autoimmune diseases are characterized by high serum IgG4 concentrations, sclerosing inflammation of numerous IgG4-positive lymphoplasma cells of varying origin, and a positive response to steroid treatment. Autoimmune pancreatitis, sclerosing cholangitis, and retroperitoneal fibrosis are representative presentations of IgG4 related autoimmune disease. Herein, we describe 2 patients (40-years-old woman and 47-years-old man) diagnosed with pulmonary involvement of IgG4-related autoimmune disease. The patients were admitted for an evaluation of the lung mass or multiple lung nodules found on chest radiography. Surgical lung biopsies were performed and pathologic finding revealed lymphoplasmacytic sclerosing inflammation with numerous IgG4 positive cells. The patients had elevated serum total IgG and IgG4 levels. Treatment consisted of high dose methylpredinisolone (1 mg/kg/day) and demonstrated good responsiveness. However, one patient experienced 2 relapses while being tapered off of steroid treatment.

        Eight new D-1.3-dioxolanyl and L-1,3-oxathiolanyl nucleosides containing 5-phenyl- selenyl pyrimidine bases which are expected to have antiviral activity were synthesized. Condensation of D-1,3-dioxolane acetate and L-1,3-oxathiolane acetate with 5-phenylselenyl pyrimidines gave anomeric mixtures of their nucleosides which were separated by silicagel column chromatography.

        청소년 운동선수의 불행지각 구성요인과 측정도구 개발

        This study was aimed at exploring the factor of conceptual structure about unfortune exercise situations youth Ssiruem players perceive, and verifying validity with measure scale. For attaining these purpose, we sampled 652 participants, and developed items through the open-ended questionnaires based on each research steps, also identified internal validity via exploratory and confirmatory factor analysis. With the verification of difference between grades of structured sport unhappiness, we performed the examination of external relation, investigated possibility as measure scale. As a result of analysis, the component factors of sport unhappiness youth Ssiruem players perceive were completely created for five factors fifteen items such as achievement frustration, negative hierarchy, competition defeat, losing friend, and exercise punishment. According to the result of external relation about sport unhappiness, high school players were aware of more unhappy due to higher achievement frustration, losing friend, and exercise punishment than middle school players. In accordance with Stepwise F Verification such factors as achievement frustration and losing friend were sport unhappiness factors to well distinguish grades.

        중증 폐혈전색전증 환자에서 Urokinase와 Alteplase의 효과 및 출혈 합병증 비교

        연구배경: 폐혈전색전증 환자에서 혈전용해제의 효과는 차이가 없는 것으로 보고되어 있으나 출혈 등의 부작용에 대해서는 잘 알려져 있지 않다. 본 연구는 한 병원에서 중증 폐혈전색전증환자들에 사용한 alteplase와 urokinase의 치료효과 및 출혈 유발 정도를 비교하였다. 방법: 2000년 1월부터 2008년 7월까지 대상 환자 40명을 urokinase군(16명)과 alteplase군(24명)으로 나누어 의무기록을 후향적으로 분석하였다. 결과: 혈전용해제 사용한 이유는 혈역학적 불안정 23명, 중증 우심실부전 7명 그리고 헤파린 사용에도 불구하고 진행하는 저산소증 10명이었다. 두 군간 나이, APACHE II, SOFA score, 혈전용해제에서 유의한 차이는 없었다. 혈전용해제를 사용 후 승압제의 사용기간에서 aletplase군이 urokinase군보다 더 짧았으나, 인공호흡기 사용 기간과 중환자실 입원기간에서는 양 군간 유의한 차이가 없었다. 입원기간 동안 사망한 환자수는 13명(30%)으로 urokinase와 alteplase군 5명, 8명으로 유의한 차이는 없었고, 폐혈전색전증과 연관된 사망한 환자수에서도 urokinase와 alteplase군에서 1명, 4명으로 유의한 차이는 없었다. 혈전용해제 사용 후 수술적 치료가 필요한 환자수도 urokinase군 2명, alteplase군 1명으로 차이가 없었다. 혈전용해제 사용 후 발생한 출혈은 14명(35%)에서 관찰되었고 urokinase군 3명(7.5%), alteplase군 11명(27.5%)으로 urokinase군에서 적은 경향을 보였다. 중증 출혈은 2명으로 모두 alteplase군에서 발생하였으며 1명은 뇌출혈로 사망하였다. 결론: 중증 폐색전혈전증환자에서 urokinase 사용은 alteplase에 비하여 치료효과는 차이가 없으면서 출혈의 위험은 낮을 것으로 사료된다. Background: The efficacy of several thrombolytic agents for treating massive pulmonary thromboembolism (PTE) has been reported to be similar. However, the difference of the bleeding complications caused by two commonly used thrombolytic agents in PTE patients is not well known. The aim of this study was to compare the therapeutic efficacy and the bleeding complications between urokinase and recombinant tissue-type plasminogen activatior (rt-PA, alteplase) in a Korean medical center. Methods: We retrospectively reviewed the clinical data of the patients who were treated with thrombolytic agents (urokinase and alteplase) because of massive PTE. Results: A total of 40 patients were included: 16 (40%) treated with urokinase and 24 (60%) with alteplase. The patients treated with alteplase showed a shorter duration of using vasopressor agents than did the patients who were given urokinase, but the duration of mechanical ventilation, the length of the ICU stay and the hospital stay were not different between the thrombolytic agents. Five patients treated with urokinase and eight patients treated with alteplase died (p=0.565): One patient in the urokinase group and four patients in the alteplase group died due to pulmonary thromboembolism. Bleeding complications after thrombolysis were observed in 3 patients (7.5%) treated with urokinase and in 11 (27.5%) patients treated with alteplase (p=0.079). Major bleeding complication occurred in 2 patients who were treated with alteplase. Conclusion: Urokinase seems to have fewer bleeding complications with an equivalent efficacy, as compared to alteplase, in Korean patients who suffer with massive pulmonary thromboembolism.

        송풍량이 음식물쓰레기 발효건조에 미치는 영향

        본 연구는 음식물쓰레기의 발효건조를 위한 최적 운전조건을 도출하기 위하여 0.75, 1.00, 1.25, 1.50 L/min·kg의 송풍조건에서 발효건조 회분식 반응기를 20일간 운전하였으며, Modified Gompertz 모델을 이용하여 발효건조 기간 중 반응기내에서의 유기물 분해반응속도를 분석하였다. 유기물 분해 반응속도 분석에서 최대 유기물 분해량 (P)은 송풍량 0.75, 1.00, 1.25, 1.50 L/min·kg에서 각각 2.31, 2.52, 2.27, 1.88 kg이었으며, 최대 유기물 분해속도 (Rm)는 송풍량 0.75, 1.00, 1.25, 1.50 L/min·kg에서 각각 0.33, 0.45, 0.28, 0.18 kg/day를 보여 송풍량 1.00 L/min·kg에서 우수한 유기물 분해효율을 보였다. 발효건조 반응기의 지체성장시간 (λ)은 송풍량 0.75, 1.00, 1.25, 1.50 L/min·kg에서 각각 2.10, 1.48, 1.15, 1.06 일로 나타나 0.75 L/min·kg의 적은 송풍조건에서 가장 긴 지체성장시간을 보여 송풍량의 증가는 지체성장시간을 단축시키는 것으로 나타났다. 음식물쓰레기 발효건조 반응기의 운전에서 수분 제거율은 발효건조 반응기 운전 초기에서 중기로 갈수록 송풍량 증가와 함께 증가하다가 발효건조 반응기 운전 말기에는 송풍량 1.00 L/min·kg에서 가장 높은 수분제거율을 보여 발효건조의 최적 송풍조건은 1.00 L/min·kg으로 나타났다. This study was carried out for 20 days in a bio-drying batch reactor under the blowing conditions of 0.75, 1.00, 1.25, and 1.50 L/min·kg in order to optimize the operating conditions for the bio-drying of food wastes. The decomposition rate of organic matter during the bio-drying operation period was analyzed using modified Gompertz model. The maximum organic degradation (P) was 2.31, 2.52, 2.27 and 1.88 kg at air flow rates of 0.75, 1.00, 1.25 and 1.50 L/min·kg, and the maximum organic degradation rate was 0.33, 0.45, 0.28, and 0.18 kg/day at 1.00, 1.25 and 1.50 L/min·kg, respectively, showing excellent organic decomposition efficiency at a air flow rate of 1.00 L/min·kg. The lag growth phase time (λ) of the bio-drying reactor was 2.10, 1.48, 1.15, and 1.06 days at 0.75, 1.00, 1.25 and 1.50 L/min·kg, respectively. The water removal rate in the operation of bio-drying reactor of food waste increased with the increase of air flow rate from the early stage of bio-drying to the middle stage, and the highest water removal rate was observed at the air flow rate of 1.00 L/min·kg at the end of bio-drying. The optimum air flow rate condition of bio-drying reactor was 1.00 L/min·kg.

        거대억새(Miscanthus sacchariflorus)의 혐기소화를 위한 메탄생산 퍼텐셜 분석

        BACKGROUND: This study was carried out to assess a biochemical methane potential of giant miscanthus (Miscanthus sacchariflorus) which was a promising candidate energy crop due to a high biomass productivity, in order to utilize as a feedstock for the biogas production. METHODS AND RESULTS: Giant miscanthus was sampled the elapsing drying time of 6 months after harvesting. TS (Total Solid) and VS (Volatile Solid) contents were 94.7 and 90.8%. And CP (Crude Protein), EE (Ether Extracts), and CF (Crude Fiber) contents of giant miscanthus were 1.4, 0.46, and 46.12%, respectively. In the organic composition of giant miscanthus, the NDF (Neutral Detergent Fiber) representing cellulose, lignin, and hemicellulose contents showed 86.88%, and the ADF (Acid Detergent Fiber) representing cellulose and lignin contents was 62.91%. Elemental composition of giant miscanthus showed 47.75%, 6.44%, 41.00%, and 0.28% for C, H, O, and N, respectively, and then, theoretical methane potential was obtained to 0.502 N㎥ kg<sup>-1</sup>-VS<sub>added</sub>. Biochemical methane potential was assessed as the range of 0.154∼0.241 N㎥ kg<sup>-1</sup>-VS<sub>added</sub> resulting the lower organic biodegradability of 30.7∼48.0%. CONCLUSION: Therefore the development of pretreatment technology of the giant miscanthus was needed for the improvement of anaerobic digestability.

        중·고등학교 운동선수들의 코치-선수관계가 심리적 욕구 및 운동행동에 미치는 영향

        이 연구는 학생선수들이 지각하는 코치-선수관계와 심리적 욕구와의 관계를 살펴보고, 이러한 심리적 욕구를 매개변수로 하여 운동행동간의 관계를 규명하는 데 목적이 있다. 이러한 연구목적을 달성하기 위하여 수도권 소재 남자 고등학교 팀 스포츠 선수 242명을 편의표본추출법을 사용하여 연구대상으로 선정하였다. 결과분석은 기술통계와 상관분석, 단계적 다중회귀 분석, 경로분석을 실시하였다. 분석결과 첫째, 학생선수들이 지각한 코치-선수관계는 심리적 욕구에 직접적인 영향(직접효과=.33)을 미치며, 둘째, 코치-선수관계는 운동행동에 직접적인 영향(직접적 영향=.11)을 미치는 것으로 나타났다. 마지막으로 심리적 욕구는 코치-선수관계와 운동행동에 대한 간접적인 영향(간접영향=07)은 나타나지 않았다. 결론적으로 청소년들이 지각하는 코치-선수관계는 심리적 욕구와 운동행동에 직접적인 인과관계가 있지만, 심리적 욕구는 코치-선수관계와 운동행동 규제를 매개변인으로 예측하지 않는 것으로 나타났다. The purpose of this study was to examine the structural relationships among coach-athlete relationship, psychological needs, and movement behavior of high-school team sports players. In order to accomplish the purpose of this study, the high-school team players of Seoul/Gyeonggi province made the population and 242 of them were sampled through the convenience sampling. The instruments for data collection were Korean Coach-Athlete Relationship Questionnaire (Kim et. al., 2008), Psychological Needs Scale (Kim et al., 2008), and Behavioral on Exercise Questionnaire-2(Chun, 2009). Collected data were put to correlation analysis and stepwise regression, and path analysis was used to the predictive and mediational role of psychological needs. The findings revealed that the direct effect of coach-athlete relationship on the psychological needs was revealed .33, the direct effect of psychological needs on movement behavior was revealed .07, and the direct effect of coach-athlete relationship on movement behavior was revealed .11. Therefore, coach-athlete relationship had significant influences on psychological needs and movement behavior, but psychological needs had no mednational role on coach-athlete relationship and movement behavior in team sports players.
