A 36-year-old man suffred from nasal obstruction due to huge invasive pituitary adenoma which invaded the sphenoid sinus to nasal cavity, optic nerve and hypothalamic area. His initial GH value was high (56. 9 ng/ml). The patient was being processed of growing pituitary mass in spite of partial hypophysectomy twice and cobalt irradiation (about 6000 rad) once. After 5 months later from cessation of irradiation, he was referred to emergency room because of impending comatose state with high fever and unilateral hemiparesis and he was indulged in down-hill course. Follow-up computed tomography showed increased mass size and hydrocephalus and involved to hypothalamic area.
파클리탁셀의 하이드로트로픽 가용화를 위한 양친매성 Poly(4-vinyl benzoic acid-co-diethylacrylamide)의 합성 및 평가

This study describes the synthesis and characterization of micelle-forming amphiphilic copolymers comprised of vinylbenzoic acid (VBA) and diethylacrylamide (DEAAm) for hydrotropic solubilization of paclitaxel. For the synthesis of the copolymer, N-hydroxysuccinimide activated VBA (NSVB) and DEAAm were randomly copolymerized via reversible addition-fragmentation chain transfer (RAFT) polymerization to obtain poly(NSVB-co-DEAAm)(PND), followed by grafting of amine-terminated poly(ethylene glycol) methyl ether (mPEG) to NSVB. The resulting poly(VB-co-DEAAm)-gmPEG(PVDP) and PND were characterized by 1H-NMR and TGA. It was found that PVDP could significantly improve water solubility of PTX with increasing concentration of PVDP. At a low concentration, the aqueous solution of PVDP containing PTX looked opaque but became transparent with increasing PVDP concentration. The water solubility of PTX was increased up to 10000-fold at 8 wt% of PVDP, as compared with PTX alone. We measured the average diameter of PVDP at this concentration using a dynamic light scattering. The result showed that PVDP had 137 ± 18 nm in diameter, indicating that the improved water solubility of PTX was probably due to the micelle-forming amphiphilic nature of hydrotropic PVDP. Therefore, we expect that the amphiphilic PVDP copolymer with a hydrotropic property can be used to improve water solubility of poorly water-soluble drugs including PTX.

Horseradish peroxidase (HRP)-catalyzed in situ forming hydrogel systems― with their tunable gelation rate and mechanical properties―have wide applications in biomedicine. HRP has moderate substrate specificity that can be tuned easily by changing the precursor reactants, among which phenol-conjugated polymers are the most studied. In this study, we investigated the relationship between the functional groups on the polymer chains and the properties of the crosslinked hydrogels produced by HRP. As phenols and catechols have similar structures, we conjugated 4-arm poly(propylene oxide)-poly(ethylene oxide) (tetronic) derivatives with catecholamine (dopamine hydrochloride, DA) and phenylamine (tyramine, TA). We prepared tetronic-dopamine (TDA) and tetronic-tyramine (TTA) hydrogels by mixing an aqueous solution of TDA and TTA with HRP in the presence of hydrogen peroxide (H2O2) and characterized and systematically compared their gelation properties: gelation rate, mechanical strength, swelling ratio, adhesive strength, and cellular activity. The gelation pathways of TDA and TTA were similar, and their gelation properties were well controlled by varying the HRP and H2O2 concentrations. Despite their similar molecular structures, the TDA hydrogels showed lower crosslinking densities but superior adhesive strength than the TTA hydrogels at the same HRP and H2O2 concentrations. Neither hydrogel showed cytotoxicity toward encapsulated human dermal fibroblast cells.
쿠싱 증후군을 보인 부신피질 갈색선종(Brown Adenoma)에 관한 연구

Cushing's syndrome consists of the various metabolic abnormalities induced by chronic hypersecretion of the adrenocortical steroid hormones from adrenal tumor or hyperplasia of the adrenal cortex. Since the adrenal tissue contained a large amount of cholesterol and cholesterol derived substances, the tumor or hyperplastic tissue usually appears yellow on tissue cut section. However, functioning brown adenomas of adrenal cortex have occasionally been reported but the clinical significance of the brown adenomas have been obscure and there was no apparent correlation with age, sex, or any specific metabolic or neoplastic disease. Recently, brown adenoma reported that tumor tissues of brown adenoma had higher radiological density and accompanied lower serum aldosterone and urinary 17-KS levels than ordinary yellow tumor. We reported clinical chatacteristics of six patients with Cushing's syndrome caused by adrenal adenoma by use of abdominal CT for radiological density, hormonal profiles, and macroscopic and microscopic morphologic findings of the adrenal gland tumor. 1) There was no difference in regard to age, sex, disease duration, and size between brown and yellow adenoma. 2) Brown adenomas were composed of more compact cells microscopically than yellow adenomas. 3) Brown adenomas were revealed more dense compared not only to yellow adenoma but to kidney in radiologic density on abdominal CT. 4) The two patients with Brown adenoma had higher 24 hours urinary 17-OHCS excretion.

현재 우리의 일반적인 식생활 관습으로는 선진국의 칼로리, 단백질 섭취수준과 상당한 차이가 있어 대부분의 사람들의 경우 필요한 양의 영양을 충분히 섭취하지 못함으로 해서 영양 부족이 염려된다. 비싼 쇠고기 소비성향에서 닭고기나 계란 등 상대적으로 싼 동물성 식품을 골고루 섭취해 섬유질 위주에서 균형된 식생활로 전환해 활동에 필요한 충분한 영양섭취를 해야 한다 간혹 지방 또는 콜레스테롤 섭취가 필요이상 과다한 경우 규칙적인 운동과 균형식을 통해 시정하는 것이 바람직하다.
